Description
本発明は、糖尿病の治療に関するものであり、より詳細には、細胞をグルコース応答性のインスリン産生細胞に形質転換するための組成物及び方法に関するものである。
本発明の範囲を限定することなく、糖尿病に関して本発明の背景を記載する。糖尿病は世界中でおよそ2億人の人が罹患しており、有病率が増加してきている(Wild S, Roglic G, Green A, Sicree R, King H. Global prevalence of diabetes: estimates for the year 2000 and projections for 2030. Diabetes Care 2004; 27:1047-53)。糖尿病は世界における死亡理由の第5位であるとされ(Roglic G, Unwin N, Bennett PH, Mathers C, Tuomilehto J, Nag S, Connolly V, King H. The burden of mortality attributable to diabetes: realistic estimates for the year 2000. Diabetes Care 2005; 28:2130-35)、心血管疾患、慢性腎疾患、失明、及び神経障害を含む、重篤な合併症をもたらす。

糖尿病治療の分野における有望な機会の1つは、Kojimaに発行された、膵臓の機能を向上させるための組成物についての米国特許第6232288号明細書により教示されている。Kojimaは、ベータセルリンタンパク質自体又はその断片を使用して、未分化膵臓細胞の、インスリン産生ベータ細胞又は膵臓ポリペプチド産生F細胞への分化を促進することを教示している。BTCタンパク質組成物は患者における耐糖能を向上させ、未分化膵臓細胞の成長を阻害した。ヒトを含む哺乳類を治療する方法も提供されたが、糖尿病状態の長期的な治療は、患者にベータセルリンタンパク質を静脈内投与しても成し遂げられなかった。
ベータセルリンタンパク質(BTC、betacelluin)は、トランスジェニックマウスに由来する膵臓ベータ腫瘍細胞により産生されるペプチド因子であり、その全アミノ酸配列はcDNA分析によって明らかにされている(Shing et al., Science, 259:1604 (1993)、Sasada et al., Biochemical and Biophysical Research Communications, 190:1173 (1993))。BTCのmRNAは脳以外の器官、例えば、肝臓、腎臓、及び膵臓において検出されており、このことは、BTCタンパク質がこれらの器官においていくつかの機能を示し得ることを示唆している。BTCタンパク質は当初、マウス3T3細胞の成長促進活性を有する因子であることが発見され、その後、血管平滑筋細胞及び網膜色素上皮細胞に対する成長促進活性を示すことが明らかにされた(Shing et al., Science, 259:1604 (1993))。

本発明は、ベータセルリン及び膵臓十二指腸ホメオボックス−1(PDX1、Pancreas Duodenum Homeobox-1)を用いて細胞をグルコース応答性のインスリン産生細胞に形質転換するための組成物及び方法を用いる。一具体例として、超音波標的マイクロバブル破壊(UTMD、ultrasound targeted microbubble destruction)を、標的又は宿主細胞にベータセルリン遺伝子及び膵臓十二指腸ホメオボックス−1(PDX1)遺伝子を送達する1又は複数の発現ベクターと共に用いて、膵腺房細胞をインビボで形質転換することができる。
より詳細には、本発明は、1又は複数の細胞を、ベータセルリン(BTC)及び膵臓十二指腸ホメオボックス−1(PDX1)を発現する構築物で形質転換することにより、細胞におけるインスリンの産生を誘導するための組成物及び方法を含み、例えば、細胞を、PDX1及びBTCを共発現する構築物で形質転換する。一実施形態において、細胞は、膵島細胞、膵腺房細胞、細胞系、1又は複数のインスリン遺伝子がコトランスフェクトされている細胞から選択される。構築物は、マイクロバブル、リン酸カルシウム−DNA共沈殿、DEAE−デキストランを用いたトランスフェクション、ポリブレンを用いたトランスフェクション、エレクトロポレーション、マイクロインジェクション、リポソーム融合、リポフェクション、プロトプラスト融合、レトロウイルス感染、及びバイオリスティック、又は当業者に知られている他の方法を用いて送達することができる。特定の一実施形態において、構築物は、マイクロバブル及び超音波標的マイクロバブル破壊を用いて送達される。
本発明の組成物及び方法を用いることにより、膵臓細胞が15日超にわたりグルコースレベルに基づいてインスリンを発現し得ること、即ち、細胞がグルコース応答性のインスリン産生細胞になることが明らかになっている。細胞は、インビボで形質転換され、15日超にわたりグルコース応答性の形でインスリンを発現することができる。一例として、細胞は、非内分泌性の膵臓細胞であり、15日超にわたり、グルコース応答性の形でインスリンを発現する。標的細胞は、マウス、ラット又はヒトのBTCから選択されたBTCと、マウス、ラット又はヒトのPDX1から選択されたPDX1との発現により、グルコース応答性のインスリン産生細胞となり得る。

本発明は、以下の特許請求の範囲の趣旨及び範囲から逸脱することなく、本明細書に記載する教示に照らして行うことが各々可能な修飾及び変化を含み得る。本発明の使用は、異なる特徴を有する構成要素を伴い得るものである。本発明の範囲は、本明細書に添付される特許請求の範囲により定義されるものであり、それにより、すべての点における同等物が完全に認識される。
本明細書では、ヌクレオチドについての「基本的に配列番号(#)で示される配列」、「に類似の配列」、「ヌクレオチド配列」という用語、及び類似の用語は、配列番号1として本明細書において同定される配列のあらゆる部分に実質的に対応する配列を指す。これらの用語は、合成分子及び天然由来の分子を指し、生物学的に、免疫学的に、実験的に、又はそれ以外の機能的に等しい活性、例えば、核酸断片によるハイブリダイゼーションに関する活性、又は、ベータセルリン若しくはPDX1の活性のすべて若しくは一部をコードする能力を有する配列を含む。当然のことながら、これらの用語は、このような配列内の情報を、その線形の順序で特定されている通りに含むものである。
本明細書では、「遺伝子」という用語は、機能的なタンパク質、ポリペプチド、又はペプチドをコードする単位を指す。当業者には理解されるように、この機能的な用語は、ゲノム配列、cDNA配列、又はそれらの断片若しくは組合せと、遺伝子産物との両方を含み、これらは人の手によって改変されている可能性のあるものも含む。精製した遺伝子、核酸、タンパク質などは、それが通常関連している少なくとも1つの汚染核酸又は汚染タンパク質から同定及び分離した場合の、これらの実体を指すために用いる。

本明細書では、「宿主細胞」という用語は、古細菌、原核生物、又は真核生物の、核酸の断片又は改変した断片を含むように操作された細胞を指す。したがって、操作された細胞又は組換え細胞は、人の手を介して組換えにより導入された遺伝子を含まない天然の細胞から区別することが可能である。
本明細書では、「制御配列」という用語は、特定の宿主生物における機能可能に結合したコード配列の発現に必要なDNA配列を指す。原核生物に適した制御配列には、例えばプロモーターが含まれ、オペレーター配列、リボソーム結合部位、及び転写ターミネーターが含まれていてもよい。成長阻害量より低いレベルにFab’ポリペプチドの合成を抑制し、その一方で例えば対数期の間に細胞培養物を成長及び成熟させる、高度に調節された誘導性プロモーターを用い得る。
本明細書では、「機能可能に結合している」という用語は、第1及び第2の核酸配列の間の機能的な関係を指す。例えば、プレ配列又は分泌リーダーのDNAは、ポリペプチドの分泌に関与するプレタンパク質として発現する場合、ポリペプチドのDNAに機能可能に結合している。プロモーター若しくはエンハンサーが、コード配列の転写に作用する場合に、コード配列に機能可能に結合しているか、又は、リボソーム結合部位が、翻訳を促進するように位置している場合に、eコード配列に機能可能に結合している。通常、「機能可能に結合している」とは、結合対象のDNA配列が連続的であること、また分泌リーダーの場合は、連続的であり且つ同一のリーディングフレーム内にあることを意味する。エンハンサーは連続的である必要はない。結合は、適当な制限部位におけるライゲーションによって行われる。そのような部位が存在しない場合、合成オリゴヌクレオチドのアダプター又はリンカーを従来の慣例に従って用いる。
本明細書では、「細胞」及び「細胞培養物」という用語は互換的に用いられ、このような表記のすべては後代を含む。したがって、「形質転換体」及び「形質転換細胞」という語は、初代の対象細胞と、移動の数に関係なく、それらに由来する培養物とを含む。また、すべての後代は、意図的な又は不慮の突然変異により、DNA含量が正確には同一ではない場合があることも理解されたい。最初に形質転換した細胞においてスクリーニングされたものと同一の機能又は生物学的活性を有する突然変異の後代が含まれる。異なる表記は文脈から明らかとなろう。
本明細書では、「プラスミド」は大文字及び/又は数字を前及び/又は後に有する小文字pによって表記される。市販の開始プラスミドは、無制限で公然に利用可能であるか、又は、このような利用可能なプラスミドから、公開された手順に従って構築可能である。加えて、他の同等のプラスミドが当技術分野において知られており、当業者に自明であろう。
本明細書では、「タンパク質」、「ポリペプチド」、又は「ペプチド」という用語は、ペプチド結合を介して結合しているアミノ酸を含む化合物を指し、互換的に用いられる。
本明細書では、「内因性」という用語は、源が細胞内に由来する物質を指す。内因性物質は細胞の代謝活性により産生される。しかし、それにもかかわらず、内因性物質は、例えば物質をコードする遺伝子を細胞に発現させるためのものといった、細胞代謝の操作の結果として産生され得る。
本明細書では、「外因性」という用語は、供給源が細胞外にある物質を指す。核酸について言及する場合、「外因性」は、細胞にとって外来の核酸配列、又は細胞に相同であるが、宿主細胞の核酸内の、通常はそのエレメントが見られない位置に存在している核酸配列を指す。にもかかわらず、外因性物質は、当業者に知られている様々な代謝手段又は誘導手段のいずれか1つにより、細胞に内在化し得る。
遺伝子のゲノム形態又はクローンは、「イントロン」又は「介在領域」又は「介在配列」と呼ばれる非コード配列が介在したコード領域を含む。イントロンは、核RNA(hnRNA)に転写される遺伝子断片であり、イントロンは、エンハンサーのような調節エレメントを含み得る。イントロンは、核又は一次転写産物から除去されるか、削除されるか、又は「スプライシング」される。したがって、イントロンは、メッセンジャーRNA(mRNA、messenger RNA)転写産物には存在しない。翻訳の際のmRNAの機能は、新生ポリペプチドにおけるアミノ酸の配列又は順序を特定することである。
イントロンを含むことに加え、遺伝子のゲノム形態は、RNA転写産物上に存在する配列の5’末端及び3’末端の両方に位置する配列も含み得る。これらの配列は「隣接」配列又は「隣接」領域と呼ばれる(これらの隣接配列は、mRNA転写産物上に存在する非翻訳配列の5’又は3’に位置する)。5’隣接領域は、遺伝子の転写を制御するか又はそれに影響を与える、プロモーター及びエンハンサーのような調節配列を含み得る。3’隣接領域は、転写の終結、転写後の切断、及びポリアデニル化を指示する配列を含み得る。

本明細書では、「形質転換」という用語は、外因性DNAをレシピエント細胞の中に入れそれを変化させるプロセスを指し、例えば、ベータセルリン及び/又はPDX1を発現するためのプロモーター及びコード配列を含む、1又は複数のプラスミドである。それは、当技術分野において周知の様々な方法を用いて、天然の又は人工的な条件下において生じさせ得る。形質転換では、外来の核酸配列を原核宿主細胞又は真核宿主細胞内に挿入するために、あらゆる既知の方法を用い得る。方法は形質転換する宿主細胞に基づいて選択され、限定はしないが、ウイルス感染、エレクトロポレーション、リポフェクション、及び粒子衝突を含み得る。このような「形質転換」細胞には、挿入されたDNAが自己複製プラスミドとして又は宿主染色体の一部として複製し得る、安定な形質転換細胞が含まれる。
本明細書では、「トランスフェクション」という用語は、外来DNAを真核細胞内に導入することを指す。トランスフェクションは、例えばリン酸カルシウム−DNA共沈殿、DEAE−デキストランを用いたトランスフェクション、ポリブレンを用いたトランスフェクション、エレクトロポレーション、マイクロインジェクション、リポソーム融合、リポフェクション、プロトプラスト融合、レトロウイルス感染、及びバイオリスティックを含む、当業者に知られている様々な手段によって行うことができる。したがって、「安定なトランスフェクション」又は「安定にトランスフェクトした」という用語は、トランスフェクトする細胞のゲノム内への外来DNAの導入及び組み込みを指す。「安定な形質転換体」という用語は、ゲノムDNA内に外来DNAを安定に組み込まれた細胞を指す。また、この用語は、挿入したDNA又はRNAを限られた時間にわたって一過的に発現する細胞も含む。したがって、「一過性トランスフェクション」又は「一過的にトランスフェクトした」という用語は、トランスフェクトする細胞のゲノム内に外来DNAが組み込まれていない細胞内に外来DNAを導入することを指す。外来DNAは数日間、トランスフェクトした細胞の核内に残存する。この時間の間、外来DNAは、染色体における内因性遺伝子の発現を管理する調節制御を受ける。「一過性トランスフェクタント」という用語は、外来DNAを取り込んではいるがこのDNAを組み込んではいない細胞を指す。
本明細書では、「ベクター」という用語は、1つの細胞から別の細胞へ1又は複数のDNA断片を移す核酸分子に関して用いる。「媒体」という用語は「ベクター」と互換的に用いる場合がある。本明細書において用いる「ベクター」という用語は、所望のコード配列と特定の宿主生物における機能可能に結合したコード配列の発現に必要な適切な核酸配列とを含む組換えDNA分子に関する発現ベクターも含む。原核生物における発現に必要な核酸配列には、通常、プロモーター、(任意の)オペレーター、及びリボソーム結合部位が含まれ、他の配列を伴うことが多い。真核細胞は、プロモーター、エンハンサー、並びに終結シグナル及びポリアデニル化シグナルを用いることが知られている。
本明細書では、「増幅する」という用語は、核酸に関して用いる場合は、当技術分野において知られているあらゆる方法によって核酸配列の多くのコピーを産生することを指す。増幅は、鋳型特異性を伴う、核酸の複製の特別なケースである。鋳型特性は「標的」特異性という用語で記載されることが多い。標的配列は、それらを他の核酸から区別されるべきであるという意味で「標的」である。増幅技術は、主にこの区別のために設計されている。
本明細書では、「プライマー」という用語は、核酸鎖に相補的なプライマー伸長産物の合成が誘導される条件下(即ち、ヌクレオチド、及びDNAポリメラーゼのような誘導剤の存在下で、且つ、適切な温度及びpH)にある時に合成の開始点として作用し得る、精製された制限消化物内にあるように自然に生じるか又は合成的に産生されるオリゴヌクレオチドを指す。プライマーは、増幅の最大の効率のためには一本鎖であり得るが、二本鎖であってもよい。二本鎖である場合、プライマーは、まず鎖を分離するための処理をし、その後、伸長産物を調製するために用いる。プライマーは、誘導剤の存在下で伸長産物の合成を開始するために十分長くなくてはならない。プライマーの正確な長さは、温度、プライマーの源、及び用いる方法を含む多くの要素に依存する。
本明細書では、「プローブ」という用語は、所望の別のオリゴヌクレオチドにハイブリダイズし得る、精製された制限消化物内にあるように自然に生じるか、又は合成的に、組換えにより、若しくはPCR増幅により産生される、オリゴヌクレオチド(即ち、ヌクレオチドの配列)を指す。プローブは一本鎖又は二本鎖であり得る。プローブは、特定の遺伝子配列の検出、同定、及び単離に有用である。本発明において用いるあらゆるプローブは、あらゆる「レポーター分子」で標識されるものであり、それにより、限定はしないが、酵素(例えば、ELISA、及び酵素に基づいた組織化学的アッセイ)、蛍光、放射能、及び発光のシステムを含む、あらゆる検出システムにおいて検出可能である。本発明は、いかなる特定の検出システム又は標識にも限定されるものではない。
本明細書では、「標的」という用語は、ポリメラーゼ連鎖反応に関して用いる場合、ポリメラーゼ連鎖反応に用いるプライマーが結合している核酸の領域を指す。したがって、「標的」は、他の核酸配列から区別されるべきものである。「断片」は、標的配列内の核酸の一領域として定義される。標的は、細胞又は組織に関して用いる場合、細胞内に核酸を送達して細胞の機能を変えるための、例えば、1又は複数のBTC遺伝子又はPDX1遺伝子を発現させるための、細胞に対して外因性のベクター(例えば、ウイルス、リポソーム、又はさらに裸の核酸)を用いた標的化を指す。
本明細書では、「ポリメラーゼ連鎖反応」(「PCR(polymerase chain reaction)」)という用語は、クローニング又は精製を行うことなくゲノムDNAの混合物における標的配列の断片の濃度を増大させるための方法を記載している、参照することにより本明細書に組み込まれる、K. B. Mullisの米国特許第4683195号明細書、米国特許第4683202号明細書、及び米国特許第4965188号明細書の方法を指す。標的配列を増幅するためのこのプロセスは、所望の標的配列を含むDNA混合物に対する、非常に過剰な2つのオリゴヌクレオチドプライマーを含み、その後、DNAポリメラーゼの存在下で正確な一連の熱サイクルを行う。2つのプライマーは、各々の、二本鎖標的配列の鎖に対して相補的である。増幅を行うために、混合物を変性し、次にプライマーをそれらに相補的な標的分子内の鎖にアニーリングする。アニーリングの後、プライマーをポリメラーゼで伸長し、相補鎖の新たな対を形成させる。変性段階、プライマーのアニーリング段階、及びポリメラーゼ伸長段階は、所望の標的配列の増幅断片を高濃度にするために何度も反復することができる(即ち、変性、アニーリング、及び伸長は1つの「サイクル」を構成し、多くの「サイクル」を行うことができる)。所望の標的配列の増幅断片の長さは、プライマー同士の相関位置によって決定され、したがって、この長さは調節可能なパラメータである。プロセスの反復態様により、この方法は「ポリメラーゼ連鎖反応」(以下では「PCR」)と呼ばれる。標的配列の所望の増幅断片は混合物における(濃度の観点から)主要な配列となるため、それらは「PCR増幅産物」と呼ばれる。PCRでは、ゲノムDNAにおける特定の標的配列の1つのコピーを、いくつかの異なる手法(例えば、標識したプローブとのハイブリダイゼーション、ビオチニル化したプライマーの組み込みの後のアビジン−酵素複合体の検出、増幅断片への32Pで標識したDCTP又はDATPのようなデオキシヌクレオチド三リン酸の組み込み)によって検出することが可能なレベルまで増幅することが可能である。ゲノムDNAに加え、あらゆるオリゴヌクレオチド配列を適切なプライマー分子のセットで増幅することができる。特に、PCRプロセス自体により生成した増幅断片自体、その後のPCR増幅のための有効な鋳型である。
本明細書では、「染色試薬」という用語は、試薬を含む核酸配列のすべてのハイブリダイゼーションパターンを指す。ゲノムの一部分に特異的な染色試薬により、標的の染色体物質と非標的の染色体物質との間でコントラストが生じる。多くの異なる異常は、1又は複数の色で検出されるゲノムの部分上のあらゆる所望の染色パターン(複数色の染色パターン)及び/又は他の指示法により検出し得る。
本明細書では、「導入遺伝子」という用語は、哺乳類のゲノム、例えば生きている動物の哺乳類細胞内に人工的に挿入し得る遺伝物質を指す。「トランスジェニック動物」という用語は、本明細書において、細胞の一部において染色体外エレメントとして存在するか又は生殖系のDNA内(即ち、ほとんどの又はすべての細胞のゲノム配列内)に安定的に組み込まれている非内因性(即ち異種)核酸配列を有する、ヒト以外の動物、通常は哺乳類を記載するために用いる。異種核酸は、当技術分野において周知の方法に従って、例えば宿主動物の胚又は胚性幹細胞を遺伝的に操作することにより、そのようなトランスジェニック動物の生殖系に導入される。
本明細書では、「導入遺伝子」という用語は、そのような異種核酸、例えば、(例えば、「ノックイン」トランスジェニック動物の作成のための)発現構築物などの形態の異種核酸、又は、(例えば、「ノックアウト」トランスジェニック動物の作成のための)標的遺伝子の発現を減少させる、標的遺伝子内若しくは標的遺伝子の近傍での挿入の際の異種核酸を指す。遺伝子の「ノックアウト」とは、標的遺伝子の機能を低下させて、好ましくは標的遺伝子の発現を検出不可能にするか又は有意でないものにする、遺伝子の配列における改変を意味する。トランスジェニックノックアウト動物は、標的遺伝子のヘテロ接合型のノックアウト、又は標的遺伝子のホモ接合型のノックアウトを含む。
本明細書では、「幹細胞」という用語は、例えば胚性幹細胞などの全能性又は多能性の幹細胞、及び、限定はしないが発生の胚盤胞段階にある細胞を含む、胚の発生の非常に早い段階にある多能性細胞を指す。本発明での使用の特定の一例として、幹細胞は、腺房細胞又はベータ細胞に分化していない膵臓細胞前駆体であり得、BTC及び/又はPDX1を発現するための標的として用いられる。
本発明者らは、超音波標的マイクロバブル破壊(UTMD)を用いて、遺伝子治療の標的を正常なラットの膵島とし得ることを明らかにした(Chen S, Ding JH, Bekeredjian R, Yang BZ, Shohet RV, Johnston SA, Hohmeier HE, Newgard CB, Grayburn PA. Efficient gene delivery to pancreatic islets with ultrasonic microbubble destruction technology. Proc Natl Acad Sci U S A. 2006; 103:8469-74)。プラスミドDNAを有する静脈内のマイクロバブルは、膵臓の微小循環において超音波によって選択的に破壊され、それにより、プラスミドを局所的に送達する。膵島の特異性は、プラスミドDNA内にラットのインスリン−Iプロモーターを組み込むことにより得られた。現在、ラットのストレプトゾトシン(STZ)誘発性の糖尿病において、ベータセルリン(BTC)を単独で及びPDX1と組み合わせて送達するためにUTMDを用い得ることが明らかになっている。標的細胞の形質転換により、BTC及びPDX1で処理したラットにおいて見られる、グルカゴンで染色した細胞の初期の膵島様塊が生じた。この研究において、塊は処理の30日後までに消失した。正常な膵島の再生は見られなかったが、ベータ細胞マーカーを用いて膵腺房細胞をインスリン産生細胞に形質転換することにより、糖尿病はUTMDの最大15日後まで回復した。
血中のグルコース、インスリン、及びC−ペプチドに対するUTMD遺伝子治療の影響。糖尿病のラットにおける血清のグルコース及びインスリンに対するBTC遺伝子治療の影響を評価するため、18匹のラットをSTZ(80mg/kg)で腹腔内処理し、その後、DsRedレポーター遺伝子(不活性の対照、n=6)、BTC(n=6)、又はBTC及びPDX1(n=6)を含むUTMDを行った。6匹のさらなる対照に対して、STZのみを付与しUTMD遺伝子治療を行わなかったか(n=3)、又はSTZを付与せずUTMDも行わなかった(正常な対照、n=3)。ベースラインの血中グルコースは111±19mg/dlであり、群間で有意には異ならなかった。
図1(上図)において示すように、血糖は、STZで処理したすべてのラットにおいて2日目までに劇的に上昇し、STZのみ、DsRedレポーター遺伝子、及びBTCのみで処理したラットにおいて10日間を通して上昇し続けた。しかし、BTC及びPDX1の両方で処理したラットにおいて、グルコースは3日目から5日目まで減少し、10日間を通しての平均値は200mg/dl未満のままとなった。ANOVAを反復して測定した結果、処理群間の差(F=89.0、p<0.0001)及び経時的なグルコースの差(F=54.7、p<0.0001)は、統計上非常に有意であった。シェフェのポストホック試験の結果、BTC及びPDX1で処理したラットにおける血糖は、STZのみの群、DsRed群、又はBTC群におけるよりも統計上有意に低かった(p<0.0001)が、対照とは統計上の差はなかった(p=0.17)。血中のインスリンレベルはベースラインで0.52±0.11ng/mlであり、群間で有意には異ならなかった。
図2(下図)に示すように、インスリンレベルは、正常な対照よりも、STZ処理したすべての群において3日目に低下した。しかし、5日目には、インスリンレベルは、正常な群よりもBTC/PDX1群の方が高かったが、この差は統計上は有意ではなかった。ANOVAを反復して測定した結果、処理群間の差(F=15.4、p<0.0001)及び経時的なインスリンの差(F=18.9、p<0.0001)は、統計上非常に有意であった。シェフェのポストホック試験を用いたところ、インスリンレベルはBTC及びPDX1で処理したラットの群において、STZのみ(p=0.0087)、DsRed(p=0.024)、又はBTCのみ(p=0.015)で処理したラットと比較して統計上有意に高いが、対照とは有意には異ならない(p=0.99)ことが分かった。
検出されたインスリンがUTMD処理により産生されたものであることを確認するために、ベースライン及び10日目のC−ペプチドのレベルを測定した。図2に示すように、C−ペプチドは正常な対照において安定を保ち、STZのみ、又はSTZとDsRed若しくはBTCとで処理したラットにおいて減少した。しかし、C−ペプチドは、BTC及びPDX1の両方で処理したラットにおいて10日目に有意に上昇した。ANOVAを反復して測定した結果、処理群間のC−ペプチドの差は統計上有意であった(F=10.5、p=0.0004)。ポストホック試験の結果、10日目のC−ペプチドのレベルは、BTC/PDX1群において、すべての他の群と比較して高かった(p<0.03)。
UTMD処理によりグルコース調節性のインスリン産生が生じるかどうかを決定するために、耐糖能試験を実施した。図3に示すように、STZで処理した動物は、BTCのみで処理したラットと同様、異常な耐糖能曲線を顕著に有した。逆に、BTC及びPDX1で処理したラットはほぼ正常な耐糖能応答を有していた。
膵島の形態の免疫組織学。図4は、FITCで標識した抗インスリン(緑)及びCY5で標識した抗グルカゴン(青)で染色した、10日目のラットの膵臓の代表的な組織学的サンプルである。低倍率(100倍)では、正常な対照(左上図)は視野当たり3〜4個の膵島を有し、中央は緑色の抗インスリンの染色であり、周辺は青色の抗グルカゴンの染色であり、正常な膵島の予想される形態と合致していた。STZのみで処理したラット(右上図)は10日目には抗インスリンの染色がほとんど検出されなかったが、膵島の残余と思われるところに抗グルカゴンのわずかな染色が時折見られた。同様の所見がDsRedで処理したラットにおいて見られた(データは示していない)。BTCのみで処理したラットにおいて(左中図)、視野当たり1〜2個の膵島様塊が見られ、そのほとんどはグルカゴン陽性(青色)の細胞からなり、インスリンの染色はほとんどなかった。BTC及びPDX1で処理したラットは、主に抗グルカゴンの染色を有する(右中図)、視野当たり3〜4個の膵島様塊を示し、右下図において高倍率(400倍)で示す。加えて、100倍の外分泌性の膵臓において、抗インスリンの実質的な染色があった。高倍率(400倍、右下図)では、多くの非膵島細胞が細胞質において抗インスリンの染色を示していることが確認される。
図5は、処理群間での膵島の形態の差を示す。正常な膵島は対照群において共通であったが(スライド当たり46±9個の膵島)、STZ処理したすべての群においてはほとんどなかった(スライド当たり3個未満の膵島)(対照に対してp<0.0001)。主にグルカゴンで染色されている細胞の異常な膵島様塊は正常な対照には存在せず、STZ処理の後にのみ見られた。これらの異常な現れ方をする膵島は、BTC及びPDX1で処理したラット(スライド当たり19±8個の塊)において、STZのみ(7±2)、DsRed(11±4)、又はBTCのみ(12±3)で処理したラットよりもより多く見られた(p<0.02)。さらに、グルカゴンで染色された細胞のこれらの膵島様塊は、BTC及びPDX1で処理したラットにおいて他の群におけるよりも高い血中グルカゴンレベルと関連していた。ベースラインでは、血中グルカゴンは95±12pg/mlであり、群間で差はなかった。10日目に、グルカゴンは、BTC及びPDX1で処理したラットにおいて263±145pg/mlに上昇したが(F=4.6、p<0.014)、他の群においては有意には変化しなかった。
外分泌性の膵臓におけるインスリン産生細胞の免疫組織学。図4に見られる外分泌性の膵臓におけるインスリン産生細胞をさらに評価するため、FITCで標識した抗インスリン及びDsRedで標識した抗アミラーゼの両方を用いて共焦点顕微鏡法を実施した(図6、左図)。1000倍の左上図に示すように、これらの細胞の細胞質においてFITCと結合した抗インスリン抗体によって測定したところ、インスリンが顕著に発現している。左下図は、シグナルが共局在している、抗インスリン(緑)及び抗アミラーゼ(赤)の共焦点画像を示しており、これらが腺房細胞であることを示している。腺房細胞がインスリンを産生していたことをさらに確認するために、正常な対照、STZのみ、並びにSTZの後にBTCのみを用いたUTMD、並びにSTZの後にBTC及びPDX1を用いたUTMDの4群のラットから、腺房細胞を単離した。次に、単離した腺房細胞画分を、多くのベータ細胞マーカーについてRT−PCRに付した(図6、右図)。ポジティブコントロールであるB−アクチンが、Nkx6.1及びneuroDと同様、すべての群において存在した。INS−1、INS−2、グルカゴン、ソマトスタチン、MIST−1、VMAT、ニューロゲニン−3、及びNkx2.2を含む多くのマーカーが、BTC及びPDX1を付与した群においてのみ検出された。
経時的な腺房細胞のインスリン産生。個別の研究において、6匹のラットをSTZで処理し、その後BTC及びPDX1でUTMDを行い、30日間追跡した。血清のグルコース及びインスリンを5日毎に測定し、それぞれ3匹のラットをそれぞれ20日目及び30日目に屠殺した。図7に示すように、腺房細胞の細胞質における抗インスリン染色は30日目までに消失し、血清インスリンは顕著に低下し、血清グルコースは上昇した。加えて、グルカゴン陽性細胞の膵島様塊は30日目にはまったく存在しなかった。
糖尿病に対する薬理学的療法は改良され続けているが、「厳格な」グルコースの制御では糖尿病の重篤な合併症はなくなっておらず(The DCCT Research Group. The effect of intensive treatment of diabetes on the development and progression of long-term complications in insulin-dependent diabetes mellitus. N Engl J Med 1993; 329:977-986、The EDIC Study Group. Sustained effect of intensive treatment in type I diabetes mellitus on development and progression of diabetic retinopathy: The Epidemiology of Diabetes Interventions and Complications (EDIC) study. Diabetes 1993; 42:1715-1720)、STZ(Guz Y, Nasir I, Teitelman G. Regeneration of pancreatic beta cells from intra-islet precursor cells in an experimental model of diabetes. Endocrinology 2001; 142:4956-4968)、又はインターフェロンガンマ(Sarvetnick NE, Gu D. Regeneration of pancreatic endocrine cells in interferon-gamma transgenic mice. Adv Exp Med Biol 1992; 321:85-89)の後に生じることが知られている、ベータ細胞塊の増加の原因が、最近の議論の対象となっている。Dor et al.(Dor Y, Brown J, Martinez OI, Melton DA. Adult pancreatic beta-cells are formed by self-duplication rather than stem cell differentiation. Beta-cell differentiation from nonendocrine epithelial cells of the adult human pancreas. Diabetes 2002; 51:1398-1408)及び肝細胞(Kojima H, Fujimaya M, Matsumura K, Younan P, Imaeda H, Maeda M, Chan L. NeuroD-betacellulin gene therapy induces islet neogenesis in the liver and reverses diabetes in mice. Nature Medicine 2003; 9:596-603)におけるインスリンの産生を誘導することが示されているベータ細胞刺激ホルモン(Shing Y, et al. Betacellulin: a mitogen from pancreatic beta cell tumors. Combined gastrin and epidermal growth factor treatment induces islet regeneration and restores normoglycemia in C57BL/6J mice treated with alloxan. Beta-cell differentiation from nonendocrine epithelial cells of the adult human pancreas. Nature Medicine 2006; 12:310-316)の最近の研究によりサポートされる。胎児の膵組織には膵島の再生のための適切な刺激が含まれると推測される。
PDX1及びBTCを用いた遺伝子治療では主にアルファ細胞を含む初期の膵島様塊が産生され、30日目までに消失した。腺房細胞をグルコース応答性のインスリン産生細胞に形質転換し得ることは、第1に、正常な膵島機能を再生するためのBTC及びPDX1を用いたUTMDを用い得ることを示す。UTMDにより、糖尿病の成体動物モデルにおける膵島の再生のための候補遺伝子を試験するための、インビボでの非侵襲性の方法が得られる。
動物における手順及びUTMD。動物研究は、NIHの推奨及び動物実験委員会の承認に従って行った。オスのSprague-Dawleyラット(200〜250g)をケタミン(60mg/kg)及びキシラジン(5mg/kg)の腹腔内投与により麻酔した。左の腹部及び頸部の毛を剃り、ポリエチレンチューブ(PE50、Becton Dickinson社製、MD)を静脈切開により右内頸静脈に挿入した。最初の実験において、24匹のラットに、1:未処理(正常な対照ラット、n=3)、2:UTMDを行わずSTZ(80mg/kg/腹腔内、Sigma社製)のみ(N=3)、3:STZ及びDsRedレポーター遺伝子をコードするプラスミドを用いたUTMD(n=6)、4:STZ及びラットBTC遺伝子をコードするプラスミドを用いたUTMD(n=6)、5:STZ並びにBTC及びPDX1の両方をコードするプラスミドを用いたUTMD(n=6)の5つの処理のうち、1つを行った。すべてのプラスミド調製物で、遺伝子の半分はRIPプロモーターを含み、残りの半分はCMVプロモーターを含んでいた。マイクロバブル又は対照溶液(0.5mlのPBSで希釈した0.5ml)をポンプを介して20分間にわたり注入した(Genie、Kent Scientific社製)。
注入の間、市販の超音波振動子(S3、Sonos 5000、Philips Ultrasound社製、Bothell、WA)を用いて、超音波を膵臓に当てた。プローブを適当な位置に固定した。次に、超音波を1.4のメカニカルインデックスの超高調波モード(送信1.3MHz、受信3.6MHz)で用いた。R波のピーク後の45〜70msの遅延を用いたECGにより、4回の収縮末期毎に超音波のバーストを4回生じさせた。これらの設定は、この装置を用いたUTMDによるプラスミドの送達に最適であることが示されている(Chen SY, Shohet RV, Bekeredjian R, Frenkel PA, Grayburn PA. Optimization of ultrasound parameters for cardiac gene delivery of adenoviral and plasmid deoxyribonucleic acid by ultrasound-targeted microbubble destruction. J Am Coll Cardiol 2003; 42:301-308)。バブル破壊は、すべてのラットにおいて視覚的に明らかであった。UTMDの後、頸静脈を結紮し、皮膚を縫合し、動物を回復させた。血液サンプルを、一晩10時間絶食させた後、ベースラインとして、並びに、処理の3、5、及び10日後に採取した。動物を、過量のペントバルビタールナトリウム(120mg/kg)を用いて10日目に屠殺した。膵臓、肝臓、脾臓、及び腎臓を、組織学、並びにウェスタンブロットによるPDX1及びベータセルリンタンパク質の評価のために採取した。血中グルコースレベルを、血中グルコース試験紙(Precision、Abbott社製)を用いて測定し、血中のインスリン、C−ペプチド、及びグルカゴンのレベルをLinco Research社製のRIAキットを用いて測定した。
腺房細胞の経時的な分化転換を評価するために設計された第2の手順において、STZ誘発性の糖尿病を有する6匹のラットを、PDX1プラスミド及びベータセルリンプラスミドの両方を含むマイクロバブルを用いてUTMDで処理した。グルコース、インスリン、C−ペプチド、及びグルカゴンについての血液サンプルを、UTMDの5、10、15、20、25、及び30日後に採取した。ラットの半分を組織学のために20日目に屠殺し(これらのラットにおいて25及び30日目の血液サンプルは採取しなかった)、残りの半分は30日目に屠殺した。
プラスミドを含む、脂質で安定化させたマイクロバブルの製造。脂質で安定化させたマイクロバブルを、本発明者らが以前に記載したように調製した(Chen S, Ding JH, Bekeredjian R, Yang BZ, Shohet RV, Johnston SA, Hohmeier HE, Newgard CB, Grayburn PA. Efficient gene delivery to pancreatic islets with ultrasonic microbubble destruction technology. Proc Natl Acad Sci U S A. 2006; 103:8469-74)。簡潔に述べると、250μlの、2.5mg/mlのDPPC(1,2−ジパルミトイル−sn−グリセロ−3−ホスファチジルコリン、Sigma社製、St. Louis、MO)及び0.5mg/mlのDPPE(1,2−ジパルミトイル−sn−グリセロ−3−ホスファチジルエタノールアミン、Sigma社製、St. Louis、MO)の溶液と、50μlの純粋なグリセロールとを、2mgの乾燥プラスミドを含む1.5mlのバイアルに加え、20μgのデキサメタゾンとよく混合し、室温で30分間インキュベートし、上部に残っている空間にペルフルオロプロパンガス(Air Products, Inc社製、Allentown、PA)を満たし、次に、歯科用混合器(Vialmix(登録商標)、Bristol-Myers Squibb Medical Imaging社製、N. Billerica、MA)によって30秒間、機械的に振動させた。マイクロバブルの平均直径及び平均密度を、粒子計数器(Beckman Coulter Multisizer III)によって測定した。

ラットのPDX1及びベータセルリンのcDNA調製物の追跡。新生ラットの膵臓サンプルを液体窒素内で急速冷凍し、−86℃で保存した。冷凍したサンプルを1mlのRNA−STAT溶液内で解凍し、Polytron 3000ホモジナイザーを用いて、10000rpmで30分間、素早く2回ホモジナイズした。RNeasy Mini Kit(QIAGEN社製)を製造者の指示に従って用いて、試料から全RNAを調製した。Sensiscript RT Kit(QIAGEN社製)をオリゴ(dT)16と共に用いて、RNA(30ng)を20μl内に逆転写した。反応混合物を42℃で50分間インキュベートし、その後さらに、70℃で15分間インキュベートした。2μlのcDNA、25μlのHotStarTaq Master Mix(QIAGEN社製)、及び20pmolの以下の各プライマーを含む50μlの容積において、GeneAmp PCR System 9700(PE ABI社製)を用いて、すべてのサンプルについてPCRを行った。

対応するPCR産物を、アガロースゲル電気泳動及びQIAquickゲル抽出キット(QIAGEN社製)によって精製した。PCR産物を、dRhodamineターミネーターサイクルシーケンシングキット(PE Applied Biosystems社製、Foster City、CA)を用いてABI3100ゲノムアナライザーで配列決定した。RIP断片をXhoI及びEcoRIで消化し、次に、pDsRed-Express-1ベクター(BD Biosciences社製)のXhoI−EcoRI部位内にライゲーションした。PDX1及びベータセルリンのcDNA断片をEcoRI及びNotIで消化し、RIP又はCMVの駆動ベクターのEcoRI及びNotI部位内にライゲーションし、ライゲーション反応は、20mMのtris−HCl、0.5mMのATP、2mMのジチオトレイトール、及び1単位のT4DNAリガーゼを含む20ml内において実施した。プラスミドのクローニング及び単離は標準的な手順によって行った。
腺房細胞の単離及びRT−PCR。腺房細胞を以前に記載されているように単離した(Guz Y, Nasir I, Teitelman G. Regeneration of pancreatic beta cells from intra-islet precursor cells in an experimental model of diabetes. Endocrinology 2001; 142: 4956-4968)。簡潔に述べると、1グラムのラット膵臓組織を、200U/mlのコラゲナーゼ、10mMのHepes、5%ウシ胎児血清、100U/mLのペニシリン、50μg/mLのストレプトマイシン、及び0.2mg/mLの大豆トリプシン阻害剤を含む20mLのRPMI-1640培地を入れた100mLのビーカー内に置いた。それをハサミで非常に小さな片に切り、滅菌したフラスコに移し、150サイクル/分の往復式の振盪をしながら37℃で40分間インキュベートした。腺房細胞の懸濁液を100μmのメッシュナイロンガーゼで濾過した。腺房細胞を、10%FBA及び4mMのストレプトゾシンを含む(残余のベータ細胞をなくした)RPMI−1640培地で、37℃、5%CO2で2時間培養した。細胞を採取し、全RNAを抽出し、cDNAプールに戻した。すべてのサンプルについて、GeneAmp PCRシステム9700(PE ABI社製)を用いて、1μlのcDNA、12.5μlのHotStarTaq Master Mix(QIAGEN社製)、及び20pmolの各プライマーを含む25μlの容積においてPCRを実施した。PCR産物を配列決定により確認した。表1にはPCRオリゴ対の例が含まれる。
免疫組織化学。厚さ5μmのクリオスタット切片を、4%パラホルムアルデヒドを用いて4℃で15分間固定し、PBS内の10mMのグリシンを用いて5分間反応を止めた。次に切片をPBS内で3回すすぎ、PBS内の0.2%Triton X-100で10分間透過化した。切片を10%ヤギ血清及び10%ウシ血清を用いて37℃で1時間ブロックし、PBSで3回洗浄した。一次抗体(Sigma社製の1:5000希釈の抗マウスインスリン、Chemicon社製の1:500希釈の抗ウサギグルカゴン、Chemicon Co社製の1:500希釈の抗ウサギpdx1及び抗ウサギベータセルリン、Abcam社製の1:500希釈の抗アルファアミラーゼ)を加え、4℃で一晩インキュベートした。5分間にわたりPBSで3回洗浄した後、二次抗体(Sigma Co.社製の1:250希釈のFITC結合抗マウスIgG、Chemicon社製の1:250希釈のCy5結合抗ウサギIgG)を加え、37℃で1時間インキュベートした。切片をPBSで10分間、5回すすぎ、その後載せた。共焦点顕微鏡を用いてFITCシグナル(488nm/510nm)及びCy5シグナル(633nm/710nm)を検出した。
データ分析。Statviewソフトウェア(SAS社製、Cary、NC)を用いてデータを分析した。結果は平均±1標準偏差で表す。差はフィッシャーのポストホック試験を用いてANOVAを反復して測定することにより分析し、p<0.05を有意であるとした。

Claims (24)
- 細胞におけるインスリンの産生を誘導する方法であって、
1又は複数の細胞をベータセルリン(BTC)及び膵臓十二指腸ホメオボックス−1(PDX1)を発現するベクターで形質転換すること
を含む方法。 - 細胞が、BTC及びPDX1を共発現する1つの構築物で形質転換される、請求項1に記載の方法。
- 細胞が、膵島細胞、膵腺房細胞、細胞系、1又は複数のインスリン遺伝子がコトランスフェクトされている細胞から選択される、請求項1に記載の方法。
- 構築物が、マイクロバブル、リン酸カルシウム−DNA共沈殿、DEAE−デキストランを用いたトランスフェクション、ポリブレンを用いたトランスフェクション、エレクトロポレーション、マイクロインジェクション、リポソーム融合、リポフェクション、プロトプラスト融合、レトロウイルス感染、及びバイオリスティックを用いて送達される、請求項1に記載の方法。
- 構築物が、マイクロバブル及び超音波標的マイクロバブル破壊を用いて送達される、請求項1に記載の方法。
- 細胞が、15日超にわたりグルコースレベルに基づいてインスリンを発現する、請求項1に記載の方法。
- 細胞が、インビボで形質転換され、15日超にわたりグルコース応答性の形でインスリンを発現する、請求項1に記載の方法。
- 細胞が、非内分泌性の膵臓細胞であり、15日超にわたりグルコース応答性の形でインスリンを発現する、請求項1に記載の方法。
- BTC、PDX1又はその両方が、マウス、ラット又はヒトから選択される、請求項1に記載の方法。
- 腺房細胞内にトランスフェクトされるとベータセルリン、PDX1又はその両方を発現する核酸発現構築物を含むベクター。
- ベクターが、超音波に晒されると破壊されるマイクロバブルと共に配置されている、請求項10に記載のベクター。
- 核酸発現構築物が、BTCの発現を制御するプロモーターを含む、請求項10に記載のベクター。
- 核酸発現構築物が、同一のプロモーターの制御下にあるBTC及びPDX1を含む、請求項10に記載のベクター。
- BTCとPDX1が別々のベクター上に存在する、請求項13に記載のベクター。
- BTCとPDX1が異なるプロモーターの制御下にある、請求項13に記載のベクター。
- プロモーターが、RIP、HIV、AMV、SV40、マウス乳腺腫瘍ウイルス(MMTV)のプロモーター、ヒト免疫不全ウイルスの長い末端反復(HIV LTR)のプロモーター、モロニーウイルスプロモーター、ALVプロモーター、サイトメガロウイルス(CMV)プロモーター、ヒトアクチンプロモーター、ヒトミオシンプロモーター、RSVプロモーター、ヒトヘモグロビンプロモーター、ヒト筋クレアチンプロモーター、及びEBVプロモーターから選択される、請求項13に記載のベクター。
- BTC、PDX1又はその両方が、マウス、ラット又はヒトから選択される、請求項10に記載のベクター。
- PDX1がGenbank受託番号NM022852であり、BTCがGenbank受託番号NM022256である、請求項10に記載のベクター。
- 構成的プロモーターの制御下にあるBTC及びPDX1を発現する外因性の核酸断片を含む宿主細胞。
- プロモーターが、RIP、HIV、AMV、SV40、マウス乳腺腫瘍ウイルス(MMTV)のプロモーター、ヒト免疫不全ウイルスの長い末端反復(HIV LTR)のプロモーター、モロニーウイルスプロモーター、ALVプロモーター、サイトメガロウイルス(CMV)プロモーター、ヒトアクチンプロモーター、ヒトミオシンプロモーター、RSVプロモーター、ヒトヘモグロビンプロモーター、ヒト筋クレアチンプロモーター、及びEBVプロモーターから選択される、請求項19に記載の宿主細胞。

- 宿主細胞が、マイクロバブル、リン酸カルシウム−DNA共沈殿、DEAE−デキストランを用いたトランスフェクション、ポリブレンを用いたトランスフェクション、エレクトロポレーション、マイクロインジェクション、リポソーム融合、リポフェクション、プロトプラスト融合、レトロウイルス感染、及びバイオリスティックにより形質転換される、請求項19に記載の宿主細胞。
- 細胞が、INS−1、INS−2、グルカゴン、ソマトスタチン、MIST−1、VMAT、ニューロゲニン−3、Nkx2.2、及びそれらの組合せから選択される膵臓ベータ細胞マーカーを発現する、請求項19に記載の宿主細胞。
