Abstract
Description
各患者は、いかにして検査を受けるかに関する命令を受け、かつ試験の開始に先立ち成功裏に完了せねばならない訓練セッションを受けた。複合反応時間(図7)は、眼の高さで、被験者からの快適な距離で位置決めされた15インチモニタ上で、同時にスクリーン全体に表示される、一対の視覚「刺激」(文字「A」及び正方形「□」)の連続かつ同時表示からなった。スクリーンのいずれか半分の上の文字の位置(すなわち、左「A □」又は右「□ A」は、各表示に関してランダムに選択され、かつ被験者は、各視覚刺激の出現後に、文字Aが現れるスクリーンの側(左又は右)に従って、左又は右マウスボタンを直ちに押すように命令された。被験者がいずれかのボタンを押し次第、又は応答がない場合に最大表示時間(1秒)が経過した後、各刺激表示は、次の刺激まで除去された(空白のスクリーンをもたらす)。各表示の終了時に、ランダムタイマが設定され、その終了で次の表示が引き起こされる。合計36の刺激が、各検査セッションで表示された。試験間の表示時間間隔は、適応を最小限に抑え、かつ能力をより良好に評価するために、集合{0.5、1.0、1.5、2.0秒}からランダムに選択された。
複合反応時間検査は、外科的モニタリング期間を通して毎日08:00〜20:00の間でのみ:a)[リアルタイム自動発作検出(Osorioら、2002)、又は事象ボタンを押すことの内、早い方を介した]発作によって、かつb)ランダムに引き起こされた。ランダム複合反応時間検査を引き起こしたタイマは、1日当たり6つの表示に関して設定され、12時間の検査期間を通して均一に分布され、発作又はランダムに引き起こされた検査後の15分間の期間内にはランダム検査が起き得ないという追加の制約を付した。疲労を最小限に抑えるために、合計で30回以下の(ランダム+発作で引き起こされた)検査が、いずれの12時間の期間にわたっても投与され得た。
下記事項は、(ミリ秒の精度により)記録され、かつコンピュータのメモリに経過記録/保存された:a)検査条件(ランダム対発作)、b)検査を開始するための被験者に対する各プロンプトに対応する呼び出し時間、c)呼び出しへの応答の待ち時間、d)刺激表示時間及び文字「A」が表示されたスクリーンの側(左対右)、及びe)全てのボタン押しの時間及び側(左対右)。これらのデータは、各被験者に関して次の尺度を導くために処理された:I.コンプライアンス、正確さにかかわらず被験者がボタンを押した各検査セッション内で表示された刺激の割合として定義される(コンプライアンススコア=応答数/表示数)。II.正解率=正解数/表示数、但し正解は、文字「A」が現れたスクリーンの側(左又は右)と同じ側のボタンを、被験者が1回のみ押したものと定義される。応答は、次の場合に不正確であるとして分類された:a)文字「A」がスクリーン上に表示された場所と反対側のマウスボタンが押された、b)右及び左マウスボタンが同時又は連続的に押された、c)正しいボタンが、1回の刺激表示当たり2回以上押された、又はd)ボタンが押されなかった。III.応答障害までの時間(TIR):各検査呼び出しと、以下に定義する最初の検査失敗前の最終の正解との間の経過した時間(秒)。応答障害までの時間(TIR)の中数、範囲及び標準偏差(SD)は、最も厳格なものから厳格でないものまでの、検査失敗の3つの異なる定義に関して算定される:A)正解であるが、待ち時間がランダム検査のそれの第90百分位数を超える(TIR−A)、反応速度は、ある種の状況において、応答の正確さと同じ位に重要である、B)上記II.に定義された何らかの不正解(TIR−B)、C)応答の待ち時間にかかわらず、3回の連続した不正解(TIR−C)、被験者が、おそらく認知障害がない時のランダム検査中でも、断続的な間違いをすることを考慮しようと努める定義である。検査中に失敗が起きなかった場合、間隔は、最終刺激に対する正解の時間で終了する。しかしながら、評価が、複合反応時間検査の持続時間(75秒)に限定されるので、検査終結後の障害の可能性は、排除できない。(以上に定義したような)最初の失敗までの時間でなく、最終正解までの時間を使用することは、応答障害までの時間(TIR)を過大評価し、それを過小評価するアプローチが、好ましい/「安全な」とみなされる。
ECoGは、市販された深部(近心側頭領域)又はグリッド/帯状電極(大脳円蓋部)電極(Ad−Tech、Racine、WI)を使用して、記録された。これらの信号は、市販のシステム(Nicolet、Madison、WI)に供給され、フィルタをかけられ(0.5〜70Hz)、デジタル化され(240Hz、10ビットの精度、0.59μV/ビット)、かつ特注のベッドサイドシステムに実装された、認証された発作検出及び数量化アルゴリズム(Osorio 1998、Osorio 2002)を使用して、更に処理された(Petersら、2001)。検出アルゴリズムは、最大発作強度(Si)、持続時間(Sd)を数量化し、発作源の部位、及び拡散範囲(Sc)は、ECoGの視覚による検討を通して判定された。この研究に関して、発作は、臨床症状の有無にかかわらず、最小持続時間D=0.84秒で強度閾値T=22に達したいずれかの自動検出と定義された。これらのパラメータ(T及びD)は、検出アルゴリズムの感度及び特異性を最適化するために選択された(Osorioら、98)。
必要に応じて、各及び全(プール)被験者に関してデータが分析された。データは、20のランダム検査を表示され、5つを片付け(25%)、対134の発作検査で内19を片付けた(14%)1人の被験者によって歪められたので、表示された複合反応時間検査を、片付けた該検査で除されるものの割合として定義されるコンプライアンスは、(全被験者からのデータをプールするのとは対照的に)個別に分析された。
全員が試験対象基準を満たした20人の被験者(人口統計、タイプ、電極埋め込み数及び部位、てんかん発生帯の局在に関して、表1を参照)は、この研究に加えられた。6人の被験者(4、6、7、9、12、13)のデータは、彼らが、最低限必要な数の発作で引き起こされた検査を受けなかったので分析から除外された。12/14の被験者において電子写真的な発症は、臨床的発作発症に先行し、被験者16及び20において、臨床が電子写真的な発症に先行し、彼らのデータは、対照として分析に含められた。
この研究は、被験者の能力が非発作性(発作のない)期間からは区別できない自動発作検出後の時間の長さを推定するため、複合反応時間検査を引き起こすために、自動発作検出を用いた。反応時間は、知覚処理、刺激の評価、及び応答の成立のために必要な時間である。(選択又は代替的としても知られる)複合反応時間は、単純反応時間検査とは異なり、2つ以上の刺激からなり、刺激数の対数によって増加する複雑さを加え、従って被験者の正確かつ適時に刺激を処理及び評価する能力を徹底的に掘り下げて調査し、かつ適切な応答を発生させる。刺激の表示及び応答の間に行われるプロセスは、発生順に記載される3つの下位プロセスに分けることができる:a)刺激登録時間、b)選択反応時間、及びc)応答する決定を構築している時間。このこと、及びそれを複数回再投与する能力により、複合反応時間検査が、発作の応答性/認識に対する影響を評価することに適するようにする。

Claims (28)
- 脳の状態に変化がある患者の応答性の程度を判定する方法であって、
トリガ事象の兆候を受信するステップと、
前記兆候に応答して、応答性検査を患者に投与するステップと、
前記検査の結果に基づき、(i)患者の応答性変化の発生時刻、(ii)患者の応答性変化の持続時間、(iii)患者の応答性変化の大きさ、(iv)事象発生の前記兆候から患者の応答性変化までの時間間隔、(v)患者の応答性変化のタイプ、(vi)発作の重症度推定、(vii)発作が臨床的又は不顕性のいずれであるかの分類、(viii)臨床的発作が単純部分、複雑部分又は全身のいずれであるかの分類、(ix)患者の病状に関する治療の有効性評価、(x)前記病気の状態の評価及び患者の予後診断の策定、(xi)患者の傷害又は死亡の危険性の推定、及び(xii)これらの2つ以上、からなる群から選択される少なくとも1つの応答性パラメータを判定するステップとを有する方法。 - 前記トリガ事象が、a)医療事象が起きている若しくは差し迫っているという医療事象検出アルゴリズムからの兆候、b)応答性検査を患者に投与するための手動信号、又はc)医療事象が起きている若しくは差し迫っているという医療事象検出アルゴリズムからの兆候がない場合に応答性検査を患者に投与する指令、からなる群から選択される請求項1に記載の方法。

- 患者から受信され、かつ脳信号、脳神経信号、脊髄信号、末梢神経信号、身体運動、位置及び力、並びにこれらの2つ以上からなる群から選択される少なくとも1組の信号を分析することによって患者の神経的指標の変化を検出するステップを更に有する請求項1に記載の方法。
- 患者がてんかんに苦しんでおり、かつ投与が複数回行われ、前記複数回の内少なくとも1回が発作性であり、かつ前記複数回の内少なくとも1回が非発作性である請求項1に記載の方法。
- 第1の難度を有する第1の応答性検査を選択するステップ、及び前記第1検査による患者の応答性に基づき、第2の難度を有する第2の応答性検査を選択かつ投与するステップを更に有する請求項1に記載の方法。
- 第1の持続時間を有する第1の応答性検査を選択するステップ、及び前記第1検査による患者の応答性に基づき、第2の持続時間を有する第2の応答性検査を選択かつ投与するステップを更に有する請求項1に記載の方法。
- 前記応答性検査が、患者の反射、運動、又は認知機能の少なくとも1つを検査する請求項1に記載の方法。
- 前記応答性検査が、患者の認知機能を検査し、前記認知機能が、注意、言語、非言語及び手続き的短期記憶、言語、非言語及び手続き的長期記憶、言葉の流暢さ及び理解、視空間機能、聴覚弁別、視覚弁別、抽象的な論理、計算、又はこれらの2つ以上からなる群から選択される請求項1に記載の方法。
- 発作事象の治療を患者に供給するステップを更に有し、前記発作事象の治療が、患者の脳神経の電気刺激、患者の脳神経の熱的操作、患者の脳の電気刺激、患者の脳の熱的操作、患者への血流、脳脊髄液を介した、又は直接脳組織への化学物質の供給、脳神経の磁気刺激、脳の磁気刺激、運動タスク、知覚タスク、認知タスク、及びこれらの2つ以上からなる群から選択される請求項1に記載の方法。
- 患者の応答性に基づき、外部装置にその操作状態を変更するよう命令するステップを更に有する請求項1に記載の方法。
- コンピュータによって実行される時に、脳の状態に変化がある患者の応答性を判定する方法であって、
トリガ事象の兆候を受信するステップと、
前記兆候に応答して、応答性検査を患者に投与するステップと、
前記検査の結果に基づき、(i)患者の応答性変化の発生時刻、(ii)患者の応答性変化の持続時間、(iii)患者の応答性変化の大きさ、(iv)事象発生の前記兆候から患者の応答性変化までの時間間隔、(v)患者の応答性変化のタイプ、(vi)発作の重症度推定、(vii)発作が臨床的又は不顕性のいずれであるかの分類、(viii)臨床的発作が単純部分、複雑部分又は全身のいずれであるかの分類、(ix)患者の病状に関する治療の有効性評価、(x)前記病気の状態の評価及び患者の予後診断の策定、(xi)患者の傷害又は死亡の危険性の推定、及び(xii)これらの2つ以上、からなる群から選択される少なくとも1つの応答性パラメータを判定するステップとを有する方法を実行する命令によってコードされるコンピュータ読み取り可能プログラム記憶ユニット。 - 前記トリガ事象が、a)医療事象が起きている若しくは差し迫っているという医療事象検出アルゴリズムからの兆候、b)応答性検査を患者に投与するための手動信号、又はc)医療事象が起きている若しくは差し迫っているという医療事象検出アルゴリズムからの兆候がない場合に応答性検査を患者に投与する指令、からなる群から選択される請求項12に記載のコンピュータ読み取り可能プログラム記憶ユニット。
- 患者から受信され、かつ心血管信号、呼吸信号、皮膚信号、瞳孔信号、温度信号、蠕動信号、自律神経又は神経節信号、及びこれらの2つ以上からなる群から選択される少なくとも1組の信号を分析することによって患者の自律神経的指標の変化を検出するステップを更に有する請求項12に記載のコンピュータ読み取り可能プログラム記憶ユニット。
- 患者から受信され、かつ脳信号、脳神経信号、脊髄信号、末梢神経信号、身体運動、位置及び力、並びにこれらの2つ以上からなる群から選択される少なくとも1組の信号を分析することによって患者の神経的指標の変化を検出するステップを更に有する請求項12に記載のコンピュータ読み取り可能プログラム記憶ユニット。
- 患者がてんかんに苦しんでおり、かつ投与が複数回行われ、前記複数回の内少なくとも1回が発作性であり、かつ前記複数回の内少なくとも1回が非発作性である請求項12に記載のコンピュータ読み取り可能プログラム記憶ユニット。
- 第1の難度を有する第1の応答性検査を選択するステップ、及び前記第1検査による患者の応答性に基づき、第2の難度を有する第2の応答性検査を選択かつ投与するステップを更に有する請求項12に記載のコンピュータ読み取り可能プログラム記憶ユニット。
- 第1の持続時間を有する第1の応答性検査を選択するステップ、及び前記第1検査による患者の応答性に基づき、第2の持続時間を有する第2の応答性検査を選択かつ投与するステップを更に有する請求項12に記載のコンピュータ読み取り可能プログラム記憶ユニット。
- 前記応答性検査が、患者の反射、運動、又は認知機能の少なくとも1つを検査する請求項12に記載のコンピュータ読み取り可能プログラム記憶ユニット。
- 前記応答性検査が、患者の認知機能を検査し、前記認知機能が、注意、言語、非言語及び手続き的短期記憶、言語、非言語及び手続き的長期記憶、言葉の流暢さ及び理解、視空間機能、聴覚弁別、視覚弁別、抽象的な論理、計算、又はそれらの2つ以上からなる群から選択される請求項12に記載のコンピュータ読み取り可能プログラム記憶ユニット。
- 発作事象の治療を患者に供給するステップを更に有し、前記発作事象の治療が、患者の脳神経の電気刺激、患者の脳神経の熱的操作、患者の脳の電気刺激、患者の脳の熱的操作、患者への血流、脳脊髄液を介した、又は直接脳組織への化学物質の供給、脳神経の磁気刺激、脳の磁気刺激、運動タスク、知覚タスク、認知タスク、及びこれらの2つ以上からなる群から選択される請求項12に記載のコンピュータ読み取り可能プログラム記憶ユニット。
- 脳の状態に変化がある患者の応答性を判定する医療装置システムであって、
トリガ事象の兆候を受信するように構成された受信ユニットと、
前記兆候に応答して、応答性検査を患者に投与するように構成された応答性検査ユニットと、
前記検査の結果を受信し、かつ(i)患者の応答性変化の発生時刻、(ii)患者の応答性変化の持続時間、(iii)患者の応答性変化の大きさ、(iv)事象発生の前記兆候から患者の応答性変化までの時間間隔、(v)患者の応答性変化のタイプ、(vi)発作の重症度推定、(vii)発作が臨床的又は不顕性のいずれであるかの分類、(viii)臨床的発作が単純部分、複雑部分又は全身のいずれであるかの分類、(ix)患者の病状に関する治療の有効性評価、(x)前記病気の状態の評価及び患者の予後診断の策定、(xi)患者の傷害又は死亡の危険性の推定、及び(xii)これらの2つ以上、からなる群から選択される少なくとも1つの判定を行うように構成された判定ユニットと、を有する医療装置システム。 - 前記検査結果又は前記判定の少なくとも1つを保存するように構成された記憶ユニットを更に有する請求項22に記載の医療装置システム。
- 患者の自律神経又は神経的指標の変化を検出するように構成された自律神経又は神経的指標変化検出ユニットを更に有する請求項22に記載の医療装置システム。
- 医療事象のための治療を患者に供給するように構成された医療事象治療ユニットを更に有する請求項22に記載の医療装置システム。
- 複数の応答性検査の少なくとも1つを選択し、かつ応答性検査ユニットに選択された検査を投与するように命令するよう構成された応答性検査選択ユニットを更に有する請求項22に記載の医療装置システム。
- 脳の状態に変化がある患者の応答性の程度を判定する方法であって、
トリガ事象の兆候を受信するステップと、
前記兆候に応答して、応答性検査を患者に投与するステップと、
前記検査の結果に基づき、(i)患者の応答性変化の発生時刻、(ii)患者の応答性変化の持続時間、(iii)患者の応答性変化の大きさ、(iv)事象発生の前記兆候から患者の応答性変化までの時間間隔、(v)患者の応答性変化のタイプ、(vi)発作の重症度推定、(vii)発作が臨床的又は不顕性のいずれであるかの分類、(viii)臨床的発作が単純部分、複雑部分又は全身のいずれであるかの分類、(ix)患者の病状に関する治療の有効性評価、(x)前記病気の状態の評価及び患者の予後診断の策定、(xi)患者の傷害又は死亡の危険性の推定、及び(xii)これらの2つ以上、からなる群から選択される少なくとも1つの応答性パラメータを判定するステップとを有する方法。 - コンピュータによって実行される時に、脳の状態に変化がある患者の応答性の程度を判定する方法であって、
トリガ事象の兆候を受信するステップと、
前記兆候に応答して、応答性検査を患者に投与するステップと、
前記検査の結果に基づき、(i)患者の応答性変化の発生時刻、(ii)患者の応答性変化の持続時間、(iii)患者の応答性変化の大きさ、(iv)事象発生の前記兆候から患者の応答性変化までの時間間隔、(v)患者の応答性変化のタイプ、(vi)発作の重症度推定、(vii)発作が臨床的又は不顕性のいずれであるかの分類、(viii)臨床的発作が単純部分、複雑部分又は全身のいずれであるかの分類、(ix)患者の病状に関する治療の有効性評価、(x)前記病気の状態の評価及び患者の予後診断の策定、(xi)患者の傷害又は死亡の危険性の推定、及び(xii)これらの2つ以上、からなる群から選択される少なくとも1つの応答性パラメータを判定するステップとを有する方法を実行する命令によってコードされるコンピュータ読み取り可能プログラム記憶ユニット。
