Abstract
Description
骨折及び関係する状態に対する方法及び組成物が記載される。副甲状腺ホルモン及び間葉系幹細胞の投与が、骨の修復に関係する治療法を改善する。
米国において、おおよそ1000万人が骨粗しょう症と診断されており、それに加えて3400万人が低骨量であると分類されている。骨粗しょう症関連脊椎圧迫骨折(OVCF)は米国における最も一般的な脆弱性骨折であり、年当たりおおよそ700,000件の傷害数を占め、これは股関節部骨折の割合の倍である。各年に、おおよそ70,000件のVCFが、患者当たり平均8日の病院への滞在での入院に繋がる。それ故に、骨粗しょう症に起因する脆弱性骨折は膨大な医療資源を消費し、骨粗しょう症患者にとって重大な健康上の危険要素となる。骨粗しょう症患者の治療は、殆どが、主としてアレンドロン酸塩及び副甲状腺ホルモン(PTH)などの比較的新しい薬剤を用いることによる、OVCFの予防に重点が置かれる。しかしながら、OVCFが実際に起こった場合に利用可能な治療の選択肢は殆どない。骨粗しょう症患者の集団において、外科手術は、病的状態になること及び移植不全の大きな危険性を伴い、患者の圧倒的多数に対して、通常は薬物治療及び固定具などの非外科的治療が推奨される。とはいうものの、多数の患者が難治性疼痛及び種々の活動への復帰ができないことを訴えている。これらの限界が、崩壊した脊柱の椎体中へのポリメタクリル酸メチル(PMMA)の経皮注射による、椎体形成術及びballoon tamp整復術の手法を含む、新たな、侵襲性を最小限にした外科的技法の開発を助長してきた。合成による非生物学的な材料は脊柱中で永続的な異物である固定具のままであるとの点で、依然として重大な欠点が存在するように思われ、また、いくつかの研究が、PMMA椎体形成術による治療は、疑似治療以上に有効ではないと報告している。従って、本技術分野において、OVCFの治療に関する有効な生物学的解決手段に関して大きなニーズが存在する。

動物MSCの単離及び培養
動物実験用に、間葉系幹細胞(MSC)は、安楽死させたブタ(体重35〜40kg、平均年齢1.5歳)の肋骨の骨髄(BM)及び皮下脂肪組織から単離することができる。例えば、脂肪組織及び肋骨BMを無菌的に採取した。上記脂肪組織を小片(5mm 5mm以下)に切断し、1%のBSA PBS(インビトロジェン社、米国カリフォルニア州カールズバッド)で洗浄し、0.075%のコラゲナーゼで、37℃において1時間処理する。BMを含有する肋骨を掻き取り、PBSで洗い、900gで10分間遠心分離する。沈渣をPBS中に再懸濁してもよく、その後これをリンパ球分離培地(ICNファーマシューティカルス社、米国オハイオ州ブライアン)上に積層し、30℃で30分間、900gで破壊することなく遠心分離する。
ヒトMSCの単離及び培養
ヒト間葉系幹細胞(hMSC)は、ヒト骨髄外科廃棄物の外植片から単離することができ、hMSCはイン・ビトロで増殖する能力を有する。例えば10mlの骨髄穿刺液を6000Uのヘパリン入りチューブ中に採取し、PBSで洗浄し、回収した細胞を900gでの遠心分離によって採取する。次いで、採取した細胞をパーコール溶液(密度1.073g/ml)にロードする。1100g(20uCで30分間)で遠心分離することによって細胞の分離を実施する。あるいは、細胞は、プラスチック付着プロトコルを用いて単離することができる。採取した有核細胞をPBSで2回洗浄し、次いで培養し、10%のウシ胎児血清(FCS)を補足したダルベッコ最小必須培地(DMEM)(低グルコース)中で培養及び継代する。Turgeman,G.et al.,Engineered human mesenchymal stem cells:a novel platform for skeletal cell mediated gene therapy. J Gene Med,2001.3(3):p.240−51に提示される例を始めとする、MSCを単離及び培養するための様々な手法がよく理解されており、該文献は参照により本明細書にその全体が援用される。
ヒトMSCの特徴づけ及び分化
ヒトMSCは、種々の抗原表面マーカーまたは他の発現産生物に従って特徴づけすることができる。骨髄由来ヒトMSCは、それらのMSC表現型を裏付けるCD90+、CD44+、CD29+、CD73+、及びCD105+を含む5種のMSC表面マーカーのパネルを示す(図1)。記載した単離技法を用いて、単離された内の95%を超える、この発現パネルを含む細胞を得ることができる。
PTHは椎骨の欠損部へのMSCのホーミングを増進
簡潔に説明すると、調査のモデルは、卵巣摘出及び4ヶ月の低カルシウム食餌を用いた免疫不全ラットにおける骨減少症を含むことができ、骨減少症ラットの腰椎において複数の脊椎の欠損部を生じさせることができ、ヒト骨髄由来のMSCが、イン・ビボでの追跡のためのレポータ遺伝子で標識される。腰椎の欠損部を有するラットを、1.標識した幹細胞の単回若しくは複数回の静脈内注射及び毎日のPTH(40ug/Kg体重)の真皮下注射、2.MSCの注射のみ、3.PTHの注射のみ、または4.対照としての生理食塩水の注射、のいずれかで治療することができる。細胞の生存及び欠損部位へのホーミングを、非侵襲的な光学的画像法及び核画像法、並びにそれらに続く免疫組織化学的分析を用いて監視するか、または脊椎骨の修復を、マイクロCT走査を用いて毎週監視する。
PTHは静脈内注射したMSCの骨形成分化を誘導
MSCの静脈内注射及びPTHの皮下投与によって治療した脊椎の欠損部を有するラットを屠殺し、手術した脊椎骨を切除し、切片化し、免疫蛍光分析に用いた。本発明者らは、抗ルシフェラーゼ抗体を用いて細胞を検出し、抗骨シアロタンパク質(BSP)または抗オステオカルシン(OC)を用いて骨形成系統への細胞の分化を実証した。共焦点画像法によって、Luc発現細胞が、骨の欠損部位に生成した新しい骨組織に一体化していることが示された。更に、同一の細胞が、BSP及びOCに対して陽性に染色されたことから、骨細胞に分化したことも明らかであった(図7)。
SDF−1/CXCR4分子はPTHによって誘導されるMSCのホーミングに関与
本発明者らはまた、MSCの脊椎の欠損部へのホーミングの背後にある機序及びこの過程に対するPTHの作用を説明することができる可能性がある、ある特定の分子の発現についても評価を行った。記載するように、SDF−1/CXCR4系は、それによって造血剤及びMSCも特定の位置に遊走することが知られる、最も広範に報告されている経路である。
MSC−PTH治療は骨粗しょう症動物に設けた脊椎の欠損部の骨修復を増進
更なる調査において、本発明者らは、骨粗しょう症ヌードラットの腰部に椎骨の欠損部(ラット当たり2箇所の欠損部)を設けた。術後1週で、ラットを、3週間、毎日のPTH皮下注射(0、0.4、4及び40ug/kg)、または5×106のhMSCを用いた注射された静脈内注射+PTH療法のいずれかによって治療するか、または治療しない。術後第1日並びにその後第2週、第8週及び第12週に、イン・ビボマイクロCTを用いてラットを走査する。
脂肪由来MSC(ADSC)及びPTHの併用治療は、椎骨骨折のブタモデルにおいて骨再生の加速を誘導
ミニブタの腰椎において骨に空隙部(直径4mm、深さ15mm)を設けた。記載のプロトコルに従ってブタADSCを単離した。各ブタに、4週間、週に1回5000万のADSCを静脈内注射する。更に、上記ブタに、PTHを40ugr/kg/日で4週間またはPBSのいずれかを注射する。更に2つの群のブタには、PBSのみまたはPTHのいずれかを与える。脊椎の欠損部の修復を監視し、且つX線画像法及び組織学的分析を用いて分析する。マイクロCT分析によって、早くも手術後4週で上記欠損部の顕著な治癒が示された(図11)。特に併用でのADSC−及び−PTH療法は、両方の単独での治療よりも大幅に効率的な形態で欠損部を再生させることに成功した。
考察
卵巣摘出及び低カルシウム食餌により、ラットの腰椎において、骨塩量が15〜20%減少し、且つ小柱の厚さが30%超低下した。光学的画像法及び核画像法によって、静脈内送達後数日で、注射した幹細胞の動物の腰部へのホーミングが検出される。更に免疫組織化学的分析によって、注射した幹細胞は脊椎の欠損部位にホーミングするだけでなく、骨形成細胞に分化することも実証される。最終的に、併用での幹細胞−及び−PTH療法で治療した骨量減少ラットの脊椎の欠損部は、MSC単独で治療した欠損部と比較した場合に、治療後2ヶ月で骨量密度が2倍増加するとの結果となった。
追加の研究設計
本発明者らは、初めに骨粗しょう症の免疫不全状態のラットモデル、その後ミニブタモデルにおいて、本発明者らの仮設を検証した(図12A、B)。ラットモデルにおいて、ldPTHまたはhdPTHのいずれかと併用で、hMSCを全身的に注射した。光学画像法及び免疫蛍光法を用いて、脊椎損傷部位へのhMSCのホーミングの度合い及びこの活性に関する可能性のある機序を評価した。ラットモデルにおいてイン・ビボμCTを用いることにより、且つミニブタモデルにおいてイン・ビボX線画像及びエクス・ビボμCT画像を用いることにより骨再生を定量し、組織学的分析を行うことによって再生を検証した。MSCの生体内分布及び本治療の可能性のある全身作用も同様に評価した。
MSCの培養及びレポータ遺伝子による標識化
新しいヒト骨髄をロンザ社(メリーランド州ウォーカーズビル)から購入し、hMSCを標準的な手順に従って単離した。簡潔に説明すると、骨髄単核細胞を採取し、2×105細胞/cm2の密度で播種した。既報に従ってpMSCをブタ脂肪組織から単離した。簡潔に説明すると、安楽死させミニブタから皮下脂肪組織を回収し、その後酵素消化を用いて単核細胞を回収し、次いで106細胞/cm2の密度で播種した。両方の細胞型に用いた培地は週2回交換した。
脊椎の欠損部を設けたモデル
この調査で記載される全ての手順は、シーダーズ・サイナイIACUCによる認可を受けた。複数の脊椎の欠損部を、骨減少症ラット及び健康なミニブタの腰椎に設けた。これらの手順は補足資料中に詳述している。
MSC及びPTHによる全身的治療
ラットに、術後第3日に開始して3週間毎日、皮下(SQ)投与によってPTHまたはPBSを与えた。2種の異なる投与量のテリパラチド(フォルテオ(商標)、イーライリリー社、インディアナ州インディアナポリス)、0.4μg/kg/日(ldPTH)または4μg/kg/日(hdPTH)を用いた。ブタには、術後第5日に開始して4週間、0.9%のNaCl溶液(pH5に調整)中に再懸濁した研究用グレードのPTH(1.75μg/kg/日)及び加熱不活性化した(56℃、1時間)2%のブタ血清(シグマ−アルドリッチ社)または薬物ビヒクルを投与した。全身的なMSC治療を受けるラットを2〜3%のイソフルランの吸入により麻酔し、これらに尾静脈を介してhMSC−Luc2を静脈内注射した。各ラットに、術後第3日に開始して3〜4日毎に、注射当たり2×106細胞で、計5回の注射を与えた。全身的なMSC治療を受けるブタには、pMSC−Lucを静脈内注射した。各ブタに、術後第5日に開始してその後週に1回、計4回の注射を与えた。各注射に関しては、50×106細胞を5mlの滅菌生理食塩水に懸濁し、耳静脈を介して注射し、追加の5mlの生理食塩水で流し込んだ。
イン・ビボでMSCが脊椎の欠損部にホーミングする際のMSCの撮像
前述したように、欠損部位におけるルシフェラーゼの発現を、BLIシステムを用いて定量化した。簡潔に説明すると、光検出の前に、100%酸素と混合した1〜3%のイソフルランの連続投与によってラットを麻酔した。撮像の10分前に、ラットに、PBS中の126mg/kgのルシフェリン(プロメガ社、ウィスコンシン州マディソン)を腹腔内注射した。IVISスペクトルを用いて発光を評価した。露光時間を自動的に設定し、生物発光を、露光時間及び対象とする領域の面積に対して正規化したシグナルの合計として定量化した。
未処置及び手術した脊椎骨のμCT分析
イン・ビボマイクロ・コンピュータ化断層撮影分析。コーンビーム・イン・ビボμCT撮像システム(vivaCT40、Scanco Medical社、スイス国ブリュッティーゼレン)を用いて脊椎骨を評価した。55kVp電位のX線管を用いてマイクロ断層撮影スライスを取得し、35μmのボクセルサイズで再構築した。

術後12週(実験の終了時点)で回収したラットの脊椎骨、及び5週後に回収したブタの脊椎骨に対して、組織学的分析を実施した。これらの脊椎骨を前述のように、形態学的分析用に、切片化しH&Eを用いて染色した。免疫蛍光染色のために、組織を脱パラフィン化し、予備加温したTarget Retrieval Solution(商標)(Dako社、カリフォルニア州カーピンテリア)中、37℃、45分間のインキュベーションによって抗原を回収した。血清を含まないブロッキング溶液(Dako社)を塗布することによって非特異的抗原をブロックした。スライドガラスを、骨形成分化を調べるためにヒトBSP及びOcに対する一次抗体で染色し、MSCホーミングの機序を判別するために、SDF1、CXCR4、EGFR、及びアンフィレギュリンで染色した。一次抗体をスライドガラスに塗布して4℃で終夜インキュベートし、PBSを用いて洗浄除去し、当該スライドガラスを二次抗体(表1)と共に室温で1時間インキュベートし、その後PBSで洗浄除去した(表1)。次いで、スライドガラスを、暗所にて5分間、4',6−ジアミジノ−2−フェニルインドール二塩酸塩(DAPI、1μg/ml)で染色し、その後これらを再度PBSで3回洗浄した。VectaMountマウンティング培地(ベクターラボラトリーズ社、カリフォルニア州バーリンゲーム)を組織に塗布した。4チャンネルレーザー走査顕微鏡780(ツァイス社、カリフォルニア州プレザントン)を、倍率20倍、zスタック、及び5×5のタイル走査で用いて、スライドガラスを撮像した。ズームイン画像に関しては、単一のzスタック画像を作成した。全ての試料を同一のゲイン及び露出設定を用いて走査した。
骨粗しょう症ラット脊椎骨骨折モデル:追加処置
骨粗しょう症の誘導:卵巣を外科的に除去した46週齢の雌の無胸腺ラット(Hsd:RH−Foxn1rnu)をハーランラボラトリーズ社(インディアナ州インディアナポリス)から購入した。本発明者らの施設に到着後、ラットを術前の4ヶ月間、0.01%のカルシウム及び0.77%のリン酸塩からなる低カルシウム食餌(LCD)(Newco Distributors社、カリフォルニア州)とし、その後通常の食餌を自由に与えた。(図19に示すデータは、上記LCDを8ヶ月間維持したラットにおいて得られたものであり、これらの動物は例外であった。)
ブタ脊椎骨折モデル
動物モデル。合計12頭の成体の雌ユカタンミニブタ(S&S Farms社、カリフォルニア州)を用いた。これらの動物の平均体重±標準偏差は45.0±3.6kg、平均月齢±は9.2±1.2ヶ月であった。
統計的分析
グラフパッドプリズム5.0bソフトウェア(グラフパッドプリズム社、カリフォルニア州サンディエゴ)を用いて上記データを解析した。結果を、平均値±標準誤差、*P≦0.05、**P≦0.01、***P≦0.001、****P≦0.0001;ns 有意ではない、として示す。反復測定及びボンフェローニ事後検定と共に1要因分散分析または2要因分散分析を用いて、長期のデータ解析を行った。有意性を評価するために、P<0.05を統計的に有意と見なした。
骨粗しょう症ラットの調査結果
卵巣切除と低カルシウム食餌(LCD)との組み合わせにより、ヒト細胞の移植を維持することができる免疫不全状態のラットにおいて、閉経後の骨粗しょう症ラットモデルを生み出した。類似のモデルは、免疫応答性ラットを用いて既報であった。卵巣摘出及び4ヶ月のLCDが十分に骨粗しょう症を誘発したことを確認した後(図19)、異なる群のラットにこの誘導プロトコルを与え、その後通常の食餌に切り替えた(LCD後)。マイクロ・コンピュータ化断層撮影(μCT)を用いて、LCDの開始前並びにLCD後第0日及び第8週及び第12週に再びこれらのラットを走査し、自発的な骨量増加を殆ど伴わない骨粗しょう症の持続を確認した(図12C〜E)。ヘマトキシリン及びエオシン(H&E)染色によって、4ヶ月のLCD後に、動物の腰椎における小柱骨成分の劇的な減少を更に検証した(図12F)。その結果、4ヶ月のLCDが、最大限の不可逆的な骨粗しょう症を得るために最適と判断され、研究を通してこれを用いた。
椎骨の欠損部へのMSCのホーミング
構成的ユビキチンプロモーター下にレポータ遺伝子ルシフェラーゼ2(Luc2)を内包するレンチウィルスベクターを、ヒトMSC(hMSC)に感染させて、ホーミング活性のイン・ビボでの撮像を可能にした。安定なhMSC−Luc2の遺伝子導入及びLuc2の構成的発現を、6継代にわたって定量的生物発光画像法(BLI)を実施することにより検証した(図13A、B)。その後の継代においてはLuc2発現の有意な変化は見られず(P>0.05)、このことはBLIがイン・ビボでhMSC−Luc2を追跡するために用いることができることを示す。
hMSC−PTH治療の椎骨の欠損部再生に対する効果
2種のPTH投与量(ldPTH及びhdPTH)を用いて、MSC−PTH併用療法の治療効果を評価した。次いで、上記効果を、各治療単独での効果及び治療なしでの効果(以下、「対照」と称する。)と比較した。イン・ビボμCTを実施することによって、術後のいくつかの時点で脊椎の欠損部における骨再生を監視した(図14A)。これらの走査を解析することにより、発明者らは、骨形成の指標である骨量密度(BVD、図14B)、及び骨質の指標である見掛け密度(AD、図14C)を定量化することができた。これらの結果は、対照群及びhMSC群においては、術後最初の2週の間に僅かに骨が再生すること及びその後の時点では更に骨は再生しないことを示した(図14C)。新骨は殆どが欠損部の縁部に形成された(図14A)。いずれの時点においても、上記2群の間に、BVDまたはADのいずれかにおける有意差は見られなかった(p>0.05)。ldPTHで治療したラットにおいて、対照群と比較して骨形成の有意な増進は見られなかった(p>0.05、図14B)。hMSC+ldPTHで治療したラットにおいてのみ、術後第2週という早い段階で、二次元断層画像上及び三次元再構成画像上の両方で、明らかな皮質骨及び小柱骨の形成が増加した(図14A)。この時点で、対照群及び単一治療群におけるよりも、hMSC+ldPTH群において、有意により高いBVD(p≦0.05、図14B)及びAD(p≦0.01、図14C)の値が見られた。両方のhMSC+PTH治療は、術後4週におけるBVDにおいて、hdPTH単独以外の全ての他の治療よりも優れていた(図14B、p≦0.01)。第8週までに、hMSC+ldPTH群における骨形成が十分となり、皮質が完全に再生し且つ脊椎骨の構造上の完全な状態を取り戻した一方、対照群のラットにおいては、皮質の不連続性及びより低い骨量が明らかに見られた(図14A)。興味深いことに、hMSC+hdPTH群とldPTH群との間にはいずれの時点でもBVDにおける有意差が見られなかった(p>0.05、図14B)一方で、第8週でこれらの2群の間にADにおける有意差が見られた(p≦0.01、図14C)。組織学的分析によって、対照群及びMSC群においては、再成長が非常に少なく、皮質または小柱帯を再生させることができないことが示された(図15A)。ldPTHで治療した動物において欠損部が部分的に再生はしたが、hMSC+ldPTHで治療した動物(図15A)においてのみ、皮質及び小柱帯の両方が再生した。hdPTH群及びhMSC+hdPTH群についても同様の傾向が見られた(図20A)。
IV注射したhMSCの行く末
脊椎の欠損部再生に対する、全身的に投与したMSCの寄与を調査するために、本発明者らは、注射の前にこれらの細胞をDiIで標識し、後に回収した組織切片を、骨形成マーカーである骨シアロタンパク質(BSP)及びオステオカルシン(Os)に対する免疫蛍光抗体で染色した。本発明者らは、ヒト骨形成マーカーに対する抗体を用いたが、ラットのタンパク質と一部交差反応が予想され、そのため、全ての骨形成細胞はDiI染色したドナーのヒト細胞と共局在化してはおらず(図15B)、このことは宿主細胞の寄与を示す。上記DiI染色細胞は全てのMSC注射を行った群の脊椎の欠損部に見られたが、細胞の存在量は、PTHによる治療も受けた動物で定性的により高いように思われた。多くの注射した細胞は海綿骨中に見られ、この海綿骨では新たな組織が形成されたが、皮質領域中には見られなかった。DiI染色した細胞とOc及びBSPとの共局在化によって、注射した細胞の一部は分化マーカーのいずれかまたは両方を発現した一方、その他の細胞はどちらも発現しなかったことが示された。骨形成マーカーに対する一部の染色がPTH治療群で観察され、これはおそらくPTHの同化作用に起因する。重要なことは、MSC−PTH治療を受けたラットの再生した欠損部に、より多くの細胞が視認されたことである。欠損領域における顕微鏡によるMSCの検出に加えて、Luc2に対するPCRを用いてこれらの細胞の全身的な生体内分布を評価した。MSCの形跡は唯一動物の肺に見られた(図20)。
細胞遊走及び化学誘引分子の発現
PTHに誘導される脊椎の欠損部への幹細胞のホーミングの機序を調べるために、本発明者らは、前に調査した細胞遊走の2種の経路の活性化を評価した。まず、本発明者らは、最も広範に研究されている幹細胞の損傷部位への動員及びホーミングの経路であり、PTH療法によって増進されることが知られているSDF1/CXCR4系を調査した。この調査において、本発明者らは、欠損部位における宿主細胞によるSDF1の発現及び、SDF1に対する受容体であり、報告によればホーミングする細胞によって発現されるとされるCXCR4の発現を測定するために、回収した脊椎骨切片を染色した。本発明者らは、PTHで治療し、且つhMSCで治療したまたはhMSCによらない治療を受けた動物の脊椎の欠損部においてSDF−1を発現する細胞を認めたが、対照群においてはSDF−1の発現は見られなかった。CXCR4に関して、本発明者らは、hMSC治療群の動物由来の組織において多少の陽性細胞を検出したことから、その一部はDiI標識ドナー細胞と共局在化することができた(図16A)。MSC−PTH治療群において、本発明者らは、多くの、CXCR4を共発現するDiI標識MSCを認めた一方、SDF1の発現は、欠損部内部及び欠損部の縁にある宿主細胞に限定されていた。本発明者らは、PTH治療群においてCXCR4の発現は認めなかった。そのPTHはまた、骨芽細胞及び骨細胞の両方において、EGF受容体(EGFR)を介してシグナルを送る上皮成長因子(EGF)様リガンドであるアンフィレギュリンの発現を刺激することによって、MSCを損傷部位へと誘引することが報告されている。ここで、本発明者らは、分泌されたリガンド(アンフィレギュリン)及び受容体(EGFR)の両方に対する抗体を用いて、脊椎の欠損部位を染色した。SDF1/CXCR4の発現と同様に、アンフィレギュリンはPTH群において検出され、EGFR染色はDiI標識MSCと共局在化していた(図16B)。
予備的なブタの調査
上記のラットの調査結果に従って、本発明者らは、免疫応答性の大型動物モデルにおける椎骨再生に対するMSC−PTH併用治療の効果を調査した。上述のものと同様にして、ミニブタの腰椎に骨の欠損部を設けた(図12B)。上記ミニブタを、週に一度で4回の、同種異系のブタMSC(pMSC)またはPBSのIV注射のいずれか、及び4週間、毎日のPBSまたは1.75μg/kgのPTHの皮下(SQ)注射で治療した。骨形成をイン・ビボX線蛍光透視法、それに続いてエクス・ビボμCT走査を用いて監視した。本発明者らは、各群(それぞれ3箇所の欠損部を有する3頭のブタ)について9箇所の欠損部を解析した。
ミニブタの脊椎の欠損部における骨再生
術後数日で得た側位X線フィルムは、大きさ及び解剖学的位置(図17A)が類似したX線透過性の欠損部を示した。術後第5週では、全ての対照群において、欠損部は明確に視認される状態を維持していたが、pMSC+PTH群においては視認されなかった。対照群において、放射線不透過性の周縁部は欠損部位の外周における骨形成を示したが、欠損部の周囲の組織よりもより放射線透過性の状態を維持していたことから、欠損部自体が治癒しているようには見えなかった。これとは対照的に、pMSC+PTHで治療したミニブタにおいては、欠損部位自体が周囲の損なわれていない組織よりもより放射線不透過性であり、このことは、以前に欠損した骨が、元来の椎骨よりもより大きな骨塩量で再生したことを示す。X線の所見はμCT分析によって裏付けられ、該μCT分析は、pMSC+PTHで治療した欠損部位のBVD(p≦0.05、図17B)及びAD(p≦0.05、図17C)が、対照において測定されたBVD及びADよりも約2倍高いことを示した。結論として、両方のpMSC+PTHで治療したミニブタのみが骨の欠損部の完全な治癒を示した一方で、単独でのいずれの治療もが、治癒過程に影響を及ぼしたとは思われなかった(p>0.05、図17B、C)。組織学的分析によって、対照群においては、欠損部の縁部で密な骨の皮質様の形成があったのに対して、MSC−PTH治療群においては、欠損部位が、周囲の損なわれていない椎骨よりもより高密度の小柱骨によって充填されたことが示された(図18A)。
IV注射したpMSCの行く末
骨の欠損部の再生に対する同種異系のブタMSCの寄与を、蛍光顕微鏡及びIV注射したDiI標識細胞の撮像を用いて検出した(図18B、C)。上記ラットモデルと同様に、投与した細胞は、殆どが欠損の領域、新たに形成された骨組織の骨髄中に認められ、小柱骨組織自体には組み込まれていなかった。また、上記ラットと同様に、細胞の一部がOs発現細胞と共局在化し、このことは骨形成分化を示す。欠損部位で認められたMSCに加えて、他のMSCが脾臓及び肝臓において検出されたが、pMSC+PTHで治療したブタにおいては検出されなかった(図21)。
骨粗しょう症を誘導するために最適な低カルシウム食餌の期間の判定
LCDの最適な期間を判定するために、卵巣摘出ラットを8ヶ月のLCDに供し、その期間に、マイクロ・コンピュータ化断層撮影法(μCT)を用いて、損傷を与えていない脊椎骨における骨塩減少を評価した。2ヶ月のLCD後に有意な骨量密度(BVD)及び見掛け密度(AD)の減少が観察された(図19)。BVDは更に2ヶ月間(すなわち、合計で4ヶ月間のLCD)減少し続けた一方、ADは2ヶ月後にほぼ一定のままであった。LCDを8ヶ月維持したところ、4ヶ月継続したLCDと比較した場合、骨密度や他の構造上のパラメータへの影響に有意差を生じなかった。従って、卵巣を摘出したヌードラットにおいて骨粗しょう症を誘発させるために、4ヶ月間のLCDが選択された。
MSCの生体内分布アッセイ
動物を安楽死させた直後に種々の器官(脳、骨髄、肝臓、肺、心筋、骨格筋、及び脾臓)を生検し、これらを急速冷凍した。次いで、上記組織を均質化し、DNA抽出キット(キアゲン社、カリフォルニア州バレンシア)を用いてDNAを抽出した。ラット及びブタの両方が、ルシフェラーゼ(Luc)を発現するように形質導入されたMSCの全身的投与を受けていることから、DNA試料を、定量的PCRを用いてLuc発現について試験し、18Sハウスキーピング遺伝子に正規化した。
代謝バイオマーカーに対するPTH治療の影響
高カルシウム血症は、PTH治療後の主要な懸念事項である。従って、上記ブタを4週間、種々の用量のPTH(最大5μg/kg)で治療し、それらの血清を分析した。いずれの治療群においても、カルシウム及びリン酸塩の血清濃度に変化は認められなかった。血清中クレアチニン濃度は治療を通じて維持され、これは正常な腎機能を示す。ブタの代謝バイオマーカーのいずれにおいても変化は観測されなかった。上記の治療はアルカリホスファターゼ(ALP)またはアルブミンの濃度に影響を及ぼさず、これらはそれぞれ、肝機能または骨代謝回転に障害がないことを示す(図22A〜E)。これらの結果は、短期間に投与される、投与量が増加するPTHは、全身的な影響を生じることなく良好な忍容性を示し、それ故に安全であることを示している。
hdPTHで治療した骨粗しょう症ラットにおけるhMSCの行く末
2種の異なる投与量のテリパラチド(フォルテオ(商標)、イーライリリー社、インディアナ州インディアナポリス)、0.4μg/kg/日(ldPTH)または4μg/kg/日(hdPTH)によって骨粗しょう症ラットを治療した。全身的に投与したhMSCの、脊椎の欠損部再生に対する寄与を調査するために、本発明者らは、注射の前にこれらの細胞をDiIで標識し、後に回収した組織切片を、骨形成マーカーである骨シアロタンパク質(BSP)及びオステオカルシン(Os)に対する免疫蛍光抗体で染色した。更に、PTHに誘導される、脊椎の欠損部への幹細胞のホーミングの機序を調べるために、本発明者らは、前に調査した細胞遊走の2種の経路、すなわち、SDF1/CXCR4及びAmp/EGFRの活性化を評価した。結果は、両方のPTHの用量において、本発明者らが、DiI標識hMSCが上記骨形成マーカー並びにCXCR及びEGF受容体を共発現することを見出したことを示した。
考察
本発明者らの結果は、骨粗しょう症ラット及び健康なブタに設けた脊椎の欠損部が、MSC+PTH併用治療の結果、効率的に修復されたことを示した。これは、単独での治療のいずれかを受けたまたは治療を受けていない動物における結果と対照的である。更に、PTHの投与は、椎骨の欠損部へのhMSCのホーミングを増進させた。
生命に危険が及ぶ肋骨骨折に対する全身的MSC及びPTH療法
本発明者らは、全身的なMSC及びPTH治療薬の静脈内投与からなる、RFを治療するための新規な併用手法を開発した。まず、本発明者らは、複数の肋骨骨折があるラットモデルを創り出し(図23A)、仮骨の顕著な形成を観測した(図23B)。それにも拘わらず、欠損部の縁は統合されなかった(図23C)。
肋骨骨折部位におけるMSCホーミング及び修復の増加
次に、ヒト骨髄からMSCを採取し、構成的ルシフェラーゼ発現を用いてタグ付けを行った(図24A)。2*106のhMSCを、術後3日、7日、10日、14日及び17日に繰り返しラット尾静脈に注射した。並行して、これらのラットに、毎日4ug/kgのPTH類似体(テリパラチド)の皮下注射を与えた。術後1週で、顕著な生物発光シグナルが骨折部位において検出され(図24B)、これは骨折部位への細胞のホーミングを示す。術後14日に、イン・ビボuCT分析によって、未治療のラットと比較して、hMSC及びPTHで治療したラットにおいて骨折の修復が見られ、未治療のラットにおいては、骨折がブリッジングされていないままであった。
Claims (31)
- 対象を選択する段階;および
