Abstract
本発明はデュアルエネルギマモグラフィに関する。乳がんとなりうる前駆体としての微細石灰化部を早期に識別するために、本発明では、乳房の隣に、既知の濃度分布及び既知の密度分布及び既知の有効原子番号分布を有する比較パターンを配置し、異なるエネルギのもとで形成された比較パターンの複数のマモグラムに基づいて、原子番号と2つの異なる放射エネルギでの相互作用なしに乳房を通過した光子数の対数差との結合パラメータ、及び、原子番号と2つの異なる放射エネルギでの相互作用なしに乳房を通過した光子数の対数比との結合パラメータを求め、当該結合パラメータに基づいて乳房における原子番号分布乃至濃度分布を視覚表示する。
Description
本発明は、請求項1の上位概念記載のデュアルエネルギマモグラフィ方法に関する。
本発明は、医療分野、特に、乳房の良性及び悪性の疾患を診断する方法に関連する。
乳房における腫瘍の前駆体として、微細石灰化部がある。これは健常組織の有効原子番号Z=6.5〜7.5を大幅に上回る有効原子番号Z=12〜14を有する。微細石灰化部の存在は基本的に腫瘍の発生に対する充分な前提と捉えることができる。ただし、大きさが200μmを下回る微細石灰化部は、現行のX線撮影による乳房画像では認識されないので、特に危険である。
悪性腫瘍も同様に高い有効原子番号を有する。これは、炭素分布と酸素分布とが異なることに関係している(Antoniassi M., Conceic,a~o A.L.C.: Study of effective atomic number of breast tissues determined using the elastic to inelastic scattering ratio// Nuclear Instruments and Methods in Physics Research Section A: Accelerators, Spectrometers, Detectors and Associated Equipment, 2011, V.652, No.1, 739頁−743頁を参照)。
本発明のデュアルエネルギマモグラフィ方法によれば、微細石灰化部を従来よりも信頼性高く、より早期の疾患段階で識別できる。また、従来の診断方法に比べ、新たな腫瘍の発生をよりシャープに表示できる。
触診不能な早期の腫瘍形状を高い信頼性で診断するために、X線スクリーニングマモグラフィが行われる。この手法は、組織ごとにX線吸収率が異なるという作用を基礎としている。つまり、そのつど相互作用なしに乳房を通過した光子量分布が、
によって可視化される。ここで、N0は出発光子数であり、μ(E,Z,x)は光線に対する全吸収係数の質量係数分布であり、ただし、質量吸収係数は出発光子のエネルギと乳房部分の有効原子番号とに依存し、ρ(x)は光ベクトルに沿った濃度分布であり、dは光ベクトルに沿った乳房密度である。なお、質量吸収係数は狭い変化領域内で有効原子番号に比例する。
このように、従来のX線マモグラムは、乳房における密度と濃度と有効原子番号との積の非線形分布を表している。
図1のaには、微細石灰化部を有する乳房を表した従来のマモグラムの例が示されている。
乳房における濃度の変動箇所(血管、リンパ管、良性組織など)は、マモグラムに場合によって存在する極小の微細石灰化部をしばしば覆ってしまう。コリメータが利用される場合にも、検出器はX線散乱光を検出するため、マモグラムでの乳房は充分にシャープには結像されない。このことは微細石灰化部の識別をさらに困難にする。確実に識別できるのは200μmを超える大きさの微細石灰化部のみである。これよりも小さな微細石灰化部は均質な人工検体(ファントム)としてしか認識されない。
有効原子番号分布に対するマモグラムの感度を高めるために、デュアルエネルギサブトラクションマモグラフィ(2エネルギ差分マモグラフィ)プロセスが適用される。このプロセスは多くの特許文献に開示されている(“Dual energy rapid switching imaging system”, US4541106, 1985; “Dual-energy system for quantitative radiographic imaging”, US5150394, 1992; “Dual energy x-ray imaging system and method for radiography and mammography”, US6683934, 2004)。
デュアルエネルギサブトラクションマモグラフィプロセスは、特に、文献Lewin, J.M., Isaacs, P.K, Vance, V., Larke, F.J.: Dual-energy contrast-enhanced digital subtraction mammography: Feasibility. Radiology, Vol.229. No.1, 2003, 261頁−268頁に詳しく説明されている。
このプロセスによれば、2つの異なるエネルギの出力光を用いて2つのマモグラムが形成される。その後、
として、対数化及び減算(差形成)が行われる。ここで、L,Hは、下方のエネルギ値と上方のエネルギ値とを表すインデクスであり、kα,aαは線形係数である。
このように、デュアルエネルギサブトラクションマモグラフィは、有効原子番号と濃度との積の線形分布を視覚化するプロセスである。
図1のbには、同じ乳房に対するサブトラクションマモグラムの例が示されている。
デュアルエネルギサブトラクションマモグラムは、散乱光が抑圧されるため、格段にシャープである。しかし、乳房の個々の部位の表示は、有効原子番号にも濃度及び密度にも依存する。よって、極小の微細石灰化部の識別はやはり困難であり、或る程度の大きさの微細石灰化部しか可視とならない。
濃度及び密度の変動がマモグラムに影響する度合を低減するために、デュアルエネルギディバイディングマモグラフィ(2エネルギ分割マモグラフィ)プロセスが提案されている(“Verfahren fuer Differenzdiagnose der Brustdruese”, RU2391909, 2008)。
当該デュアルエネルギディバイディングマモグラフィプロセスは、特に次の文献に詳しく説明されている。

デュアルエネルギディバイディングマモグラフィプロセスでは、上述した対数比が
図1のcには、同じ乳房のディバイディングマモグラムの例が示されている。
密度及び濃度の変動は、ディバイディングマモグラムではサブトラクションマモグラムよりも不鮮明となる。実際、サブトラクションマモグラムでは、乳頭は不可視である。乳頭はきわめて小さい密度しか有さないため、ディバイディングマモグラムでは乳房とほぼ同じ光密度となってしまうのである。
また、図1のcのマモグラムには、血管及びリンパ管及びその他の濃度変動箇所が示されている。これは、上掲の式(3)が均等なエネルギを有する光スペクトルにしか当てはまらないことに関連している。特徴光及び制動光の双方を含むX線管の実際のスペクトルでは、マモグラムにおける密度及び濃度の変動を抑圧することは不可能である。
乳房疾患の有効診断には、有効原子番号及び濃度及びこれらの凸結合(コンベクスコンビネーション)の視覚化が前提となる。
したがって、本発明の課題は、従来よりも小さな微細石灰化部を識別可能なデュアルエネルギマモグラフィ方法を開発することである。
この課題は請求項1記載の特徴によって解決される。
本発明では、濃度変動に対して不変の有効原子番号分布、及び、有効原子番号の変動に対して不変の濃度分布、及び、有効原子番号と濃度との凸結合の分布を有するマモグラムを取得するために、デュアルエネルギサブトラクションディバイディングマモグラフィ(2エネルギ差分分割マモグラフィ)のための方法を提案する。
本発明を以下に図示の実施例に則して詳細に説明する。
スペクトルが連続している場合、上述した対数差及び対数比の双方の数値は、一定密度での濃度及び狭い変化領域内の有効原子番号に線形に関連する。したがって、有効原子番号及び濃度の分布の可視化は、式
にしたがって可能となる。
これらの単位量の凸結合の分布は、

文献WO99/45371には、検査すべき人体部分の隣に比較パターンを配置して行うコンピュータトモグラフィ方法が記載されている。
有効原子番号及び濃度の分布を数値として再現すること及びその効果は、本発明の方法の主たる特徴である。
本明細書において、N,β,α,Z,ρ,Zn,ρn,λは、相応の特性量の各値をマモグラムの検出器のi番目及びj番目のピクセルとして定める行列である。
図1のdには、2つのマモグラムに基づく本発明のデュアルエネルギサブトラクションディバイディングマモグラフィプロセスを用いて計算された有効原子番号分布の例が示されている。2つのマモグラムはX線管の2つの異なるプレート電圧のもとで形成されたものである。比較パターンとして、種々の密度のアルミニウム条片(微細石灰化部のシミュレーション)を含むグラファイトプリズム(乳房組織のシミュレーション)が用いられる。
サブトラクションディバイディングマモグラムでは実際には血管乃至リンパ管は可視とならない。乳房の健常部位が同じ光密度で表示されるからである。
図2では、aに従来のデュアルエネルギサブトラクションディバイディングマモグラムが示されており、bに本発明のデュアルエネルギサブトラクションディバイディングマモグラム、すなわち、有効原子番号及び濃度の分布を有するマモグラムが示されている。大きな微細石灰化部は従来のマモグラムにおいても良好に識別できる。しかし、極小の微細石灰化部はここでは不可視である。この場合、従来のサブトラクションディバイディングマモグラムでは、極小の微細石灰化部が多く集中した箇所が大きな顆粒のように見えているのである。
悪性腫瘍も微細石灰化部も、大きな有効原子番号に加え、高い濃度を有する。よって、これらの単位量(正規化値)の凸結合によりこうした影響を良好に識別できる。
図3には、識別された単位量である有効原子番号及び濃度につき、これらの凸結合を可視化することの有効性が示されている。図3から、従来のスクリーニングマモグラフィでは認識不能であり、ディバイディングマモグラフィでは認識困難であった極小の微細石灰化部が、単位量である有効原子番号及び濃度の凸結合分布において充分に有効に識別されることがわかる。
このように、サブトラクションディバイディングマモグラフィによれば、微細石灰化部の発生による乳房疾患を発生早期に認識できる。
本発明は次のようなステップで実行される。
1)マモグラフィシステムのプレート上の乳房の隣に、既知の濃度分布及び既知の密度分布及び既知の有効原子番号分布を有する比較パターンを配置する。
2)低いアノード電圧と高いアノード電圧とで2つのマモグラムを撮影する。
3)上掲の式4の係数を比較パターンに基づいて計算する。
4)乳房における有効原子番号及び濃度及びこれらの凸結合の分布を、計算された係数に基づいて視覚表示する。

Claims (2)
- 2つの異なる放射エネルギを用いたマモグラム形成によるデュアルエネルギマモグラフィ方法において、
乳房の隣に、既知の濃度分布及び既知の密度分布及び既知の有効原子番号分布を有する比較パターンを配置し、
異なるエネルギのもとで形成された比較パターンの複数のマモグラムに基づいて、原子番号と2つの異なる放射エネルギでの相互作用なしに前記乳房を通過した光子数の対数差との結合パラメータ、及び、原子番号と2つの異なる放射エネルギでの相互作用なしに前記乳房を通過した光子数の対数比との結合パラメータを求め、
前記結合パラメータに基づいて、前記乳房における原子番号分布を、式
ここで、Zは有効原子番号であり、ρは有効濃度であり、α,βは対数差α及び対数比βであり、N0は出発光子数であり、Nは検出された光子数であり、L,Hは低エネルギ及び高エネルギを表すインデクスであり、
は線形係数である、
ことを特徴とするデュアルエネルギマモグラフィ方法。 - 前記有効原子番号及び前記有効濃度の分布に基づき、凸結合を、

により求められ、Zmin,Zmax,ρmin,ρmaxは乳房の有効原子番号及び乳房の有効濃度の最小値及び最大値を表す、
