本願は、米国仮特許出願第61/227,284号(名称「System for Treatment and Imaging Using Ultrasonic Energy and Microbubbles and Related Method Thereof」、2009年7月21日出願)、米国仮特許出願第61/253,435号(名称「System for Treatment and Imaging Using Ultrasonic Energy and Microbubbles and Related Method Thereof」、2009年10月20日出願)および米国仮特許出願第61/298,741号(名称「System for Treatment and Imaging Using Ultrasonic Energy and Microbubbles and Related Method Thereof」、2010年1月27日出願)の利益を主張し、これらの出願の各々は、その全体が本明細書に参照により援用される。

最近の研究が、標的化超音波造影マイクロバブルの、疾患の血管内発現を検出する手段としてのフィージビリティを調査した。病理にはしばしば、影響を受けた血管の内皮細胞層内層の変質が伴う。この機能異常は微小循環において生じる場合があり、血管内皮表面上の或る分子の選択的発現または上方制御によって特定される。アテローム性動脈硬化[15(特許文献15)]、移植拒絶反応[16(特許文献16)]、炎症および虚血再灌流障害[17(特許文献17)]等の病態に対応する内皮機能不全の分子マーカーの多くは良く特徴付けられている。しかし、現在のところ、このような血管病変の大きさおよび位置を評価するための臨床的に認められた非侵襲的技法はない。所望の標的に対して特異的な表面結合リガンドを含有する標的化マイクロバブルの実験用製剤が、血管内に注入され、短い循環期間の後、標的部位において蓄積することが観察される。それに続く超音波イメージングが、標的病態の位置および大きさの決定を可能にする[18(特許文献18)]。この技法は、「標的化造影増強超音波」として知られ、高空間分解能、リアルタイムイメージング、ならびに接着マイクロバブルと受信信号との間の線形または他の測定可能な相関を達成する場合がある。
空間的に局所化され、集中された非侵襲的/低侵襲的処置は、周囲の解剖学的構造との関連で選択された標的部位に対する治療(集中)領域の局所化を誘導するために、適切な非侵襲的リアルタイムイメージングを必要とする。この点は簡単に見えるかもしれないが、それは非侵襲的処置のために深い意味を有する。このパラダイムは、集中処置区域が、非侵襲的イメージングが用いられるどんなものとも正確に且つ高い信頼性をもって整列することを確実にすることに注意が払われなければならないことをさらに示唆する。理想的なモデルは、画像平面が治療点、線または平面と一致するというものであろう。しばしば、より大きい治療用アレイから「切り取られた」アパーチャ内の中央に小さいイメージング用アレイが設置される。Rosenschein[21]は、内部に7.5MHzの環状アレイが同心状に設置される直径94mmの治療用アレイを記載している。システムはウシ動脈部におけるインビトロ血栓溶解のために用いられ成功した。Unger[22]は、共通の前面平面を有する治療用およびイメージング用アレイ素子を組み込むトランスデューサ設計を(少なくともコンセプトを)記載している。この例では、治療用アレイはイメージング用アレイ内の孔内に設置される。アレイアパーチャ内の大きな中央「孔」が、近距離領域の死角、および歪んだサイドローブパターンを生じさせる(通例、アパーチャ内の「孔」の故に潜在する不十分な空間サンプリングが原因のグレーティングローブが関連する)。現在まで、多くの研究は、イメージング用アレイを治療用集束トランスデューサ/アレイに対して固定することを含んでいた[23〜25]。例えば、統合されたイメージングおよび治療用アレイがワシントン大学によって記載された[26]。しかし、このような「統合された」アレイが、本願明細書において提案される通りの厳密に一致する「治療用」アレイとイメージング用アレイとを含むと考える理由はない。要求される「統合」のレベルを定義する正確な必要性は特定の用途の関数である。
上述されたように、ラパマイシンの化学的特性および生物学的特性と、それが、インビボにおける血管損傷に伴うSMC増殖を防ぐためのベンチマーク試薬である理由。これが、超音波誘発マイクロバブル運搬体放出のためにラパマイシンを選ぶ根拠を確立した。複数のグループが、ラパマイシンを用いた培養SMCの処理がSMC増殖を減少させることを示している[12、30]。しかし、マイクロバブル運搬体からの超音波誘発放出を介したラパマイシンの送達は遂行されていない。
外殻内にラパマイシンを含有する変更超音波マイクロバブルとともに超音波がラット平滑筋細胞に印加された。マイクロバブルはバージニア大学の共同発明者A. L. Klibanovによって製作された。無論、本発明はそのように限定されるものではない。なぜなら、本願明細書において記載されているマイクロバブルは他の製造者によって製作されることができるからである。マイクロバブルは、以前に記載されている手順[78]と同様に、ラパマイシン(0.2mg/ml)および/または極微量の蛍光色素DiI(分子プローブ、Eugene、OR)を有するホスファチジルコリン(2mg/ml)およびステアリン酸ポリエチレングリコール(Polyethylene Glycol、PEG)(2mg/ml)の水性ミセル混合物内におけるデカフルオロブタンガスの超音波分散中の脂質単分子層の自己集合によって形成された。マイクロバブル媒介体のみが細胞への効果を生じさせたのではないことを確実にするための対照として、蛍光標識されたDiIマイクロバブルが用いられた。100%エタノール中に溶解したラパマイシン薬物も、ラパマイシンマイクロバブルの効果を比較する対象の対照として用いられた。我々は、いずれの細胞を平板培養する前にもフィブロネクチンを用いて細胞を24時間平板培養することによって、OPTICELL(商標)(Biocrystal、Westerville、OH)フラスコへの細胞の強い粘着を確実にした。ラットのSMCが低密度で平板培養され、12枚のOPTICELL(商標)の各々の内部のDF10培地内で48時間成長することを許された。ベースライン条件を確立する処理の5時間前に細胞のデジタル位相顕微光画像が撮影された。画像はすべて4×倍率で撮影された。平板培養の24時間後、培地は、DiIマイクロバブル(媒介体対照)、ラパマイシン薬物(薬物対照)、またはラパマイシンマイクロバブルのいずれかを含有する新鮮な培地と交換された。マイクロバブル(DiIまたはラパマイシン)は10×106バブル/mlの濃度でOPTICELL(商標)に加えられ、ラパマイシンは10ng/mlの濃度で加えられた。マイクロバブルの濃度は、加えられたマイクロバブルの数が、同等量のラパマイシン、約10ng/ml、を含有するように選ばれた。我々は、OPTICELL(商標)フラスコの半分を取り、それらに2時間だけ処置を施すことによって、たとえ長期の曝露を用いなくとも薬物は効果を生じることを確実にした。2時間後、薬物/バブル含有培地は新鮮な培地と交換された。OPTICELL(商標)フラスコ内の細胞は以下の6種の処置のうちの1つを受けた:DiIバブル 48時間、ラパマイシン薬物 48時間、ラパマイシンバブル 48時間、DiIバブル 2時間、ラパマイシン薬物 2時間、ラパマイシンバブル 2時間。条件はすべて二重に試験された。
図5(A)および5(B)において、我々は、超音波誘発放出フォームマイクロバブル運搬体による約10ng/mlのラパマイシンの送達がSMCの増殖を防いだことを示しており、それは、同等数のマイクロバブル運搬体からの蛍光膜色素、DiI(Invitrogen)の放出と比較した細胞数の変化として表現されている。さらに、図5(D)における定量分析は、超音波誘発放出によるマイクロバブル運搬体からのラパマイシン(10ng/ml)の送達は、細胞培養培地内で遊離ラパマイシン薬物(10ng/ml)を用いて処置された細胞とは違わなかったことを示す。同様の結果が、超音波の後、2時間だけ処理され、その後、48時間成長することが許された6枚のOPTICELL(商標)のセット内で観察された(図5(C)および5(E))。これらの結果は、(とりわけ、)ラパマイシンおよび不活性細胞標識色素(DiI、図5(C))が、超音波誘発によるマイクロバブル運搬体放出によってSMCに送達されることができることを示す。
二重機能イメージング療法のための例示的なトランスデューサソリューションは、トランスデューサ素子が両方の作業(高周波(HF)イメージングおよび低周波(LF)バブル操作/破壊)を遂行することができるほど十分融通性があるものである。これは、イメージング平面および治療平面が一致する設計を可能にする。これらの以前の設計における欠陥は、優れたソリューションの必要性を示唆する。
利用可能な種々の選択肢の中で、SonixRP(Ultrasonix, Richmond, BC, カナダ)が、本発明を実装するための基本計装を用いるために汎用性のあるプラットフォームである。当然、当業者に周知のように、他のスキャナプラットフォームが調達され得または設計/構築され得る。RP、およびその研究能力(高レベルのソフトウェア/ハードウェアアーキテクチャを含む)、が最近の刊行物に詳細に記載されている[79]。
血管内注入による標的造影剤を用いる場合に直面する問題は、非常に小さい血管内を除き、リガンドと受容体との間の所望の分子結合を形成するわずかな可能性を有するためであっても、注入された物質のほんのごく一部しか十分に近接(1μm未満)しないことである。P−セレクチンの基質上への標的化マイクロバブル接着のインビトロ研究は、生理的流れ条件の下では灌流マイクロバブルのごく一部しか具体的に保持されなかったことを報告している[例えば、Klibanov[80]。マイクロバブル単独の検出は可能であるが[81]、マイクロバブル標的化の効率の低さのせいで、そうでなければ必要とされるよりも大きな投入量のマイクロバブルが必要となる。マイクロバブルは赤血球のものと同様のレオロジー挙動を呈し[82]、血管の中心に向かって遊走する傾向がある。ほとんどの内皮プロテインは内皮から数ナノメートルしか延在しないので[83]、標的血管系を流れるマイクロバブルの多くが所望の分子標的と接触することはありそうにない。マイクロバブルの付着効率は、循環するマイクロバブルを動かして血管壁と接触させ、マイクロバブル:標的接着事象の頻度を増加させることによって高められることができる。Dayton[84]およびその他[85]は以前に、血管内皮へのマイクロバブル接着は、自由に流れるマイクロバブルを、超音波放射力を用いて血管壁に向かって進ませることによって促進される可能性があると仮説を立てた。アビジン:ビオチンモデル系内での印加音圧下におけるマイクロバブル[86]および音響活性リポソーム[87]の接着が調べられており、培養内皮細胞への標的化マイクロバブルの接着が報告されている[86]。

(単一素子トランスデューサ(一般的に、イメージング能力を持たない))
我々の予備データは、単純な軸対称集束単一素子トランスデューサを用いる有望な初期結果を提供した。必要とされるものは、二重機能(低周波バブル「破裂」プラス高周波イメージング)トランスデューサおよび関連計装である。
例示的な設計は、一方が他方の上に積層される1:3複合材圧電セラミック−エポキシ能動層を含み得る。(「1:3」複合材は、ポリマーマトリクス内に埋め込まれた圧電性セラミック柱を含む。すなわち2つの構成要素は電気的且つ機械的に「並列」になっている。1:3構成は主流の複合材構成であり、広く商業利用されている。).複合材料は約50%のセラミック体積含有率を持ち、約15MRaylの音響インピーダンスを持つ。高密度のタングステン粒子を充填された裏当てブロックが用いられる。12MHz動作のために整合された波長の約4分の1の薄い整合層が頂部上に用いられる。高さ焦点を得るためには、従来の充填シリコーンゴムレンズが用いられることになる。高さ焦点深度は約15mmである。具体的に、我々は、200μm未満の方位分解能および距離分解能を生じさせる約12MHzのBモードイメージングを用いる。この周波数において、λは125μmである。その結果、実用的なf数について、(すなわち1〜2)200μmの分解能が実現可能となる。アレイシステムは十分すぎる走査フレームレートを提供する(選択された小さい視野−例えば15mm×15mmに対し100フレーム/秒超)。集束超音波放出は1〜2MHzにおいて放出される。我々は、治療効果が得られる領域を約3λ(すなわちおおよそ2mmのスポットサイズ)まで制御することができる。
目的は、ユーザ制御の下での解剖学的Bモードイメージング能力、バブル特定イメージングおよびバブル破壊パルスの印加を提供することである。解剖学的Bモードイメージングは、標準的なBモード画像形成技法(すなわち最適化されたアパーチャアポディゼーション、固定焦点送信、ダイナミック受信焦点調節、信号検出および走査変換)を用いて遂行される。バブル特定イメージングは、「パルスインバージョン」(Pulse Inversion、PI)[92](すなわち1、−1送信極性/振幅、それに続き、線形成分を無くすための「1」+「−1」処理)を用いることによって提供されることになる。必要であれば、振幅スケーリング等の他のバブル特定技法(すなわち1,2送信極性/振幅;それに続き、(2x「1」)−「2」処理[93])ならびにPIおよび振幅スケーリングの組み合わせ(例えば−1,2,−1送信極性/振幅、それに続き、「−1」+「2」+「−1」処理[47])。
これらのモードはアレイの低周波素子を用いる。トランスデューサの実施形態の設計は、合計128個未満の素子(96個は12MHz素子、24個は2MHz素子)を含む。このように、利用可能な128個のトランスデューサコネクタチャネルから選択されたトランスデューサアパーチャを単に再プログラムするだけで、我々は動作のイメージングモードと治療モードを切り替えることができる。
(バブル作製およびラパマイシン取り込み)
マイクロバブルは、以前に記載されている手順[95]と同様に、ラパマイシン(0.2mg/ml)および/または極微量の蛍光色素DiI(分子プローブ、Eugene、OR)を有するホスファチジルコリンおよびステアリン酸PEG(2mg/ml)の水性ミセル混合物内におけるデカフルオロブタンガスの超音波分散中の脂質単分子層の自己集合によって製作された。場合によっては、グリセロールトリオレアートの追加によって膜の増厚が達成される(マイクロバブル外殻がより厚くなればより多くの量のラパマイシンを収容することになる)[96]。バブル外殻内に取り込まれていない遊離脂質、色素およびラパマイシンは、試薬を保存するべく最初の洗浄液を再利用する一連の(3×)遠心浮遊(100×g、5分)によって除去される。
ロバスト且つ高感度の高性能液体クロマトグラフィ(high performance liquid chromatography、HPLC)の手順が臨床検定のための文献に記載されている。我々は自分たちの研究所において利用可能なHPLCを有しており、このような手順を実施する[97]。手短に言えば、試験されるサンプル(マイクロバブルまたは媒体)は、凍結乾燥されてクロロブタノール中に再溶解され、沈積物を除去するべく遠心分離される。サンプルはオートサンプラバイアル内に配置され、既知の量のラパマイシンを有する較正曲線に対するUV検出を用いてHPLCが遂行される。
ラパマイシン含有マイクロバブルの水性食塩水分散(106〜107/ml粒子濃度)がOPTICELL(商標)(USA Scientific, Ocala, FL)内に10mlの体積で入れられる。我々は、細胞培養研究用に記述された条件で超音波によってバブルを破壊し、OPTICELL(商標)からマイクロバブル粒子を除去し、残留マイクロバブルが(それが存在する場合には)サンプルから除去されることになることを証明するべくそれらを遠心浮遊にかける。次に、我々は、上述された通りのHPLC技法によって、バブルのない下澄液中のラパマイシンの定量化を遂行する。
マイクロバブル表面への抗VCAM−1抗体の結合が、[91]記載されているストレプトアビジン結合技法によって遂行される。手短に言えば、マイクロバブルの製作の間に、2mol%のビオチン−PEG3400−ホスファチジルエタノールアミンが脂質混合物に添加される。他の抗体のために以前に記載されている通り[80、98]、マイクロバブル表面へのビオチン化抗VCAM−1抗体の結合のためにストレプトアビジン架橋法が適用される。抗体分子のビオチン化は、DPBS緩衝液中でビオチンN−ヒドロキシスクシンイミドエステル試薬を用いてpH7.5において遂行される。抗体ビオチン化の程度は、以前に記載されている通り、HABA検定を用いて調べられる[例えば、Klibanov[80]]。抗体−to−ビオチン−NHSの調整によって、1抗体当たり約1ビオチンのインキュベーション比のカップリングが達成されることになる。ビオチン化が抗体を不活性化しないことを確認するために、VCAM−1抗原に対するELISA試験が用いられる。ストレプトアビジン−バブル(109/ml)が、ビオチン化抗体を用いて氷上で30分間インキュベートされ;バケット−ロータ遠心機内での、脱気DPBS緩衝液を用いた3回の遠心浮遊洗浄(lOO×g、5分)によってバブルから遊離抗体分子が除去される。浮遊の繰り返しの後、抗体をコーティングされたバブルの平均サイズは通常、約2.5umとなる。粒子の99%超は8um未満である(粒子のサイズおよび濃度はコールター Multisizer He計器(Beckman Coulter, Miami, FL)を用いて測定される。1つのバブル当たりの付着抗体の量は、以前に記載されている通り、蛍光分光標識によって調べられ、通例、1つのマイクロバブル当たり約105個の抗体分子がこの技法によって付着される[98]。
・イメージング用トランスデューサは走査式の単一素子またはアレイであり得る。

・バブルはコンセプトとしてはポートを介して注入される。
・バブルは同じアクセスカテーテル(すなわち約2mmチューブまたは所望に応じたもの)を介して自由に注入され得る。
・バブルはカテーテル先端付近に1回切りの高濃度の形態で蓄えられ得る。これは我々がより少数のバブルを用いることを可能にし得る。バブルを高濃度(すなわち低い外方拡散速度)に保つことはそれらが時間安定的となることを可能にする。
・バブルは単分散(すべて同じサイズ)であり得る。ただし、必ずしもそうでなくてもよい。
・原理的に、バブルのばらつきは選別されることができる。
1.高められた融通性が外殻組成を変えることができる(すなわち場合により薬物/遺伝子積載量および濃度を「オンザフライで」)。
2.さもなければ実現不可能なバブルを可能にする。先端においてバブルを作製することは安定性問題が緩和されることを意味する。バブルは治療的送達前の数秒間存続しさえすればよい。これは、より不安定な化学組成またはより不安定なバブル(すなわち外殻/ガス)の置換を可能にし得る。現在のところ、ガスは、非常に遅い拡散速度(すなわち高い分子量)を有するものに限られている。新しい設計は新しいガスまたは少なくとも軽いガスの使用を可能にする。我々の見解では、この領域はまだしっかりと研究されていない。
3.複雑な取り扱いを必要とするバブル安定性の既存の問題が回避される。
本発明におけるマイクロフルイディクスデバイスの使用に伴う重要な実用上の問題は、吸入液体およびガスチューブとマイクロフルイディクスデバイスとの間のシールを達成することに関連する。図15A〜15Eは、吸入流体およびガスチューブとマイクロフルイディクスデバイスとの間の適切且つ持続的なシーリングを促進するために提供される、本発明の種々の態様を示す。重要なことは、失敗したりまたは漏出したりすることなく内部加圧に耐えることができるシールを達成することである。
デバイス132の一実施形態は、Dow Corning Sylgard 184ポリ(ジメチルシロキサン)(PDMS)等の軟質のゴム様材料を用い得る。この材料はシリコン、ガラスまたはSU8フォトレジストフィーチャからの成形がしやすい。しかし、高圧力に曝されると、それは破れてしまう場合がある。従って、代替の材料が適切であり、種々の実施形態のために実装され得る。これらの材料としては、シリコン、ガラス(全形態、ソーダ石灰、ホウケイ酸塩等)およびSU8等のフォトレジスト/ポリマーが挙げられ得る。他のポリマー類(ポイメチルメタクリラート(poymethylmethacrylate、PMMA)等)も可能である。これらの後者の材料の或るものは、より複雑なデバイス形状形成を必要とすることになる。例えば、シリコンは化学エッチング、反応性イオンエッチング(reactive ion etching、RIE)またはレーザマイクロマシニングを必要とする場合がある。本発明はデバイス形成の特定の材料または方法に特化するものではない。
C4F10は、外殻を通し取り囲んでいる水ベースの媒体にコアから入るその拡散速度が非常に低い故に、商業生産されている超音波マイクロバブルにおいて最も普及しているガスコアである。C4F10の場合でさえ、いくらかの拡散がなお存在し、これは、収容バイアル内において、造影剤の上方の上部空間をC4F10が飽和充填したマイクロバブルの長期貯蔵を示唆する。しかし、使用直前にマイクロバブルが形成されるという本発明の種々の実施形態の状況では、長寿命は問題とならない場合がある。実際、体内における短い寿命が大きな利点となる場合がある。例えば、我々は、バブルが所望の治療区域から逃げてしまう場合に急速溶解する空気充填マイクロバブルを用いることを構想している。急速溶解するマイクロバブルを用いることによって、我々は肺内等の所望されない下流の塞栓のリスクを軽減することができる。不安定なバブルを用いるアプローチは、はるかにより大きなバブル(10〜20μmの範囲内等)の使用を可能にする場合もある。我々は、より大きいバブルは、超音波によって誘発される分裂のプロセス中の血管壁内へのより高い浸透によって、より大きな治療効果をもたらすことを述べた。
外殻液の粘度を高めることは、生産されるバブルの寸法を小さくする効果がある(他の変数はすべて一定に保たれる)。この効果を達成するために種々の外殻液体添加物が考えられる。このようなものとしては、スクロース、グリセロールおよびデキストランが挙げられる。加えて、場合によっては、外殻液の粘度の熱依存性を利用することによって外殻の粘度を変更することが可能である。例えば、ペルチェ効果デバイスが、マイクロスケール、またはマイクロスケールに近いスケールの局所冷却を提供するために用いられ得る。マイクロフルイディクスデバイスを冷却するためのペルチェ効果は、Maltezos、他、’’Thermal management in microfluidics using micro−Peltier junctions,’’ Appl Phys Lett, vol. 87, pp. 154105(1−3), 2005によって記載されている。同文献はその全体が本願明細書においてそれへの参照により援用される。Marlow Industries(Dallas, TX)がマイクロエレクトロニクス業界用のペルチェベースのサーモクーラを供給している。NL1010T−01等のデバイスが、マイクロフルイディクスデバイスを冷却する目的のために適合されることができる(しかし、この特定の品番は、ほとんどの場合に適合性のあるカテーテルではないだろう)。
少なくとも1つの実施形態では、本発明は、一方の管腔は既存のカテーテルの通路専用である二重管腔カテーテルを含み得る。好ましくは、光コヒーレンストモグラフィ(OCT)イメージング用カテーテルである。LigMab LLCが、Helios閉塞カテーテルおよびImageWire OCTイメージング用カテーテルと呼ばれるOCTカテーテルシステムを開発した。OCT/IVUSと薬物/遺伝子のジョイント送達システムの実装は、専用超音波カテーテルを作り出すとことになり、専用超音波カテーテルは、適切なサイズになされたカテーテルチューブ(直径約1mmのサイズになされる等)の先端付近に搭載される治療用単一素子であって、該圧電セラミックの単一素子の頂部および底部へのワイヤを有する素子を含むであろう。超音波カテーテルは、また、食塩溶液中のマイクロバブルのカテーテルの全長を通る通路を提供する管腔およびポートと、超音波素子に対して近位の血管内への出口とを有する必要があろう。ImageWire OCTイメージング用カテーテルが、接着剤(エポキシ等)を用いて、超音波カテーテルに接合される。ImageWireは、Helios閉塞カテーテルと連動させて、通常のOCTの使用法によるのと同様に用いられることができる。OCT走査の間、超音波素子1412ならびに(マイクロバブル管腔1433および放出ポート1434を介した)バブル注入は、OCTイメージングに応答してユーザがスイッチを入れるのに従って、有効になっている。OCTの円周方向視野の一部は超音波カテーテルによって隠される場合があるが、死角は、カテーテルの対を回転させることによって回避されることができる。図16の概略図では、OCTカテーテル1450は超音波カテーテル1417の管腔1420の内部に搭載されている。画像平面(円周方向)は超音波治療区域に対して横方向に(遠位方向に)オフセットされている。理想的には、OCT画像平面のオフセットは、超音波デバイス/トランスデューサ1438とカテーテル1417の端部との間の物理的距離1422を最小化することによって最小になり、従って、OCTカテーテル1450の最近位側の有効なイメージング平面はわずかに約1mmオフセットされるだけになる。代替的に、OCT平面は超音波カテーテルの内部にあることができるが、この場合には何らかのイメージングアーチファクトが生じ得る。
本発明によって開示されている通りのマイクロバブル発生用マイクロフルイディクスを含む統合カテーテルシステムでは、マイクロフルイディクスデバイス1332へのガスおよび外殻流体の送達のために提供される構造を単純化するための種々の実施形態が提供される。図17Aは、マイクロフルイディクスデバイス1332が、カテーテル1720内の、カテーテルシステム1702の遠位端部内に収容される一実施形態を概略的に示す。図17A〜17Cの3つのすべての実施形態において、マイクロフルイディクスデバイス1332はカテーテルの遠位先端内に設置されている。それにより、射出ポート1333はカテーテル1720の遠位端1706の開口部1704に接するので、射出ポート1333はマイクロバブル1334をシステム1702から直接射出する。例えば、射出されたバブルは自由に流れてデバイス1332/射出ポート1333から出、カテーテル1720および血管の中心軸付近でカテーテル管腔1704から出て血流に入り得る。ただし、ここに記載されている機構は、図17A〜17Cに示されている態様の遠位先端におけるデバイス1332の設置に限定されるものではないことに留意されたい。なぜなら、デバイス1332は遠位先端の近位に据えられることができようし、射出ポート1332はカテーテルチューブ材1720の側面の開口部と接するように構成され得、あるいはチューブを介して、開口部1704、またはカテーテルチューブ材1720の側壁の開口部に接続され得るからである。
(エレクトロポレーション)

標的組織への薬物/遺伝子送達を向上させるために局所的組織加熱が用いられることができる。Gao、他、’’Intravascular Magnetic Resonance/Radiofrequency May Enhance Gene Therapy for Prevention of In−stentNeointimal Hyperplasia,’’ Acad. Radiol., pp. 526−530, 2006、および、Yang、他への米国特許第7,422,568号(両文献はその全体が本願明細書においてそれへの参照により援用される)は、ガイドワイヤが無線周波数(Radio Frequency、RF)電源に取り付けられる変更されたカテーテルを記載している。RF信号は通例、l〜3GHzの範囲内である(ただし、より低い周波数(200MHz前後等)が用いられることができる)。有効性を高めるには中程度の温度上昇が必要とされるだけであることが見いだされている。一般的に、加熱は、薬物/遺伝子送達のための単独方法であるよりは、薬物/遺伝子送達を強化することが観察される。すなわち、加熱は、エレクトロポレーションまたは集束超音波/マイクロバブルと組み合わせて最もうまく用いられると考えられる。Gaoの論文およびYangの特許の内容は、RFベースの加熱を実装するために必要な技術の詳細な概説(いくつかの設計およびモデル化の詳細を含む)を提供し、これらの参照文献は本文書において参照により援用される。

最適なlacZノックダウン結果を得るために、以下の構造的特徴が、lacZのための操作されたpre−miRNAをエンコードするdsオリゴ内に組み込まれた。1)ベクターと相補的である4ヌクレオチド、5’オーバーハング(TGCT)(ディレクショナルクローニングのために必要)、2)5’G+lacZのために抽出された短い21ヌクレオチドアンチセンス配列(成熟miRNA)(GACTACACAAATCAGCGATTT)、これに続き、3)末端ループを形成する19個のヌクレオチドの短いスペーサ、および、4)内部ループを作り出すために、2つのヌクレオチドが除去された(Δ2)を有する短いセンス標的配列、5)ベクターと相補的である4ヌクレオチド、5’オーバーハング(CAGG)(ディレクショナルクローニングのために必要)。ベクターは、本願明細書において記載されている本発明の技術を用いる、インビボにおける将来の遺伝子標的有効性確認試験のためにmiR−lacZを所望のmiRNAと置換する固有の制限酵素部位を考慮して設計される。
陽イオン性脂質マイクロバブルは自己集合によって形成される。手短に言えば、ホスファチジルコリン、インビボにおける寿命を延ばすために用いられるステアリン酸ポリエチレングリコール−40(PEG)、およびジステアリルトリメチルアンモニウムプロパンの水性ミセル混合物が、デカフルオロブタンガスを散布しながら音波処理される。遠心浮遊によってマイクロバブルが未反応成分から分離される。マイクロバブルのサイズおよび濃度がコールターカウンタ上でエレクトロゾーンセンシングによって測定される。p−miR−lacZプラスミドが、以前に示されている通りに静電荷結合によって、陽イオン性脂質外殻を持つマイクロバブルと結合され、結合効率が、中性または陰イオン性マイクロバブルと比較して評価される。マイクロバブルに結合されたプラスミドの量は、260nmにおける吸収度を用い、標準曲線と対照して算出される。実験のためのマイクロバブルに対するp−miR−lacZの比は1μgDNA/5×106マイクロバブルに維持される。図22は、デカフルオロブタンガスコア2202、陽イオン性脂質外殻2204、ならびにそこに付着させたプラスミドDNA2206およびPEGスペーサ2208を有する、マイクロバブル1833の1つの実施形態を示す。
lacZを発現する内皮および平滑筋細胞系は、我々の研究所によって日常的に遂行されているように、ROSA26マウス大動脈から培養されることになる。細胞は、光学的に透明且つ音響的に透過的なOPTICELL(商標)2302内で平板培養される。細胞が平板培養された24時間後に、各OPTICELL(商標)2302内の培地は、以下の試薬の3つの組み合わせのうちの1つを含有する新鮮な培地と交換される:30μgのp−miR−lacZプラスミドと結合されたマイクロバブル(1.5×108)(各OPTICELL(商標)内の最終溶液、3μg/ml)、30μgp−miR−lacZプラスミドのみ(マイクロバブルなし)、または同等体積の純食塩水(プラスミドもマイクロバブルもなし)。図23に示される超音波照射装置2300は、単一素子の集束式1MHzトランスデューサ2304(Panametrics)およびそれを正確に位置付けるためのMM3000モーションコントローラリニアステージ2306の使用を含む。波形、パルス繰返し周波数、および振幅のセットが、自動波発生器(automatic wave generator、AWG)2310、パルス発生器2308および増幅器2312を用いて、個々のパルス長およびピーク圧力を変化させながら、一定のデューティサイクルを維持するように選ばれる。各OPTICELL(商標)2302は、吸音体2318によって裏当てされたOPTICELL(商標)保持具2316内に保持され、バブルが細胞に向かって上昇する時間を与えるために、超音波照射の前に、再循環ヒータ(不図示)(モデル73A0A11B,Polyscience,Niles,IL)を用いて37°Cに保たれたウォーターバス2314中に2分間浸漬される。超音波照射の後、OPTICELL(商標)2302は、リン酸緩衝食塩水(phosphate buffered saline、PBS)を用いてすぐにフラッシュされ、新鮮血清含有成長培地を補給される。p−miR−lacZのトランスフェクションの有効性が処理の24時間後に単位面積当たりのeGFP蛍光細胞の数として測定される。LacZのノックダウンが、処理の72時間後に、細胞を固定し、X−galを用いてlacZ信号を処理することによって評価される。lacZ抑圧の有効性は、eGFP陽性細胞と比較した、非eGFP細胞におけるlacZの強度を定量化することによって求められる。細胞死および増殖がBrdU取り込みならびにTUNELまたはアネキシンV染色によって分析される。

該処置部位に薬物または遺伝子治療を投与するために該マイクロバブルを破裂させることと
を含む、方法。 - 前記破裂させることは、前記マイクロバブルに音響放射力を印加することによって遂行され、該力は、前記カテーテル内に配置されたトランスデューサから印加される、請求項84に記載の方法。
- 前記カテーテル内に配置されるトランスデューサから前記処置部位に超音波エネルギーを印加することによって提供される超音波イメージングを介して、該処置部位を見ることをさらに含む、請求項84に記載の方法。
- 前記処置部位は、血管、臓器、実質組織、間質組織または管路の少なくとも一部である、請求項84に記載の方法。
- 前記処置部位は、血管の少なくとも一部である、請求項87に記載の方法。
- 前記被検体の心周期のECG波形を検知することであって、前記ペース調整は、検知された該ECG波形に従うものである、ことをさらに含む、請求項84に記載の方法。
- 前記被検体の前記心臓からの前記処置部位の距離に基づいて、投与することの開始に先だって、前記ECG波形に関する遅延期間を追加することをさらに含む、請求項89に記載の方法。
- 前記マイクロバブルは、前記カテーテルを通って延在する管腔を通して送達される、請求項84に記載の方法。
- 前記マイクロバブルは、前記カテーテルの前記遠位端部において形成され、そこから投与される、請求項84に記載の方法。
