Abstract
Description

Claims (15)
- 後角、後根、及び後柱を含む神経系を有する患者に神経変調を印加するためのシステムであって、
後角刺激を前記後角に送出するように構成された変調出力回路と、
複数の変調パラメータを使用して神経変調アルゴリズムを実行することにより、前記後角刺激の前記送出を制御するように構成された変調制御回路であって、
前記変調制御回路が、
前記後角の前記刺激に対する1又は2以上の反応を示す反応情報を受信するように構成された反応入力と、
前記反応情報を使用して前記複数の変調パラメータのうちの1又は2以上の変調パラメータを調節するように構成されたパラメータ較正器と、
を含む、前記変調制御回路と、
を含むことを特徴とするシステム。 - 前記変調制御回路は、前記後柱の上の前記後角を選択的に刺激する知覚以下後角刺激を送出するための神経変調アルゴリズムを実行することにより、前記後角刺激の前記送出を制御するように構成されることを特徴とする請求項1に記載のシステム。
- 前記反応入力は、各々が前記後角刺激に対する前記1又は2以上の反応のうちの反応の定量的尺度である1又は2以上の反応パラメータを含む前記反応情報を受信するように構成され、前記パラメータ較正器は、前記複数の変調パラメータのうちの前記1又は2以上の変調パラメータを調節して該1又は2以上の反応パラメータをターゲット値範囲内に維持するように構成されることを特徴とする請求項1又は請求項2に記載のシステム。
- 疼痛に関連付けられた生理的機能を各々が示す1又は2以上の神経生理学的信号を感知するように構成された1又は2以上のセンサと、
前記1又は2以上の神経生理学的信号から前記1又は2以上の反応パラメータを導出するように構成されたパラメータ発生器と、
を更に含むことを特徴とする請求項3に記載のシステム。 - 前記変調出力回路は、後根刺激を前記後根に送出するように更に構成され、前記変調制御回路は、該後根刺激と交互配置された前記後角刺激を送出するための試験アルゴリズムを実行することにより、該後角刺激の前記送出を制御し、かつ該後根刺激の前記送出を制御するように構成され、
前記後柱内の前記後根刺激に対する反応を感知するように構成されたセンサであって、該反応が前記後角刺激によって変調される前記センサ、
を更に含むことを特徴とする請求項3に記載のシステム。 - 前記センサは、前記後柱内の軸索内の局所電場電位(LFP)を感知するように構成され、前記パラメータ発生器は、該感知されたLFPから前記1又は2以上の反応パラメータのうちの1又は2以上のパラメータを導出するように構成されることを特徴とする請求項5に記載のシステム。
- 前記センサは、前記後柱内の軸索内の誘発複合活動電位(eCAP)を感知するように構成され、前記パラメータ発生器は、該感知されたeCAPから前記1又は2以上の反応パラメータのうちの1又は2以上のパラメータを導出するように構成されることを特徴とする請求項5に記載のシステム。
- 前記変調出力回路と、前記反応入力と、前記変調制御回路とを含む埋込可能な変調デバイスを含み、
前記後角刺激に対する前記1又は2以上の反応をモニタして前記反応情報を生成するように構成されたモニタデバイスを更に含む、
ことを特徴とする請求項1から請求項7のいずれか1項に記載のシステム。 - 前記モニタデバイスは、埋込可能なモニタデバイスを含むことを特徴とする請求項8に記載のシステム。
- 前記モニタデバイスは、携帯型モニタデバイスを含むことを特徴とする請求項8に記載のシステム。
- 前記モニタデバイスは、着用可能なモニタデバイスを含むことを特徴とする請求項10に記載のシステム。
- 前記埋込可能な変調デバイスは、無線リンクを通じて前記モニタデバイスに通信的に結合されることを特徴とする請求項8から請求項11のいずれか1項に記載のシステム。
- 前記モニタデバイスは、前記埋込可能な変調デバイス内に統合されることを特徴とする請求項8に記載のシステム。
- 前記モニタデバイスは、定量的感覚試験、脳波検査(EEG)、皮質脳波検査(ECoG)、拡散光学撮像、及び機能的磁気共鳴映像法(fMRI)のうちの1又は2以上を実行するように構成されることを特徴とする請求項8に記載のシステム。
- 前記変調出力回路は、前記後角に電気パルスを送出するように構成されることを特徴とする請求項1から請求項14のいずれか1項に記載のシステム。
