Description

The present invention relates to a mounting aid, a balloon mounting kit for an endoscope, and an endoscope system used when mounting a balloon made of an elastic material on a distal end portion of an insertion portion of an endoscope.

Examples of the endoscope used with the balloon attached include an ultrasonic endoscope, a balloon endoscope, a cell sheet conveying endoscope, and the like.
For example, when attaching a balloon made of an elastic material to a distal end portion having an ultrasonic probe of an insertion portion of an ultrasonic endoscope, conventionally, a hand or a mounting aid (mounting jig) disclosed in Patent Document 1 is used. Then, the balloon was expanded, and the expanded balloon was put on the tip (ultrasonic probe).

Endoscopes in recent years have a tendency to reduce the diameter of the insertion portion in order to reduce the burden on the patient, and in particular, endoscopes for trachea are remarkably reduced in diameter. Therefore, the diameter of balloons attached to these endoscopes has been reduced.
However, since it is not easy to spread a small-diameter balloon using a hand or a conventional attachment aid, it is becoming difficult to attach the small-diameter balloon to the distal end portion of the small-diameter insertion portion. In particular, in the case of an endoscope using a balloon, it is necessary to fit the opening end of the balloon in a mounting groove (annular groove) for preventing balloon drop formed at the distal end portion of the insertion portion, which is particularly difficult to mount.

The present invention relates to an endoscopic balloon mounting aid, an ultrasonic endoscopic balloon mounting kit, and an endoscope, in which a small-diameter balloon made of an elastic material can be easily mounted on the distal end portion of the endoscope. A connecting portion that is connected in a state where the sides are spaced apart by a predetermined distance, located on the other end side of the pair of opening and closing pieces, separated from each other when the opening and closing pieces and the connecting portion are in a free state, and the opening and closing At least one of the piece and the connecting portion approaches each other by elastic deformation, a tip insertion portion that can be inserted into and removed from the opening of the balloon, continuous to the tip insertion portion, and the opening and closing piece and the connection portion are free An endoscope passing portion that is spaced apart from each other when in the state, and extends from the endoscope passing portion in a direction opposite to the distal end insertion portion so as to be inclined with respect to the extending direction of the distal end insertion portion, and to the connection portion It has an operation part to be connected.

The pair of opening / closing pieces preferably have a plate shape that is substantially parallel to each other, and the width of the distal end insertion portion is preferably larger than the dimension in the direction perpendicular to the width direction of the distal end portion of the endoscope.

When a pair of distal end insertion portions are inserted into the balloon at the proximal end portion of the distal end insertion portion, a mouthpiece portion that is thicker than the other portion formed at the peripheral portion of the opening portion of the balloon is engaged. It is preferable to form an engaging groove.

A pair of the opening and closing pieces is formed in a plate shape that is substantially parallel to each other, the endoscope passage part is wider than the distal end insertion part, and a distal end surface that is continuous with the distal end insertion part in the endoscope passage part is defined as the distal end It is preferable to use a stopper surface that restricts the movement of the balloon toward the endoscope passing portion side opposite to the opening of the balloon when the insertion portion is inserted into the balloon.

A mounting aid for mounting the balloon on the tip, and the pair of opening / closing pieces, when the opening / closing pieces and the connecting portion are in a free state, the opening of the balloon in a free state A distal end insertion portion that is spaced apart from each other at a wider interval, and that approaches the distal end insertion portion until the opening and closing piece and at least one of the connection portions are elastically deformed to be inserted into the opening portion of the balloon. An endoscope passage portion that is continuous and spaced apart from each other when the opening and closing piece and the connection portion are in a free state, and an extension direction of the distal end insertion portion in a direction opposite to the distal end insertion portion from the endoscope passage portion It extends to be inclined with respect to And it is characterized by having an operation unit, to be connected to the connection portion.

When an external force other than the elastic force of the balloon is removed from the mounting aid with the pair of tip insertion portions inserted into the balloon, the distance between the pair of opening and closing pieces is parallel or toward the tip insertion portion side. It is preferable to set the elasticity of the mounting aid and the balloon so that the distance between the pair of opening and closing pieces increases as it goes.

An ultrasonic endoscope system according to the present invention includes an endoscope, a balloon made of an elastic material that is detachably placed on the distal end portion of the endoscope, a pair of opening and closing pieces, and a connecting portion that connects the pair of opening and closing pieces. A mounting aid for mounting the balloon to the tip portion, and the pair of opening and closing pieces is in a free state when the opening and closing pieces and the connecting portion are in a free state. Until the opening of the balloon and the distal end of the endoscope are spaced apart from each other at a wider interval, and at least one of the opening / closing piece and the connecting portion is elastically deformed to be inserted into the opening of the balloon. An approaching distal end insertion portion, an endoscope passing portion that is continuous with the distal end insertion portion and that is spaced apart from the distal end portion by a wider interval when the opening / closing piece and the connection portion are in a free state, and the endoscope Direction opposite to the tip insertion part from the passing part It extends so as to be inclined with respect to the extending direction of the tip insertion portion, and is characterized by having an operation unit, to be connected to the connection portion.

According to the first, sixth, and eighth aspects of the present invention, the pair of distal end insertion portions approach each other by elastically deforming at least one of the pair of opening / closing pieces and the connection portion of the mounting aid. When the mounting aid is returned to the free state after being inserted into the opening, the pair of tip insertion portions are separated (opened) to elastically expand the balloon opening, and the distance between the pair of insertion tips And the space | interval of a pair of endoscope passage part spreads. Therefore, if the distal end portion of the insertion portion of the endoscope is inserted between the pair of endoscope passage portions from the extending direction of the distal end insertion portion, the distal end portion of the insertion portion does not come into contact with the pair of operation portions. It can be inserted into the opening. Then, if the pair of tip insertion portions of the mounting aid is moved in the direction of escape from the inside of the balloon, the pair of tip insertion portions can be moved from the inside of the balloon without contacting the mounting aid and the insertion portion of the endoscope. Pulled out. Then, since the balloon is elastically restored and automatically attached to the distal end portion of the endoscope, the small-diameter balloon can be easily attached to the distal end portion of the small-diameter insertion portion.
Moreover, since the operator can perform the operation of inserting the distal end portion of the endoscope into the balloon and the operation of extracting the distal end insertion portion from the balloon while the operator removes the force from the wearing aid, the burden on the operator can be reduced. Is possible.

According to the second aspect of the present invention, when the mounting aid is in a free state with the pair of tip insertion portions inserted into the balloon, the width dimension and width of the opening of the balloon expanded by the pair of tip insertion portions Since the dimension in the direction orthogonal to the endoscope becomes larger than the distal end portion of the insertion portion of the endoscope, it becomes easier to insert the distal end portion of the endoscope into the balloon.

If comprised like Claim 3, it will become possible to hold | maintain the mounting | wearing state of the balloon with respect to a pair of front-end | tip insertion part reliably by engaging the mouth wound part of a balloon with the engagement groove | channel of a pair of front-end | tip insertion part. . Therefore, it is easier to attach the balloon to the distal end portion of the endoscope.

According to the fourth aspect of the present invention, since the balloon attached to the pair of distal end insertion portions can be reliably prevented from moving toward the endoscope passage portion side, the operation of attaching the balloon to the distal end portion of the endoscope is further improved. It becomes easy.

If it comprises like Claim 5, the connection part which connects the base end part of a pair of opening-and-closing piece becomes wider than the space | interval of a pair of opening-and-closing piece. Therefore, it becomes easy to grasp the mounting aid by hand and to prevent the mounting aid from falling out of the operator's hand.

If comprised like Claim 7, when the opening part of a balloon is expanded by the elastic force of the opening direction of a pair of tip insertion part (opening-closing piece), as a space | interval of a pair of opening-and-closing pieces goes to the tip insertion part side. Compared to the case of narrowing, it becomes easier to pass the distal end portion of the endoscope between the pair of distal end insertion portions and the endoscope passage portion.

Hereinafter, an embodiment of the present invention will be described with reference to FIGS. In the following description, the front-rear direction defines the distal end side of the insertion portion 12 of the ultrasonic endoscope 10 as “front” and the proximal end side of the insertion portion 12 as “rearward”.
An ultrasonic endoscope 10 shown in FIG. 1 is a medical endoscope for trachea, and includes an operation unit 11, an insertion unit 12 extending forward from the operation unit 11, and both the operation unit 11 and the insertion unit 12 on the opposite side. An extending universal tube 13 and an ultrasonic image transmission tube 14 are provided. The ultrasonic image transmission tube 14 is connected to an ultrasonic diagnostic apparatus (not shown), and the universal tube 13 is connected to a processor (image processing apparatus / light source apparatus, not shown). The processor and the processor are both connected to a monitor (not shown).
The portion that forms the vicinity of the distal end portion of the insertion portion 12 is a bending portion 17 that bends in the vertical and horizontal directions in accordance with the rotation operation of the bending operation lever 15 provided in the operation portion 11. Is a flexible tube portion 18 that is bent by its own weight or a direct operation of an operator.
As shown in FIGS. 1 and 2, the distal end portion of the insertion portion 12 with respect to the bending portion 17 is a distal end portion 19 made of hard resin. An inclined surface 20 is formed in the latter half of the tip 19, and an objective lens 21, an illumination lens 22, and the like are provided on the inclined surface 20. An ultrasonic probe 23 located immediately before the inclined surface 20 is formed in the front half of the tip 19. Further, a rear annular groove 24 is formed over the entire circumference at a portion located between the inclined surface 20 of the tip 19 and the ultrasonic probe 23. Further, a cylindrical tip protrusion 25 protrudes from the tip of the tip 19, and a front annular groove 26 is recessed at the base end of the tip protrusion 25.

The ultrasonic endoscope 10 is used by covering the front half (the portion where the ultrasonic probe 23 is formed) of the distal end portion 19 with a balloon B1 made of an elastic material (for example, silicon rubber) shown in FIGS. To do. Specifically, the mouth wound part B1c and the mouth wound part B1d (annular part thicker than the other part formed by rolling up the peripheral part) are formed at the peripheral parts of the opening parts B1a and B1b before and after the balloon B1. The rear annular groove 24 and the front annular groove 26 are respectively elastically fitted (closely attached), and the space formed between the front half of the tip 19 and the balloon B1 is filled with water for use.
Since this balloon B1 is smaller than the front half of the distal end portion 19 in a free state, the balloon B1 is covered with the distal end portion 19 while being elastically deformed so that the mouth wound portion B1c and the mouth wound portion B1d are enlarged. However, since the diameter of the distal end portion 19 of the ultrasonic endoscope 10 which is an endoscope for trachea is extremely small (for example, about 7 mm), the balloon B1 designed in accordance with the ultrasonic endoscope 10 is in a free state. The diameter is also extremely small (for example, about 1 mm to 3 mm in inner diameter). Therefore, it is difficult for the surgeon to cover the front half of the distal end portion 19 with the balloon B1 while expanding the mouth winding portion B1c and the mouth winding portion B1d by hand.
Therefore, in this embodiment, the balloon B1 is attached to the front half of the distal end portion 19 by using the attachment aid 30 described below.

As shown in FIGS. 2, 3, 4, and the like, the mounting aid 30 is formed by bending a single piece punched from a metal plate, and includes a pair of plate-shaped opening / closing pieces 31 that are substantially parallel to each other, and an opening / closing piece. And a connection portion 39 having a substantially C-shape in a plan view for connecting the base ends of 31.
The opening / closing piece 31 includes an operation portion 32 that extends linearly from both ends of the connection portion 39, an endoscope passage portion 33 that extends from the tip of the operation portion 32 in a direction inclined with respect to the opening / closing piece 31, and an endoscope passage A distal end insertion portion 34 that extends in the same direction as the endoscope passage portion 33 from the distal end of the portion 33 and has a narrower width than the endoscope passage portion 33 whose front half is tapered. The distal end surface of the endoscope passage portion 33 is a pair of stopper surfaces 35 for preventing excessive insertion of the balloon B1 and guiding the balloon B1 to an appropriate mounting position with respect to the distal end portion 19 of the ultrasonic endoscope 10. In addition, a pair of engaging grooves 36 that are respectively continuous with the corresponding stopper surface 35 are recessed in the proximal end portion of the distal end insertion portion 34 (portion continuing to the endoscope passage portion 33). From the intermediate portion of the distal end insertion portion 34 toward the distal end, the taper is tapered again.
As shown in the drawing, the maximum width of the connecting portion 39 is larger than the interval between the base end portions of the two open / close pieces 31. Further, the entire surface of the wearing aid 30 is coated with a fluororesin.
In the mounting aid 30 having such a structure, the pair of opening / closing pieces 31 are separated from each other in the free state (the state shown in FIGS. 6 and 9), and the pair of endoscope passage portions 33 and the pair of distal end insertion portions 34 are mutually separated. Is wider than the width of the distal end portion 19 of the ultrasonic endoscope 10. Furthermore, as shown in FIG. 9, the distance between the pair of distal end insertion portions 34 at this time is wider than the distance between the base end portions of the operation portion 32 and wider than the diameter of the balloon B1 in the free state. When the pair of operation parts 32 are held with fingers and pressed in a direction approaching each other (elastically deformed), the pair of opening / closing pieces 31 has its tip insertion part 34 as shown in FIG. It is possible to approach to a position where they come into contact with each other. Even if the mounting aid 30 is elastically deformed to a position where the pair of distal end insertion portions 34 come into contact with each other, the base end portions of the opening / closing piece 31 are hardly brought close to each other, and substantially at a predetermined interval (of the ultrasonic endoscope 10). The distance is slightly larger than the outer diameter of the tip 19 (for example, about 1.5 cm).

Next, a procedure for mounting the balloon B1 on the front half of the distal end portion 19 of the ultrasonic endoscope 10 using the mounting aid 30 having such a structure will be described.
First, the operator's hand (not shown) grips the pair of operation portions 32 of the mounting aid 30 and elastically deforms the pair of opening / closing pieces 31 in a direction approaching each other (see FIG. 3). Then, since a pair of front-end | tip insertion parts 34 contact, as shown in FIG. 3, the mounting assistance tool 30 is made to approach the balloon B1 in a free state. Further, as shown in FIG. 4, the pair of distal end insertion portions 34 is inserted into the balloon B1 in a free state from the mouth-wrapping portion B1c, and the mouth-wrapping portion B1c is brought into contact with the stopper surface 35. Since the width (height in FIG. 10) of the distal end insertion portion 34 is slightly larger than the opening B1a of the balloon B1 in the free state, when the pair of distal end insertion portions 34 are inserted into the balloon B1 from the opening B1a, FIG. As shown, the balloon B1 narrows in the width direction in plan view and extends in the height direction (a direction perpendicular to the paper surface in FIG. 4). Further, the balloon B1 is elastically deformed in this manner, so that the wound portion B1c is engaged with the engagement groove 36 of the distal end insertion portion 34.

When the pair of distal end insertion portions 34 are thus inserted into the balloon B1, the gripping force applied to the pair of operation portions 32 is released. Then, the pair of opening and closing pieces 31 are substantially parallel as shown in FIG. 5 by the elastic force of the balloon B1 and the mounting aid 30 (in FIG. 5, the distance between the pair of distal end insertion portions 34 is the distance between the proximal ends of the opening and closing pieces 31). The pair of opening and closing pieces 31 opens so that the elasticity of the balloon B1 and the mounting aid 30 may be set so that the pair of opening and closing pieces 31 are parallel to each other. The insertion portion 34 expands the balloon B1 in the width direction of the wearing aid 30. As shown in FIG. 5, in a state where the balloon B1 is mounted on the mounting aid 30, the clearance between the pair of opening and closing pieces 31 is an ultrasonic endoscope over the entire range from the pair of distal end insertion portions 34 to the proximal end portion. 10 is ensured to be slightly larger than the outer diameter of the distal end portion 19, so that the distal end portion 19 of the ultrasonic endoscope 10 can be easily passed. If the distance between the pair of distal end insertion portions 34 is too larger than the distance on the proximal end side of the opening / closing piece 31, the distance from the distal end portion 19 of the ultrasonic endoscope 10 also becomes larger, and the attachment of the balloon B1 and the distal end insertion portion are increased. It becomes difficult to pull out 34.
Next, as shown in FIGS. 6 and 7, the bending portion 17 of the ultrasonic endoscope 10 is set in a substantially linear state, and the distal end portion 19 of the ultrasonic endoscope 10 is paired with a pair of endoscope passing portions 33. Positioned immediately after. Further, at this time, as shown in FIG. 7, the distal end portion 19 and the bending portion 17 are positioned on an extension line (extension direction) L of the distal end insertion portion 34 in a side view. When the mounting aid 30 is in a free state, the distance between the pair of endoscope passage portions 33 is wider than the distal end portion 19 of the ultrasonic endoscope 10 (see FIGS. 6, 8, and 9). When 19 (the ultrasonic endoscope 10) is linearly moved forward along the extension line L from the position of the solid line in FIG. 7, the distal end portion 19 passes between the pair of endoscope passage portions 33. Since the operation unit 32 extends in a direction inclined with respect to the extension line L direction, even if the distal end portion 19 of the ultrasonic endoscope 10 is passed between the pair of endoscope passage portions 33 in this way, the connection is possible. The portion 39 and the operation portion 32 do not interfere with the insertion portion 12 (the tip portion 19 and the bending portion 17).
As shown in FIGS. 7 and 8, the width and height of the opening B1a (mouth wound portion B1c) of the balloon B1 expanded by the pair of distal end insertion portions 34 is the front half of the distal end portion 19 (the ultrasonic probe 23). Therefore, when the distal end portion 19 of the ultrasonic endoscope 10 is further linearly moved forward, the distal end portion 19 enters the balloon B1 from the wound portion B1c, and the distal end portion 19 The protrusion 25 protrudes forward of the balloon B1 through the mouth wound part B1d (see the phantom line in FIG. 7).
Then, the wound part B1c of the balloon B1 is removed from the engagement groove 36 from this state, and the pair of distal end insertion parts 34 are moved backward from the inside of the balloon B1 as shown in FIGS. If B1c is elastically fitted into the rear annular groove 24 of the ultrasonic endoscope 10, the mounting aid 30 comes into contact with the insertion portion 12 (the distal end portion 19 and the bending portion 17) of the ultrasonic endoscope 10. It can be withdrawn from the balloon B1 without making it. Further, when the pair of distal end insertion portions 34 are pulled out from the inside of the balloon B1 in this way, the balloon B1 is automatically brought into close contact with the front half of the distal end portion 19 while being contracted by its own elasticity, and the wound portion B1d is The front annular groove 26 is automatically fitted.

As described above, the small-diameter balloon B1 can be easily mounted on the small-diameter distal end portion 19 of the ultrasonic endoscope 10 by using the mounting aid 30 of the present embodiment.
In particular, when the ultrasonic endoscope 10 is passed between the pair of endoscope passage portions 33 and the pair of distal end insertion portions 34, the insertion portion 12 does not interfere with the operation portion 32 and the connection portion 39. The distal end portion 19 of the endoscope 10 and the distal end insertion portion 34 to which the balloon B1 is attached can be easily visually recognized at the same time, and the insertion operation of the distal end portion 19 into the balloon B1 can be performed smoothly. Further, when the distal end insertion portion 34 is pulled out from the balloon B1, the mounting aid 30 and the insertion portion 12 do not interfere with each other, so that the distal end insertion portion 34 can be pulled out from the balloon B1 smoothly.
Furthermore, the operator removes the force from the attachment assisting tool 30 during the insertion operation of the distal end portion 19 of the ultrasonic endoscope 10 into the balloon B1 and the extraction operation of the distal end insertion portion 34 from the balloon B1 (attachment assistance). This is possible in a state where the tool 30 is not held and elastically deformed), so that the burden on the operator can be reduced.

Further, when the pair of distal end insertion portions 34 are inserted into the balloon B1, the stopper surface 35 restricts the movement of the balloon B1 toward the endoscope passage portion 33. The balloon B1 can be prevented from covering the endoscope passage part 33 when inserted into B1.
In addition, since the mounting aid 30 of the present embodiment is obtained by punching a metal plate and molding the entire surface, the entire surface of the mounting aid 30 is smooth. Therefore, it is possible to smoothly insert and remove the distal end insertion portion 34 from the balloon B1, and the surface (inner surface) of the balloon B1 is not damaged during the insertion and removal of the balloon B1. In order to smooth the entire surface of the wearing aid 30, the surface of the wearing aid 30 may be polished instead of applying the fluororesin coat, but the fluorine tree coat is superior in terms of surface protection. .

As mentioned above, although this invention was demonstrated based on the said embodiment, this invention is not limited to the said embodiment, It can implement, giving various deformation | transformation.
For example, the balloon that can be mounted by the mounting aid 30 is not limited to the above. For example, as shown in FIG. 11, the balloon B2 having only one opening B2a and the mouth wound part B2b (no opening corresponding to the mouth wound part B1d) is attached to the distal end part 19 of the ultrasonic endoscope 40. Of course, it can also be used.
In the above-described embodiment, the bending portion 17 is substantially linear when the distal end portion 19 of the ultrasonic endoscope 10 is inserted into the balloon B1 and when the distal end insertion portion 34 is pulled out from the balloon B1. This operation may be performed in a state where the bending portion 17 is bent in a direction that does not interfere with the operation portion 32.
Furthermore, FIG. 12 shows a modification of the wearing aid. As shown in the figure, the mounting aid 50 includes a base portion 51 that extends substantially linearly from the base end portion of the distal end insertion portion 34 to the connection portion 39 and a connection portion 39 (shown) that connects the base end portions of the pair of base portions 51 to each other. Abbreviation). The base 51 is a part corresponding to the operation part 32 and the endoscope passage part 33 of the wearing aid 30, and the part located on the tip side of the two-dot chain line written in FIG. 12 corresponds to the endoscope passage part. The part located on the base end side from the alternate long and short dash line corresponds to the operation unit.

Further, only the connection part 39 of the wearing aid 30 is elastically deformed to bring the pair of opening / closing pieces 31 closer to each other, or the operation part 32 and the connection part 39 are simultaneously elastically deformed to bring the pair of opening / closing pieces 31 closer to each other. Also good.
Furthermore, although the said embodiment is a thing when inserting a balloon in the front-end | tip part of the ultrasonic endoscope 10, this invention is a system which mounts | wears with a balloon like a balloon endoscope or a cell delivery system. Is also applicable.
