Claims (13)
- 拡張装置であって、
管状支持体と、
膨張可能な剛性付与部分と、
前記管状支持体に取り付けられた膨張可能な拡張部分と、
インフレーションルーメンとを含むことを特徴とする拡張装置。 - 前記剛性付与部分及び前記拡張部分が、コンプライアント材料、ノンコンプライアント材料、またはセミコンプライアント材料、あるいはそれらの組合せで作られていることを特徴とする請求項1に記載の拡張装置。
- リテーナ部分をさらに含むことを特徴とする請求項2に記載の拡張装置。
- 前記拡張部分及び前記リテーナ部分が、各々異なる材料で作られていることを特徴とする請求項3に記載の拡張装置。
- 当該装置が、内視鏡のワーキングチャンネルを通して挿入され得ることを特徴とする請求項1に記載の拡張装置。
- 当該装置が、前記内視鏡のワーキングチャンネルを通して抜去され得ることを特徴とする請求項5に記載の拡張装置。
- 前記剛性付与部分が、カテーテルチューブ内に挿入されかつ、膨張させられた後に摩擦を用いて前記チューブをしっかり保持するように構成されていることを特徴とする請求項1に記載の拡張装置。
- 前記チューブが、当該装置の前記剛性付与部分に対して摩擦により保持された状態で、非血管内腔内へ押し込まれることを特徴とする請求項7に記載の拡張装置。
- 前記チューブが、当該装置の前記剛性付与部分に対して摩擦により保持された状態で、非血管内腔内へ引き込まれることを特徴とする請求項7に記載の拡張装置。
- 当該装置を配置する前に、前記剛性付与部分が少なくとも15psiまで膨張させられることを特徴とする請求項7に記載の拡張装置。
- 当該装置を貫通して延在する連続経路をさらに含むことを特徴とする請求項1に記載の拡張装置。
- 経腸栄養チューブをストーマ内に留置する方法であって、
患者の非血管内腔へ内視鏡を進めて送気し、かつ触診により経腸栄養チューブ設置部位を確認するステップと、
前記非血管内腔に針を刺入し、該針を通してガイドワイヤを導入するステップと、
前記内視鏡のワーキングチャンネルを通して、標準的な内視鏡用鉗子、内視鏡スネア、または取付具を挿入するステップと、
前記ガイドワイヤを把持し、該ガイドワイヤを、前記内視鏡の前記ワーキングチャンネルを通して引き寄せて前記患者の口腔から引き出すステップと、
インフレーションルーメンを備えた拡張装置を前記ガイドワイヤの端部に固定し、前記拡張装置を、前記ガイドワイヤを用いて前記ワーキングチャンネルを通して胃内へ引っ張るステップと、
前記ガイドワイヤを穿刺経路内に残したまま前記針を前記非血管内腔から抜去するステップと、
前記拡張装置を前記穿刺経路内へ引き込みかつ部分的に前記穿刺経路を通り抜けさせるステップと、
剛性付与部分を含む前記拡張装置の遠位端に被せるようにカテーテルチューブを配置するステップと、
制御された量の流体を前記拡張装置の膨張可能な拡張部分及び剛性付与部分に徐々に導入することにより、前記両部分内の圧力を上昇させて前記両部分を膨張させ、それによって、前記拡張部分により管腔壁を腹壁に接触させながら前記穿刺経路を滑らかにかつ徐々に拡大させてストーマ路を形成し、かつ前記チューブ内で前記剛性付与部分を拡張させて前記剛性付与部分及び前記チューブを互いに摩擦によって結合するステップと、
前記チューブ及び前記拡張装置を前記非血管内腔内へ進め、前記チューブを前記ストーマ内に留置するステップと、
前記拡張装置の前記両部分を完全に収縮させ、かつ、前記内視鏡の前記ワーキングチャンネルを通して前記拡張装置を引き抜くか、前記非血管内腔から前記内視鏡及び前記拡張装置をともに引き戻すことによって前記拡張装置を引き抜くか、または、前記経腸栄養チューブを通して前記拡張装置を引き抜くステップとを含むことを特徴とする方法。 - 前記拡張装置の前記遠位端に被せるように前記カテーテルチューブを配置した後、前記拡張装置の前記剛性付与部分を0psi以上に膨張させることを特徴とする請求項12に記載の方法。
