本発明は、不定形骨補填材に関し、より詳細には、リン酸カルシウム系化合物を主成分とし、人工骨、人工歯、骨類の補填等に好適に使用することができる不定形骨補填材に関する。

【従来の技術】
近年、歯科、脳外科、形成外科、整形外科等の分野において、人工骨、人工歯、骨類の補填等に用いられる材料(以下、「不定形骨補填材」という)に関する研究が盛んに行われるようになってきている。
例えば、生体内に人工骨等の不定形骨補填材を埋入する際には、該不定形骨補填材は、毒性、生体為害性がなく、かつ、十分な機械強度を有しており、しかも、生体組織に対する親和性に優れ、骨組織の細胞や血管組織と結合しやすい材料であることが要求される。さらに、生体内において、組織に吸収される等によって自然に消滅し、新生骨と経時的に置換されるものであることが好ましい。

上記のような不定形骨補填材の材料としては、これまでに、リン酸三カルシウム、ハイドロキシアパタイト等のリン酸カルシウム系化合物からなるものが種々提案され、また、使用されている。
このようなリン酸カルシウム系化合物からなる骨補填材の中でも、気孔がほとんど存在しない緻密質のものは、その緻密で強固な構造のため、生体内での骨組織の侵入および形成が速やかに行われず、治癒するまでに長期間を要する。
そのため、生体組織と結合しやすい、すなわち、開気孔内に骨組織が侵入しやすいという観点から、骨補填材としては、多孔質のリン酸カルシウム系化合物が、好適に用いられている。

従来、上記のようなリン酸カルシウム系化合物からなる骨補填材の形態としては、機械加工等による成形品、粉砕された顆粒品、ペースト状の製品等が使用されている。

【発明が解決しようとする課題】
しかしながら、成形品や顆粒品は使用の際の初期固定が難しい。すなわち、成形品は、補填部位に適合するような精巧な加工が必要となる。また、顆粒品は、補填部位の形態に関わらず、必要量を補填することができるが、生体内において脱落してしまうおそれがある。
一方、ペースト状の製品は、微粒の粉体が主成分であるため、生体内で硬化させた後は、緻密質の固化体となる。このため、骨芽細胞等が侵入し難く、大きな欠損部位においては、自家骨に置換するのに長時間を要する。

したがって、骨補填材の性状として、使用時に、所望の形状に自在に成形することができる可塑性を有しており、成形直後は、その形状を保持することができる程度の自立強度および適度の弾性を有していることが好ましい。
さらに、生体内に固定された後は、十分な強度を有するとともに、生体組織との親和性に優れ、かつ、骨芽細胞等が侵入しやすく、経時的に生体内に吸収されて自家骨に置換されやすいことも要求される。

本発明は、上記技術的課題を解決するためになされたものであり、生体組織との親和性に優れ、大きな部位においても、任意の形状で補填成形することができ、しかも、経時的に生体内に吸収されて自家骨に置換されやすい性状を有する不定形骨補填材を提供することができる。

【課題を解決するための手段】
本発明に係る不定形骨補填材は、リン酸カルシウム系化合物の多孔質粒状体と生体内分解吸収物質の粉体と増粘剤とリン酸セメントとが配合されている骨補填材において、前記リン酸カルシウム系化合物の多孔質粒状体と生体内分解吸収物質の粉体との合計配合量100重量部のうち、リン酸カルシウム系化合物の多孔質粒状体が20重量部以上90重量部以下、その残部が生体内分解吸収物質の粉体であり、前記リン酸カルシウム系化合物の多孔質粒状体と生体内分解吸収物質の粉体との合計配合量100重量部に対して、増粘剤は0.01重量部以上5重量部以下、リン酸セメントは5重量部以上50重量部以下配合されていることを特徴とする。
このような構成からなる不定形骨補填材は、容易に成形加工することができ、任意の形状に自在に成形することができ、しかも、生体内に固定された後は、迅速に硬化して、十分な強度を有し、脱落等も生じ難いことから、大きい欠損部位の補填や、複雑な形状の骨格部分の補綴にも好適に用いることができる。

前記不定形骨補填材においては、前記リン酸カルシウム系化合物の多孔質粒状体は、CaHPO4、Ca(H2PO4)2、Ca5(PO4)3OH、Ca4O(PO4)2、Ca10(PO4)6(OH)2、Ca3(PO4)2、CaP4O11、Ca(PO3)2、Ca2P2O7、Ca(H2PO4)2・H2Oから選ばれた少なくとも1種を主成分とするセラミックスからなることが好ましい。
このようなリン酸カルシウム系化合物からなる多孔質粒状体は、生体組織との親和性に優れているため、経時的に生体内に吸収されて、自家骨に置換されやすく、基材として好適である。

また、前記多孔質粒状体は、多孔質ブロックの粉砕物からなり、前記多孔質ブロックは、連球状開気孔を有しており、気孔率が55%以上90%以下、球状気孔の孔径が150μm以上500μm以下、前記球状気孔間の連通部分の平均孔径が50μm以上400μm以下であり、かつ、前記連通部分の孔径が10μm以上である気孔が全気孔体積の40%以上であることが好ましい。
前記リン酸カルシウム系化合物の多孔質粒状体は、このような多孔質構造を有していることが、基材内に骨芽細胞等の侵入、定着をより一層容易とする観点から好ましい。

さらにまた、前記生体内分解吸収物質の粉体には、リン酸カルシウム系化合物が含まれ、かつ、β−TCP、キチン、キトサン、ポリ乳酸から選ばれた少なくとも1種が含まれていることが好ましい。
このような生体内分解吸収物質は、生体内での分解吸収速度が適度であり、前記骨芽細胞の侵入、定着促進の観点から、好適に用いられる。

また、前記増粘剤は、キサンタンガム、ペクチン、アルギン酸から選ばれた少なくとも1種であることが好ましい。
生体為害性がなく、成形時の作業性向上等の観点から、これらの増粘剤が好適に用いられる。

【発明の実施の形態】
以下、本発明をより詳細に説明する。
本発明に係る不定形骨補填材は、リン酸カルシウム系化合物の多孔質粒状体と生体内分解吸収物質の粉体と増粘剤とリン酸セメントとが配合されているものである。
そして、これらの配合組成が、前記リン酸カルシウム系化合物の多孔質粒状体と生体内分解吸収物質の粉体との合計配合量100重量部のうち、リン酸カルシウム系化合物の多孔質粒状体が20重量部以上90重量部以下、その残部が生体内分解吸収物質の粉体であり、さらに、前記リン酸カルシウム系化合物の多孔質粒状体と生体内分解吸収物質の粉体との合計配合量100重量部に対して、増粘剤は0.01重量部以上5重量部以下、リン酸セメントは5重量部以上50重量部以下であることを特徴とするものである。

具体的には、前記不定形骨補填材は、例えば、リン酸カルシウム系化合物の多孔質粒状体を基材とし、その粒界に、生体内分解吸収物質の粉体、増粘剤、リン酸セメントとが含まれたスラリーが介在した構成からなる練土状のものである。
このため、前記不定形骨補填材は、容易に成形加工することができ、任意の形状に自在に成形することができる。
そして、生体内に固定された後は、迅速に硬化して、十分な強度を有する補填人工骨が形成される。

したがって、本発明に係る不定形骨補填材は、大きい欠損部位の補填や、複雑な形状の骨格部分の補填にも好適に用いることができ、しかも、切削等の整形による微調整加工を必要とすることなく、かつ、初期固定にも優れており、脱落等を生じることもない。
また、前記不定形骨補填材は、リン酸カルシウム系化合物の多孔質粒状体を基材としているため、生体組織との親和性に優れており、気孔内に骨芽細胞等が侵入しやすく、しかも、経時的に生体内に吸収されて、自家骨に置換されやすいものである。

前記多孔質粒状体を構成するリン酸カルシウム系化合物としては、CaHPO4(リン酸一水素カルシウム)、Ca(H2PO4)2(リン酸二水素カルシウム)、Ca5(PO4)3OH(ハイドロキシアパタイト)、Ca4O(PO4)2(オキシアパタイト)、Ca10(PO4)6(OH)2、Ca3(PO4)2(リン酸カルシウム)、CaP4O11(テトラポリリン酸カルシウム)、Ca(PO3)2(メタリン酸カルシウム)、Ca2P2O7(ピロリン酸カルシウム)、Ca(H2PO4)2・H2O(リン酸二水素カルシウム・一水和物)等が挙げられる。
前記不定形骨補填材においては、これらのリン酸カルシウム系化合物うちの少なくとも1種を主成分とするセラミックスからなる多孔質粒状体が、基材として用いられることが好ましい。

また、前記リン酸カルシウム系化合物の多孔質粒状体は、多孔質ブロックの粉砕物からなり、前記多孔質ブロックは、連球状開気孔を有しており、気孔率が55%以上90%以下であることが好ましく、より好ましくは、60%以上85%以下であり、かつ、球状気孔間の連通部分の平均孔径は50μm以上400μm以下、特に、100μm以上400μm以下であることが、基材内に骨芽細胞等が侵入、定着しやすいことから好ましい。

さらに、前記多孔質粒状体は、球状気孔の孔径が150μm以上500μm以下であり、より好ましくは、200μm以上500μm以下であり、かつ、前記連通部分の孔径が10μm以上である気孔が全気孔体積の40%以上、特に、50%以上であることが、上記と同様に、骨芽細胞等の定着性等の観点から好ましい。

上記のような多孔質粒状体の母材となる多孔質ブロック等は、撹拌起泡により形成されることが好ましい。
撹拌起泡による多孔質体の形成は、気孔径の均整化を図りやすく、骨芽細胞等が侵入しやすい気孔を形成する上で好適である。

前記不定形骨補填材において、前記リン酸カルシウム系化合物の多孔質粒状体の配合量は、上記のように、該多孔質粒状体と生体内分解吸収物質の粉体との配合量の合計100重量部のうち、20重量部以上90重量部以下、より好ましくは、30重量部以上70重量部以下であり、その残部が生体内分解吸収物質の粉体である。
前記多孔質粒状体の添加量が20重量部未満である場合、基材内に骨芽細胞等が侵入し難く、定着することが困難となる。
一方、前記添加量が90重量部を超える場合、不定形骨補填材が練土状とならず、作業性に劣ることとなる。

また、前記多孔質粒状体の粒度は、特に制限されないが、骨芽細胞等の定着密度、粒子自体の強度保持とのバランス等の観点から、トップ粒として1000μm以上5mm以下であることがより好ましい。

前記不定形骨補填材に配合される生体内分解吸収物質の粉体は、混練および使用時における作業性を向上させるため、および、生体内に埋入後、溶出して、骨芽細胞等を多孔質基材に侵入、定着しやすくさせるために添加される。
この生体内分解吸収物質の粉体には、少なくともリン酸カルシウム系化合物が含まれ、かつ、β−TCP(β−リン酸三カルシウム)、キチン、キトサン、ポリ乳酸等が含まれたものが、生体内での分解吸収速度が適度であり、前記骨芽細胞の侵入、定着促進の観点から、好適に用いられる。これらのうちでも、特に、ポリ乳酸が好ましい。
その他にも、生体内分解吸収性を有するリン酸カルシウム系化合物であれば、限定されることなく用いることができる。

前記生体内分解吸収物質の粉体の粒度は、特に制限されないが、生体への吸収性や作業性等の観点から、1μm以上300μm以下であることが好ましく、より好ましくは150μm以下である。

また、前記不定形骨補填材に配合される増粘剤は、作業性を向上させるために添加されるものであり、生体為害性のないものとして、例えば、キサンタンガム、ペクチン、アルギン酸等が好適に用いられ、これらのうちでも、特に、キサンタンガムが好ましい。

前記増粘剤の配合量は、上記のように、リン酸カルシウム系化合物の多孔質粒状体と生体内分解吸収物質の粉体との合計配合量100重量部に対して、0.01重量部以上5重量部以下であることが好ましく、より好ましくは0.05重量部以上3重量部以下である。
前記配合量が0.01重量部未満である場合は、添加効果がほとんど認められない。
一方、前記配合量が5重量部を超える場合、粘度が高くなりすぎ、作業性に支障をきたすこととなる。

また、前記不定形骨補填材に配合されるリン酸セメントとしては、例えば、α−リン酸三カルシウムと第1リン酸カルシウムの混合粉末にNaF、KF等のフッ化物塩含有液を混和させて硬化させるリン酸カルシウムセメント、α−リン酸三カルシウムと第二リン酸カルシウムとポリ乳酸粉粒の混合粉に水を添加するリン酸カルシウムセメント、β−TCPとリン酸モノカルシウムおよびそれら以外のリン酸カルシウム系化合物混合粉末と水とからなる水硬性セメント等が挙げられる。
本発明においては、このように、常温で硬化し、外科用または歯科用等の生体為害性がなく、かつ、水硬性のリン酸セメントが好適に用いられる。

前記リン酸セメントの配合量は、上記のように、リン酸カルシウム系化合物の多孔質粒状体と生体内分解吸収物質の粉体との合計配合量100重量部に対して、5重量部以上50重量部以下であることが好ましく、より好ましくは、10重量部以上30重量部以下である。
前記配合量が5重量部未満である場合、不定形骨補填材の初期強度が低く、補填部位から脱落しやすくなる。
一方、前記配合量が50重量部を超える場合、マトリックスが緻密となり、骨芽細胞等が、不定形骨補填材の内部まで侵入して定着することが困難となり、自家骨と一体化されるまでに長時間を要することとなる。

本発明に係る不定形骨補填材は、上記した各配合物の混合物に、混和性、粘性等を調節するために液体を添加し、混練して成形される。
このときに添加される液体としては、主に注射用水等の滅菌水を用いることが好ましく、添加量は、作業性に応じて適宜調整される。
なお、前記液体には、例えば、ペプチド系化合物、非ペプチド系低分子化合物等からなる骨形成促進物質を添加してもよい。

また、前記不定形骨補填材を調製する際は、リン酸カルシウム系化合物の多孔質粒状体の気孔を十分に確保するために、予め水やコラーゲン等を該多孔質粒状体の気孔内に充填しておくことが好ましい。その後、他の配合物と迅速に混練させることにより、該多孔質粒状体の気孔が確保され、骨芽細胞等の侵入が容易となる。

【実施例】
以下、本発明を実施例に基づきさらに具体的に説明するが、本発明は下記の実施例により制限されるものではない。
[実施例1]
粒径2〜5mmのハイドロキシアパタイトの多孔質粒状体30重量部と、粒径1〜2mmの同多孔質粒状体20重量部と、粒径1mm未満の同多孔質粒状体20重量部と、生体内分解吸収物質としてβ―TCPの粉体10重量部と、増粘剤としてキサンタンガム0.1重量部と、リン酸セメント20重量部とを混合したものに、注射用水35重量部を添加して混練し、不定形骨補填材を調製した。
得られた不定形骨補填材を、内径20mm、高さ20mmの円筒状の型に注型して成形し、30分後に脱型した。
そして、この不定形骨補填材の成形体を、人工体液に3日間浸漬した後、強度試験機(島津製作所製 Autograph AG−10Kni)を用いて、圧縮強度を測定した。
この結果を表1に示す。

[実施例2〜6]
表1の実施例2〜6に示す配合組成からなる不定形骨補填材を、それぞれ、実施例1と同様にして調製し、成形した。
そして、これらの不定形骨補填材の各成形体について、実施例1と同様にして、人工体液に3日間浸漬した後、圧縮強度を測定した。
これらの結果を表1に示す。

[比較例1〜6]
表1の比較例1〜6に示す配合組成からなる不定形骨補填材を、それぞれ、実施例1と同様にして調製し、成形した。
そして、これらの不定形骨補填材の成形体について、実施例1と同様にして、人工体液に3日間浸漬した後、圧縮強度を測定した。
これらの結果を表1に示す。

表1に示したように、実施例1〜6の不定形骨補填材は、いずれも作業性に優れており、体液中に保持された後も、5MPa以上の十分な圧縮強度を有していることが認められた。
また、骨芽細胞等が侵入して定着するのに十分な気孔が残存していることも確認された。

一方、多孔質粒状体の配合量が90重量部を超える場合(比較例2)、増粘剤の配合量が5重量部を超える場合(比較例3)、または、同配合量が0.01重量部未満である場合(比較例4)は、粘性が適当でなく、作業性に劣るものであった。
また、多孔質粒状体の配合量が90重量部を超える場合(比較例2)、リン酸セメントの配合量が5重量部未満である場合(比較例5)は、体液中に保持された後の成形体の圧縮強度は、5MPa未満と劣るものであった。

なお、比較例1および6の成形体は、作業性および圧縮強度はいずれも良好であったが、比較例1の不定形骨補填材は、多孔質粒状体の配合量が20重量部未満と少ないため、生体内に埋入した場合に骨芽細胞等が定着するのに十分な気孔率を有するものではなかった。
また、比較例6の不定形骨補填材は、リン酸セメントの配合量が50重量部以上と多すぎたため、多孔質粒状体の気孔が目詰まりしてしまい、生体内に埋入した際に、骨芽細胞等が十分に侵入することができる性状とは言えなかった。

【発明の効果】
以上のとおり、本発明に係る不定形骨補填材は、生体組織との親和性に優れ、任意の形状で成形することができ、しかも、初期固定にも優れており、脱落等を生じ難いことから、大きい欠損部位の補填や、複雑な形状の骨格部分の補填にも好適に用いることができる。
しかも、生体内に固定され、一旦硬化した後は、十分な強度を有するとともに、骨芽細胞等が侵入しやすい細孔を多く有していることから、経時的に生体内に吸収されて、短期間で自家骨と置換されることが可能である。
したがって、本発明に係る不定形骨補填材は、骨欠損部位が大きく、複雑な形状である場合であっても、該骨欠損部位の治療に好適な材料として用いることができる。[0001]

However, molded products and granules are difficult to fix at the time of use. That is, the molded article requires elaborate processing so as to fit the filling portion. In addition, the granules can be replenished in a required amount regardless of the form of the replenishment site, but may fall off in vivo.
On the other hand, a paste-like product has a fine powder as a main component, and after being cured in a living body, becomes a dense solidified body. The amorphous bone filling material according to the present invention is the bone filling material in which a porous granular material of a calcium phosphate compound, a powder of a biodegradable and absorptive substance, a thickener and a phosphate cement are blended. BEST MODE FOR CARRYING OUT THE INVENTION
Hereinafter, the present invention will be described in more detail.
The amorphous bone substitute according to the present invention is a mixture of a porous granular material of a calcium phosphate compound, a powder of a biodegradable and absorbent substance, a thickener, and a phosphate cement.
And, the composition is such that the porous granules of the calcium phosphate-based compound are 20 parts by weight or more in the total blended amount of 100 parts by weight of the porous granules of the calcium phosphate-based compound and the powder of the biodegradation-absorbing substance. 90 parts by weight or less, the balance being the powder of the biodegradable and absorbing substance, and further based on 100 parts by weight of the total blended amount of the porous particulate body of the calcium phosphate compound and the powder of the biodegradable and absorbing substance. The thickener is 0.01 to 5 parts by weight, and the phosphate cement is 5 to 50 parts by weight.
