Abstract
Description
The present invention allows a guide wire to be easily inserted into a catheter used for treatment of tubular organs such as blood vessels and ureters, and can be easily attached to the guide wire. The present invention relates to a guide wire insertion / operation instrument that improves the performance.
In recent years, for inspection and treatment of human tubular organs such as blood vessels, ureters, bile ducts, trachea, etc., a catheter is inserted to administer contrast agents, etc., or a portion of tissue is collected with forceps through the catheter. Has been done.
When inserting the catheter, first, a guide wire is inserted into the tubular organ, the distal end of the guide wire is made to reach the target location, and then the catheter is inserted along the outer periphery of the guide wire. In this case, in a state where the guide wire is inserted through the catheter, a method of causing the catheter to follow and follow the guide wire while advancing the tip of the guide wire first is often employed.
In such catheter insertion work, for example, an insertion device as shown in FIGS. 6 and 7 is used. In FIG. 6,
By the way, when the
In a conventional guide wire insertion device for a catheter, when the
Next, the
However, in the conventional insertion operation as described above, the
On the other hand, the wire rod operating instrument of Patent Document 1 has a tapered catheter fitting portion at the tip. However, with regard to the procedure for attaching to the guide wire, a wire (guide wire) is inserted into the catheter, the tip of the wire is set to be arbitrarily extended from the tip of the catheter, and the operation instrument is passed from the rear end of the wire. There is only an explanation of clamping at a predetermined position. That is, even in Patent Document 1, the conventional method shown in FIGS. 6 and 7 is adopted as the operation procedure, and the problem that the operation procedure is complicated and the operation takes time cannot be solved. .
Accordingly, an object of the present invention is to provide a guide wire insertion / operation instrument that can insert a guide wire into a catheter and attach an operation instrument for operating the guide wire in a short time with good workability. There is.
In order to achieve the above object, a first aspect of the present invention is a guide wire insertion / operation device that serves as a guide for inserting a guide wire into a catheter and also serves as a gripping portion for operating the guide wire. A cylindrical main body having an insertion hole for the guide wire therein, a chuck portion provided at the tip of the main body, and a cap screwed on the outer periphery of the tip of the main body to tighten the chuck portion, The insertion hole of the main body and the inner periphery of the chuck portion are not formed with a stepped diameter portion when viewed from the base end side of the main body, and the base end of the catheter is formed at the tip of the cap. A guide wire insertion / manipulation instrument, characterized in that a tapered guide tube is provided for being inserted into a hub provided on the hub and for introducing the guide wire into the reduced diameter hole portion of the hub. It is intended to provide.
According to the above invention, when inserting the guide wire into the catheter, first, the cap of the insertion / operation instrument is loosened so that the guide wire can be inserted, and the guide wire is inserted into the hub provided at the proximal end of the catheter. Insert the guide tube of the operating instrument, and then insert the guide wire into the insertion hole from the proximal end side of the main body of the operating instrument. At this time, since the insertion hole of the main body and the inner periphery of the chuck portion do not have a stepped diameter portion when viewed from the base end side of the main body, the guide wire is caught in the middle. Instead, it passes through the inner circumference of the chuck portion, passes through the guide tube, is inserted into the hub, and is inserted from the hub into the catheter. Thus, the guide wire can be inserted into the catheter using the insertion / operation instrument as an inserter.
Next, when the guide wire is projected from the distal end of the catheter by a predetermined length, the insertion / operation instrument is moved to an appropriate position for operating the guide wire, and in that state, the cap is turned around the distal end of the main body. Screw in. Then, the diameter of the chuck portion is reduced and the outer periphery of the guide wire is tightened, so that the insertion / operation instrument can be fixed to the guide wire. Then, the operator can insert the guide wire into the tubular organ while grasping the insertion / operation instrument and performing the push-pull or rotation operation of the guide wire. Thus, since it is not necessary to use an inserter, it is possible to insert the guide wire into the catheter and attach the operation tool for operating the guide wire in a short time with good workability.
In the wire manipulator of the above-mentioned patent document 1, since the holding chuck member is inserted and set in the hole provided in the wire manipulating grip (gripping portion), it is viewed from the proximal end side of the grasping portion. Sometimes, there is a step between the clamping chuck member and the hole, and there is a problem that even if an attempt is made to insert the wire from the proximal end of the gripping part into the hole, it is caught in the step. It is thought that it is difficult to use it by a simple operation method.
According to a second aspect of the present invention, in the first aspect, when the guide tube is inserted into the hub, the foremost end of the guide tube abuts on the inner periphery of the reduced diameter hole portion, and the abutment position thereof. The guide wire insertion / operation instrument according to claim 1, wherein an inner diameter of the reduced-diameter hole portion is less than 2.5 times an outer diameter of the guide wire.
According to the above invention, when the leading end of the guide tube is inserted until it contacts the inner periphery of the reduced diameter hole of the hub, the inner diameter of the reduced diameter hole at the contact position is 2.5 times or less the outer diameter of the guide wire. Therefore, when the guide wire is protruded from the guide tube, there is no large gap between the guide wire and the inner diameter of the reduced diameter hole portion, and the guide wire can be smoothly guided to the catheter with less play. be able to.
According to the guide wire insertion / operation instrument of the present invention, the inner periphery of the insertion hole of the main body has no step where the guide wire is caught, and the tip of the cap has a hub provided at the proximal end of the catheter. Since a tapered guide tube that can be inserted into the reduced diameter hole is provided, the distal end of the guide tube is inserted into a hub provided at the proximal end of the catheter, and the distal end of the guide wire is inserted into the insertion hole from the rear end of the main body. Thus, the guide wire can be smoothly inserted into the catheter simply by continuing to push in as it is. When the guide wire has been inserted, the guide wire insertion / operation device can be securely attached to the guide wire simply by rotating the cap, thus completing the guide wire insertion / operation device installation work quickly and efficiently. can do. In addition, there is no need to insert a guide wire using a conventional inserter, and the insertion work of the inserter can be omitted. Therefore, the workability when inserting the guide wire into the catheter is further improved, and the burden on the operator is improved. Can be reduced.
Hereinafter, with reference to FIGS. 1-4, one Embodiment of the guide wire insertion and operation instrument of this invention is described.
The guide wire insertion /
Referring also to FIG. 2, the guide wire insertion /
The
Further, as shown in FIG. 2B, an
The
In the case of this embodiment, the inner diameter of the
Therefore, when the inner diameter of the
The
The
As shown in FIG. 3, when the leading end of the
A method of using the guide wire insertion /
First, the
And the front-end | tip of the
As described above, when the leading end of the
When the
Thus, according to the guide wire insertion /
Further, the insertion work of the
When the guide wire insertion /
FIG. 5 shows another embodiment of the guide wire insertion /
5A, the
Unlike the above-described embodiment, the guide wire insertion /
According to the present invention, a guide wire can be easily inserted into a catheter used for treatment of blood vessels and the like, and can be easily attached to the guide wire, and the operability of the guide wire can be improved.
