Abstract
Description
(a)上記被験体におけるネイティブな(native:天然)アンフィレグリンに対する単離抗体、抗体フラグメント又は抗体模倣物、
(b)上記被験体におけるネイティブなアンフィレグリン前駆体に対する単離抗体、抗体フラグメント又は抗体模倣物、
(c)上記被験体におけるEGFRに対して遮断するような単離抗体、抗体フラグメント又は抗体模倣物、
(d)被験体におけるアンフィレグリンに感受性がある別の受容体に対して遮断するような単離抗体、抗体フラグメント又は抗体模倣物、
(e)EGFR阻害剤、及び、
(f)被験体におけるアンフィレグリンの発現を減少させることが可能な低分子干渉RNA(siRNA)剤、
からなる群から選択される。
(a)単離アンフィレグリン又はその生物学的に活性な断片若しくは誘導体、

調査対象母集団
屈折異常及び近視に関するコンソーシアム(Consortium of Refractive Error and Myopia)(CREAM)から、25個の研究からのヨーロッパ系の40036人の個体と、9個の研究からのアジア系の10315人の個体とを含む、全部で34個の研究を採用した。20歳未満の個体を除外するだけでなく、白内障手術、レーザー又は屈折を変えることができるその他の眼内処置を受けた個体も除外した。これらの研究の多くは、等価球面度数に対する以前のCREAMのゲノムワイド関連解析(GWAS)にも含まれていた。全ての研究は、ヘルシンキ宣言の主義に準拠しており、その地元の研究倫理委員会によって承認されている。全ての参加者は、研究開始前に書面によるインフォームドコンセントを提出している。
含まれる研究における参加者は、完全な眼科学的調査を受けた。非瞳孔散大屈折(Non-dilated refraction)は、他覚的屈折及び/又は自覚的屈折によって測定した。等価球面度数は、各々の眼について、球面度数に円柱度数の半分を足したものとして計算した。右眼と左眼の平均等価球面度数は、定量的アウトカムとして使用した。片眼だけからのデータしか利用することができない場合に、その眼の等価球面度数を使用した。教育について、被験体は、修了した教育の最も高いレベル又は学校教育の年数を、自己報告によるアンケートを通じて又はインタビューにおいて報告した。
それぞれの研究は、GWASのために厳格な品質管理フィルタを適用した。一般的に、重複、低いコールレート(95%未満)、性別ミスマッチ又は集団異常値を反映している個体を除外した。SNPは、低いジェノタイピングコールレート(5%を上回る欠測)、単型SNP(MAF<1%)の場合に、又はハーディー・ワインバーグ不平衡(p値<10−6)において除外した。品質管理(QC)フィルタリングの後に、それぞれの研究のアレイ遺伝子型を、リファレンスパネルとしての1000個のゲノムプロジェクトデータ(ビルド37、フェーズ1リリース、2012年3月)を使用してソフトウェアMinimac又はIMPUTEでインピュートした。インピュテーション品質閾値(MACH:r2>0.5又はIMPUTE情報スコア>0.5)に合格したマイナーアレル頻度≧5%を有するSNPを、メタ解析のために持ち越した。
それぞれの研究について、それぞれジェノタイプ又はインピュートされたSNPで線形回帰モデルを、アウトカムとして平均等価球面度数を用いて構築した。相加的遺伝子モデルは、リスクアレルの数が直接的にジェノタイプされたSNPについて順序変数(0、1又は2)である場合、又はインピュートされたSNPについてアレル数の見込み(allele dosage probability)が0から2までの範囲の連続的な変数である場合に仮定した。主要解析モデルは、SNP、教育、SNP×教育相互作用という用語、並びに共変量としての年齢及び性別を含むものとした。ゲノムマーカーのバリエーションの上位の主要成分についての追加の調節は、適用することができるならば(すなわち、集団階層化の証拠が存在する場合に)個々の研究において実施した。

(式中、Yは、平均等価球面度数であり、教育は、二分法変数(0は、低教育群であり、かつ1は、高教育群である)であり、Covは、年齢、性別のような一式の共変量であり、適用することができるならば最初の上位5つの主要成分である)を、SNP(βSNP)及びSNP×教育相互作用(βSNP×教育)の、平均等価球面度数に対する共同効果について検定するために使用した。家系ベースの研究のために、血縁行列を、SNPデータから経験的に推定し、一般化混合モデルにおけるランダム効果として含めた。SNP×教育相互作用の効果を検定するために、βSNP×教育を、モデル1から評価した。

(式中、変数の定義は、モデル1と同じである)を使用して調節することによって検定した。
固定効果モデルによる等価球面度数に関する主SNP効果及びSNP×教育相互作用の両方を、SNP及びSNP×教育回帰係数並びに各研究からのβの共分散行列を用いて同時に検定するために共同メタ解析(JMA)アプローチを採用した。自由度2を有するカイ二乗分布に従うWald統計を、SNP及びSNP×教育回帰係数の同時有意性を検定するために使用した。JMAは、METAL(http://www.sph.umich.edu/csg/abecasis/metal/)によってスクリプトパッチを用いて実施した。コクランのQ検定を、SNP及び相互作用効果についての研究にわたるβ係数の異質性を評価するために使用した。SNPと教育との間の相互作用について検定するために、等価球面度数に関するSNP×教育相互作用の効果の二次メタ解析(自由度1)を、固定効果モデルで逆分散法を用いてMETALにおいて実施した。これは、SNP×教育相互作用自体を調査するための古典的なメタ解析である。低教育群及び高教育群における、SNP(βSNP)の等価球面度数に対する効果及び標準誤差(βSNP+βSNP×教育)が、JMA出力から導き出された。
位置情報及び多様体識別名は、NCBIのB37(hg19)ゲノムビルドに報告され、UCSCゲノムブラウザを用いてアノテーションされる。1000個のゲノムプロジェクト(トップSNPに隣接する100Kb;hg19)のヨーロッパ系集団及びアジア系集団における連鎖不平衡(LD)ブロック(r2>0.8)のそれぞれの範囲内の多様体を同定して、転写調節の実験的証拠により、HaploReg(http://www.broadinstitute.org/mammals/haploreg/haploreg.php)及びEncyclopedia of DNA Elements(ENCODE)のデータを用いて機能アノテーションを適用した。
ヒト組織における遺伝子発現を評価するために、眼組織データベース(https://genome.uiowa.edu/otdb)及びEyeSAGEデータベースを調査した。推定された遺伝子及びエクソームレベルの存在度は、オンラインで入手可能である。遺伝子発現の正規化は、GCバックグラウンド補正を伴うプローブ対数強度誤差(Probe Logarithmic Intensity Error)(PLIER)法を使用した。
教育及びその等価球面度数に対する主効果
本発明者らのメタ解析における34個の研究からの50351人の参加者のベースライン特性は、全体で40036人の被験体がヨーロッパ系であり、10315人がアジア系であり、参加者の年齢が、20歳から99歳までの範囲であったことを示している。ヨーロッパ系のなかで、後期中等教育を終えた参加者の割合は、16.0%から94.4%までの範囲であり、平均は50.7%であった。アジア人においては、後期中等教育を終えた個体の割合は、6.7%から75.9%までの範囲であり、平均は30.0%であった。全体の研究を通じて、高教育群の個体は、平均で0.59のジオプトリー(D)である等価球面度数屈折異常を有しており、低教育群のそれと比較してより近視性が高く、すなわち、より遠視性が低かった(β=−0.59;95% CI:−0.64、−0.55)。高い教育レベルは、アジア人の個体ではヨーロッパ系と比較して、2倍も高い近視性の等価球面度数と関連していた(アジア系:β=−1.09、95% CI:−1.20、−0.98;ヨーロッパ系:β=−0.49、95% CI:−0.54、−0.44)。
ヨーロッパ人における共同メタ解析
40036人のヨーロッパ系の個体におけるJMAによるSNP主効果及びSNP×教育相互作用についてのゲノムワイド共同解析は、9種の以前から関係があるとされていた座位で等価球面度数との関連を示した。さらに、等価球面度数と関連した4種の以前には報告されていなかった座位も同定され、ゲノムワイド有意性に達した(JMAについてのP<5.0×10−8;表1及び図3A):FAM150B、LINC00340、FBN1及びDIS3L−MAP2K1。それらのうちの2種(FAM150B及びDIS3L−MAP2K1)を、アジア人において繰り返した(JMAについてのP<0.05;後続のセクション参照)。JMAにおける異質性の証拠は、研究全体を通して殆ど示されなかった(Q検定:Phet≧0.086)。ヨーロッパ人におけるこれらの座位でのJMAについての有意な関連は、主として、低教育層及び高教育層の両方におけるSNP効果によるものであった(それぞれ、4.40×10−8≦P≦1.35×10−6、7.61×10−11≦P≦1.75×10−6)。SNP×教育相互作用は、有意差無しであった(相互作用についてのP≧0.208)。等価球面度数に対するSNP効果の推定される効果サイズは、教育層全体にわたって非常に似通っていた。
アジア人における共同メタ解析
アジア人コホートからの10315人の個体における等価球面度数についてのJMAは、3種の遺伝子AREG、GABRR1及びPDE10Aについてのゲノムワイドの有意な関連を特定した(JMAについてのP<5.0×10−8;表2及び図3B)。等価球面度数と関連したSNP×教育相互作用効果は、全ての3種の座位で観察された(相互作用についてのP≦8.48×10−5)。遺伝子型及び表現型の関連は、高教育層において高度に有意であったが(1.97×10−10≦P≦8.16×10−8)、低教育層においてはかなり弱かった(0.008≦P≦0.243)。AREG、GABRR1又はPDE10A内のインデックスSNPでの研究間異質性の証拠は存在しなかった(Q検定:Phet≧0.122)。
全てのコホートの共同メタ解析
引き続き、組み合わせメタ解析を、全部で34個の研究のヨーロッパ人及びアジア人の両方の被験体を含めて実施した。この解析により、2種の追加のSNP:ARID2(JMAについてのP=4.38×10−8)及びSLC14A2(JMAについてのP=2.54×10−8)が明らかになった。両方の座位は、ヨーロッパ人コホートにおける等価球面度数と示唆的な関連を示したが、その関連はアジア人コホートにおいては弱まった(表1)。ゲノムワイドの有意な関連は、以前のCREAM GWASで同定された17種の既知の座位についての等価球面度数でも検出された。
GWASで既知の座位についての遺伝子及び教育の相互作用
最近の2つの大規模GWASから同定された39種の座位での以前に報告された等価球面度数との関連に関して、教育との相互作用を評価した。2つのSNP×教育相互作用は、名目的に有意差があった:ヨーロッパ人におけるTJP2(rs11145488;相互作用についてのP=6.91×10−3)及びアジア人におけるSHISA6−DNAH9(rs2969180;相互作用についてのP=4.02×10−3)。一般的に、39種の座位で検定されたインデックスSNPは、等価球面度数に対してのより大きなSNP×教育相互作用効果を有していた(倍率変化に関するメタ回帰のP<0.001)。同じ方向の相互作用効果を有する20種のSNPについて、相互作用効果の規模は、アジア人においてヨーロッパ人よりも平均して4倍大きかった(メタ回帰のP=0.003)。
ヒト組織における遺伝子発現
眼組織データベースを使用して、関連遺伝子の発現を、20人の正常なヒトドナーの眼において調査した。同定された遺伝子の大部分は、ヒトの網膜、強膜又は網膜色素上皮(RPE)で発現していた(表3)。これらの遺伝子のなかで、GABRR1は、網膜において最も高い発現を有した。網膜におけるGABRR1の発現のPLIERで正規化したレベルは、121.7であり、強膜においては21.5の発現値を有し、それは、GABRR1の発現が、網膜内でより優勢であることを示唆している。FAM150Bは、脈絡膜/RPEにおいて多く存在することが分かったが(333.3の発現値)、網膜中では大幅にそれより低いレベルで発現していた(29.9)。MAP2K1は、網膜、強膜及び脈絡膜中で広く発現しており、85.7より高い発現値を有していた。
上記データは、屈折異常に関する遺伝子及び教育の相互作用の今日で最も包括的なゲノムワイド解析を表す。屈折異常に関連する新規の遺伝子座を、大規模な多民族集団におけるSNP及びSNP×教育の効果の共同的寄与を検定することによって同定した。3種の遺伝子(AREG、GABRR1及びPDE10A)は、アジア系の集団において教育との強い相互作用を示した。17種の以前に発表された座位での等価球面度数との既知の関連を確認するだけでなく、組み合わされた多民族コホートを使用して、等価球面度数と有意に関連する6種の新たな座位(FAM150B、LINC00340、FBN1、DIS3L−MAP2K1、ARID2及びSLC14A2)が同定された。
Claims (15)
- 被験体における上皮成長因子受容体(EGFR)のシグナリング及び/又はアンフィレグリンに感受性がある別の受容体のシグナリングを直接的又は間接的に減少させることができる、前記被験体における近視の予防的処置又は治療的処置において使用するための作用物質。
- 前記作用物質は、
(a)前記被験体におけるネイティブなアンフィレグリンに対する単離抗体、抗体フラグメント又は抗体模倣物、
(b)前記被験体におけるネイティブなアンフィレグリン前駆体に対する単離抗体、抗体フラグメント又は抗体模倣物、
(c)前記被験体におけるEGFRに対して遮断するような単離抗体、抗体フラグメント又は抗体模倣物、
(d)被験体におけるアンフィレグリンに感受性がある別の受容体に対して遮断するような単離抗体、抗体フラグメント又は抗体模倣物、
(e)EGFR阻害剤、及び、
(f)被験体におけるアンフィレグリンの発現を減少させることが可能な低分子干渉RNA(siRNA)剤、
からなる群から選択される、請求項1に記載の使用のための作用物質。 - 前記抗体フラグメントは、Fabフラグメント、F(ab’)2フラグメント及びFab’フラグメントからなる群から選択される、請求項2に記載の使用のための作用物質。
- 前記抗体模倣物は、単鎖可変フラグメント(scFv)、単独ドメイン抗体、アフィボディ、アフィリン、アフィマー、アフィチン、アンチカリン、DARPin、モノボディ及びペプチドアプタマーからなる群から選択される、請求項2又は3に記載の使用のための作用物質。
- 硝子体内、角膜上、経角膜、経強膜、経結膜、結膜下、眼内、又は全身に投与される、請求項1〜4のいずれか一項に記載の使用のための作用物質。
- 被験体における上皮成長因子受容体(EGFR)のシグナリング及び/又はアンフィレグリンに感受性がある別の受容体のシグナリングを増大させることができる、前記被験体における遠視の予防的処置又は治療的処置において使用するための作用物質。
- 前記作用物質は、
(a)単離アンフィレグリン又はその生物学的に活性な断片若しくは誘導体、
(b)単離アンフィレグリン前駆体又はその生物学的に活性な断片若しくは誘導体、
(c)前記被験体における上皮成長因子受容体(EGFR)に対して活性化するような単離抗体、抗体フラグメント又は抗体模倣物、
(d)被験体におけるアンフィレグリンに感受性がある別の受容体に対して活性化するような単離抗体、抗体フラグメント又は抗体模倣物、及び、
(e)EGFR活性化剤、
からなる群から選択される、請求項6に記載の使用のための作用物質。 - 前記アンフィレグリンは、配列番号1に示されるアミノ酸配列を含む、請求項7に記載の使用のための作用物質。
- 前記アンフィレグリンは、配列番号1に示されるアミノ酸配列からなる、請求項7又は8に記載の使用のための作用物質。
- アンフィレグリンの前記生物学的に活性な断片又は誘導体は、配列番号1に示されるアミノ酸配列に対して、少なくとも50%の配列同一性を有するアミノ酸配列を含む、請求項7〜9のいずれか一項に記載の使用のための作用物質。
- 前記抗体フラグメントは、Fabフラグメント、F(ab’)2フラグメント及びFab’フラグメントからなる群から選択される、請求項7〜10のいずれか一項に記載の使用のための作用物質。
- 前記抗体模倣物は、単鎖可変フラグメント(scFv)、単独ドメイン抗体、アフィボディ、アフィリン、アフィマー、アフィチン、アンチカリン、DARPin、モノボディ及びペプチドアプタマーからなる群から選択される、請求項7〜11のいずれか一項に記載の使用のための作用物質。
- 硝子体内、角膜上、経角膜、経強膜、経結膜、結膜下、眼内、又は全身に投与される、請求項6〜12のいずれか一項に記載の使用のための作用物質。
- 被験体において近視若しくは遠視、又は近視若しくは遠視を発症する傾向を診断する方法であって、
(a)該被験体から生物学的試料を得る工程と、
(b)前記試料中のアンフィレグリンレベルを測定する工程と、
(c)工程(b)で測定されたレベルと、正視の被験体又は正視になりかかっている被験体において見られるアンフィレグリンレベルとを比較する工程と、
(d)工程(b)で測定されたレベルが正視の被験体又は正視になりかかっている被験体において見られるアンフィレグリンレベルより高い場合に、前記被験体が近視であるか、又は近視を発症する傾向にあると判定し、また工程(b)で測定されたレベルが正視の被験体又は正視になりかかっている被験体において見られるアンフィレグリンレベルより低い場合に、前記被験体が遠視であるか、又は遠視を発症する傾向にあると判定する工程と、
を含む、方法。 - アンフィレグリン又はその断片若しくは多様体と会合する作用物質を同定する方法であって、
