Abstract
【解決手段】

Description
〔発明の要約〕
本発明は中でも、安定した低レート心室頻脈を有する患者に繰り返してショック治療を送出する問題に対処する。本発明の態様は、特に、患者が血行力学的に弱化していないと判断されると、再発性で遅いVTを有する患者への苦痛を伴うショック治療の送出を遅延させることを含む。本発明のさらなる態様は、ショック治療送出の時刻を制御することであって、それによって、不可欠でないショック治療は、患者が、損傷または苦痛のさらなるリスクがない時に送出されるようにする、時刻を制御すること、および、おそらく、抗頻脈ペーシング治療を続けて試みることによって、遅延されたショック治療を送出する前に異常調律を終わらせることが可能になることによって、ショック治療の必要を避けることを含む。
Claims (18)
- 心室頻脈を経験する心臓患者へのショック治療の送出を制御する方法であって、
心室頻脈を検出すること、
前記心室頻脈が検出されると、抗頻脈ペーシング治療を送出すること、
前記抗頻脈ペーシング治療が送出された後、前記心室頻脈が終了したかどうかを判断すること、および
血行力学的状態の尺度に基づいてプログラムされたショック治療を遅延させること、
を含むショック治療の送出を制御する方法。 - 前記血行力学的状態の尺度は、血行力学的しきい値基準を設定することを含む請求項1に記載の方法。
- 前記血行力学的しきい値状態の尺度は、血行力学的安定状態を設定することを含む請求項2に記載の方法。
- 血行力学的しきい値基準を設定すること、
患者の血行力学的状態を検出すること、および
患者の前記血行力学的状態に基づくか、または、前記血行力学的しきい値基準に対して患者の前記血行力学的状態を比較して、前記プログラムされた治療を遅延させること、
をさらに含む請求項1に記載の方法。 - 遅延されたショック治療を送出するための条件を規定すること、および
ショック送出についての前記条件が満たされると、前記遅延されたショック治療を送出すること、
をさらに含む請求項4に記載の方法。 - 遅延されたショック治療を送出するための前記条件は、前記血行力学的しきい値基準を満たさない血行力学的状態を検出することを含む請求項5に記載の方法。
- 遅延されたショック治療を送出するための前記条件は、指定された長さの経過時間を監視することを含む請求項5に記載の方法。
- 遅延されたショック治療を送出するための条件を規定すること、および
送出についての前記条件が満たされると、前記遅延されたショック治療を送出すること、
をさらに含む項1に記載の方法。 - 患者の姿勢を検出することをさらに含む請求項8に記載の方法。

心室頻脈を検出する手段と、
遅い心室頻脈が検出されるかどうかを特定する手段と、
プログラムされた抗頻脈ペーシング治療を送出する手段と、
前記心室頻脈の終了を検出する手段と、
遅い心室頻脈を持続して再検出する手段と、
血行力学的不安定性を調べる手段と、
プログラムされたショック治療を遅延させる手段と、
患者通知を生成する手段と、
を備えるデバイス実施式ソフトウェアシステム。 - 終了を検出する前記手段はさらに、持続する遅い心室頻脈を調べる手段と協働する請求項15に記載のシステム。
- 血行力学的不安定性を検出する前記手段は、心筋虚血を検出する手段、睡眠を検出する手段、所定の時間経過にマークを付ける手段、および、ショック条件が満たされているか、ショック条件が満たされていないかのいずれであるかを確認する手段と協働する請求項16に記載のシステム。
- 患者通知を生成する前記手段はさらに、高レベル警報モードを信号で送る手段を含む請求項15に記載のシステム。
