Abstract
Description
The present invention relates to a biological condition measuring device suitable for measuring the condition of a human body, particularly a pulse wave analyzer used for diagnosis of a circulatory system of the human body, and a biological device measured on a peripheral side of the human body. The present invention relates to a blood pressure measurement device for evaluating vascular resistance and compliance in a central part and a peripheral part, and estimating a central-side blood pressure based on the state of (1).
When diagnosing the condition of the circulatory system of the human body, blood pressure and heart rate are most commonly used. However, in order to make a more detailed diagnosis, it is necessary to measure so-called circulatory dynamic parameters such as viscosity resistance and compliance of blood vessels. When these parameters are modeled and represented, a lumped four-element model is used as a model that describes the behavior of the arterial system.
However, in order to measure such circulatory parameters, it is necessary to measure the pressure waveform and blood flow at the aortic root and the notch. That is, a method of directly measuring by inserting a catheter into an artery or a method of indirectly measuring by ultrasonic waves or the like is adopted. However, in the former method, since the measurement is invasive, the apparatus becomes large-scale. On the other hand, the latter method allows non-invasive observation of the blood flow in the blood vessel, but this method requires skill and also requires a large-scale measurement device. there were.
Therefore, the present inventor has found a method of approximately calculating the parameters of the four-element lumped parameter model only by measuring the pulse waveform of the radial artery and the stroke volume. By using this technique, a pulse wave analyzer capable of non-invasively and easily evaluating circulatory parameters has been proposed (JP-A-6-205747, title: pulse wave analyzer). ).
However, the above method does not employ a model that handles the compliance of blood vessels separately in the central part and the peripheral part of the arterial system. Therefore, when a circulatory agonist is administered to a patient during exercise or when a circulatory agonist is administered to a patient, the effect cannot be evaluated separately for the central part and the peripheral part.
Next, the measurement of blood pressure described above will be briefly described.
Conventional non-invasive blood pressure measuring devices are generally used. A cuff (arm band) is attached to a subject's upper arm or the like, and pressure is applied to the cuff to detect a subject's pulse wave, thereby detecting a blood pressure value. Is measured. As described above, as an apparatus for measuring the blood pressure in the peripheral part of a subject, for example, Japanese Patent Application Laid-Open No. 4-276234 is cited. In other words, as shown in FIG. 29, the cuff 110 is wound around the upper arm of the subject's upper limb and attached, the
By the way, when the central blood pressure value and the peripheral blood pressure value in the arterial system of the human body are actually measured, there is a difference between the central blood pressure value and the peripheral blood pressure value, especially regarding the systolic blood pressure value. Moreover, the degree of the difference varies depending on the shape of the pulse wave observed on the peripheral side. FIGS. 22 to 24 illustrate how the blood pressure value varies depending on the shape of the pulse wave. These figures show the aortic pressure waveform and systolic / diastolic values on the central side, and the radial artery pressure waveform and systolic / diastolic values on the peripheral side.
It is a pulse wave waveform of the first type shown in FIG. 22, and the respective systolic blood pressure values obtained from the aortic pressure waveform shown by the dotted line and the radial artery waveform shown by the solid line are almost equal to each other although the radial artery side is slightly higher. good. However, in the pulse waveform of the second type shown in FIG. 23, the systolic blood pressure difference is 14.9 mmHg, which is considerably larger than that in FIG. Further, when the pulse waveform of the third type shown in FIG. 24 is obtained, the systolic blood pressure difference becomes even larger at 26.1 mmHg, and in contrast to the pulse waveforms of the first and second types, the aortic pressure waveform becomes Is much larger than the radial artery waveform as a whole. Incidentally, from these figures, it can be said that the diastolic blood pressure value on the radial artery side is substantially the same irrespective of the shape of the pulse.
Here, the above-described first to third types of pulse waves will be briefly described.
The first type of pulse waveform is a pulse of a normal healthy person, and is characterized by a slow and relaxed waveform, a constant rhythm and little disturbance. The second type of pulse wave waveform is characterized in that it rapidly rises, then falls immediately, and the aortic notch is deeply cut, while the subsequent diastolic peak is considerably higher than usual. Further, the third type of pulse wave waveform is characterized in that it rapidly rises, does not immediately fall thereafter, and maintains a high blood pressure state for a certain period of time.
It can be deduced from these figures that the blood pressure value in the aortic root, that is, the central side, may be low even if the blood pressure value in the peripheral side such as the radius and the upper arm is high, and conversely, the blood pressure value in the peripheral side may be low. The central blood pressure value may be high even if the blood pressure value on the central side is low. Such a relationship differs depending on the shape of the pulse wave waveform, and these relationships clearly appear in the shape of the pulse wave waveform.
For example, suppose that a hypotensive agent is administered to a patient for the treatment of hypertension, and the effect of the drug is evaluated based on the blood pressure in the radial artery. In such a case, even if the blood pressure measured on the peripheral side decreases, the blood pressure on the central side may not actually decrease. Therefore, it can be said that there is a case where it is difficult to correctly grasp the medicinal effect only from the peripheral blood pressure. On the contrary, even if the peripheral blood pressure does not change, if the aortic pressure waveform changes and the central blood pressure decreases, the burden on the heart is actually reduced. That is. In such a case, the effect of the drug is sufficiently manifested without forcibly lowering the peripheral blood pressure, but it is difficult to judge this from only the peripheral blood pressure.
In this way, the measurement of blood pressure is originally correct to see how much the burden on the heart is, and if it is judged from the blood pressure value measured on the peripheral side as conventionally performed, The burden on the heart may be overestimated, or on the contrary, underestimated.
The present invention has been made in view of the above points, and a first object of the present invention is to provide a device capable of non-invasively evaluating circulatory parameters, and particularly to a device for vascular compliance and vascular resistance. It is an object of the present invention to provide a pulse wave analyzer capable of separately evaluating a central part and a peripheral part of the arterial system. It is a second object of the present invention to provide a blood pressure measurement device capable of obtaining a central blood pressure value from a pulse wave waveform measured on a peripheral side.
In a first aspect of the present invention, a measuring means for measuring a state of a living body, and a circulating dynamic parameter representing a circulating dynamic of an arterial system from a central part to a peripheral part of the living body based on the state of the living body are provided. Analyzing means for calculating circulatory parameters including viscoelasticity of the aorta. As a result, the circulatory dynamics parameters including the viscoelasticity of the aorta in the arterial system of the living body were calculated, so that the circulating dynamics of the arterial system from the central part to the peripheral part of the living body were divided into the central part and the peripheral part, The effect of being able to evaluate precisely is obtained.
In one embodiment, the circulatory dynamics of the living artery are simulated by an electric circuit based on a lumped five-element model to determine the circulatory dynamics parameters. As a result, an effect is obtained that the circulatory dynamic parameters more suitable for the state of the human body can be easily calculated as compared with the four-element lumped parameter model.
In another embodiment, a pulse wave in the peripheral part of the arterial system is used as the state of the living body, and when the waveform of the left ventricular pressure of the living body is given to the five-element lumped parameter model, the waveform of the pulse wave becomes Since each element constituting the model is obtained as described above, it is possible to obtain an effect that it is possible to determine a circulatory dynamic parameter well suited to a peripheral pulse wave actually measured from a living body.
Further, in another embodiment, the electric signal corresponding to the left ventricular pressure is approximated by a sine wave, so that the circulatory kinetic parameters that more accurately represent the actual state of the central part of the arterial system of the living body are determined. The effect that can be obtained is obtained.
Further, according to a second aspect of the present invention, a measuring means for measuring a state of a living body in a peripheral part of an arterial system, and a circulating dynamic parameter representing a circulating dynamic state of the arterial system are determined from the state of the living body. Blood pressure calculating means for calculating the aortic pressure waveform of the living body based on dynamic parameters. As described above, according to the present invention, the aortic pressure waveform is calculated from the state of the living body in the peripheral portion, so that an effect is obtained that the blood pressure value of the aortic portion can be estimated only from the peripheral state. Thereby, for example, there is no danger of making an erroneous judgment that the administered drug has no effect unless the blood pressure measured on the peripheral side changes, and it is possible to correctly determine the medicinal effect of the administered drug, etc. It also has the effect of being very useful for drug selection.
Further, in one embodiment, since the parameters including the viscoelasticity of the aorta in the arterial system of the living body are selected as the circulatory dynamic parameters, the circulatory dynamics of the arterial system can be evaluated separately for the central part and the peripheral part, The effect is obtained that the blood pressure can be estimated more precisely.
Further, in another embodiment, the circulatory dynamics of the arterial system of the living body is simulated by an electric circuit based on a five-element lumped constant model to determine the circulatory dynamics parameters. The effect of obtaining the blood pressure in the aorta in a manner more suited to the condition of the human body is obtained.
In another embodiment, a pulse wave in the peripheral part of the arterial system is used as the state of the living body, and when the waveform of the left ventricular pressure of the living body is given to the five-element lumped parameter model, the waveform of the pulse wave becomes As it is obtained, since each element constituting the model is obtained, the effect of being able to determine the blood pressure of the aorta using the circulatory dynamic parameters well suited to the peripheral pulse wave actually measured from the living body is obtained. can get.
In another embodiment, the blood flow in the aorta is estimated by determining the circulatory parameters without detecting the stroke volume, so that the subject does not have to feel troublesome in measuring the blood pressure. The effect is obtained.
In another embodiment, the value of the circulatory dynamic parameter is adjusted such that the calculated stroke volume obtained from the aortic pressure waveform matches the actual stroke volume measured from the living body. As a result, an effect is obtained that the blood pressure in the aorta can be more accurately estimated.
Further, in another embodiment, since the work of the heart is calculated based on the aortic pressure waveform, even when a remarkable change is not seen in the blood pressure value obtained in the peripheral part of the arterial system. Thus, it is possible to quantitatively show the true burden on the heart, and it is possible to obtain an effect that the evaluation of antihypertensive therapy or the like can be performed more finely.
<First embodiment>
Hereinafter, a first embodiment of the present invention will be described with reference to the drawings.
FIG. 1 is a block diagram showing a configuration of the blood pressure measurement device according to the embodiment. In the present embodiment, the circulatory parameters of the arterial system of the human body are evaluated based on information obtained from the human body by a non-invasive sensor. The specific contents of the circulatory parameters will be described later.

# 67 denotes a CRT display, which displays various waveforms such as a radial artery waveform, a left ventricular pressure waveform, and an aortic pressure waveform.
Here, the significance of performing a warning display on the
As described above with reference to FIGS. 22 to 24, there are three types of the systolic blood pressure difference between the estimated aortic pressure waveform and the radial artery waveform. Then, the subject whose pulse wave waveform is the first type (FIG. 22) is likely to be a healthy person, and in the case of the second and third types, the subject often has some kind of disease.
For example, the second type (FIG. 23) is caused by abnormal blood flow conditions, and is likely to have diseases such as edema, hepatic renal disease, respiratory disease, gastrointestinal disease, and inflammatory disease. The third type is caused by an increase in the tension of the blood vessel wall, and is likely to have hepatobiliary disease, skin disease, hypertension, painful disease and the like. Therefore, in the present embodiment, when the systolic blood pressure difference is considered to be abnormal, a red LED is turned on to display a warning.
In the above example, the diagnosis is performed based on the systolic blood pressure difference between the aortic pressure waveform and the radial artery waveform, but the diastolic blood pressure difference or the average blood pressure difference may be used instead of the systolic blood pressure difference. Further, it goes without saying that the diagnosis may be made using all of the systolic blood pressure difference, the diastolic blood pressure difference and the average blood pressure difference.
Next, in this embodiment, a “five-element lumped parameter model” is newly adopted as an electrical model of the arterial system. In this five-element lumped parameter model, among the factors that determine the behavior of the circulatory system of the human body, the four-element lumped parameter model disclosed in Japanese Patent Application Laid-Open No. 6-205747 (title: pulse wave analyzer) is used. Inertia due to blood in the central part, vascular resistance due to blood viscosity in the central part (viscous resistance), compliance of blood vessels in the peripheral part (viscoelasticity), vascular resistance in the peripheral part (viscous resistance) Then, aortic compliance was imposed, and these parameters were modeled as an electric circuit. Note that compliance is a quantity representing the softness of a blood vessel.

FIG. 3 (a) shows a circuit diagram of a four-element lumped parameter model, and FIG. 3 (b) shows a circuit diagram of a five-element lumped parameter model. Hereinafter, the correspondence between each element and each parameter constituting the five-element lumped parameter model will be described.

Here, the current flowing through each part of the electrical circuit i, i p, i c, i s is equivalent to a blood flow flowing through the respective units, each corresponding [cm 3 / s]. Among them, the current i is the aortic flow, current i s is the blood flow pumped out from the left ventricle. The input voltage e corresponds to the left ventricular pressure [dyn / cm 2 ], and the voltage v 1 corresponds to the pressure at the start of the aorta [dyn / cm 2 ]. Further, the terminal voltage v p of the capacitance C corresponds to the pressure [dyn / cm 2 ] at the radial artery. In addition, the diode D shown in FIG. 3B corresponds to the aortic valve, and is turned on (in a state where the valve is open) during a period corresponding to the systole, and is turned off (valve opened) during the period corresponding to the diastole. Is closed).
As described later, in the present embodiment, rather than would calculate these five parameters at a time, using a four element concentration constant model disclosed the parameters except capacitance C c in the aforementioned literature After the calculation, the capacitance Cc is determined. Therefore, first, the behavior of the four-element lumped parameter model shown in FIG. 3A will be theoretically described.
υp=exp(st) (5)
υ p = exp (st) (5)
Substituting equation (5) into equation (4), equation (4) is transformed as follows.
Section of 0 ≦ t < tp1 :
Then, a response waveform v p (that is, a radial artery wave) when the voltage v 1 represented by the equations (17) and (18) is input to the equivalent circuit of FIG. 3A is as follows.
Section of 0 ≦ t < tp1 :
Here, E min is the lowest blood pressure value in the radial artery waveform measured by the pulse wave detection device 1 (see FIG. 11 described later).
The third term on the right-hand side in equation (19) and the second term on the right-hand side in equation (20) are the damped oscillation components of equation (5), and α and ω in these terms are expressed by equations (15) and ( 16). Note that B, t b , D m1 , and D m2 are constant values calculated according to a procedure described later.
First, E min is obtained from equation (27) as follows.
a.式(19)のvp(tp1)と式(20)のvp(tp1)とが一致すること
a. V p (t p1) and v p (t p1) that a match b of formula (20) in equation (19). V p (t p ) in equation (20) matches v p (0) in equation (19) c. The differential coefficient of t = t p1 in equation (19) and equation (20) matches d. A differential coefficient at t = 0 of the formula (19), that the differential coefficient at t = t p of formula (20) coincide i.e., D m1 and theta 1 is
D m2 and θ 2 are
In this way, the constants of Expressions (19) and (20) were obtained.
Generally, R p is about 10 3 [dyn · s / cm 5 ], C is about 10 -4 [cm 5 / dyn], and ω is the angular frequency of the vibration component superimposed on the pulse wave. (Rad / s) or more. Therefore, the lower limit of Expression (40) can be regarded as approximately 1 / (ω 2 C). Therefore, for simplicity, L is approximately
In addition, from the relationship between Expressions (41) and (42), the attenuation constant α in Expression (15) is
First, the average value E 01 of the pressure wave in the aortic root is given by the following equation.
Next, an implicit explanation of a method for calculating circulatory dynamic parameters based on the five-element lumped parameter model will be given. As mentioned above, R c , R p , C, and L among the circulatory dynamic parameters are determined using a four-element lumped model, and based on these parameters, the capacitance C c is determined. Determine the value. Therefore, a certain current i, current i s, the voltage v 1, is necessary to obtain the voltage v p and the like in FIG. 3 (b).
Hereinafter, as shown in FIG. 4, the time t is to be described with a case divided into diastole t 1 ≦ t <t 2 systolic and t 2 ≦ t <(t p + t 1). Here, time t 1 and time t 2 are times at the intersection of the left ventricular pressure waveform and the aortic pressure waveform.
In this case, v 1 = e holds, and equations (1) and (2) hold for the voltage v 1 and the current i, respectively. Therefore, the following differential equation is established from Expressions (1) to (3) and Expressions (12) to (14) and Expression (51).
Eventually, equation (65) becomes:
Then, as the current i,
In addition, the current i s is,
In the diastole, the diode D is turned off, the left ventricular pressure e is not applied to the circuit on the cathode side of the diode D, and the current flowing through the capacitance C C is equal to the current i, and Current. Accordingly, the voltage v 1 is represented by the above-described equation (1), and the current i and the current ic are respectively represented by the following equations.
Then, the voltage v p is further assumed as in the following equation.
Next, the operation of the pulse wave analyzer according to the present embodiment will be described. 6 to 10 are flowcharts illustrating the operation of the pulse wave analyzer according to the present embodiment. FIG. 11 is a waveform diagram of an average waveform obtained by the above-described averaging process. FIG. 12 is a waveform diagram comparing a radial artery waveform obtained by a parameter calculation process described later with an average waveform obtained by an averaging process. The operation will be described below with reference to these drawings.

(4) the parameter calculation process, first, on the basis of a four-element lumped model, the determination of four circulatory state parameter excluding capacitance C c.
Thus, the processing of the
Hereinafter, the processing contents of these routines will be described. First, the
Next, in order to simplify the process, performs the normalization process of the blood pressure values y 1 ~y 3 with a blood pressure value y 0 of the point A shown in FIG. 13 (step S102, S103), the value of point B Initially, (y 0 /2)−0.1 is set (step S104).
Next, the optimum values of B, t b , α, and ω are determined according to the following procedure.
a) First, B a "(y 0/2) ~y 0" is changed in a range of, at the same time, changing the t b in the range of "(t p / 2) ~t p". At this time, B and t b are both changed at +0.1 intervals. Then, for each of B and t b , | v p (t 1 ') -y 1 |, | v p (t 2 ') -y 2 |, | v p (t 3 ') -y 3 | Α and ω are obtained.
b) Among B, t b , α, and ω obtained in a), | v p (t 1 ′) −y 1 |, | v p (t 2 ′) −y 2 |, | v p (t 3 ') Find B, t b , α, ω that minimizes −y 3 |.
c) b) in the obtained B, and based on t b, B ± 0.05 for B, and the range of t b ± 0.05 for t b, above a), the process b) re Execute.
d) In the above processes a) to c), α is changed in the range of 3 to 10 at intervals of 0.1, and the optimum ω is calculated for each α. Further, ω is obtained by using the bisection method at the point where dv p (t 2 ′) / dt = 0 at each α (see the flowchart of FIG. 10). In calculating the value of v p in each of the above processes, the initial value v 01 of equation (33) is set to zero.
By the above processing, B, t b , α, ω are finally determined.
e) t p1, E m, E o the formula (28) to (30), is calculated based on the equation (44) to (46) (step S123, S124).
f) Using equation (50), the value of L is calculated based on the measured stroke volume SV (step S125), and the remaining parameters R C , R p , and C are calculated using equations (44) to (44). It is obtained from (46) (step S126).
Then, based on the five elements lumped model to determine the capacitance C C is the last circulatory state parameters. At that time, as a method of determining the electrostatic capacitance C c so measured values and calculated values of stroke volume SV matches, diastolic blood pressure calculation pulse wave and the diastolic blood pressure measured pulse wave coincides A method of determining the capacitance Cc is considered. Therefore, each method will be described separately for each case.
First, the value of the capacitance C C is estimated as in the following equation based on the capacitance C calculated by the four-element lumped constant model. The values of the other circulatory dynamic parameters, ie, R C , R P , C, and L, obtained by the four-element lumped model are used.
Next, stroke volume SV values calculated in the manner described above, to determine the value of the capacitance C C to match the measurements taken from the
On the other hand, if the measured value of the stroke volume and the calculated value do not agree with each other only by adjusting the value of the capacitance C C , one time in the adjusted value of the capacitance C C. The value of the capacitance C C at which the difference between the measured value of the stroke volume and the calculated value is the minimum is defined as the final value. Then, by changing each 1mmHg values of systolic blood pressure E m 'in the range of ± 3 mmHg, check for agreement between the calculated values and measured values of stroke volume as well. If there is a matching systolic blood pressure Em ', the value is set as the final systolic blood pressure Em ', and the parameter calculation process ends.
On the other hand, even if adjusting the value of systolic blood pressure E m ', when not seen yet match the calculated and measured values of stroke volume is adjusted further the value of the resistor R p. Therefore, 'in the value of the highest blood pressure value E m the difference between the calculated and measured values of stroke volume is the smallest' systolic blood pressure E m adjusted to the value of the final value . Next, the resistance R p is increased or decreased in steps of, for example, 10 [dyn · s / cm 5 ], and the value having the smallest difference between the measured value of the stroke volume and the calculated value is determined as the final value of the resistance R p . decide.
FIG. 31 shows an example of a flowchart for realizing the process described above. Note that, for parameters that fluctuate within a predetermined range in the program, a subscript “v” is added to the original parameter name.

Next, the diastolic blood pressure of the calculated pulse wave and the diastolic blood pressure of the actually measured pulse wave should be matched. It describes a method of determining the electrostatic capacitance C c.
In this method, it is assumed that the systolic time QT has been obtained in advance from the subject's electrocardiogram. With respect to the systolic time QT obtained in advance, the left ventricular pressurization time t s v is set to “0.01 [sec]” in the range of “QT + 0.1 [sec]” to “QT + 0.2 [sec]”. varied in the interval, changing the "1mmHg" interval the systolic blood pressure E m v 'in the range of "E o + E m -20 [mmHg]" - "E o + E m +20 [mmHg]" at the same time.
That is, for each of these left ventricular pressurizing time t s v and systolic E m v ', so that the combination of 451 kinds are contemplated. In each of these combinations, the capacitance Cc is calculated such that the diastolic blood pressure of the calculated pulse wave matches the diastolic blood pressure of the actually measured pulse wave.

(6) Second Output Processing Further, in step S4, an aortic pressure waveform is obtained based on the values of the circulatory dynamic parameters L, C, C C , R C , R P and the like. That is, in the systole using equation (51), by using the formula In the diastole (140), one beat waveforms of voltages v 1 (i.e.,
As described above, according to the present embodiment, the central systolic blood pressure, the diastolic blood pressure, and the aortic pressure waveform, as well as various circulatory parameters, can be displayed to the diagnostician or the subject. Since the equation (51) shows the left ventricular pressure waveform itself, the left ventricular pressure waveform may be displayed on the
<Second embodiment>
In the first embodiment, each value of the circulatory dynamic parameter is calculated from the radial artery waveform and the stroke volume. However, as described above, since the subject needs to wear the cuff band S1 to detect the stroke volume once, it can be said that the subject is troublesome. Therefore, in the present embodiment, the central blood pressure value and the like are estimated by noting the phenomenon that the aortic pressure changes depending on the shape of the radial artery waveform, and representing the waveform shape as a distortion factor. That is, in the present embodiment, the circulatory dynamic parameters are derived based on the distortion rate d obtained from the radial artery waveform.
Next, a circulatory dynamic parameter is estimated from the obtained distortion factor d. The estimation is performed based on the finding that there is a considerable degree of correlation between the distortion rate of the radial artery waveform and each value of the circulatory dynamic parameters. That is, the distortion rate d and the circulatory dynamic parameters are measured in advance for a large number of subjects, and a relational expression between the distortion rate and each of the circulatory dynamic parameters is derived. Here, an example of the correlation between the distortion factor d and the measurement results of the circulation dynamic parameters R C , R P , L, and C is shown in FIGS. 25 to 28. Although not shown with respect to aortic compliance C C, it can be obtained with equation other four parameters as well as the correlation coefficient.
Then, the circulatory dynamic parameters R C , R P , L, C, and C C are calculated based on the distortion rate d calculated by the above equation (153) and the relational expressions shown in FIGS. Then, in the same manner as in the output processing of (5) and (6) in the first embodiment, a waveform for one beat of the aortic pressure waveform is obtained from the calculated circulatory dynamic parameters, and the diastolic blood pressure value at the aortic root is calculated. Eo and systolic blood pressure value Em are calculated and displayed on
<Third embodiment>
In the present embodiment, in addition to the systolic blood pressure value and the diastolic blood pressure value at the aortic root, in addition to the blood pressure waveform of the aortic root determined as described above, the heart work (hereinafter referred to as cardiac work) is obtained. This is calculated and displayed. The cardiac work is one index representing the burden on the heart, and is defined by the product of the stroke volume and the aortic pressure, and is obtained by converting the cardiac output per minute to the work volume. Here, the stroke volume is defined by the blood flow volume sent out from the heart in one beat, and corresponds to the area of the blood flow waveform coming out of the heart. The stroke volume has a correlation with the area of the aortic pressure waveform during the systole, and the stroke volume can be determined by applying the systolic area method to the aortic pressure waveform.
That is, first, the area S of the pulse waveform corresponding to the systole of the heart is calculated. This will be described with reference to the pulse wave waveform in FIG. 29. The area of the region from the rising portion of the pulse wave to the depression (notch), that is, the hatched portion in FIG. Next, assuming that a predetermined constant is K, the stroke volume SV can be calculated by the following equation.
Stroke volume SV [ml] = area S [mmHg · s] × constant K
On the other hand, the cardiac output is defined by the blood flow sent from the heart in one minute. Therefore, the cardiac output is obtained by converting the stroke volume to one minute. That is, the cardiac output is determined by the product of the stroke volume and the heart rate.
In the present embodiment, in the output processing of (5) of the first embodiment or the second embodiment, the
First, when the W s defined in e · i s, which is the formula (51), equation (88), is calculated from equation (89) as follows.
That is,
Then, the equation (157) becomes the following equation.
The meaning of displaying the cardiac work as described above in addition to the systolic blood pressure value and the diastolic blood pressure value at the aortic root is as follows.
That is, in the conventional blood pressure measurement device, the blood pressure is measured on the peripheral side such as the radial artery and the upper arm, and it can be said that this is a method of indirectly measuring the burden on the heart. However, the change in the burden on the heart is not always reflected in the blood pressure on the peripheral side, and it is not always accurate to see the burden on the heart on the peripheral side. For this reason, the present invention pays attention to the fact that the blood pressure waveform at the central part is particularly important in observing the burden on the heart, and the blood pressure waveform at the origin of the aorta (the central part of the arterial system) is located on the peripheral side. It is determined by estimating from the measured pulse waveform. Then, if the systolic blood pressure value and the diastolic blood pressure value at the aortic root are calculated from the estimated aortic pressure waveform, these blood pressure values can be indexes directly representing the burden on the heart.
Further, by obtaining the above-mentioned cardiac work based on the aortic pressure waveform, a useful index different from the systolic blood pressure value and the minimum blood value value at the aortic root is provided as an index representing the burden on the heart. It is also possible. Therefore, the usefulness of the amount of cardiac work will be described below with reference to examples.
Now, consider a case in which a hypertensive is treated by administering a hypotensive agent to a patient. Normally, if the drug is effective, the systolic blood pressure value and the diastolic blood pressure value measured in the radial artery change, and the effect of the drug can be confirmed. However, even when the systolic blood pressure value and the diastolic blood pressure value do not change, the medicine is actually effective, and the load on the heart itself may be reduced. This is because the role of the antihypertensive agent is to reduce the load on the heart somewhere in the arterial system, and the blood pressure in the radial artery does not necessarily need to be reduced. In this manner, even when a remarkable change is not seen in the blood pressure value in the peripheral part of the arterial system such as the radial artery, by calculating the cardiac work obtained from the blood pressure waveform of the aortic root, It is possible to know the true burden of the heart.
By the way, such changes in the burden on the heart can be found by examining the blood pressure waveform at the origin of the aorta in detail, but by calculating the amount of cardiac work, the subtle changes in the waveform can be quantitatively determined. Be able to express. Therefore, by calculating and displaying not only the systolic blood pressure value and the diastolic blood pressure value but also the amount of cardiac work, it is possible to more precisely evaluate antihypertensive therapy. The results of calculating the amount of cardiac work for each of the above-described first to third types of pulse wave shapes are shown in FIGS.
<Modified example>
The present invention is not limited to the embodiments described above, and various modifications are possible, for example, as follows. For example, a form in which a circulatory dynamic parameter is obtained without measuring the stroke volume SV is also conceivable.
That is, according to this embodiment, the inductance L among the circulatory parameters is a fixed value, and the values of the other circulatory parameters are calculated based only on the waveform of the radial artery pulse wave measured from the subject. To do. In this way, the stroke
By the way, if the value of the inductance L is fixed in this way, the accuracy of the obtained circulatory dynamic parameters is lower than the method using the actually measured stroke volume. To compensate for this, as shown in FIG. 17, the radial artery waveform (measured waveform) W1 obtained by measurement and the radial artery waveform (calculated waveform) W2 obtained by calculation are superimposed and displayed on the
As a model of the pressure waveform at the aortic root, a trapezoidal wave may be used instead of the above-described triangular wave. By doing so, the waveform becomes closer to the actual pressure waveform as compared with the case where it is approximated by a triangular wave, so that more accurate circulatory dynamic parameters can be calculated.
Further, the measurement positions of the pulse wave and the stroke volume are not limited to the positions shown in FIGS. 2 and 16, but may be any parts of the subject's body. That is, in the above-described embodiment, the measurement mode is such that the cuff band S1 is attached to the upper arm of the subject. However, considering the convenience of the subject, it is preferable that the cuff band is not used.
As an example, a form in which both the radial artery waveform and the stroke volume are measured at the wrist can be considered. As an example of this type of configuration, as shown in FIG. 18, a
Further, a form in which a pulse wave and a stroke volume are measured with a finger is also conceivable. FIG. 19 shows a configuration example of an apparatus according to this form. As shown in the figure, a
Furthermore, by combining these two measurement forms, a form in which the stroke volume is measured at the wrist and a pulse wave is measured at the finger, a stroke volume at the finger is measured, and a radial artery wave is measured at the wrist. It becomes possible to realize the form which does.
The configuration without the cuff band as described above eliminates the need for the subject to roll his arms, and reduces the burden on the subject in measurement.
On the other hand, a configuration shown in FIG. 20 is considered as a mode using only the cuff band. As shown in the figure, a sensor 32 composed of a sensor for measuring blood pressure and a sensor for measuring stroke volume, and a
In the above embodiment, the pulse wave is used to calculate the circulatory dynamic parameters. However, the present invention is not limited to this, and it goes without saying that other biological conditions can be used.
