本発明は、音響聴診器と同等な音質で聴診音を増幅又は録音再生する電子聴診器に関する。

【従来の技術】
器官、特に心臓および肺からの音を聴いて病気を診断するために医師によって使用される音響聴診器は、図4に示すように、受音具100で捉えた音を導音管101、左右一対の副導音管102および左右一対のイヤーチップ103を経て使用者の左右の外耳道に導くよう構成されている。
しかし、音響聴診器の使用者が難聴、例えば老人性難聴のために聴力低下を来した場合、この種の音響聴診器を使用して、呼吸音などの微弱な音を聴取するのは困難である。このような問題を解決するために、図5に示すように、音響聴診器の受音具105にマイクロホン106を設け、このマイクロホン106の出力を増幅器107で電気的に増幅し、イヤホン108を介して装用者に聴取させる、いわゆる電子聴診器が提案されている。
また、聴診の教材とするため、或いは患者の記録のために受音具105に設けたマイクロホン106の出力を録音することも行われている。更に、複数の医師が同時に聴診を行って聴診の実習をするときに、一人が操作する受音具105のマイクロホン106の出力を分岐し、それを他の人が同時に聴取することもある。

しかし、この種の電子聴診器は、従来型の音響聴診器と音質が異なるため、従来型の音響聴診器を使用して学習をし、経験を積んだ使用者にとって違和感があり正確な診断ができないという問題点を有していた。即ち、従来型の音響聴診器においては、図4に示すように、受音具100が捉えた音が導音管101および左右一対の副導音管102が有する音響特性の影響を受けて装用者の外耳道に到達するのに対し、電子聴診器においては、図5に示すように、受音具105が捉えた音が直接マイクロホン106で電気信号に変換されるため、導音管101および左右一対の副導音管102が有する音響特性の影響を受けることがない。
それに加えて電子式聴診器ではイヤホン108から音が送出させるため、使用するイヤホン108固有の電気音響特性及びそれと使用者の耳の形状との相互作用により音質が変化する。これらが両者の音質に差が生じる主な原因となり、音響聴診器で経験を積んだ使用者が正確な診断を行うことを困難にしていた。

そこで、イヤーチップ内にマイクロホンとイヤホンを配置し、導音管及び副導音管を通じて到達した音をマイクロホンで集音し、それをイヤホンにより外耳道内に出力することにより音響聴診器の音質に近づける聴診器が開示されている(例えば、特許文献1参照)。
また、音響聴診器の伝達関数を模擬するフィルタを備え、複数の音響聴診器の伝達関数を測定してそれを模擬するフィルタ係数を記憶し、それを切り替えて使用する電子聴診器が開示されている(例えば、特許文献2参照)。

【発明が解決しようとする課題】
しかし、特許文献1に記載の発明では、導音管及び副導音管内の音の伝播特性はイヤーチップがマイクロホンで閉塞されることにより音響聴診器と同じにはならない。また、イヤホンから外耳道内に音を出力する際の電気音響特性も、導音管等を伝播した音が直接外耳道に導かれる場合とは異なっていた。
また、特許文献2に記載の発明では、使用するイヤホン固有の電気音響特性及びそれと使用者の耳の形状との相互作用による音質の変化を補正することができないため、音響聴診器と同等の音質を得ることは困難であった。また、音響聴診器の伝達関数をあらかじめ計測した上で、それを模擬するフィルタ係数を記憶させるのは手間のかかることであった。

本発明は、従来の技術が有するこのような問題点に鑑みてなされたものであり、その目的とするところは、使用するイヤホンと耳の形状に自動的に適応して、常に音響聴診器と同等な音質で聴診音を聴取できる電子聴診器を提供しようとするものである。

上記課題を解決すべく請求項1に係る発明は、受音具で集音された聴診音を信号処理した後に使用者の耳に装着されたイヤホンから出力させる電子聴診器であって、前記受音具で集音した聴診音を検出する第1マイクロホン又は前記受音具で受ける振動を検出する振動センサと、前記受音具に導音管を介して接続された擬似耳内に設置された第2マイクロホンと、前記イヤホンから使用者の外耳道内又は外耳道近傍に出力される音を検出する第3マイクロホンと、前記第1マイクロホン又は振動センサの出力信号を信号処理して前記イヤホンに出力する適応フィルタを備え、前記第3マイクロホンの出力信号が前記第2マイクロホンの出力信号に近似するように前記適応フィルタを適応制御するものである。

以下に本発明の実施の形態を添付図面に基づいて説明する。ここで、図1は本発明に係る電子聴診器の第1の実施の形態の構成図、図2は同じく第2の実施の形態の構成図、図3は同じく第3の実施の形態の構成図である。

本発明に係る電子聴診器の第1の実施の形態は、図1に示すように、受音具1内に設置され受音具1で集音した聴診音を検出する第1マイクロホン2と、受音具1に導音管3を介して接続された擬似耳4内に設置された第2マイクロホン5と、第1イヤーピース6内に設置された第1イヤホン7から使用者の外耳道内又は外耳道近傍に出力される音を検出する第3マイクロホン8と、第1マイクロホン2の出力信号を信号処理して第1イヤーピース6内に設置された第1イヤホン7及び第2イヤーピース9内に設置された第2イヤホン10に出力する適応フィルタ11を備えている。

使用者が第1イヤーピース6と第2イヤーピース9をそれぞれ左右の耳に装着し、受音具1を患者の体表に接触させると、受音具1で集音された聴診音は、第1マイクロホン2で電気信号に変換され、次いでA/D変換器16でデジタル信号に変換された後に適応フィルタ11で信号処理され、増幅器15で適度に増幅されて第1イヤーピース6内に設置された第1イヤホン7及び第2イヤーピース9内に設置された第2イヤホン10から出力される。
