Claims (10)
- 心臓チャンバの電気活動を表す感知信号を生成し、その感知信号が指定された固有の検出閾値を超えた場合にチャンバ・センスを検出する感知チャネルと、
抗頻拍ペーシング治療を施すペーシング・チャネルと、
前記感知チャネルとペーシング・チャネルにインターフェースをとるコントローラと
から構成され、前記コントローラは、
検出されたチャンバ・センスのレートが指定された頻拍閾値を超えた場合に心臓チャンバの頻拍症を検出し、
頻拍症が検出されると、抗頻拍ペーシング・プロトコルに従って１つまたは複数のペーシング・パルスのバーストを送出し、前記バーストがチャンバ・センスに関する指定のカップリング間隔後に出力され、
前記ペーシング・パルスの出力に後続する捕捉検出ウィンドウ中に誘発反応が生じたかどうかを検出することによって、ペーシング・パルスが心臓チャンバを捕捉したかどうかを判断し、
前記バースト中に捕捉を行った連続ペーシング・パルスの数をカウントすることによって心臓チャンバが同調された程度を決定するようにプログラミングされていることを特徴とする心リズム管理装置。 - 前記コントローラは、前記バースト中に捕捉を行った連続ペーシング・パルスの数と同調閾値数Ｍを比較するようにさらにプログラミングされていることを特徴とする請求項１に記載の心リズム管理装置。
- 前記コントローラは、Ｍ個の連続ペーシング・パルスが捕捉を行った場合に前記バーストを停止するようにさらにプログラミングされていることを特徴とする請求項２に記載の心リズム管理装置。
- 前記コントローラは、頻拍性不整脈が存続する場合に後続のバーストに関する同調閾値Ｍを増加するようにさらにプログラミングされていることを特徴とする請求項３に記載の心リズム管理装置。
- 前記コントローラは、頻拍性不整脈が存続して前のバーストで捕捉を行った連続ペーシング・パルスの数がＭ未満の場合に、後続のバーストに関するカップリング間隔を大きくするようにさらにプログラミングされていることを特徴とする請求項２に記載の心リズム管理装置。
- 前記コントローラは、頻拍性不整脈が存続して前のバーストで捕捉を行った連続ペーシング・パルスの数がＭ未満の場合に、後続のバーストに関するカップリング間隔を小さくするようにさらにプログラミングされていることを特徴とする請求項２に記載の心リズム管理装置。
- 前記送出されるバーストは、指定されたサイクル長で分離された一連のペーシング・パルスであることを特徴とする請求項１に記載の心リズム管理装置。
- 前記バーストはランプタイプ・バーストであって、ペーシング・パルス間のサイクル長が前記バースト中のパルスごとに次第に短縮されるようになっていることを特徴とする請求項７に記載の心リズム管理装置。
- 前記送出されるバーストは指定されたサイクル長で分離された一連のペーシング・パルスであり、前記コントローラは、頻拍性不整脈が存続して前のバーストで捕捉を行った連続ペーシング・パルスの数がＭ未満の場合に、後続のバーストに関するサイクル長を調整するようにさらにプログラミングされていることを特徴とする請求項２に記載の心リズム管理装置。
- 前記送出されるバーストは指定されたサイクル長で分離された一連のペーシング・パルスであり、前記コントローラは、頻拍性不整脈が存続して前のバーストで捕捉を行った連続ペーシング・パルスの数がＭ未満の場合に、後続のバーストに関するペーシング・パルス・エネルギを調整するようにさらにプログラミングされていることを特徴とする請求項２に記載の心リズム管理装置。
