Description
核酸増幅方法およびその方法を行うための組成物およびプロセスを提供する。
メッセンジャーRNA(mRNA)種の発現レベルを、多数の方法により定量することができる。伝統的な方法には、ノザンブロット解析が含まれる。転写物の定量を容易にする優れた核酸検出方法が開発された。これらの方法には、例えば周知の定量的逆転写ポリメラーゼ連鎖反応(QRT-PCR)法等の多数の核酸増幅反応の一つによる、mRNA種の増幅が関与する。有用なPCR法の事例は、参考文献としてその全体を本明細書中に援用する米国特許出願No. 10/090,326(US 10/090,326、US特許公開No. 20030017482)中に記載される。所定のmRNAの発現レベルを調べるためのその他の核酸増幅方法には、等温性増幅アッセイまたは検出アッセイおよびアレイ技術が含まれる。

変性工程およびアニーリング工程が関与するPCR反応やRT-PCR反応などの多重化核酸増幅反応をバランス化する際に有用な方法および組成物を提供する。この方法は、アンプリコンやプライマーの異なるTmを利用して、より好ましい増幅を消去し(quench)、それにより、それほど好ましくない反応を、リンパ節中の微小転移を含む乳癌細胞および肺癌細胞を同定する反応資源と競合することなく、またはわずかな競合を伴って進行させることができる。初期の転移検出は、典型的には、生存している患者に関連する。非常に小さな転移は、リンパ節バイオプシーの組織学的研究においてはしばしば検出されず、患者の生存の可能性の低下を引き起こす偽陰性の結果を生じる。本明細書中において記載する核酸検出アッセイは、ほとんどの事例における組織学的研究における識別性(わずかな優秀な組織学者はリンパ節切片中の微小転移を同定することができる)よりも、かなり高い識別力があり、そして最小限しかトレーニングを積んでいないテクニシャンの手にも、強固でありそして反復可能である。本明細書中に記載された方法および組成物は、必然的に特異的mRNAマーカーの発現を含むものが提示されるが、特異的マーカーおよびその他の当該技術分野において既知のマーカーの組合せを含む方法および組成物を排除するものとみなされるべきではないことは、理解されるべきである。この目的のため、限定的ではない一態様において、乳癌において発現される多数の分子マーカーが同定される。これらのマーカーは、特定の癌型を生じ、そして典型的にはリンパ組織中で少なくとも同一レベルまでは発現されていない、組織特異的なマーカーである。前哨リンパ節組織における1またはそれ以上のこれらのマーカーの発現の存在および/または上昇は、癌の診断と強力に相関するリンパ系組織中の転移細胞の指標である。

以下の限定的ではない事例において説明されるように、2種の標的遺伝子(TACSTD1およびPIP)を、前哨リンパ節における乳癌検出のためのマーカーとして選択し、そして1つのハウスキーピング遺伝子(β-GUS)を、内在性対照として選択した。これらの3つの遺伝子が多重化アッセイにおいて検出されるアッセイを開発した。オプションとして、合成内部陽性対照の増幅のための第4の反応を計画する。これが必要な場合、多重化反応は、三重化反応よりも複合的である。多重化PCR反応において、1つの遺伝子シグナルが他のものよりも4サイクル以上早く検出される場合、第二の反応の効率を大幅に低下させる。偽陰性の結果を回避するため、上述したように、2つの“消去”ストラテジーを、このアッセイにおいて使用するために利用した。両ストラテジー共に、異なるデザインのプライマー対、および装置(例えば、GeneXpert(登録商標)またはSmartCycler装置)ソフトウェア内での“次の段階へ進む(Advance to Next Stage;ANS)”の能力に依存している。2つの消去ストラテジーを組み合わせるため、“分岐した(Bifurcated)ANS”アプローチが以下に記載されるように必要とされる可能性がある。
(l)いずれかのプロトコルにおけるいずれかのサイクリング段階のモニタリング;
(2)1つの段階における複数のリポーターのモニタリング。一態様において、異なるリポーターが閾値を越えた場合、同一段階または異なる段階のいずれかに進むことができる;
(3)閾値を越えた後、新たな段階に進む前に、追加的な数のサイクルを行うこと(各リポーターについて設定することができる);
(4)1またはそれ以上のモニタリングしたリポーター、あるいはすべてのモニタリングしたリポーターが、閾値を越える場合および/または閾値を越えることができない場合、モニタリング段階の最後でプロトコルを終了させること;そして
(5)新たな段階の最後にプロトコルを終了させること。

潜在的なマーカーの同定
広範囲に及ぶ文献および公共データベースのサーベイを行って、いずれかの潜在的マーカーを同定した。このサーベイのための資源には、PubMed、OMIM、UniGene(http://www.ncbi.nlm.nih.gov/)、GeneCards(http://bioinfo.weizmann.ac.il/cards)、およびCGAP(http://cgap.nci.nih.gov)が含まれた。サーベイ分類は、いくらか柔軟であったが、しかしながら目的は、腫瘍中で中程度〜高度に発現しているが正常リンパ節では低度にしか発現していない遺伝子を同定することであった。さらに、腫瘍中で亢進制御されていることが報告された遺伝子および限定的な組織分布を有する遺伝子は、潜在的に有用であるとみなした。最終的に、癌精巣抗原およびhTERTなどの癌特異的であると報告された遺伝子を評価した。

スクリーニングは、2相でおこなった。すべての潜在的マーカーを、一次スクリーニング相に導入し、そして発現を、癌を有さない患者から取得した6個の原発腫瘍および10個の良性リンパ節(1プール当たり2個のリンパ節RNAを有する5個のRNAプール)において解析した。リンパ節転移検出に関して良好な特徴を示したマーカーを、二次スクリーニング相に回した。二次スクリーニングは、20〜25個の原発腫瘍、20〜25個の組織学的に陽性のリンパ節、および21個の癌を有さない良性リンパ節についての発現解析から構成された。
RNAを、RNeasy minikit(Qiagen, Valencia, CA)を、本質的には製造者により記載されたように使用して単離した。唯一の修飾は、溶解試薬の容量を二倍にし、そして2工程でカラムにかけた点である。おそらくは、組織切片中のOCTを完全に希釈する結果として、このことにより、より良好なRNA収率および純度が得られることが見いだされた。100μLの反応容量中で、表C中に示されるプローブを使用したランダムヘキサマープライミングまたは配列特異的プライミングのいずれかを使用して、そしてSuperscript II(Invitrogen, Carlsbad, CA)逆転写酵素を使用して、逆転写を行った。一次スクリーニングのため、それぞれ500 ngのRNAを含む3つの逆転写反応を行った。cDNAを組合せ、そして反応当たり20 ng RNAの等量を使用してQPCRを行った。二次スクリーニングのため、原発腫瘍および陽性リンパ節用のRNA入力は、共に500 ngであった。しかしながら、良性リンパ節のため、RNA入力は2000 ngであり、結果としてQPCR反応当たり80 ng等量のRNAが得られた。
すべての定量的PCRを、ABI Prism7700配列検出装置(Applied Biosystems, Foster City, CA)を用いて行った。マーカー遺伝子の相対的配列を、上述したΔ-CT法を使用して、そしてβ-グルクロニダーゼを内在性対照遺伝子として使用して、計算した。すべてのアッセイを、5'ヌクレアーゼハイブリダイゼーションプローブと共に使用するために設計したが、費用を削減するため、一次スクリーニングをSYBER Green定量を使用して行った。ABI Primer Express Version 2.0ソフトウェアを使用してアッセイを設計し、そして可能な場合には、cDNAの特異性をもたらすため、アンプリコンがエクソンジャンクションをまたぐように設計した。すべてのプライマー対を、60、62、および64℃のアニーリング温度で、増幅特異性について試験した(ゲル上に1本のバンドを生成)。さらに、PCR効率性を、SYBER green定量を使用して評価し、その後一次スクリーニングにおいて使用した。二次スクリーニングにおいて使用されるすべてのアッセイ用の5'ヌクレアーゼプローブを用いて、効率のさらなる最適化およびより精密な評価を行った。
SYBR Green I-ベースのQPCRについて、各50μLの反応物は、1×TaqManバッファーA(Applied Biosystems)、300nMの各dNTP、3.5 mMのMgCl2、0.06 units/μLのAmplitaq Gold(Applied Biosystems)、0.25×SYBR Green I(Molecular Probes, Eugene, OR) および200 nMの各プライマーを含有した。増幅プログラムは、最初に95℃のTaq活性化段階を12分間行った後に、95℃の変性を15秒、60、62または64℃のアニール/伸長を60秒間および増幅される特異的PCR生成物のTmよりも2〜4℃低い温度で10秒間のデータ回収工程を1サイクルとして、40サイクル行う、2段階を含む(TB Morrison, et al. 1998)。増幅の後、60℃〜95℃の温度まで20分間かけて上昇させつつ、蛍光データを回収することにより、融解曲線解析を行った。
プローブベースのQPCRを、以前に記載されたように行った(Godfrey, et al., Clin. Cancer Res. 2001 Dec., 7(12):4041-8)。簡単に説明すると、200 nMのプローブ濃度および60℃、62℃、または64℃で60秒のアニーリング/伸長相、により、反応を行った。二次スクリーニングにおいて評価した遺伝子用のプライマーおよびプローブ(IDT, Coralville, IAから購入)の配列を、以下の表Bに列挙した。
一次スクリーニングにおいて、溶解曲線から得たデータを、ABI Prism 7700 Dissociation Curve Analysis 1.0ソフトウェア(Applied Biosystems)を使用して解析した。溶解曲線の一次導関数を使用して、生成物のTmを決定し、ならびに各サンプル中の特異的生成物の存在を確認した。一般的には、サンプルを、二重にしたPCR反応中で解析し、そして平均Ct値を発現解析において使用した。しかしながら、二次スクリーニングにおいては、各個別の良性リンパ節について三重にした反応を行い、そしてサンプルについてのバックグラウンド発現の最高値を取得するため、最低のCt値を相対発現の計算に使用した。
CK7、CK19、MGB1、MGB2、PIP、およびTACSTD1の発現レベルを、実施例1に記載した方法により決定した。図7は、原発腫瘍、腫瘍陽性リンパ節および良性リンパ節におけるCK7、CK19、MGB1、MGB2、PIP、およびTACSTD1の発現レベルを示す散布図である。図8A-Oは、2マーカーシステムが、リンパ節中の良性細胞と悪性細胞とを識別する能力を示す散布図を提供する。表DおよびEは、図7および図8A-Oのグラフを生成した際の生データを提供する。このデータは、前哨リンパ節中でのCK7、CK19、MGB1、MGB2、PIPおよびTACSTD1マーカー単独の発現または組合せの発現が、前哨リンパ節中の乳癌に由来する悪性細胞の存在と強力な相関を有することを示す。
材料と方法
実施例2において概説したように、広範囲な文献サーベイおよびデータベースサーベイにより、乳癌におけるリンパ節転移の検出のための潜在的なmRNAマーカーを同定した。一次スクリーニングにより、6個の原発性乳房腫瘍および癌を有さない患者から得た10個の良性リンパ節において、43個の潜在的なマーカーの相対発現を解析した。6個のマーカーが、リンパ節転移検出についての良好な特徴を示したため、25個の原発腫瘍、27個の組織学的に陽性のリンパ節、そして癌を有さない患者から得た21個の良性リンパ節(73人の別個の患者)において発現を解析する、二次スクリーニング段階へと進行した。分類特性に基づいて、迅速な多重化リアルタイムPCRアッセイを使用して、乳癌を有する別個の患者に由来する90個のSLNの外部評価研究のために、4個のマーカーを選択した。最終的に、9個の組織学的に陰性のリンパ節および9個の組織学的に陽性なリンパ節を、完全に自動化しそして迅速なRNA単離およびリアルタイムPCRアッセイを使用して、GeneXpert(登録商標)上で解析した。
マーカースクリーニングおよびGeneXpert(登録商標)研究のための組織は、University of Pittsburgh Medical Centerの組織バンクから取得し、そしてマーカー検証実験のためのSLNは、Medical University of South Carolinaで始められたMinimally Invasive Molecular Staging of Breast Cancer Trial (MIMS)から得た。
すべての組織を、液体窒素中で瞬間凍結し、そして-80℃で使用するまで保存し、使用時に、クリオスタットでの凍結切片作成のため、それらを最適切断温度(OCT)コンパウンド中に包埋した。マーカースクリーニングおよびGeneXpert(登録商標)研究のため、40個の5-ミクロンの切片をRNA単離のために切り出した。RNA単離用の切片の最初、中間、そして最後から得た追加の切片を、H&E解析およびIHC解析のために切り出した。各試料から得た3つのH&Eスライドすべてについて、2人の病理学者により、病理学的検討を行った。すべての非染色スライドを、-20℃で保存し、そしてH&E組織構造を確認するために必要である場合と同様に、IHC評価のために使用した(AE1/AE3汎サイトケラチン抗体カクテルを使用した)。
スクリーニングおよび検証実験のため、製造者により記載された様にRNeasyミニキット(Qiagen, Valencia, CA)を使用して、RNAを単離した。唯一の変更点は、溶解試薬の容量を二倍にし、そして2工程でカラム上に充填した点であった。すべてのRNAは、Ambionから入手したDNA不含キットを使用して、DNAse処理された。
マーカースクリーニング研究のため、cDNAをランダムヘキサマーを使用して合成した。定量的リアルタイムPCRを、ABI Prism 7700 Sequence検出装置上で行い、各マーカー遺伝子の発現をΔCt計算を使用して、内在性対照遺伝子β-グルクロニダーゼに対して測定した。コストを抑えるため、一次スクリーニングをSYBR greenを用いた定量を使用して行った。二次スクリーニングにおいて、5’ヌクレアーゼハイブリダイゼーションプローブを使用して、アッセイの特異性を上昇させた。すべてのアッセイを、ABIPrimer Express Version 2.0ソフトウェアを使用して設計し、そして可能である場合、cDNA特異性を提供するため、アンプリコンはエクソン結合部にかかっていた。陰性対照を、各PCRプレート中でインキュベートした。Universal Human Reference RNA(Stratagene, La Jolla, CA)とヒト胎盤、甲状腺、心臓、結腸、PCI13細胞株、およびSKBR3細胞株から得たRNAとの混合物が、マーカースクリーニングプロセスにおけるすべての遺伝子についてのユニバーサル陽性発現対照として機能した。
24枚のOCT包埋組織の5-μm切片を、800μlのGeneXpert(登録商標)溶解バッファー(Cepheid, Sunnyvale, CA)中で切断した。溶解バッファーは、0.22-μmシリンジフィルター(Osmonics Inc, West borough, MA)を通して濾過し、そしてGeneXpert(登録商標)カートリッジ中に充填した。RNA単離、逆転写、そしてQRT-PCRの自動化プロセスは、GeneXpert(登録商標)上でおこなった。
マーカーを評価するために使用した特徴は、感度、特異性、分類精度、そして陰性適中率および陽性適中率であった。評価には、マーカーの分布を特徴付け、そしてlog-正規分布に対してデータを適合させることを試験することが含まれた。個別のマーカーについては、分類精度を最大にするカットオフ値を決定した。分類精度が100%であった場合、最大に発現する良性のリンパ節と最低に発現する組織学的に陽性のリンパ節との中間点にカットオフ値を設定した。マーカーを対結合(paired combinations)においても評価し、そして各対について線形予測規則(linear prediction rule)を生成した。規則は、線形境界の上および下での近似確率(fitted probabilities)を均一にする線形予測値(linear predictor)に対応していた。すなわち、境界線上の点は、陽性のリンパ節および陰性のリンパ節について割り当てられた数値スコアの中間の予測確率を有した。
一次マーカースクリーニング:
原発腫瘍中および良性リンパ節中での中央値相対発現を、一次スクリーニングにおいて含まれた43個の潜在的マーカーのすべてについて計算した(表F)。
二次スクリーニングにおいて使用された25個の原発性乳癌試料の組織学的評価から、75%の中央値腫瘍%(5〜95%の範囲)が示された。27個の組織学的に陽性のリンパ節中の中央値腫瘍%は80%であった(5〜95%の範囲)。乳腺腫瘍、陽性リンパ節、および良性リンパ節における二次スクリーニングに含まれた6個のマーカーの相対的発現は、図9Aに示される。各マーカーの分類特性(病理学的検討と比較した)は、表Gにまとめる。
二次スクリーニングにおいて試験された選択されたマーカーの分類精度をさらに検証するため、90個の乳癌前哨リンパ節の別個の検証用セットを、予め解析した(図11Aおよび11B)。さらに、手術中解析の可能性を示すため、迅速な多重化QRT-PCRを使用して、SmartCycler(登録商標)装置(Cepheid)でこの研究を行った。サーモサイクラープラットフォームにおけるこの変化に由来する、相対発現計算値におけるわずかな差異が観察されたが(データは示さず)、QRT-PCR解析の頑健性を間接的に評価するための努力において、二次スクリーニングからの分類アルゴリズムを補正率(correction factors)なしで検証用セットデータに対して適用した。
別個の患者から得た18個のリンパ節試料を、マーカーTACSTD1およびPIPについて、完全に自動化されたQRT-PCRアッセイを用いて評価した(図11Aおよび11B)。組織学的検討の結果、9個の陽性リンパ節(60〜95%腫瘍)および9個の陰性リンパ節から構成されたことが確認された。二次スクリーニングセットから得たデータに基づいて、決定規則を使用して線形決定規則または等確率等高線解析のいずれかにより予め解析された場合、多重化GeneXpert(登録商標)アッセイは、18個の試料すべてを、アッセイあたり35分以内に正確に(100%)特徴づけた。完全に自動化された迅速なQRT-PCRアッセイは、転移性乳癌の存在についてリンパ節を正確に特徴づけたと我々は結論づけられる。
プライマーおよびプローブの設計
3種すべてのプライマー/プローブセットを、エクソンジャンクションをカバーするように設計して、ゲノムDNAを増幅する可能性を減少させ、そしてその他の既知遺伝子に対する全体の相同性が低い領域にあるようにした。2種の標的遺伝子用のプライマー対を、内在性対照遺伝子の変性温度よりも少なくとも4℃高い変性温度のアンプリコンを生成する様に設計した。最初に、2種の標的遺伝子用のプライマー対を、内在性対照遺伝子と同様のアニーリング温度を有するように設計した。次いで、標的遺伝子用のプライマーを、“GCクランプ” を付加することにより修飾して、アニーリング温度を内在性対照よりも少なくとも4℃高くした(表I)。
アニーリング温度の研究を、25μL反応チューブ中でSmartCycler(登録商標)(Cepheid)プラットフォームにて、一重で行った。プラスミド、pENTR-TACSTD1、pENTR-PIP、およびpOT7-βGUSを、Invitrogenから入手し、そして低コピー数および高コピー数について、反応あたり適切な4E3および3E6コピー数でそれぞれ使用した。すべてのプローブを、200 nMで使用した。PIPおよびTACSTD1用のプライマーを、400 nMで使用した。β-GUS用のプライマーを、600 nMで使用した。反応を、4 mM MgCl2および200μM dNTPを添加した製造者のバッファー中、2.5U Platinum Taqにより行った。β-GUS増幅用のサイクリング条件は、2段階プロトコルを使用して行った。第1段階において、サンプルを95℃で30秒間変性した。第2段階において、95℃で1秒間の変性工程、56℃〜68℃まで変化させて4秒間のアニーリング工程、そして68℃で6秒間の光学的読み取り/伸長工程からなる3温度プロトコルを40サイクル使用した。
一重の研究のため、プラスミド3000 pg〜0.092 pgまでTEバッファー中で連続的に希釈し、そしてPCR反応においてテンプレートとして使用した。反応を、25μL反応チューブを使用して、SmartCycler(登録商標)プラットフォーム上で行った。プライマー濃度、プローブ濃度、酵素濃度、MgCl2濃度、およびdNTP濃度は、アニーリング温度の研究のために使用したものと同じであった。
変性温度の研究は、上述した一重反応設定で行った。使用されたサイクリングプロトコルは、第1段階、95℃で30秒間を1サイクル;第2段階、95℃で1秒間、60℃で4秒間、そして光学系を“on”にしながら68℃で6秒間;そして第3段階、80℃〜95℃の様々な温度で1秒間、60℃で4秒間、そして光学系を“on”にしながら68℃で6秒間のサイクルを20サイクル、の3段階プロトコルでから構成された。図15は、β-GUS増幅が、通常は84℃低度の変性温度で進行するが、PIPおよびTACSTD1の反応は、88℃またはそれ未満では進行できないことを示す。
様々な比率のプラスミド混合物を、表Kに示すように調製した。三重の反応を、上述したように三重で設定した。サイクリングプロトコルは、第1段階、95℃で30秒間を1サイクル;第2段階、95℃で1秒間、60℃で4秒間、そして光学系を“on”にしながら68℃で6秒間のサイクルを20サイクル;そして第3段階、95℃で1秒間、そして光学系を“on”にしながら68℃で6秒間のサイクルを30サイクル、からなる3段階プロトコルであった。得られたCt値の平均および標準偏差を、図16に示す。これらのデータは、2種の標的遺伝子(PIPおよびTACSTD1)のダイナミック・レンジが、少なくとも6 Ctの発現の倍差異を伴う、高い効率の直線であることを示す。
MDA-MB-453細胞(Stratagene)またはヒトリンパ節(Ambion) から単離した1μgの全RNAを、RNA精製後にRT-PCRを行うように設計されたカートリッジ中、GeneXpert(登録商標)で行われたRT-PCR反応において、テンプレートとして使用した。各サンプルについて、RNAを溶解バッファー(50 mM Tris、pH 7.0、4 Mグアニジンチオシアネート、125 mM DTT)に添加し、そしてエタノールと混合し、その後カートリッジにローディングした。RNAを固相材料により捕捉し、そして0.2M KCl、10%PEG 8000、20 mM Tris、pH 8.0にて洗浄し、その後、30μL DEPC-処理水で溶出した。溶出物を、1×PCRバッファー(Invitrogen)、6mM MgC12、267μM dNTP、80 nMの各RTプライマー、40U Roche RNAse Protector、そして2.6U Omniscript(Qiagen)からなるRT反応構成要素と混合した。RT反応を48℃で5分間行い、そして引き続いて、95℃で30秒間不活性化した。不活性化工程を全部で2回繰り返した。この混合物に対して、PCRプライマーおよびプローブを、上述したような最終濃度、そして0.1 U/μL Eppendorf Taq + 15μMホットスタート阻害剤となるように添加した。さらに10×PCRバッファーを容量変化を修正するために添加した。
