Abstract
【解決手段】上記課題は、肺ストレスを評価する方法において、圧力制御された呼吸ガスの気流を発生させ、後続の流量を測定し、圧力と流量とに基づいて抵抗とコンプライアンスを求め、圧力、流量、抵抗、及びコンプライアンスに基づいてストレス指数値を求めるようにすることにより解決される。

Description
特許文献1では、肺ストレス評価の方法は、定流量の吸気期間に為されるP−t測定に基づいている。特許文献1に開示されている方法の1つの実施形態では、吸気流量が一定であると仮定してつぎの関係式が用いられている。
Claims (9)
- 肺ストレスを評価する方法において、
圧力制御された呼吸ガスの気流を発生させ、後続の流量を測定し、圧力と流量とに基づいて抵抗とコンプライアンスを求め、圧力、流量、抵抗、及びコンプライアンスに基づいてストレス指数値を求めることを特徴とする肺ストレスを評価する方法。 - ストレス指数値を呼吸ごとに求める、請求項1または2記載の方法。
- ストレス指数値を複数の呼吸にわたる平均値に基づいて求める、請求項1または2記載の方法。
- 呼吸ガス圧を調整するための少なくとも1つのガス調整器(14,32)と、少なくとも1つの流量計(18,26)と、前記ガス調整器(14,32)を所定の制御パラメータに基づいて制御するための制御ユニット(34)とを備えた呼吸装置において、
前記制御ユニット(34)は請求項1から4のいずれか1項記載の方法に従ってストレス指数値を求めるように構成されている、ことを特徴とする呼吸装置。 - 前記制御ユニット(34)はさらに、求められたストレス指数値(b)を所定のインターバルと比較し、求められたストレス指数値(b)が前記所定のインターバル内にある場合には、被検者の肺に最小の肺ストレスが存在することを示し、求められたストレス指数値(b)が前記所定のインターバルを下回る場合には、肺胞の開閉に起因する肺ストレスが存在することを示し、求められたストレス指数値(b)が前記所定のインターバルを上回る場合には、肺胞の過拡張が存在することを示す、請求項5記載の呼吸装置。
- 求められたストレス指数値bに対する前記所定のインターバルは、0.5〜0.95の下限と1.05〜1.5の上限とから成る、請求項6記載の呼吸装置。
- 前記呼吸装置はさらに、該装置の動作に関する情報をオペレータに対して表示する表示ユニット(40)と警報ユニットとを有し、
前記制御ユニット(34)は、ストレス指数値(b)に基づいて、
前記警報ユニットにおいて警報を発する動作;
肺ストレスの存在を知らせる警告を前記表示ユニット(40)に表示する動作;
少なくとも1つの制御パラメータの変更を決定する動作;
決定された変更を前記表示ユニット(40)に表示する動作;
決定された変更に従って自動的に制御パラメータを再設定する動作;
漸増措置の勧告を前記表示ユニットに表示する動作;
自動的に漸増措置を実行する動作のうちの少なくとも1つの動作を実行するように構成されている、請求項5から7のいずれか1項記載の呼吸装置。 - 前記制御パラメータは、呼気終末陽圧(PEEP)、吸入酸素濃度(FiO2)、及び1回換気量(VT)のうちの1つに関係する、請求項8記載の呼吸装置。
