Abstract
Description
−骨形成:移植片又は宿主に由来する細胞からの新しい骨の合成。それは骨を生成することができる細胞を必要とする。
−骨誘導:移植片のマトリックスに由来する成長因子によって調節される細胞の分化及び動員プロセスであり、その骨形成活性は骨ミネラル抽出によって刺激される。
−骨伝導:移植片の生物学及び宿主−移植片接合部分の力学的環境によって決定される予測可能なパターンにより、移植片の材料が新しい骨の付加に適した物理的材料、構造又は環境を提供するプロセス。
A.自家移植片:患者自身から得られた骨を意味し、従って抗原性はほとんどない。それは、小さな欠損部への使用では口腔内の部位(顎、上顎結節、上行枝)から得られ、多くの量が必要とされる時には口腔外の部位(腸骨稜、脛骨又は頭蓋窩)から得られる。各アプローチは、骨空洞のタイプ、大きさ及び形状、並びに専門家の臨床経験及び好みに応じて選択される。
B.同種異系移植片又は同種移植片:同種の個体に由来するが、遺伝的に異なる。これらは、処理に応じて次の通り分類されることができる。
−凍結した同種移植片
−凍結乾燥(フリーズドライの)同種移植片
−凍結乾燥及び脱灰された同種移植片
−放射線照射骨
C.異種移植片:これは、別の(動物)種からの天然の骨ミネラルを含有する天然移植片である。例えば、ウシ骨及びサンゴ派生物(Ostrix、Osteogen、Bio-oss、Interpore)である。
D.アロプラスチック又は人工移植片:これらは、合成により製造された材料から得られる。それらには、様々な形、大きさ及び構造がある。骨の生物学的反応は、製造技術、結晶化度、多孔度及び再吸収の程度に依存する。
−チタンは不活性である。組織と接触する酸化被膜は不溶性であるため、有機分子と反応する可能性のあるイオンは放出されない。
−生体組織中のチタンは、骨が成長して金属に付着する表面を示す。

・良好な機械的特性を有する。その引っ張り強さは、荷重を受ける外科用プロテーゼに使用されるステンレス鋼の引っ張り強度と非常に類似している。象牙質又はいずれの骨皮質よりもはるかに強く、インプラントが重い荷重に耐えることを可能にする。
・この金属は柔らかく可鍛性であり、それは衝撃荷重を吸収するのに役立つ。
−細胞閉塞:閉塞性バリア(1)は、傷の空間におけるフィブリン血餅の成熟から手術中に開かれた皮膚弁の歯肉組織から分離される特性を有する。

Claims (7)
- 骨再生用の閉塞性バリア(1)を製造するための製造方法であって、
−断層画像を取得するステップと、
−前記断層画像を処理してデジタル化するステップと、
−前記断層画像のノイズを低減するステップと、
−前記断層画像を三次元CADファイルに変換するステップと、
−前記三次元CADファイルをCADモデリングソフトウェアへインポートするステップと、
−前記閉塞性バリア(1)に対応する領域を定めるステップと、
−前記閉塞性バリア(1)に対応する表面を生成するステップと、
−CAMソフトウェアによって前記閉塞性バリア(1)をレイヤーに変換するステップと、
−前記レイヤーに従ってチタンで前記閉塞性バリア(1)を形成するステップと、
−前記閉塞性バリア(1)に熱処理を施すステップと、
−前記閉塞性バリア(1)にサンドブラスト処理を施すステップと、
−前記閉塞性バリア(1)のオリフィス(4)、厚さ及び寸法を光学式ゲージで検査するステップと、
を含むことを特徴とする製造方法。 - チタンにおいて前記閉塞性バリア(1)を形成する前記レイヤーは、30又は60μmの厚さを有することを特徴とする請求項1に記載の製造方法。
- 前記閉塞性バリア(1)に滅菌処理を施すことを更に含むことを特徴とする請求項1又は2に記載の製造方法。
- 前記閉塞性バリア(1)が1つの部品で形成されることを特徴とする請求項1〜3のいずれか一項に記載の製造方法。
- 前記閉塞性バリア(1)が少なくとも2つの部分で形成されることを特徴とする請求項1〜3のいずれか一項に記載の製造方法。
- 前記部品は、相補的であり、歯(6)が収容されることができる少なくとも1つの貫通孔を共同して定めることを特徴とする請求項5に記載の製造方法。
- 請求項1〜6のいずれか一項に記載の製造方法によって得られる、骨再生のための閉塞性バリア(1)。
