<イントロダクション>
複数の生理学的パラメータ及び代謝パラメータを連続的に監視することができるスマートカテーテルが開発された(6〜10)。ここで、スマートカテーテル流量センサ(SCF)の開発を、脳潅流のリアルタイムの連続的な測定に関して説明する。好ましい一実施形態では、SCFは、リードワイヤの影響を排除し、定温モードで動作するように4ワイヤ構造を使用する。SCFの基本構造が、図1(a)に示されている。SCFは、加熱期間中の温度補正のために「熱的影響」の範囲外に配置された温度センサと、例えば、媒体よりも2℃高い温度に加熱される流量センサとの2つの構成要素を備える。図1(b)は、好ましい操作手順を示している:(1)センサを十分に冷却する(領域I);(2)自己加熱しない小電流を加えることによって媒体の温度を測定する(領域II);(3)最初の加熱期間中に、続く媒体の熱伝導率の補正のためにピーク出力をサンプリングする(領域III);(4)センサを基準温度よりも2℃高く加熱し、出力を、組み込み温度センサを用いて基準温度変化に対して補正する(領域IV);及び(5)熱伝導率の補正を用いて流量を求める(領域V)。
流量センサの微小電極は、厚さ7.5μmのKapton(登録商標)膜の上にTiの接着層(150Å)と共にAu層(1200Å)を堆積させて形成し、次いで、標準的な薄膜リソグラフィ及びエッチング処理を行った。続いて、流量センサの導線を、その抵抗を低減するためにCuで2μmの厚さに電気めっきした。最後に、Parylene(登録商標)膜を5μmの厚さに堆積させた。マイクロセンサを備えた膜を、適切なサイズにカットして、本発明者らの前の作業に基づいて金属ロッドに螺旋状に巻き付けて、心室内カテーテルを形成した(9)。開発した流量センサ及び信号調整インターフェイス回路が図2に示されている。
電気的に加熱された熱抵抗センサの温度が上昇すると、この熱抵抗センサは、移動している流体の存在下で熱平衡に達するまで、その周囲に熱出力を損失する。これは、以下の動的熱エネルギー平衡式によって定義される:

式中、αは開発したAu膜の抵抗の温度係数(TCR)である。
(1)このセンサは、自己校正し(例えば、5〜10秒毎)、長期間の連続的な監視に対して実質的にドリフトが0に近づく。これにより、許容差がバルク材料ほど高くないMEMSベースの薄膜センサで非常に信頼できるデータが得られる。
(2)このセンサは、4ワイヤ構造を使用してリードワイヤの影響を排除し、レシオメトリック測定(ratiometric measurement)を利用してセンサの抵抗を低減することができる。従って、このセンサは、ブリッジ型熱拡散流量センサと比較してより精度が高い。
(3)このセンサは、組み込み温度センサを用いて加熱期間中の基準温度変化を補正し;そして媒体の熱伝導率の変化も補正して、より正確な測定を実現する。
(4)熱拡散流量測定の原理は、センサと組織との間の完全な熱接触の存在を必要とする。しかしながら、脳組織は動くため、時としてセンサと組織とが分離する可能性が高い。本明細書の方式は、数秒毎に再校正し、そしてセンサが組織と良好に接触しているか否かによって異なり得る熱伝導率を記録する。
(5)1つのパラメータのみを監視できる現在の熱拡散流量測定と比較して、新規なセンサは、複数のマイクロセンサと一体にして多様な監視の目標を達成することができる。

Claims (56)
- 生体組織中の血流を監視するセンサであって、(a)第1の微小電極を有する流量センサであって、前記微小電極を通る電流の伝導によって加熱することができる流量センサと、(b)第2の微小電極を有する温度センサとを備え、(a)及び(b)が可撓性基板上に配置され、(a)が、前記センサが配置される組織の温度よりも0.5℃〜3℃高い安定した目標温度に加熱されるときに、(b)が、(a)によって生じる温度場の範囲外となるように(a)が(b)に対して配置されている、血流を監視するセンサ。
- 第1の微小電極を備える前記流量センサが4ワイヤ構造を有する、請求項1に記載の血流を監視するセンサ。
- 前記流量センサが定温流量センサである、請求項1又は2に記載の血流を監視するセンサ。
- 前記温度センサが、前記血流を監視するセンサが配置される媒体の温度を定量し、前記血流を監視するセンサが、前記流量センサの出力が前記温度センサの出力によって前記媒体の温度変化に対して補正されるような電気回路の構成要素である、請求項1〜3のいずれか1項に記載の血流を監視するセンサ。
- 前記電気回路がインターフェイス回路を含む、請求項1〜4のいずれか1項に記載の血流を監視するセンサ。
- 前記媒体が生体組織を含む、請求項1〜5のいずれか1項に記載の血流を監視するセンサ。
- 前記媒体が、前記生体組織中の血液を含む、請求項1〜6のいずれか1項に記載の血流を監視するセンサ。
- 前記微小電極が導電ワイヤに接続され、前記各導電ワイヤが、そのリード線抵抗を低減する材料で電気めっきされている、請求項1〜7のいずれか1項に記載の血流を監視するセンサ。
- 前記導電ワイヤが、銅で電気めっきされている、請求項8に記載の血流を監視するセンサ。
- 前記微小電極が金を含む、請求項1〜9のいずれか1項に記載の血流を監視するセンサ。
- 前記微小電極がチタンを含む、請求項1〜10のいずれか1項に記載の血流を監視するセンサ。
- 前記微小電極及び/又は前記導電ワイヤが可撓性絶縁被膜をさらに備える、請求項1〜11のいずれか1項に記載の血流を監視するセンサ。
- 前記可撓性絶縁被膜が、ポリ(4,4’−オキシジフェニレン−ピロメリトイミド)を含む、請求項12に記載の血流を監視するセンサ。
- 前記導電ワイヤが、1種以上のポリ(p−キシリレン)ポリマーをさらに含む、請求項1〜13のいずれか1項に記載の血流を監視するセンサ。
- 20秒以下の連続時間に亘って前記センサが配置される前記組織の温度よりも0.1℃〜3.5℃高い温度に(a)を加熱することができる、請求項1〜14のいずれか1項に記載の血流を監視するセンサ。
- 前記可撓性基板が、ポリイミド、ポリ(p−キシリレン)、及びポリフッ化ビニリデントリフルオロエチレン(PDVF−TrFE)からなる群から選択される1種以上のポリマーを含む、請求項1〜15のいずれか1項に記載の血流を監視するセンサ。
- 前記センサが、圧力センサ、pHセンサ、グルコースセンサ、微小透析プローブ、酸素センサ、乳酸センサ、ピルビン酸センサ、グルタミン酸センサ、及び二酸化炭素センサの1種以上をさらに備えるアセンブリに組み込まれている、請求項1〜16のいずれか1項に記載の血流を監視するセンサ。
- 前記センサが、螺旋状に巻かれた可撓性ポリマーの微小管として製造される、請求項1〜17のいずれか1項に記載の血流を監視するセンサ。
- 前記センサが、血管用のカテーテルとして製造されるか、又は血管用のカテーテル内に配置されるように製造される、請求項1〜18のいずれか1項に記載の血流を監視するセンサ。
- 前記温度センサへの入力電流及び/又は入力電圧を制御し、且つ前記流量センサへの入力電流及び/又は入力電圧を制御する制御装置に動作可能に接続されている、請求項1〜19のいずれか1項に記載の血流を監視するセンサ。
- 前記流量センサ及び/又は前記温度センサの出力を受け取って処理する1つ以上のデータプロセッサに動作可能に接続されている、請求項1〜20のいずれか1項に記載の血流を監視するセンサ。
- 前記1つ以上のデータプロセッサが、前記温度センサの前記出力から決定される前記媒体の温度変化に対して前記流量センサの前記出力を補正する、請求項21に記載の血流を監視するセンサ。
- 前記1つ以上のデータプロセッサが、(a)が加熱されている間に前記流量センサのピーク出力から決定される前記媒体の熱伝導率の変化に対して前記流量センサの前記出力をさらに補正する、請求項22に記載の血流を監視するセンサ。
- 前記媒体の前記熱伝導率が、前記センサが配置される前記組織の温度よりも1℃〜3℃高い安定した目標温度に(a)を加熱するために必要なピーク初期電流のサンプリングから決定される、請求項23に記載の血流を監視するセンサ。
- 前記ピーク初期電流は、前記流量センサの温度が前記安定した目標温度よりも0.05℃〜0.3℃高いときにサンプリングされる、請求項24に記載の血流を監視するセンサ。
- 前記ピーク初期電流は、25ミリ秒〜200ミリ秒の間にサンプリングされる、請求項24又は25に記載の血流を監視するセンサ。
- 前記ピーク初期電流は、75ミリ秒〜125ミリ秒の間にサンプリングされる、請求項26に記載の血流を監視するセンサ。
- 前記媒体の前記熱伝導率が、2つのピーク初期電流のサンプリングの平均の二乗から決定される、請求項24〜27のいずれか1項に記載の血流を監視するセンサ。
- 前記血流量が、前記流量センサの前記出力から決定され、前記媒体の前記熱伝導率に対して補正され、且つ前記媒体の温度変化に対して補正される、請求項1〜28のいずれか1項に記載の血流を監視するセンサ。
- 前記媒体が、生体組織及び/又は前記生体組織中の血液である、請求項1〜29のいずれか1項に記載の血流を監視するセンサ。
- 前記第1の微小電極が熱抵抗微小電極を含み、及び/又は前記第2の微小電極が熱抵抗微小電極を含む、請求項1〜30のいずれか1項に記載の血流を監視するセンサ。
- 前記センサを、前記組織の温度よりも1℃〜3℃高い安定した目標温度に加熱し、続いて5〜10秒の全期間内に前記組織の温度に冷却することができる、請求項1〜31のいずれか1項に記載の血流を監視するセンサ。
- 電圧の出力が前記血流量に比例する、請求項1〜32のいずれか1項に記載の血流を監視するセンサ。
- 前記血流が脳血流であり、且つ前記組織が脳組織である、請求項1〜33のいずれか1項に記載の血流を監視するセンサ。
- 対象の組織に配置される、請求項1〜34のいずれか1項に記載の血流を監視するセンサを用いて前記血流を測定することを含む、前記対象の前記組織中の血流を決定する方法。
- 前記血流を監視するセンサに非加熱電流を加えて、前記センサの出力を測定することによって基準組織温度を決定することを含む、請求項35に記載の方法。
- 前記非加熱電流が0.1〜20秒の間加えられる、請求項36に記載の方法。
- 前記流量センサの温度を、前記センサに加熱電流を加えることによって、前記センサが配置される前記組織の温度よりも1℃〜3℃高い安定した温度にすることを含む、請求項35〜37のいずれか1項に記載の方法。
- 前記流量センサの温度を、前記センサに加熱電流を加えることによって、前記センサが配置される前記組織の温度よりも1.9℃〜2.1℃高い安定した温度にすることを含む、請求項35〜38のいずれか1項に記載の方法。
- 前記加熱電流が0.1〜20秒の間加えられる、請求項38又は39に記載の方法。
- 前記加熱電流が加えられる期間の最後の5秒間、前記流量センサの出力を測定することを含む、請求項38、39又は40に記載の方法。
- 前記加熱電流が加えられる期間の最後の1/4の期間、前記流量センサの出力を測定することを含む、請求項38、39又は40に記載の方法。
- 前記流量センサを前記脳組織の温度よりも1℃〜3℃高い安定した温度に加熱するために必要な前記ピーク初期電流をサンプリングすることによって前記脳組織の熱伝導率を決定することをさらに含む、請求項35〜42のいずれか1項に記載の方法。
- 前記組織中の血流を、前記血流を監視するセンサを用いて複数の異なる時点で測定する、請求項35〜43のいずれか1項に記載の方法。
- 前記血流を監視するセンサを、第1の加熱電流の終了から第2の加熱電流が加えられるまでの間に冷却することができる、請求項35〜44のいずれか1項に記載の方法。
- 前記血流を監視するセンサを冷却できる期間が2〜20秒である、請求項45に記載の方法。
- 前記血流を監視するセンサに非加熱電流を加えることによって基準組織温度を決定すること、及び前記血流を監視するセンサを冷却できる期間中に前記センサの出力を測定することを含む、請求項35〜46のいずれか1項に記載の方法。
- 前記血流を監視するセンサを冷却できる期間の最後の部分の間、且つ加熱電流が加えられる直前に基準脳組織温度を決定することを含む、請求項35〜47のいずれか1項に記載の方法。
- 前記血流を監視するセンサを、前記対象の脳血管又は他の脳組織に配置し、脳血流を決定する、請求項35〜48のいずれか1項に記載の方法。
- 前記血流を監視するセンサを、前記対象の脳血管に配置することをさらに含む、請求項49に記載の方法。
- 前記加熱期間及びそれに続く冷却期間の合計が5〜10秒である、請求項35〜50のいずれか1項に記載の方法。
- 対象の血流を監視するシステムであって、

保存された命令を有する前記1つ以上のデータ処理装置に接続されたコンピュータ可読媒体であって、前記命令が、前記1つ以上のデータ処理装置によって実行されると、前記1つ以上のデータ処理装置が、請求項35〜51のいずれか1項に記載の方法を実行して対象の血流を監視する、コンピュータ可読媒体と
を備える、システム。 - 前記血流が脳血流である、請求項52に記載のシステム。
- 保存された命令を含む非一時的コンピュータ可読媒体であって、前記命令が、データ処理装置によって実行されると、前記データ処理装置が、請求項35〜51のいずれか1項に記載の方法を実行して対象の血流を監視する、非一時的コンピュータ可読媒体。
- 前記血流が脳血流である、請求項54に記載のシステム。
- 前記対象が、脳損傷を負っているか、又は脳の外科手術若しくは治療的介入を受けている、請求項1〜55のいずれか1項に記載の発明。
