Description

The present invention relates to a medical stapler used for suturing a wound.

In general, medical staples are used in addition to sutures as means for suturing wounds in surgery and the like. This medical staple is composed of a crown portion and needle leg portions that curve downward from the bent portions on both sides thereof, and uses a medical stapler. The medical stapler includes a stapler body having a head portion loaded with medical staples, and an operation lever provided on the stapler body so as to be operable. The operation lever is normally biased in an opening direction away from the slider body. Then, the operating lever is squeezed firmly and rotated in the closing direction, the medical staple is protruded from the head portion and inserted into the skin, and the wound is brought close and folded while the both side needle tip portions are bent inward.

When stitching with sutures, the threading holes on both sides of the wound are tied together with sutures, so there are many cases where the thread marks remain after the thread is pulled. The crown portion of the medical staple does not come into contact with the skin simply by bringing the needle into contact. For this reason, after removing the medical staple, only the scar of the wound remains, and the surgical scar is generally clean. Therefore, the spread of suture surgery using a medical stapler is expected.

By the way, the medical stapler is basically disposable. After a suture operation, regardless of how many medical staples remain in the medical stapler, the surgical stapler will be disposed of when the operation is completed.
Japanese Patent No. 3054202

However, even after trying to dispose of the medical stapler without any treatment after the suturing operation (after using the medical stapler), the operation lever is biased in the opening direction so that it is in an open state with respect to the stapler body, It protrudes away from the stapler body and is bulky. The cost of treating medical waste at risk of infection is higher than normal waste disposal, and reducing the volume of waste is a major challenge in saving hospital costs and reducing costs.

In addition, all of the loaded medical staples are not used up in the suturing operation, and the medical staples often remain in the medical stapler to be discarded. An external force is applied to the control lever during the processing of the garbage bag containing the medical stapler, and the staples thus punched through the garbage bag, exposing the medical staples contaminated with the patient's body fluid. End up being unsanitary.

For such a problem, there is a method in which all the medical staples remaining in the medical stapler after the suturing operation are blanked and the medical staples and the medical stapler are disposed of separately. However, there are cases where the operation room is contaminated with unnecessary medical staples that are punched due to blanking, and the medical staples that are punched out may accidentally adhere to the patient. is there.

It is also possible to disassemble and dispose of the medical stapler, but there are inconveniences that must be dealt with fine medical staples during disassembly, and there are small parts in the operating room during the operation. May not be an effective means.

The present invention eliminates the above-mentioned problems, and can be easily and reliably performed by reducing the volume of medical waste and disposing of the remaining staples by discarding the lever with the operating lever rotated in the closing direction. The challenge is to provide a stapler.

The invention according to claim 1 is provided with a stapler body provided with a head portion for ejecting medical staples at one end, and an operation lever that is supported by the stapler body so as to be operable, and the head of the operation lever is provided. The grip portion located on the opposite side to the head side is urged in the opening direction away from the stapler body, and when the medical staple is driven, the operation lever is rotated in the closing direction approaching the stapler body. In the medical stapler that moves the staple from the head portion and folds the both side needle tip portions of the medical staple inward, and sutures the wound, the operation lever is a normal suture for the stapler body and the operation lever. A stopper mechanism that restricts the rotation in the closing direction from the amount of rotation required for the rotation, and the stopper mechanism is released during suturing The operating lever when rotated in the closing direction than the principal pivoting amount, characterized in that a locking mechanism for locking engages in the stapler body.

According to a second aspect of the present invention, in the first aspect, the stopper mechanism includes a slider that is slidably movable along the longitudinal direction of the stapler body, and an operation lever that is closed by an amount necessary for normal sewing. And an engaging portion formed so as to be engageable with the slider when rotated in a direction, and the slider is biased in a direction to engage with the engaging portion.

According to a third aspect of the present invention, in the first or second aspect, the lock mechanism includes a locking portion formed on one of the operation lever and the stapler body, and an engagement provided on the other. The stopper portion is formed to be able to be locked with each other.

According to a fourth aspect of the present invention, in the second aspect, the locking mechanism includes a locking portion provided on one of the slider and the operation lever, and a locking receiving portion formed on the other. The slider is locked so that the operation lever cannot be rotated in the opening direction, and the slider is locked so as not to slide.

According to the first aspect of the present invention, since the operation lever is configured to be locked to the stapler body by the lock mechanism in a state in which the medical staple is bent in the closing direction after the suturing is finished, the operation lever is the stapler. Since the state which approached the main body is maintained, the whole volume becomes small. Therefore, the volume when discarded can be reduced, and the volume of medical waste can be reduced.

In addition, during normal sewing, the operating lever is restricted from rotating in the closing direction by a stopper mechanism, and the lock mechanism cannot be operated without an operation to release the stopper mechanism. Therefore, it is not necessary to operate with particular attention to the amount of rotation and the amount of force during the suturing operation.

Further, the operation lever is finally locked in the stapler body in a state of being rotated in the closing direction in which the medical staple is bent, and is locked so as not to be rotatable in the opening direction. The needle tip portion of the first medical staple is fixed in a state of being bent inward. Therefore, since the needle tip portion of the medical staple does not protrude from the stapler body, the medical staple is safely discarded. Even if an external force is applied to the control lever during the processing of the garbage bag, the needle tip of the medical staple is bent inward, so there is no risk that the extruded medical staple will break through the garbage bag and be exposed. , Disposal work can be done safely. Further, since the remaining staples can be discarded without being disassembled while remaining in the stapler body, there is no possibility of contaminating the operating room.

According to the second aspect of the present invention, the stopper mechanism is rotated in the closing direction by the amount of rotation necessary for the normal stitching and the slider provided so as to be slidable along the longitudinal direction of the stapler body. Since the slider is urged in a direction to engage with the engagement portion, the operation lever has a rotation amount necessary for normal stitching. Therefore, it is possible to reliably regulate the rotation in the closing direction.

According to the invention of claim 3, the lock mechanism locks the locking portion formed on one of the operation lever and the stapler body and the locking receiving portion provided on the other. Since it was formed, the operating lever cannot be rotated in the opening direction even if the slider is moved after the locking portion and the locking receiving portion are locked. Therefore, the closed state of the operation lever can be reliably held.

According to the invention of claim 4, the lock mechanism locks the locking portion provided on one of the slider and the operation lever and the locking receiving portion formed on the other. In this case as well, the operation lever cannot be rotated in the opening direction, and the slider can be locked so that it cannot slide.

Embodiments of the present invention will be described below with reference to the drawings. In each embodiment, the stapler main body and the operation lever are the same, and therefore, are denoted by the same reference numerals, and detailed description is omitted except for the first embodiment.

FIG. 1 is a side view showing a part of one embodiment of a medical stapler in cross section, and FIG. 2 is an exploded perspective view of the stapler. The medical stapler is provided with an operation lever 2 that can be opened and closed with respect to a stapler body 1 made of a synthetic resin, and the operation lever 2 is rotated in a closing direction in which the operation lever 2 is housed inside the storage portion 8. The staple is protruded from the head portion 3 of the stapler body 1 and stabbed into the skin, and the wound is brought close and stitched while the both side needle tip portions are bent inward.

The side surface of the head portion 3 is formed in a trapezoidal shape, and a medical staple launching portion 4 is provided at a lower portion of the front end (left side in the figure). The stapler body 1 is configured such that several to several tens of medical staples are loaded and supplied to the launching unit 4 in order from the leading one by feeding means (not shown). A driver plate 5 is provided on the upper portion of the projecting portion 4 so as to be slidable in the vertical direction, and is urged by a spring (not shown) so as to be always at the upper position. A base portion 13 is formed long in the front and rear at the lower portion of the head portion 3.

The lower end of the driver plate 5 is formed in an inverted concave shape, and as shown in FIG. 3A, projecting pieces 6 are formed on both sides. An anvil 7 is formed on the inner upper portion of the launching portion 4, and the central portion of the leading staple S 1 is placed on the anvil 7.

The operation lever 2 is in a state in which the front portion located on the head portion 3 side is housed in a space (housing portion 8) inside the stapler body 1, and the rotary shaft 10 provided on the operation lever 2 side is moved. At the center, the grip portion 11 on the opposite side to the head portion 3 is pivotably attached so as to open and close. Normally, the grip portion 11 protrudes from the storage portion 8 and is biased by a spring (not shown) in an opening direction away from the stapler body 1. In this state, the grip portion 11 protruding from the storage portion 8 is formed with a finger hook portion 12 on which four fingers from the index finger to the little finger can be put.

Next, the stapler body 1 and the operation lever 2 are sewn with a stopper mechanism that restricts the rotation of the operation lever 2 in the closing direction from the amount of rotation necessary for normal stitching, and the stopper mechanism is released. A lock mechanism is provided for locking the operation lever 2 to the stapler body 1 when the operation lever 2 is rotated in the closing direction more than the required rotation amount.

That is, as shown in FIGS. 1 and 2, the base portion 13 provided at the lower portion of the head portion 3 is formed in a rectangular tube shape, and a slider 14 is slidably disposed along the longitudinal direction therein. Has been. The slider 14 is composed of a guide portion 16 of the coil spring 15, an engagement piece 17 projecting upward and downward from its base portion, and an engagement portion 18 at the rear portion thereof, and is engaged rearward from the upper portion of the back surface portion 20 of the engagement portion 18. A jaw portion 19 is formed to protrude. The lower surface of the engaging jaw portion 19 is formed to be inclined rearward and downward. Further, the back portion 20 is also formed so that the lower side is slightly inclined forward. Further, a coil spring 15 is disposed in the base portion 13, and one end of the coil spring 15 is wound around the spring guide portion 16 of the slider 14, and an engagement piece 17 of the slider 14 is formed above and below the rear portion of the base portion 13. By engaging with the engaging hole 21, the slider 14 is slidably attached within the length of the engaging hole 21 and is always urged rearward by the coil spring 15.

The base portion 13 is formed with locking pieces (locking receiving portions) 22 protruding rearward at positions on both sides of the slider 14.

On the other hand, as shown in FIG. 2, the operating lever 2 is provided with an engaging jaw portion 19 that is biased by the coil spring 15 and is positioned at the end portion of the engaging hole 21 when rotating. A guide groove 23 is formed so as to be slidably guided. An engaging projection 25 (see FIG. 1) that can engage with the engaging jaw 19 of the slider 14 is formed at the lower end of the guide groove 23.

Further, on both sides of the guide groove 23, locking portions 24 are formed at positions corresponding to the locking pieces 22 arranged on both sides of the slider 14. The upper surface of the locking portion 24 is inclined forward and downward.

The stopper mechanism is constituted by the engaging jaw portion 19 of the slider 14 of the stapler body 1 and the engaging protrusion 25 of the operation lever 2, and the operation lever 2 is rotated to a rotation amount necessary for normal sewing as will be described later. The upper surface of the engaging protrusion 25 of the operation lever 2 is set so as to engage with the lower surface of the engaging jaw 19 of the slider 14 when rotated.
Further, the lock mechanism is constituted by a locking piece 22 of the base 13 attached to the stapler body 1 and a locking portion 24 of the operation lever 2, and the operation lever 2 exceeds the rotation amount necessary for normal sewing as will be described later. The locking portion 24 of the operation lever 2 is set so that the locking piece 22 of the base 13 attached to the stapler body 1 is elastically deformed and gets over the upper surface side of the locking piece 22 when it is rotated.

Next, the operation mode of the medical stapler will be described. When the wound is sutured, the tip of the head portion 3 is pressed against the skin a on both sides of the wound, and the operation lever 2 is moved to the position shown in FIGS. As shown in FIG. 5B, the staple S1 supplied to the launching portion 4 of the head portion 3 is pressed by rotating in the closing direction of housing in the housing portion 8. At this time, the center of the staple S1 is supported by the anvil 7 and cannot move further downward. On the other hand, since the protruding pieces 6 formed on both sides of the driver plate 5 press both sides of the staple S1, only the needle tip portion of the staple S1 is pushed down, and the needle tip portion of the staple S1 is centered on both ends of the anvil 7. Turn to get closer to each other. Thereafter, while the operating lever 2 is moved to the moving end, the needle tip portion is inserted into the skin a, and the wound is pulled and sutured while the needle tip portions on both sides are bent inward.

Next, as shown in FIG. 1, the staple S <b> 1 is discharged from the head portion 3 by releasing the force on the operation lever 2 and returning the grip portion 11 to the opening direction that protrudes from the storage portion 8 of the stapler body 1. Then, the suture of one wound is finished. This stitching operation is appropriately repeated according to the wound to complete the stitching of the entire wound.

At the time of suturing, the operation lever 2 may be rotated to the amount necessary for normal suturing as described above. At this time, the operation mechanism 2 is engaged by the stopper mechanism rotated upward as shown in FIG. Since the upper surface of the protrusion 25 engages with the lower surface of the engaging jaw 19 of the slider 14 of the stapler body 1 and cannot be rotated any more, the operating lever 2 is closer to the closing direction than the amount of rotation required for normal stitching. It is possible to reliably regulate the rotation.

Then, when the medical stapler is discarded after all the stitches are finished, as shown in FIG. 6 showing a cross section on the line AA in FIG. The slider 14 is pushed forward against the urging force of 15 to release the stopper mechanism. While maintaining this state, the operating lever 2 is grasped and rotated in the closing direction in which it is stored in the storage portion 8. At this time, since the engagement protrusion 25 of the operation lever 2 does not engage with the slider 14 of the stapler body 1, the operation lever 2 can be rotated in the closing direction rather than the amount of rotation necessary for sewing. At this time, as shown in FIG. 7 showing a cross section along line BB in FIG. 4B, the upper surface of the locking portion 24 of the operation lever 2 elastically deforms the locking piece 22 on the stapler body 1 side by the locking mechanism. Since the locking portion 24 rides over the upper surface side of the locking piece 22, the locking portion 24 of the operating lever 2 is not locked with the locking piece 22 on the stapler body 1 side even if the grip of the operating lever 2 is released thereafter. Since it is locked to, it is locked at the locking position and cannot be rotated further downward.

As described above, since the operation lever 2 is housed in the housing portion 8 of the stapler body 1 and the state in which the stapler body 1 and the operation lever 2 are approached is maintained, the overall volume is reduced. Therefore, the volume when discarded can be reduced, and the volume of medical waste can be reduced.

Further, since the operation lever 2 is finally locked in a state of being rotated in the closing direction in which the staple S1 is bent, the needle tip portion of the leading staple S1 remaining inside the head portion 3 is shown in FIG. As shown in b), it is fixed in a folded state inside. Accordingly, since the needle tip portion of the staple S1 does not protrude from the stapler main body 1, even if an external force is applied to the operation lever 2 during the processing of the garbage bag in which the medical stapler is discarded, the pushed out staple S1. There is no risk that the bag will break through the garbage bag, and the disposal work can be performed safely. In addition, since the remaining staples can be discarded without being disassembled, there is no risk of contaminating the operating room. Therefore, it is possible to perform sanitary disposal not only for medical institution workers but also for all workers involved in the disposal of medical waste including medical instrument waste disposal companies.

Further, the operation lever 2 is restricted from rotating in the closing direction by the stopper mechanism during the normal stitching, and if there is no operation to release the stopper mechanism, the lock mechanism can be operated. This is not possible, and there is no need to operate with particular attention to the amount of rotation of the operation lever 2 and the force applied during the suturing operation.

In addition, the locking mechanism is configured such that a locking piece 22 provided on the stapler body 1 separately from the slider 14 and a locking portion 24 formed on the operation lever 2 are locked with each other. Even if the slider 14 is moved after the locking piece 22 and the locking portion 24 are locked, the operation lever 2 does not rotate in the opening direction. Therefore, the closed state of the operation lever 2 can be reliably held.

In contrast to the above-described embodiment, the slider / locking piece may be disposed on the operation lever side, and the engagement protrusion / locking portion may be disposed on the stapler main body side.

Further, the locking mechanism is not limited to the above example. For example, the operating lever cannot be rotated in the opening direction by engaging an engaging portion provided on one of the slider and the operating lever and an engaging receiving portion formed on the other. Further, the slider may be configured to be locked so as not to slide.
