Description

The present invention relates to an endoscope hood that is attached to a distal end portion of an endoscope.

In recent years, endoscopes have been used for surgery and examination of human body cavities, particularly the large intestine. Since the large intestine has a narrow tube and a narrowed part such as a fold, a bent part, etc., in general, when inserting an endoscope into the large intestine, in order to assist the insertion of the endoscope and secure a visual field An endoscope hood that is attached to the distal end portion of an endoscope is used (see, for example, Patent Document 1).

A conventional endoscope hood 101 shown in FIG. 5A includes a cylindrical mounting portion 102 that is detachably mounted on an outer peripheral surface of a distal end portion of an endoscope E that is inserted into a body cavity, and a mounting portion. And a protrusion 103 extending forward from the tip of 102. The protrusion 103 is formed of a curved surface that bulges outward and is tapered, and has an opening 104 at the tip.

According to the endoscope hood 101, as shown in FIG. 5 (b), when the endoscope E is attached to the distal end portion of the endoscope E and inserted into the body cavity, the narrowed portion is caused by the tapered protrusion 103. Wa can be revealed and the insertion of the endoscope E can be assisted. Further, the outer peripheral surface of the protruding portion 103 abuts on the body cavity wall Wb and pushes the body cavity wall Wb, thereby ensuring a visual field.

However, as shown in FIG. 5B, the endoscope hood 101 has a protruding portion 103 when the narrowed portion Wa is present in front of the body cavity wall Wb in the region expanded by the outer peripheral surface of the protruding portion 103. May damage the body cavity wall Wb of the narrowed portion Wa, or may penetrate through the narrowed portion Wa to form a perforation in the body cavity wall Wb.

4)Japanese Patent No. 3748994 (FIGS. 3 and 4)

An object of the present invention is to provide an endoscope hood capable of assisting insertion of an endoscope without damaging a body cavity wall in order to eliminate such inconvenience.

In order to achieve the above object, the present invention provides a cylindrical mounting portion that is detachably mounted on an outer peripheral surface of a distal end portion of an endoscope that is inserted into a body cavity, and a front side from the distal end of the mounting portion. In the endoscope hood, the projection hood includes a projecting portion that is a curved surface that is extended toward the outside, bulges outward, and is tapered, and an opening that opens at the tip of the projecting portion. The protrusion protrudes with a length in the range of 20 to 60% with respect to the outer diameter of the mounting portion, and the opening is separated from the body cavity wall when the outer peripheral surface of the protrusion contacts the body cavity wall. It is characterized by being positioned.

According to the present invention, the outer peripheral surface of the protrusion is a curved surface that bulges outward. Therefore, the insertion of the endoscope can be assisted by the outer peripheral surface abutting against the body cavity wall and pushing the body cavity wall. At this time, the protruding portion only protrudes with a length in the range of 20 to 60% with respect to the outer diameter of the mounting portion, and is formed to be tapered, so that the outer peripheral surface of the protruding portion is a body cavity. When coming into contact with the wall, the opening that opens at the tip of the protrusion is positioned away from the body cavity wall. Therefore, even when the stenosis part exists in front of the body cavity wall with which the outer peripheral surface of the protruding part abuts, the opening part does not contact the stenosis part, and damage to the body cavity wall can be prevented.

At this time, if the protruding length of the protruding portion is less than 20% with respect to the outer diameter of the mounting portion, the outer peripheral surface of the protruding portion cannot be sufficiently bulged outwardly, It becomes difficult to assist the insertion of the endoscope. On the other hand, when the projecting length of the projecting portion exceeds 60% with respect to the outer diameter of the mounting portion, when the constricted portion exists in front of the body cavity wall with which the outer peripheral surface abuts, the opening portion is the constricted portion. May damage the body cavity wall.

Moreover, it is preferable that the endoscope hood of the present invention includes a groove portion that is recessed along the axial direction on the outer peripheral surface of the protruding portion and in which a water supply tube is mounted. According to the endoscope hood of the present invention, the water supply tube can be mounted in the groove portion by providing the groove portion recessed along the axial direction on the outer peripheral surface of the protruding portion. Further, the tip of the water supply tube can be accommodated in the groove, and the water supply tube can be prevented from coming into contact with the body cavity wall when the outer peripheral surface of the protrusion comes into contact with the body cavity wall.

Further, in the present invention, the water supply tube has a diameter that is reduced from the proximal end side toward the distal end side, or has an opening portion that is inclined at a predetermined angle with respect to the shaft at the distal end portion. Is preferred. According to any one of the above configurations, when the distal end portion of the water supply tube is stored in the groove portion, the distal end portion is immersed in the groove portion, and the water supply tube can be further reliably prevented from coming into contact with the body cavity wall. it can.

Next, embodiments of the present invention will be described in more detail with reference to the accompanying drawings. An endoscopic hood 1 shown in FIG. 1A includes a cylindrical mounting portion 2 that is detachably mounted on an outer peripheral surface of a distal end portion of an endoscope E that is inserted into a body cavity, and the mounting portion 2. And a projecting portion 3 extending forward from the tip. The protrusion 3 is formed of a curved surface that bulges outward and is tapered, and has an opening 4 at the tip. In this specification, “to bulge outward” means that the curved surface bulges outward from a straight line connecting the end on the mounting portion 2 side and the end on the opening 4 side. Means.

The mounting portion 2 is made of, for example, a soft resin such as polyvinyl chloride resin. As shown in FIG. 1B, a small inner diameter portion 5 to which the endoscope is detachably mounted and a protruding portion 3 are mounted. And a large inner diameter portion 6. In the present embodiment, the outer diameter D of the mounting portion 2 is, for example, 14.8 mm. The small inner diameter portion 5 includes four anti-slip portions 9 that are provided on the inner peripheral surface so as to protrude perpendicular to the axial direction. Further, the small inner diameter portion 5 includes a tapered surface portion 10 having a larger diameter on the proximal end side of the inner peripheral surface into which the distal end portion of the endoscope E is inserted.

The anti-slip portion 9 is formed such that its convex surface is slightly smaller in diameter than the outer diameter of the endoscope, and is in close contact with the mounted endoscope E to prevent its falling off.

The protruding portion 3 is made of, for example, a hard resin such as ABS resin, and is formed to protrude with a length L in the range of 20 to 60% with respect to the outer diameter D of the mounting portion 2. The protruding length L of the protruding portion 3 is, for example, 5 mm in this embodiment. As a result, when the outer peripheral surface comes into contact with the body cavity wall, the protrusion 4 is positioned so that the opening 4 is separated from the body cavity wall.

The protruding portion 3 includes a small outer diameter portion 11 on the proximal end side, and is fitted to the large inner diameter portion 6 of the mounting portion 2 by the small outer diameter portion 11. The small outer diameter portion 11 is bonded to the large inner diameter portion 6 with an adhesive formed by, for example, mixing cyclohexanone and dichloroethane at a ratio of 1: 1.

The endoscope hood 1 also includes a groove 12 that is recessed along the axial direction on the outer peripheral surface of the protrusion 3. A water supply tube 13 is attached to the groove portion 12. The water supply tube 13 is formed of a flexible member, and a terminal 14 connected to a water supply source (not shown) is provided on the proximal end side. The water supply tube 13 includes an opening 13 a that is inclined at a predetermined angle with respect to the shaft at the tip, and the tip is housed in the groove 12.

The mounting portion 2 on the proximal end side of the groove portion 12 is provided with two regulating members 15, 15 protruding along the axial direction. The restricting members 15 and 15 restrict the movement of the water supply tube 13 in the lateral direction by fitting the water supply tube 13.

Next, a method for using the endoscope hood 1 of the present embodiment will be described with reference to FIG. In the present embodiment, a case where the inside of the large intestine is observed with the endoscope E will be described. First, as shown by an arrow in FIG. 1, the endoscope hood 1 is attached to the distal end portion of the endoscope E, and the distal end surface of the endoscope E is placed between the small inner diameter portion 5 and the large inner diameter portion 6. To a position corresponding to the wall surface 16 (see FIG. 1B).

Next, the endoscope E to which the endoscope hood 1 is attached is inserted into the large intestine. The endoscope hood 1 opens the body cavity wall Wb by the outer peripheral surface bulging outward of the projecting portion 3 abutting against the body cavity wall Wb of the large intestine, thereby opening the path and opening the endoscope E. Insertion can be assisted. At this time, since the protruding portion 3 only protrudes with a length L in the range of 20 to 60% with respect to the outer diameter D of the mounting portion 2, the outer peripheral surface of the protruding portion 3 contacts the body cavity wall Wb. At this time, the opening 4 is positioned away from the body cavity wall Wb. Therefore, in the endoscope hood 1 of the present embodiment, even when the narrowed portion Wa is present in front of the body cavity wall Wb with which the outer peripheral surface of the protruding portion 3 abuts, the opening 4 can contact the narrowed portion Wa. In addition, damage to the body cavity wall Wb can be prevented.

At this time, when the protruding length L of the protruding portion 3 is less than 20% with respect to the outer diameter D of the mounting portion 2, the outer peripheral surface of the protruding portion 3 is sufficiently bulged outward. It becomes difficult to assist the insertion of the endoscope E. On the other hand, when the protruding length L of the protruding portion 3 exceeds 60% with respect to the outer diameter D of the mounting portion 2, when the narrowed portion Wa exists in front of the body cavity wall Wb with which the outer peripheral surface abuts, The opening 4 may contact the stenosis Wa and damage the body cavity wall Wb.

In addition, the endoscope hood 1 of the present embodiment includes the groove portion 12 that is recessed along the axial direction on the outer peripheral surface of the protruding portion 3, so that the water supply tube 13 can be attached to the groove portion 12. At this time, since the distal end portion of the water supply tube 13 is housed in the groove portion 12, the water supply tube 13 is prevented from coming into contact with the body cavity wall Wb when the outer peripheral surface of the protrusion 3 comes into contact with the body cavity wall Wb. be able to.

Furthermore, in the endoscope hood 1 of the present embodiment, the water supply tube 13 is provided with the opening 13a that is inclined at a predetermined angle with respect to the shaft at the distal end portion. When the distal end portion is stored, the distal end portion is immersed in the groove portion 12, and the water supply tube 13 can be further reliably prevented from coming into contact with the body cavity wall Wb.

As shown in FIG. 1, the endoscope hood 1 of the present embodiment is provided with a water supply tube 13 having an opening 13 a that is inclined at a predetermined angle with respect to an axis at the tip. However, as shown in FIG. 3, the water supply tube 13 may not be attached.

Further, although not shown in the drawings, the endoscope hood 1 of the present embodiment may be equipped with a water supply tube whose diameter decreases from the proximal end side toward the distal end side, instead of the water supply tube 13. Further, although not shown, a water supply tube having a diameter that decreases from the proximal end side toward the distal end side and that is inclined at a predetermined angle with respect to the shaft may be attached to the end portion.

Further, the endoscope hood 1 of the present embodiment can further use a guide tube 17 in addition to the water supply tube 13. The endoscope hood 1 shown in FIG. 4 includes two regulating members 18 and 18 on which the guide tube 17 is fitted on the opposite side of the groove portion 12 in which the water supply tube 13 is mounted. The guide tube 17 includes a terminal 19 on the proximal end side, and an opening portion 17a that is inclined at a predetermined angle with respect to the shaft at the distal end portion.

The endoscope hood 1 including the guide tube 17 is provided with the guide tube 17 so that medical instruments such as forceps, a local injection needle, a snare, a high-frequency knife, and a clip are inserted into the guide tube 17. Various procedures can be performed. Further, since the guide tube 17 includes the opening 17a that is inclined at a predetermined angle with respect to the axis at the distal end portion, the guide tube 17 can be prevented from contacting the body cavity wall Wb.
