【発明の属する技術分野】
本発明は経皮的血管形成術(Percutaneous Transluminal Angioplasty)に用いられるPTAカテーテル、経皮的冠状動脈血管形成術(Percutaneous Transluminal Coronary Angioplasty)に用いられるPTCAカテーテル等の血管内で使用される薬剤注入カテーテル及び薬剤注入カテーテル付きバルーンカテーテルの改良に関するものである。

【従来技術および発明が解決しようとする課題】
狭窄した血管の治療として、PTAカテーテルやPTCAバルーンカテーテルを用いて狭窄部を拡張する経皮的動脈血管形成術が行われている。またバルーンカテーテルで拡張した後、再狭窄を防ぐため、ステントの留置も日常的に行われている。しかし、ステント留置後もステント内の狭窄が起こり問題となっている。

ステント内の再狭窄の治療にバルーンに金属ブレードの付いたカッティングバルーンカテーテルが使用されるが、このカテーテルは、硬い病変に使用するように設計された特殊なカテーテルであり、汎用品でないため、高価である。また、ステント表面に薬剤をコーティングしたコーティングステントが開発されようとしているが、コーティングする薬剤量や放出される薬剤量等の問題があり、まだ実用化にいたってない。

例えば特許文献1には、導入針の先端を脈管壁の病巣部位に確実に密着させ薬剤等を確実に注入することができる脈管用薬剤注入カテーテルが記載されている。
当該薬剤注入カテーテルは、シャフト2とこれに着脱可能な導入針11から構成されている。
前記シャフト2は導入針11を挿入するディバイス導入ルーメン3を有するシャフト本体2HとガイドワイヤーGWを挿入する誘導ルーメン4Lを有する前方シャフト2Fから形成され、前記前方シャフト2Fと前記シャフト本体2Hは誘導壁5により区画され、前記誘導壁5近傍のシャフト本体2Hに側孔6を形成し、前記前方シャフト2Fには前記側孔6の反対側に開口部9を形成している。
前記導入針11は先端が屈曲した針部12とコネクター10bから形成されている。
前記誘導壁5の壁面5aは、ディバイス導入ルーメン3に導入針11を挿入して前方に押し込んだ時、該導入針11の針部12の先端が突き当たり、前記側孔6から突出できるような形状に形成し、前記ディバイス導入ルーメン3の側孔6の反対側に拡張部を配置し、該拡張部を拡張した時、前記側孔6から突出した導入針11の針部12の先端が、脈管壁に押し付けられ局所的に薬剤等が注入できるように形成されている。
また特許文献2には、先端を鋭利に形成した穿刺部材をカテーテルのシャフトチューブの側孔から、脈管壁に繰り出して薬剤の注入を行う発明が開示されている。
しかしながら特許文献1と特許文献2に記載の発明は、穿刺部材を誘導する専用のシャフトチューブが必要になり冠状動脈等の体の深部に位置する細く屈曲の多い血管ではディバイスが大きくなるため、目的とする部位に到達しにくく、また穿刺部材のコントロールが困難になり、穿刺部材の先端で穿刺するため穿刺箇所が1箇所になると共に、穿刺の深さを調整しにくい等の課題が指摘される。

【課題を解決するための手段】
そこで本発明者は以上の課題を解決するために鋭意検討を重ねた結果以下の発明に到達した。
[1]本発明は、本体(2、12、22、32)の途中に、薬剤放出部(3、13、23、33)を配置し、当該薬剤放出部(3、13、23、33)の側面に薬剤放出用の鋭利な突起(7、17、27、37)を形成した薬剤注入カテーテル(1、11、21、31)を提供する。
[2]本発明は、本体(2、12、22、32)の先端に円筒またはリング状のガイドワイヤー挿入部(4、14、24、34)を装着した[1]に記載の薬剤注入カテーテル(1、11、21、31)を提供する。
[3]本発明は、薬剤放出部(3、13、23、33)を保護するためのスリーブ(35)を装着した[1]ないし[2]に記載の薬剤注入カテーテル(1、11、21、31)を提供する。
[4]本発明は、本体(2、12、22、32)と薬剤放出部(3、13、23、33)に造影マーカー(36)を装着した[1]ないし[3]に記載の薬剤注入カテーテル(1、11、21、31)を提供する。
[5]本発明は、前記本体(2、12、22、32)、ガイドワイヤー挿入部(4、14、24、34)及び薬剤放出部(3、13、23、33)を金属で形成した[1]ないし[4]に記載の薬剤注入カテーテル(1、11、21、31)を提供する。
[6]本発明は、薬剤放出部(3、13、23、33)の側面に複数の突起(7、17、27、37)を形成した[1]ないし[5]に記載の薬剤注入カテーテル(1、11、21、31)を提供する。
[7]本発明は、バルーン(52、62)とシャフトチューブ(53、63)内にガイドワイヤーGWを挿入し、[1]ないし[6]に記載の薬剤注入カテーテル(1、11、21、31)の薬剤放出部(3、13、23、33)をバルーン(52、62)に装着した薬剤注入カテーテル付きバルーンカテーテル(51、61)を提供する。

【発明の実施形態】
図1から図4は本発明の薬剤注入カテーテルの一例を示す概略図、図5から図6は本発明の薬剤注入カテーテル付きバルーンカテーテル51、61の一例を示す概略図、図7は薬剤放出部3、13、23、33の拡大断面図である。
[基本構造]
本発明の薬剤注入カテーテル1、11、21、31は、本体2、12、22、32の先端にガイドワイヤー挿入部4、14、24、34、前方に薬剤注入部3、13、23、33、後端に注入用ハブ8、18、28、38をそれぞれ装着することにより構成される。

[本体2、12、22、32]
本体(注入用チューブともいう)2、12、22、32は、耐圧性が高くトルク性の高いものが良く、血管造影カテーテルに使用されるブレード入りのプラスチックチューブや金属製のチューブが使用するのが良い。
本体2、12、22、32の先端部と薬剤放出部3、13、23、33を連結する部分は血管を傷つけないようにガイドワイヤーのように柔軟に形成するのが良い。例えば先端部側を基端部側よりも弾性を有する材質または細径に形成する等により。
[薬剤放出部3、13、23、33]
薬剤放出部3、13、23、33は、図7に示すように側面に薬剤放出用の鋭利な突起7、17、27、37が突設され、当該突起7、17、27、37から薬剤が放出されるようになっている。

突起7、17、27、37とはノズルの先端を血管壁に穿刺できるように鋭利に形成したものである。突起の数、高さ、形状等は治療目的に応じて自由に設計変更することができる。突起の数は、一個よりも複数形成するほうが好ましい。

[ガイドワイヤー挿入部4、14、24、34]
既にバルーンカテーテル51、61に挿入しているガイドワイヤーGWに沿って本体2、12、22、32を挿入できるように、本体2、12、22、32の先端部に、ガイドワイヤー挿入部4、14、24、34を装着している。ガイドワイヤー挿入部4、14、24、34は、金属または合成樹脂製の円筒状パイプ(図3)、または楕円(図1)または真円状(図2)のリングを採用するのが良い。
これらガイドワイヤー挿入部4、14、24、34の形状は、複数のガイドワイヤー挿入部4、14、24、34が連結できる形状に形成するのが良い。
[スリーブ35]
また病変部に薬剤放出部3、13、23、33が届くまで、血管壁やバルーンカテーテル52、62を傷つけないように、薬剤放出部3、13、23、33を覆うようにスリーブ35を装着し、バルーン52、62の拡張時にスリーブ35をはずし薬剤放出部3、13、23、33を露出させることも可能である。

さらに本体2、12、32、33と薬剤放出部3、13、23、33に造影マーカー36を装着しても良い。造影用マーカー36を本体2、12、22、32の先端及び薬剤放出部3、13、23、33の突起7、17、27、37近傍に付けることによって、本体2、12、22、32と薬剤放出部3、13、23、33の位置を正確に知ることが可能となる。

[バルーンカテーテル51、61]
本発明のバルーンカテーテル51、61は、バルーン52、62とシャフトチューブ53、63内にガイドワイヤーGWを挿入し、前記ガイドワイヤー1、11、21、31の薬剤放出部3、13、23、33をバルーン52、62に装着している。
図5のように一本の薬剤注入カテーテル1、11、21、31、必要に応じて図6のように二本(複数本)の薬剤注入カテーテル1、11、21、31を同時に使用できるようにしている。
本発明の薬剤注入カテーテル1、11、21、31は、バルーンカテーテル51、61のバルーン52、62の外側に位置しバルーン52、62を拡張することにより、鋭利な薬剤放出部3、13、23、33を血管壁に穿刺して薬剤を注入用ハブ8、18、28、38から本体2、12、22、32、突起7、17、27、37を経て、薬液(薬剤)を血管壁に注入することができる。

【発明の作用効果】
(1)本発明の薬剤注入カテーテルは、専用の誘導カテーテル(シャフトチューブ)を必要とせず、ガイドワイヤーを伝わって容易に目的部位に到達でき、薬剤放出部の突起が側面方向に付いているため、突起の数や高さを容易に変えられ、さらに複数本の本発明の薬剤注入カテーテルを用いることによって同時に複数箇所の穿刺、薬剤投与が可能となる。また突起の高さが穿刺の深さとなるため、血管壁等の薄い部分でも安全に穿刺できる。
(2)ステント内の再狭窄を防止するため、拡張する場合、高価なカッティングバルーンカテーテルの使用や薬剤をコーティングしたコーティングステントが検討されているが、本発明の薬剤注入カテーテルをバルーンカテーテルに装着することによって、直接、血管壁に薬剤を注入することができ、より効果的な治療が可能となる。また局所的に遺伝子等の導入が可能となり新しい治療が開発できる。
