Abstract
Description

本発明は、これより記載する実施例に限定されると解釈すべきでなく、むしろ本発明は本明細書に提示するあらゆるすべての応用例、及び当業者の技能範囲に含まれるすべての同等の変更例を含むと解釈されるものと認識される。
本試験では、すべての収集時点において便サンプル検査を実施し、組成及び多様性に関して腸内微生物叢の特徴づけを行った。(図1は、治験デザインを示し、またV3、V4、V6の時点を定義する)。試験期間中、対象は、ベースライン時及び非盲検によるリファキシミン550mg、1日3回を2週間投与後の各時点において便サンプルを提供したが、便サンプルは、リファキシミン550mg、1日3回、又はプラセボを用いた二重盲検再治療を実施する前、及び2週間実施した直後に提供され、また便サンプルは、試験終了時にも提供された。便サンプルを、ポリプロピレンクライオバイアル内で2mLの一定分量に分割し、臨床施設において≦-20℃で保管し、ドライアイス同梱で発送し、≦-70℃で長期保管する。

GI微生物叢の特徴付け
本試験では、治験母集団の約10%を対象として、すべての収集時点において便サンプルを検査し、また組成及び多様性に関して腸内微生物叢の特徴づけを行った。
GI細菌培養及び耐性
本試験(図1の治験デザインを参照)では、治験母集団を対象として、すべての収集時点において便サンプルを検査し、また臨床的に注目されている抗生物質に対するグラム陰性桿菌及びC.ディフィシルを含む便微生物叢の感受性についても特徴づけを行った。微生物叢試験の記載に従い、便サンプルを収集した。

皮膚スワブ
下痢を伴う過敏性腸症候群(IBS-D)の患者に関する本試験では、IBS-Dの患者の皮膚スワブサンプルから培養されたスタフィロコッカス系統の増殖性及び抗生物質感受性について、特徴づけを行った。
・左前腕及び右前腕の皮膚:合計約10回のストロークを用いて前腕を単一のスワブでスワブ処理した。スワブのすべての側面が皮膚に曝露されるように、スワブをストローク毎に回転させた。各腕について1つのスワブを用いた。
・掌:掌全体をスワブ処理し、すべての側面が皮膚に曝露されるように、スワブを回転させた。両方の掌を同じスワブを用いてスワブ処理した。
・鼻孔:スワブの先端を各鼻孔に慎重に挿入し、2回捻った。スワブは外鼻孔のみをサンプリングするように意図されたので、スワブが鼻孔内に深く挿入されないよう徹底するために注意を払った。各鼻孔につき、約1/4インチを超えてスワブ挿入は行わなかった。両方の鼻孔を、同じスワブを用いてサンプリングした。
・直腸周辺:直腸領域周辺をスワブでスワイプし、スワブのすべての側面が皮膚に曝露されるよう徹底するために、ストローク毎に回転させた。スワブの直腸挿入は行わず、直腸外部周辺のみをスワブ処理した。

微量液体希釈法を用いて、スタフィロコッカス属の単離物を、抗生物質のパネルに対する感受性について試験した。最低阻止濃度(MIC)測定の場合、約3〜5例の単離されたコロニーを継代培養プレートから選択して、CLSIガイドラインに基づき、0.5のマクファーランド標準に近づけた懸濁液を作製した。インキュベーションプレートの播種直前に、細菌懸濁液を調製した。抗生物質のストックを調製し、陽イオン調整ミューラー・ヒントン液体培地で希釈した。各試験プレートを、35±1℃、CO2中でしかるべき時間インキュベーションした。必要に応じて、品質管理用の系統が試験プレートに含まれた。純度対照プレート並びに陽性及び陰性増殖対照が各サンプル測定時に含まれた。

試験のOL相に限定で登録し、治療2のみを受けた対象において、治療2来院時に培養された(第1日目)ブドウ球菌単離物合計373例が認められた。治療2におけるブドウ球菌単離物のリファキシミンMIC50値は0.015μg/mLであり、またMIC90値は0.03μg/mLであった。EOT2来院(第2週目)において、リファキシミンのMIC50は0.015μg/mLであったが、MIC90値は32μg/mLまで増加した。MIC値の全体的な範囲は、第2週目においてより広がり、≦0.001μg/mL〜128μg/mLであった。第7週〜第32週の間で生じたOL最終評価来院では、リファキシミンのMIC90値は、第7週〜第10週の期間中の2μg/mLから、第11週目以降のMIC90値0.03μg/mLまで低下した。観察された最大MIC濃度は、64μg/mLから0.06μg/mLに低下し、MIC濃度範囲は、リファキシミン治療から時間が経つにつれて狭まった。
リファンピンのMICを、CLSIにより定義されたブレークポイントに基づき、スタフィロコッカス属について分析したが、この場合、MIC≧4μg/mLは、耐性とみなされた。分析の目的からすると、<4μg/mLであるすべてのMIC値は、リファンピンに対して感受性を有するとみなされた。試験のOL相に登録した対象は、治療2の第1日目に≦0.015〜0.12μg/mLのMIC範囲を有したが、リファキシミンを用いた治療前の治療2来院時では、MIC50及びMIC90は、≦0.015μg/mLであった。リファキシミンを用いた治療後、第2週目において、MIC値が≧4μg/mLである単離物39例、及びMIC値が<4μg/mLである単離物297例が認められた。リファンピン耐性単離物は、直腸周辺に主に位置したが(耐性単離物32例)、耐性単離物4例が掌から培養され、また耐性単離物3例が左前腕スワブから培養された。EOT2におけるMIC50は≦0.015μg/mLであり、MIC90は16μg/mLであった。第7週〜第32週の間で生じたOL最終評価来院では、ブドウ球菌単離物合計406例のうち16例が、≧4μg/mLのリファンピンMIC値を有した。第7週〜第10週の期間において、リファンピンのMIC90は0.5μg/mLであったが、第11週目以降では≦0.015μg/mLまで低下した。リファンピン耐性単離物7例(単離物の9.5%)が、第7週〜第10週の間に、皮膚スワブサンプルから単離されたが、大部分の耐性単離物は直腸周辺に局在した。第11週〜第14週の間で、耐性単離物7例が皮膚スワブから培養された(単離物の2.8%)。第15週〜第32週では、リファンピン耐性単離物は、皮膚スワブサンプルにおいて認められなかった。
治験デザイン
第3相トライアルに関する全体的な治験デザインを図1に示す。
対象は、10(±3)日間の治療を含むスクリーニング/治療1相に参入し、この期間中、対象は単盲検プラセボの投与を1日3回受け、日々の症状に関する日記を作成した。スクリーニング/治療1相の後、適格性を有する対象が、治療2相に参入し、非盲検リファキシミン550mg、1日3回、2週間の投与と共に、治療を行わない4週間のフォローアップを受けた。治療2相の終了時に、4週間のフォローアップ期間のうち少なくとも2週間、IBS関連の腹痛及び便の硬さの両方において治療の奏功を実現した対象が、反応例として分類され、治療を行わないメンテナンス相1に参入した。治療2相での非反応例は試験から撤退させ、リファキシミンを用いた治療に反応する対象の母集団が増すようにした。
選択された対象から得た便の一定分量を、ゲノム配列決定用として凍結発送した。試験時に、サンプルを解凍した。DNAを便から抽出した。次世代配列決定技術(Illumina HiSeq2500)を、16SリボソームRNAのV4領域を配列決定するのに用いた。そうするために、286塩基対(bp)領域を、F515(フォアワード): GTGCCAGCMGCCGCGGTAA、及びR806(リバース): GGACTACHVGGGTWTCTAATプライマー(17)を用いて増幅した。
データを、Illuminaのアダプター情報を用いて最初に多重分離し、次に多重分離の第2ラウンドを実施した。
Shannon多様性インデックスを、各来院時において便微生物叢の多様性を調べるのに用いた(存在する細菌ファミリーの数だけでなく、細菌ファミリーの相対的存在量も考慮する)。便微生物叢の多様性について、試験のOL相で行われたリファキシミン治療による実質的変化は認められなかった。
試験のDB相のベースライン時において、プラセボ又はリファキシミンの投与を受けるようにランダム化する前では、DBプラセボ群及びDBリファキシミン群のShannon多様性は類似しており、それぞれ1.733及び1.786であった。2週間の治療期間後、Shannon多様性は、プラセボ群で1.743、及びDBリファキシミン群で1.698であったが、治療間で有意差は認められなかった(p値=0.4335)。Shannon多様性は、フォローアップ期間中、実質的に不変のままであり、治療群間で有意差は認められなかった。
RFIB3053のOL相及びDB相の両コースにおける微生物の豊富度の変化を、リファキシミン(対象37例)又はプラセボ(対象36例)の投与を受けるように試験のDB相にランダム化された対象において調査した。DBプラセボ治療対象は、試験のOL相においてリファキシミンの投与を受けたが、同対象は、OL相の第2週目において豊富度の減少を示したものの、DB相の開始までに回復する(図15)。更に、豊富度は、OL相開始時とDB相終了時で類似した。
