Description

The present invention relates to a branch device for infusion blood collection used in an extracorporeal circuit.

In order to inject a drug solution or the like into the blood main circuit of the extracorporeal circuit or collect blood from the blood main circuit, it is known to provide a branch pipe using a T-shaped connector in the blood main circuit. If no fluid injection or blood sampling is performed, it is necessary to close the branch. Therefore, there are cases where such branch pipes are crushed and sealed with forceps (see Patent Document 1). In some cases, a three-way cock is connected to the open end of the branch pipe, and when the liquid injection is not performed, the cock of the three-way cock is closed to seal the branch pipe. Further, there is a case where a rigid connector with a luer is connected to the blood main circuit to constitute a branch pipe, and a three-way cock is connected to the luer of the hard connector to perform liquid injection or blood collection.

Japanese Utility Model Publication No. 58-73248

When the branch pipe is sealed with forceps, blood may enter and stay in the branch pipe up to the place sealed with the forceps, so that a thrombus may be formed. In addition, when connecting a three-way stopcock to the open end of a branch pipe or connecting a three-way stopcock to a luer of a rigid connector, a thrombus is formed because blood enters and stays between the blood main circuit and the three-way stopcock. There is a risk.

Then, an object of this invention is to provide the branch pipe apparatus for liquid injection blood collection which can suppress formation of a thrombus.

The branch pipe device (1) for liquid injection and blood collection according to the present invention includes a connection passage (2) that functions as a part of the blood circuit when connected to the blood circuit (A), and a first branch branched from the connection passage. A branch pipe (3) and a first seal portion (7) provided in a movable state inside the first branch pipe and capable of sealing the connection portion (6) between the connection passage and the first branch pipe ) And a second seal portion (8) that is provided at a position away from the first seal portion and seals between the inner surface of the first branch pipe, and the connection portion A second branch pipe (4) that is branched from the first branch pipe at a position between the first seal section and the second seal section in a sealed state and used for liquid injection or blood collection, 1st mechanism (12,15,16) which can fix the said rod in the closed position where one seal part seals the said connection part The second mechanism (18) restricts the movement of the rod to a release position in a state where the seal of the connecting portion by the first seal portion is released and the second seal portion is housed inside the first branch pipe. 19).

According to this liquid injection / blood collection branch device, the first seal portion and the second seal in a state where the second branch tube used for liquid injection or blood collection seals the connection portion between the connection passage and the first branch tube. It branches from the 1st branch pipe in the position between parts. Therefore, even when the connection portion is opened by the first seal portion and the second branch pipe and the connection passage are communicated with each other, the movement of the rod to the release position where the second seal portion is accommodated inside the first branch pipe is the second mechanism. Therefore, leakage of the liquid or blood to the first branch pipe from the first seal part to the opposite side can be prevented by the second seal part. Thereby, since the amount of blood flowing into the first branch pipe is reduced, the formation of thrombus can be suppressed. Further, when the rod is fixed at the closed position by the first mechanism and the connection portion is sealed by the first seal portion, the liquid added with the anticoagulant can be injected using the second branch pipe. Since the liquid stays in the region between the first seal part and the second seal part and does not flow into the blood circuit side, the extracorporeal circulation is safely performed without increasing the active coagulation time (ACT) of the patient's blood. it can.

In one aspect of the branch device for blood injection and blood collection according to the present invention, the connection portion is formed in a tapered shape with an opening area gradually decreasing toward the connection passage, and the first seal portion is closed. It may be formed in a tapered shape that gradually narrows toward the connection passage so that the connection portion can be sealed at a position. According to this aspect, since each of the first seal portion and the connection portion is formed in a tapered shape, the degree of adhesion between the first seal portion and the connection portion when the connection portion is sealed by the first seal portion. Will improve. Thereby, the amount of blood or the like that permeates between the first seal part and the connection part is small, and a reliable seal of the connection part by the first seal part can be realized.

In one aspect of the branch pipe device for liquid injection and blood collection of the present invention, the first branch pipe and the second branch pipe may have a permeability allowing the inside to be visually observed. According to this aspect, it is possible to always check the sealing state of the connection portion by the first seal portion. For this reason, it is possible to detect and cope with an unexpected seal failure including a change with time of the first seal portion. Thereby, the risk of thrombus formation can be reduced.

As described above, according to the branch device for blood injection and blood collection of the present invention, the movement of the rod is restricted by the second mechanism to the release position where the second seal portion is accommodated inside the first branch tube. The second seal portion can prevent leakage of liquid or blood from the first seal portion to the first branch pipe toward the opposite side. Thereby, since the amount of blood flowing into the first branch pipe is reduced, the formation of thrombus can be suppressed.

In addition, in the above description, in order to make an understanding of this invention easy, the reference sign of the accompanying drawing was attached in parenthesis, but this invention is not limited to the form of illustration by it.

The cross-sectional schematic diagram which showed the state of the rod of the branch pipe apparatus for liquid injection blood collection which concerns on one form of this invention in the closed position. The cross-sectional schematic diagram which showed the state of the rod of the branch pipe apparatus for liquid injection blood collections of FIG. 1 in the open position.

As shown in FIG. 1, a branch pipe device for liquid injection and blood collection (hereinafter referred to as a branch pipe device) 1 is mainly incorporated in an extracorporeal circuit and connected to a blood main circuit A which is a blood circuit. It is used for injecting a liquid such as a drug into the circuit A or collecting blood from the blood main circuit A. The branch pipe device 1, when connected to the blood main circuit A, has a connection passage 2 that functions as a part of the blood main circuit A, a first branch pipe 3 that branches from the connection passage 2, and a branch from the first branch pipe 3. The second branch pipe 4 is provided. The connection passage 2, the first branch pipe 3 and the second branch pipe 4 are integrally formed of a permeable material such as plastic having a permeability allowing the inside to be visually observed.

A rod 5 is inserted into the first branch pipe 3 in a movable state. The rod 5 includes a first seal portion 7 that can seal the connection portion 6 between the connection passage 2 and the first branch pipe 3, and an inner surface 3 a of the first branch pipe 3 that is provided away from the first seal portion 7. A second seal portion 8 that seals the gap is provided. The connection portion 6 is open in a circular shape in cross section, and is formed in a tapered shape whose opening area gradually decreases toward the connection passage 2. The first seal portion 7 is formed in a tapered truncated cone shape that gradually narrows toward the connection passage 2 so that the connection portion 6 can be sealed. For this reason, since the adhesion degree of the 1st seal part 7 and the connection part 6 when the connection part 6 is sealed in the 1st seal part 7 improves, it is between the 1st seal part 7 and the connection part 6. It is possible to realize a reliable seal of the connection portion 6 by the first seal portion 7 with a small amount of blood or the like that permeates. The second seal portion 8 is configured by fitting an O-ring 11 into a groove portion 10 formed in the rod 5. As shown in FIG. 1, the second branch pipe 4 branches from the first branch pipe 3 at a position between the first seal part 7 and the second seal part 8 in a state where the connection part 6 is sealed, Used for liquid injection and blood collection.

The branch pipe device 1 is provided with a plurality of mechanisms for restricting the movement of the rod 5 in the direction of the center line CL. A cap 12 is fitted into the end portion 5a of the rod 5 so as to be relatively rotatable around the center line CL. The rod 5 and the cap 12 are connected by a so-called snap fit. A female screw 15 is formed on the inner periphery of the cap 12, and the female screw 15 can mesh with a male screw 16 formed on the outer periphery of the first branch pipe 3. The cap 12 can be operated by the operator's hand, and an anti-slip process may be applied to the outer periphery of the cap 12 in order to increase the grip force during operation. The state of FIG. 1 shows the closed position in which the cap 12 is screwed to the maximum and the connection portion 6 is sealed by the first seal portion 7, and the end of the male screw 16 of the first branch pipe 3 is the cap. The rod 5 is fixed in the closed position by striking the inner periphery of 12. In this embodiment, the cap 12, the internal thread 15 and the external thread 16 that mesh with each other function as the first mechanism according to the present invention.

Furthermore, in order to reduce the stagnation and vortex of the blood flow in the connection passage 2, it is preferable that the tip of the first seal portion 7 coincides with the internal shape of the connection passage 2 in the closed position of FIG. Therefore, when the passage cross section of the connection passage 2 is circular, it is desirable that the tip of the first seal portion 7 is formed to be a part of the circular shape of the passage cross section of the connection passage 2. Therefore, it is necessary to determine the rotational position of the first seal portion 7 at the tip end of the first seal portion 7 having a curvature shape equivalent to that of the connection passage 2. The branch pipe device 1 is provided with a rotation restricting mechanism 20 for restricting the rotation of the rod 5 around the center line CL. The rotation restricting mechanism 20 is formed on the inner periphery of the first branch pipe 3 and extends in the direction of the center line CL. The rotation restricting mechanism 20 is fitted in the groove portion 21 so as to be movable in the direction of the center line CL. And a protrusion 22 extending in the direction. The position of the rod 5 is determined in the rotational direction by the groove 21 and the protrusion 22. The groove portion 21 is provided on the first branch pipe 3 and the protrusion 22 is provided on the rod 5. Conversely, the groove portion 21 may be provided on the rod 5 and the protrusion 22 may be provided on the first branch pipe 3. .

On the other hand, when the cap 12 is rotated counterclockwise from the state shown in FIG. 1, the rod 5 moves upward in FIG. 1 to release the seal of the connecting portion 6, and the second seal portion 8 is connected to the first branch 3. It reaches the release position that is in the inside. The state shown in FIG. 2 shows a state where the cap 5 is further lifted after the engagement between the female screw 15 and the male screw 16 is disengaged, and the rod 5 moves most in the direction away from the connecting portion 6. When the flange 18 formed on the outer periphery of the first branch pipe 3 and the projecting portion 19 formed on the inner periphery of the cap 12 and projecting toward the center face each other, the rod 5 moves beyond the position shown in the figure. Is limited. In this embodiment, the flange 18 and the protrusion 19 function as the second mechanism according to the present invention.

When the branch pipe device 1 performs liquid injection into the blood main circuit A or blood collection from the blood main circuit A, the rod 5 is moved from the position of FIG. 1 to the position of FIG. In this case, the seal of the connection portion 6 is released by the first seal portion 7 and the second branch pipe 4 and the blood main circuit A communicate with each other, so that liquid injection and blood collection are possible. The rod 5 is maintained in a state where the inner surface of the first branch pipe 3 is sealed by the second seal portion 8 from the position of FIG. 1 to the position of FIG. It is possible to prevent blood or the like from leaking to the end portion 5a side of the rod 5 beyond the second seal portion 8. When the seal of the connecting portion 6 is released by the first seal portion 7 from the closed position of FIG. 1, the rod 5 does not reach the position of FIG. 2, that is, the position between FIG. 1 and FIG. Even so, liquid injection and blood collection are sufficiently possible.

When liquid injection and blood collection are finished and these are not performed, the rod 5 is moved from the position of FIG. 2 toward the connecting portion 6 and the cap 12 is screwed after the female screw 15 and the male screw 16 begin to engage with each other. Thus, it is further moved in the same direction and fixed in the closed position shown in FIG. By this operation, blood that has entered between the first seal portion 7 and the second seal portion 8 can be pushed out. When performing this operation, the rod 5 may be moved while injecting a predetermined liquid (the heparin that is an anticoagulant may be added to the liquid) from the second branch pipe. Since the first branch pipe 3 and the second branch pipe 4 are formed of a permeable material, it is possible to confirm the extrusion of blood that has entered between the first seal portion 7 and the second seal portion 8. . Moreover, since it is possible to always check the sealing state of the connecting portion 6 by the first seal portion 7, it becomes possible to detect and deal with an unexpected seal failure including the time-dependent change of the first seal portion 7 and the risk of thrombus formation. Can be reduced. Further, even if heparin is added between the first seal portion 7 and the second seal portion 8 in the closed position of FIG. 1, it stops without flowing out to the blood main circuit A side, so the risk of thrombus formation in this portion is reduced. . Further, since heparin does not flow into the blood main circuit A from the branch pipe device 1, extracorporeal circulation can be safely performed without increasing the ACT of the patient's blood.

Thus, according to the branch pipe device 1, the second branch pipe 4 used for liquid injection or blood collection is between the first seal part 7 and the second seal part 8 in a state where the connection part 6 is sealed. Since the first branch pipe 3 is branched at the position, even when the connection part 6 is opened by the first seal part 7 and the second branch pipe 3 and the connection passage 2 are communicated with each other, the second seal part 8 is the first seal part 8. Since the movement of the rod 5 is restricted by the flange 18 and the projecting portion 19 to the release position within the branch tube 3, the liquid or blood to the first branch tube 3 going from the first seal portion 7 to the opposite side. Leakage can be prevented by the second seal portion 8. Thereby, since the amount of blood flowing into the first branch pipe 3 is reduced, the formation of thrombus can be suppressed.
