Abstract
本発明は、心血管性事象のリスクを減少させるために、非糖尿病のヒト個人の尿細管性蛋白尿および/またはα1−ミクログロブリンを減少させる医薬組成物の製造におけるACE阻害剤またはアンギオテンシン受容体アンタゴニストの使用に関する。
Description
冠状動脈性心臓病は、依然として米国では罹患率および死亡率の主要な原因である。心血管性疾患のリスク因子は、直接的に原因であることがあり、より基礎的な内在する代謝異常の二次発現であることがあり、または該疾患の初期症状を示すことがある。いくつかのリスク因子は、心血管性疾患の発生率の増加に関係しており、そして該リスク因子には喫煙、高血圧、脂質異常血、糖尿病、ミクロアルブミン尿、肥満、座りがち生活形態、性別および貧しい栄養が挙げられる。成人でのリスク因子の作用は付加的であり、高リスク因子の数が多く存在すればする程、心血管性疾患のリスクは大きくなる。
可逆的であり、直接的原因であるリスク因子のいくつかに対する介入(intervention)は、進行しつつある心血管性疾患のリスクを低下させ、そして卒中または心筋梗塞のような心血管性事象の発病率を低下させるのに有効に使用されている。
糸球体性蛋白尿の一つの形態であるミクロアルブミン尿が、心血管性リスク因子および心血管性疾病率のインジケイターであることは知られている。
今や、意外なことに、尿細管性蛋白尿およびα1−ミクログロブリンの高められた尿中レベルが、進行しつつある心血管性疾患および心血管性死亡率の増加されたリスクを独立的に示すものであるということが見出された。
さらに、尿細管性蛋白尿および/または尿中α1−ミクログロブリンの減少により心血管性事象の発病率が低下することも見出された。
すなわち、本発明によれば、アンギオテンシン変換酵素(ACE)阻害剤またはアンギオテンシン受容体アンタゴニストのようなアンギオテンシン/レニン−系のモジュレータは、尿細管性蛋白尿およびα1−ミクログロブリンを低下させるのに有効に使用され得るということが示された。
さらに、本発明によれば、尿細管性蛋白尿または高められた尿中α1−ミクログロブリンを示す個人に対するこのような治療介入が、心血管性事象のリスクを有効に減少させるということが、大規模での介入試用で確立された。
上記の研究結果によって、本発明は個人の心血管性リスク評価のための新規なスクリーニング方法を提供する。さらに、本発明による心血管性リスク評価のためのスクリーニング方法は、尿細管性蛋白尿およびα1−ミクログロブリンの測定値が標準手法により測定され得るので、心血管性事象の予報値(predictor)としてミクロアルブミン尿を使用するスクリーニング方法よりもより容易で、より速くそしてより信頼性がある。
さらに別の特徴において、本発明はまた、非糖尿病患者の尿細管性蛋白尿および/または尿中α1−ミクログロブリンを減少させるためのアンギオテンシン変換酵素(ACE)阻害剤またはアンギオテンシン受容体アンタゴニストのようなアンギオテンシン/レニン−系のモジュレータを含有する、心血管性事象のリスクを減少させるため医薬組成物も提供する。
本発明の診断スクリーニング方法による診断後の治療介入は、無症候性の健康な個人並びに高血圧または明白な心血管性疾患を有する患者について実施され得る。
本発明による好ましいACE阻害剤は、ラミプリル、ペリンドプリル、トランドラプリル、リシノプリル、エナラプリルまたはカプトプリルおよびそれらの製薬的に許容し得る塩および誘導体である。
本発明により使用するためのアンギオテンシンアンタゴニストとしては、ロルサルタン、カンデサルタン、バルサルタン、イルベサルタン、オルメサルタン、エプロサルタンまたはテルミサルタンおよびそれらの製薬的に許容し得る塩および誘導体が好ましい。
尿分析はいずれかの標準的な実験室試験により実施され得る。尿中アルブミンの測定は定量的蛋白測定(比濁法)およびSDS−電気泳動法により実施するのが好ましい。
商業的にテストキットは入手可能であり、例えば濁度試験用にRoche Diagnosticsのチナカント アルファー−ミクログロブリン(Tinaquant alpha-microglobulin)がある。
尿細管性蛋白尿においては、レチノール−結合蛋白、α1−ミクログロブリン、β2−ミクログロブリン、リゾチーム、L鎖、ヘモグロビンおよびミオグロビンを含む小さな分子量の蛋白が検出され得る。
Claims (5)
- 心血管性事象のリスクを減少させるための、非糖尿病のヒト個人の尿細管性蛋白尿および/またはα1−ミクログロブリンを減少させる医薬組成物の製造におけるACE阻害剤またはアンギオテンシン受容体アンタゴニストの使用。
- ACE阻害剤がラミプリル、ペリンドプリル、トランドラプリル、リシノプリル、エナラプリルまたはカプトプリルから選択される請求項1に記載の使用。
- アンギオテンシンアンタゴニストがロルサルタン、カンデサルタン、バルサルタン、イルベサルタン、オルメサルタン、エプロサルタンまたはテルミサルタンである請求項1に記載の使用。
- 心血管性事象が卒中または心筋梗塞から選択される請求項1に記載の使用。
- 尿中のβ−ミクログロブリンまたはその他の尿細管性蛋白およびα1−ミクログロブリンのレベルを測定することからなる、心血管性疾患または心血管性事象を罹患するヒト個人のリスクを評価するためのインビトロ診断方法。
