Abstract
Description
本願は、2015年5月28日に出願されたUS62/004,144の非仮出願であり、全ての目的のために完全に引用している。
2014年5月28日に作成された16,655キロバイトの446850SEQLIST.txtと称する配列表を引用している。
Tat-NR2B9c(NA-1)は、PSD-95を阻害する薬剤であることから、N-メチル-D-アスパラギン酸受容体(NMDAR)と神経型一酸化窒素合成酵素(nNOS)との結合を破壊して、脳虚血によって誘導される興奮毒性を低減する。治療により、梗塞サイズ及び機能障害は、低減される。TAT-NR2B9cは、第二相試験が良好であった(WO 2010144721及びAarts et al., Science 298, 846-850 (2002) (Hill et al., The Lancet Neurology, 11:942 - 950 (2012))を参照)。
活性成分と同様に、凍結乾燥製剤は、構成成分に関する以下のクラスを1又は複数含むことができる。上記クラスは、排他的でなく、言い換えれば、同じ薬剤の構成成分は、複数のクラスに含まれる。
I.概要
本発明は、活性薬剤、特にTAT-NR2B9cの凍結乾燥製剤を提供する。かかる製剤は、室温で安定であることから、疾患又は事故の現場、又は、かかる現場と医療施設との間において投与する救急隊員又は救急車等においてかかる製剤の供給の維持管理が容易になる。かかる製剤は、冷蔵庫又は冷凍庫が利用出来ない場合であっても、周囲温度で保存することができることから、病院外の場所でも投薬できる。
多くの記述では、活性薬剤TAT-NR2B9cを例として示しているが、後述するように、別の活性薬剤を、TAT-NR2B9cについて記載されている原理に従って調製することができる。TAT-NR2B9cの特定の濃度は、他の薬剤と同じように用いることができる又は他の薬剤とTAT-NR2B9cとの等モル濃度となるように変換することができる。活性薬剤は、PSD-95に結合することによって、PSD-95と1又は複数のNMDAR(例えば、2A、2B、2C又は2D)又はnNOS(例えば、Swiss-Prot P29475)との間の相互作用を阻害する。かかる薬剤は、脳卒中及び、NMDAR興奮毒性によって少なとも部分的に媒介される他の神経学的状態による1又は複数の損傷作用を低減するのに有効である。かかる薬剤には、NMDA受容体のPLモチーフ又はPSD-95のPDZドメインを含む、又は、それに基づくアミノ酸配列を有するペプチドが含まれる。かかるペプチドは、PSD-95とnNOSと他のグルタミン酸受容体(例えば、カイニット受容体又はAMPA受容体)、例えばKV1.4とGluR6との間の相互作用を阻害することもできる。好ましいペプチドは、シナプス後肥厚-95タンパク質(PSD-95)(ヒトアミノ酸配列は、Stathakism, Genomics 44(1):71-82 (1997)が提供)のPDZドメイン1及び2と、ニューロンN-メチル-D-アスパラギン酸受容体(Mandich et al., Genomics 22, 216-8 (1994))のNR2Bサブユニットを含む1又は複数のNMDA受容体2サブユニットのC末端PL配列との間の相互作用を阻害する。NMDAR2Bは、GenBank ID 4099612、C末端20アミノ酸FNGSSNGHVYEKLSSIESDV(配列番号:11)及びPLモチーフESDV(配列番号:12)を有する。好ましいペプチドは、ヒト形態のPSD-95ヒトNMDAR受容体を阻害する。しかしながら、阻害は、タンパク質の種変異体からも示すことできる。使用可能なNMDA及びグルタミン酸受容体のリストを以下に示す。
活性薬剤は、遊離酸又は塩基として提供することができるが、より一般的には、塩として提供される。上述のタイプのペプチドは、一般的に、固相合成によって作られる。固相合成では、トリフルオロアセテート(TFA)を用いて保護基を除去するか樹脂からペプチドを除去するため、ペプチドは一般的に、トリフルオロ酢酸塩として最初は生産される。トリフルオロアセテートは、例えば、固体担体(例えばカラム)にペプチドを結合するステップと、カラムを洗浄して既存のカウンターイオンを取り除くステップと、新たなカウンターイオンを含む溶液でカラムを平衡化させるステップと、例えば、疎水性溶媒(例:アセトニトリル)をカラムに導入することによってペプチドを溶出させるステップによって、別のアニオンと置換することができる。アセテートへのトリフルオロアセテートの置換は、ペプチドを別の従来の固相合成法で溶出させる前に、最終工程としてアセテート洗浄で行なうことができる。そのようなもとして、WO2010144721又はPCT/US2013/071755等に記載されているように、現在までに、合成ペプチドTAT-NR2B9cでのケースは存在している。トリフルオロアセテート又はアセテートは、同時係属出願057769-446849にて説明したように、他の塩(例えば塩化物)と置換することもできる。あるイオンによるそのカウンターイオンとの置換は効率的に生じるが、最終的なカウンターイオンの純度は100%未満であってもよい。したがって、特定のカウンターイオンを有する塩としてのTAT-NR2B9c又は他の活性薬剤に関する言及は、特定のカウンターイオンが塩の凝集体に存在する同じ極性の他の全てのカウンターイオンに対して、重量又はモル当たりで主要なイオンであることを意味する。言い換えると、特定のカウンターイオンは、塩に存在する(同じ極性の)全てのカウンターイオンの50重量又はモル%超、好ましくは75重量又はモル%、95重量又はモル%超、99重量又はモル%超又は99.9重量又はモル%超で構成されている。
凍結乾燥製剤は、様々な疾患及び状態、特に神経疾患及び特に興奮毒性によって部分的には媒介される疾患を治療するのに有効である。かかる疾患及び状態として、脳卒中、てんかん、低酸素症、くも膜下出血、脳卒中(例えば外傷性脳損傷及び脊髄損傷)と関連していないCNSに対する外傷、他の脳虚血、アルツハイマー病及びパーキンソン病が挙げられる。興奮毒性と関連していることが知られていない、本発明の薬剤によって治療可能な他の神経疾患及び状態には、不安及び痛みが含まれる。
0:症状が全くない。
1:症状にもかかわらず重大な障害がない;全ての通常業務及び活動が実施可能。
2:軽度の障害;前の全ての活動が実施不可能であるが、補助なく自身のことに気を配ることができる。
3:中等度の障害ある程度の補助は必要であるが、補助なしで歩くことが可能。
4:中等度から重度の障害;補助なしで歩くことができない、且つ、補助なしで自身の身体の要求に対応することができない。
5:重度の障害;寝たきり、失調及び一定の看護及び配慮が必要。
再構成後、凍結乾燥製剤は、治療する疾患を患っている患者の疾患の少なくとも一つの兆候又は症状の更なる悪化を治癒するか、低減させるか、又は阻害するのに有効な量、頻度及び投与経路にて活性薬剤(例えば、NR2B9c)が投与されるように投与される。治療上有効量は、著しく本発明の薬剤で治療されない、疾患又は状態を患っている患者(又は、動物モデル)のコントロール群の損傷と比較して、本発明の薬剤で治療される疾患を患っている患者(又は、動物モデル)群において治療される疾患又は状態の少なくとも一つの兆候又は症状の更なる悪化を治癒するか、低減させるか又は阻害するのに十分な活性薬剤の量を意味する。上記量は、個々に治療した患者が本発明の方法によって治療されていない比較患者のコントロール群における平均転帰より良好な転帰を達成する場合、治療的に有効であるとも考えられる。治療的に有効な投与計画には、本発明の目的を達成するの必要な投与頻度及び経路で治療的に有効な量を投与することが含まれる。
5つの液体毒性製剤を調合して、20mg/mLの濃度のTAT-NR2B9Cとした。表1には、ビヒクル組成、ロット番号及び、調合時の力価、純度並びにpHを挙げている。約5mLの各製剤を、試験用バイアル瓶に入れた。バイアル瓶を-20℃で凍結させて、輸送又は液体保管条件をシミュレーションした。
2製剤#2の純度は、他の製剤より顕著に低かった。
3ホスファート緩衝製剤のpHは、初期緩衝pHの7.0から顕著にそれた。
虚血の3-軟膜血管閉塞モデル
実験をラットに実施した。以前記載したように、永続的な3-軟膜血管閉塞(3PVO)を実施した[Angiogenic protection from focal ischemia with angiotensin II type 1 receptor blockade in the rat. Forder et al., Am J Physiol Heart Circ Physiol. 2005 Apr;288(4):H1989-96]。要するに、250g〜350gのラットに、ケタミン(100mg/kg)、アセプロマジン(2mg/kg)及びキシラジン(5mg/kg)を0.5ml/kg筋内注射して麻酔した。必要に応じて初回量の1/3を追加した。肛門温度プローブを挿入して、動物を約37℃に維持した加温パッドに置いた。頭蓋骨を正中切開を経て露出させて、組織がないように掻き取った。解剖顕微鏡及び空気式歯科用ドリルを用いて、頭蓋骨に長方形を穿設し、頭蓋骨の断片から持ち上げつつ硬膜を完全に保ちながら6〜8mmの頭蓋窓を右体性感覚皮質上に作製した(前頂部に対して尾側に2mm、横方向に5mm)。3-軟膜細動脈の中大脳動脈分岐を、軸周りに焼灼した。皮質を選択して、硬膜を通じて電気的に焼灼した。焼灼後、頭皮を縫合した。ラットが完全に復帰するまで、各ラットを、体温を維持するための加温灯下の個々のケージに戻した。食品及び水を供給した。3PVO虚血の1時間後、ラット重量に基づいて〜0.45mL生理食塩水中に3nmol/gのNA1製剤をラットに導入した。5分間かけて容量を投与した。
賦形剤スクリーニングのために、10種類の緩衝剤を調合して1mg/mLのTAT-NR2B9Cとした。サンプルを、25℃/60%の相対湿度(RH)及び40℃/75%のRHにて保存した。t=0及びt=1週目の安定性(純度)を、RP-HPLC(TFA及びMSA方法)による純度に関してサンプルを試験した。結果を表2及び3に示す。
この研究の目的は、液剤中のTAT-NR2B9C安定性及びpHに対する塩化ナトリウム(NaCl)の影響を証明することとした。1mg/mLのTAT-NR2B9Cを有する緩衝作用をもつ製剤を表5にリストしている。pHに関する結果は表6に提供している。このデータは、研究の間、かなり一貫した結果を示している。しかしながら、顕著なシフトが、NaClを追加したシトラートで起こり、緩衝能に影響を与えて、pHが〜0.2ユニット低下した。選択したpH6.0及び6.5は、シトラートの理想的な緩衝範囲(pH 2.5-5.6)の外側にあるため、調合プロセスの間の様々な添加剤の添加が困難になる可能性があり、製剤の頑丈さを評価する際には考慮すべきである。
凍結乾燥の際に良好な固形物にして安定性を向上させる増量剤を同定するために、表8で概説されるように、50mMのヒスチジン緩衝剤、増量剤及びNaCl中の20mg/mLのTAT-NR2B9C溶液をいくつか調合した。凍結乾燥プロセスの間にTAT-NR2B9C製剤にさらす時間及び取扱い温度をシミュレーションするために、これらのサンプルを、-20℃(コントロール)及び40℃/75%RH(試験)で保存し、HPLC(MSA法)による純度及びpHに関して1週間の保管後に分析した。pH安定性に関する結果は、表9で概説しており、種々の液剤中のTAT-NR2B9Cの安定性に関する結果は、表10及び図3に示している。
マンニトール、トレハロース及びデキストラン-40は、pHを6.5で良好に維持され(表9)、高温での保存の際には液体の形態として1週間にわたる純度の低下は約1%である(表10)。TAT-NR2B9C凍結乾燥充填溶液の化学的安定性に関して、マンニトール及びトレハロースは、デキストラン-40溶液よりも良好な安定性をTAT-NR2B9Cに与えるため、好ましい増量剤である(図3)。
TAT-NR2B9C凍結乾燥製剤からの凍結乾燥サイクル開発の一部として、増量剤サンプルの表(表8)に提示する充填溶液を、製剤におけるガラス転移(Tg)を含む温度特性に関して示差走査熱量測定(DSC)によって評価した。結果を表11にリストしている。DSCのトレースを図4A-6Bに示している。
TAT-NR2B9C製剤の小さいバッチを凍結乾燥させて、25℃、40℃及び60℃で1週間後に固体状態安定性を評価した。3つの異なるビヒクル中に、20mg/mLの有効濃度のTAT-NR2B9Cを調整した。t=0及びt=1週目における、サンプルの、HPLC(MSA法)による純度、量、pH、再構成及び外観を評価した。含水量は、t=0だけ評価した。
TAT-NR2B9C製剤の小さいバッチを凍結乾燥させて、主乾燥の間、5℃での棚温度のセットを評価した。50mMのヒスチジン(pH 6.5)及び120mMのトレハロース中に27mg/mLの有効濃度のTAT-NR2B9Cを調整した。サイクルパラメーターは、表17で概説している。4つの20mLガラス凍結乾燥バイアルを10mLで満たした。2つのバイアルは、温度プローブによって調べた。
小スケール3の充填溶液を調整する前に、緩衝剤(50mMのヒスチジン、pH 6.5)中の90mg/mLのTAT-NR2B9CのpHを評価した。溶液のpHは、6.04であった。TAT-NR2B9Cの濃度を増加させると、緩衝強さも増加させる必要があることを測定した。溶液を、150mM、100mM、75mM及び水で調製して、pHを評価した。pHは、表18にリストしている。小スケール3を、pH 6.5の100mMのヒスチジン緩衝剤に調合して、TAT-NR2B9Cの追加後、pHを6.5に再調整した。
小スケール4の凍結乾燥サイクル開発を開始して、3mL充填のための固形物の外観及び凍結乾燥条件を試験した。試験したサンプルは、120mMのトレハロース及び90mg/kgのTAT-NR2B9Cを有するpH 6.5の100mMのHis、又は、トレハロースを除いた同一のサンプルとした。保存的な4日間のサイクルを、表20にて説明しているように実施した。プラセボ及び活性バイアルを、以前の実験で使用した小バイアル瓶の代わりに、20mLのガラス凍結乾燥バイアル瓶に、3mLの充填構成と共に入れた。能動的温度プローブを用いて、凍結乾燥サイクル中の温度を確認した。得られた活性バイアルを図7Aに示している。
小さいスケール5を実施して、治験にマッチするプラセボバイアルを明らかにするために調査して、可能性がある商業スケール(270mg/バイアル瓶)で製剤をするための再懸濁時間を調査した。10個のプラセボ製剤と1つの活性製剤について、外観及び再構成時間を評価した。活性固形物は、洗練された白色の固形物であり、バイアル瓶の壁の表面周辺に亀裂が入った収縮が僅かに見られた。プラセボ固形物は、白色であり、トレハロースの含有量が多い固形物においてはより多くの亀裂が見られた。
プラセボによりマッチさせるために、様々な濃度のトレハロースを、3又は5mLの充填量でTAT-NR2B9C有り無しで試験した。
TAT-NR2B9C製剤の小さなバッチは、20mMのヒスチジン(pH 6.)及び120mMのトレハロース中における90mg/mLで調製して、凍結乾燥し、-20℃、40℃及び60℃で4週間後に固体状態安定性を評価した。表25は、凍結乾燥条件を示している。
安定性、再懸濁時間及び凍結乾燥時間に基づくと、TAT-NR2B9Cに好ましい商用製剤は、20-100mMのヒスチジン、120mMのトレハロース(pH 6.5)である。トレハロース濃度は、安定性又は固形物の高品質を失うことなく上げることができるが、再懸濁時間は、トレハロース濃度の増加によって増加する。
凍結乾燥薬品の調製
TAT-NR2B9c薬品の少量バッチを、緩衝剤中で90mg/mLに調製した。この緩衝剤は、上述した20mMのヒスチジン、120mMのトレハロース、pH6.5の緩衝剤とほとんど同一のものであるが、優先的にアセチル化可能な追加のアミン基を提供するリジンを720mM追加し、緩衝能を増加させるためにヒスチジン濃度が50mMとなるにようにした緩衝剤である。この製剤は、以前の実験とは異なるロットのTAT-NR2B9cを使用して実行した。その結果、不純物プロファイルが僅かに異なっていた。この製剤(製剤#2)及びコントロールTAT-NR2B9c製剤(製剤#1、20mMのヒスチジン、120mMのトレハロース、pH 6.5)を270mg/バイアルで同時に凍結乾燥して、-20℃、40℃及び60℃にセットした温度制御インキュベーターにおける4週間の保管の後、固体状態の安定性を評価した。2つの半直交型(semi-orthogonal)HPLCアッセイによる種々の相対的停留時間で、新たなペプチド不純物の存在が以下の表27において明らかである。
Claims (19)
- 配列番号:6若しくは配列番号:7のアミノ酸配列を有するペプチド又は最高5つの置換、欠失若しくは挿入でそれとは異なるペプチドと、ヒスチジンと、トレハロースと、アセチル化スカベンジャーをpH6-7で含む未凍結乾燥製剤。
- 前記ペプチドは、TAT-NR2B9c(配列番号:6)である、請求項1に記載の未凍結乾燥製剤。
- 前記アセチル化スカベンジャーは、リジンである、請求項1又は2に記載の未凍結乾燥製剤。
- 前記TAT-NR2B9cは、70-120mg/mlの濃度であり、前記ヒスチジンは、15-100mMの濃度であり、前記トレハロースは、80-160mMの濃度であり、前記リジンは、300-900mMの濃度である、請求項3に記載の未凍結乾燥製剤。
- 前記TAT-NR2B9cは、70-120mg/mlの濃度であり、前記ヒスチジンは、20-100mMの濃度であり、前記トレハロースは、100-140mMの濃度であり、前記リジンは、500-800mMの濃度である、請求項1から4のいずれかに記載の未凍結乾燥製剤。
- 前記Tat-NR2B9cは、70-120mg/mlの濃度であり、前記ヒスチジンの濃度は、20-50mMであり、前記トレハロースの濃度は、100-140mMであり、前記リジンの濃度は、700-750mMである、請求項5に記載の未凍結乾燥製剤。
- 前記ヒスチジンの濃度は、20mMであり、前記トレハロースの濃度は、100-200mM、好ましくは120mMであり、前記TAT-NR2B9cの濃度は、90mg/mlであり、前記リジンの濃度は、720mMである、請求項6に記載の未凍結乾燥製剤。
- 請求項1-7のいずれかに記載の未凍結乾燥製剤を凍結乾燥することによって調製された凍結乾燥製剤。
- 請求項8に記載の凍結乾燥製剤と水溶液との組み合わせによって調製された、再構成製剤。
- 前記水溶液は、水又は通常の生理食塩水である、請求項9に記載の再構成製剤。
- 前記再構成製剤の量は、未凍結乾燥製剤の量の3-6倍である、請求項10に記載の再構成製剤。
- pH 6-7での50-900mMの濃度におけるリジン、20-30mMの濃度におけるトレハロース、4-20mMの濃度におけるヒスチジン及び15-25mg/mlの濃度におけるTAT-NR2B9cを含む、再構成製剤。
- 周囲温度で少なくとも1週間、請求項4に記載の凍結乾燥製剤サンプルを保存するステップと、
前記凍結乾燥製剤を再構成するステップと、を有する、製剤調製方法。 - 前記保管は、前記温度が37℃を超える期間を含む、請求項13に記載の方法。
- 任意に、通常の生理食塩水又は水での更なる希釈の後、再構成された製剤を患者に投与するステップを更に有する、請求項13に記載の方法。
- 前記患者は、CNSに対する脳卒中又は外傷を患っている、請求項13から15のいずれかに記載の方法。
- 前記凍結乾燥サンプルは、救急車に保存される、請求項16に記載の方法。
- 前記患者は、くも膜下出血を患っている、請求項13から17のいずれかに記載の方法。
- 前記患者は、動脈瘤に関する血管内修復を受けている、請求項13から18のいずれかに記載の方法。
