【発明の詳細な説明】
【０００１】
【発明の属する技術分野】本発明は整形外科用プロテー
ゼに関する。さらに具体的には、本発明は、例えば患者
の腕のような四肢の機能を回復させるためのプロテーゼ
に関する。特に、本発明は患者の長骨の代替に適する。
【０００２】
【従来の技術】２０世紀後半には、人工プロテーゼによ
って代替される人間の骨格部分の数が増大した。年月を
重ねるにつれて、これらのプロテーゼは、骨または関節
の単なる物理的代替品からさらに精巧な高機能プロテー
ゼへと発展してきた。例えば、プロテーゼは、肩関節ま
たは肘関節の代替品として周知である。DePuy, Inc.に
譲渡された米国特許に係る特許文献１に従ってデザイン
されたモジュール式の肩プロテーゼは、患者の肩に存在
する関節窩に一体化することができる。このプロテーゼ
は、患者の既存の上腕骨の内部に埋設されるように形成
された下方心棒部を有する。やはりDePuy, Inc.が所有
する米国特許に係る特許文献２は、上記に類似したモジ
ュール式の肘プロテーゼを開示する。このプロテーゼ
は、上腕骨と尺骨の髄内管に埋め込まれる複数の心棒部
を有する。類似の人工関節は、尻、膝、および足首の関
節の代替のために既に存在する。
【０００３】損傷すなわち傷害のある間接を代替するた
めの装置は多数存在するが、骨、特に長骨の代替は非常
にやっかいである。指接骨のプロテーゼは徐々に成功し
てきているとはいうものの、特に患者の手足の機能を確
保するという点で長骨はプロテーゼで容易に代替できず
にいるのが現状である。
【０００４】長骨の代替に伴う問題は多数であり種類も
様々である。おそらく最大の障害は、極度の荷重に耐え
なければならないという長骨の性質にある。この問題を
解決する一つの方法は、骨を代替するのではなく補完す
ることであった。例えば、特許文献３に記載されている
ように、大腿部の縦の骨を通るように人工大腿骨の骨幹
を延長する。この人工の骨幹の一端部は人工の膝部品と
して終結しており、他端部は人工の尻部品として終結す
る。特許文献３のプロテーゼは、既存の骨を保持し、耐
荷重性の一部をその骨に依存する。
【０００５】長骨プロテーゼに伴うさらなる問題点は、
患者によって個々の長骨の長さは異なるということであ
る。大腿骨、脛骨、橈骨、および上腕骨の長さは、患者
の身長が異なるのと同様に異なる。上記の特許文献３の
技術は、プロテーゼの尻関節部品と膝関節部品の間の全
体の距離を調節するための可動スクリューと外部に螺刻
されたスリーブ面を設けることにより、この長さの相違
に対処する。人工関節のため多くの類似の方法が実行さ
れている。これらのプロテーゼによれば、例えば、心棒
部を骨の髄内管に埋め込むことにより、関節が既存の長
骨に固定されることに注目すべきである。特許文献４〜
特許文献８には、様々な長さ調節機構が開示されてい
る。これらの長さの調節機構は、特許文献４の斜歯歯車
と外方のキー構造から、特許文献８の装置のモールスの
テーパ圧力ロック機構に至るまで多岐に及ぶ。

【発明が解決しようとする課題】これらの従来の長さ調
節機構すべてに共通する問題は、プロテーゼの長さを本
来の場所で調節できないことである。すなわち、上記の
従来の装置では、プロテーゼの所望の長さは予め決めら
れていて、患者の身体内に埋め込まれる前にプロテーゼ
は調節される。典型的な外科処置では、罹患した手足に
Ｘ線が照射されてプロテーゼの所望の長さが決定され
る。しかし、場合によっては、どんなにＸ線を精確に照
射して計測しようとしても、ある程度の誤りが生じて、
長骨の長さの精確な計測が困難になることがある。例え
ば、患者の上腕骨のＸ線検査を行うのは著しく困難なこ
とがある。通常の手順では、上腕骨の周囲の筋肉および
脂肪組織ならびに他の生体力学的な妨害のために、上腕
骨を完全にＸ線写真乾板に平行に向けることはできな
い。従って、上腕骨は多くの場合にはＸ線写真乾板に対
して非平行状態に配置され、最も多くの場合にはＸ線写
真乾板から外側に向かってわずかに傾斜させられる。こ
のような平行度の誤差により、上腕骨の長さを精確に測
定するのはほとんど不可能である。
【０００８】長骨プロテーゼの不精確な測定は、もちろ
ん四肢の長さの不適切という結果を招く。この長さの不
精確な調節は、運動障害および筋肉障害を引き起こすこ
とがある。さらに、不精確なプロテーゼの長さは、プロ
テーゼの周囲の組織の締まり具合にも悪影響を与えるこ
とがある。長さが小さ過ぎる場合には、周囲の組織は不
要なまでに弛緩する。プロテーゼが長すぎる場合には、
どうにかしてプロテーゼを埋め込むことができたにして
も、組織が硬くなり過ぎることがある。
【０００９】従来のプロテーゼ装置は、長い間、骨また
は関節の障害を持つ患者を助けることに使われてきた
が、達成されていない要望がいくつか残っている。一つ
の要望は、存続能力のある長骨プロテーゼつまり代替品
である。他の一つの要望は、患者の身体内に埋め込まれ
た後に、本来の場所で簡単に素早く調節することができ
る長さの調節可能なプロテーゼである。

【課題を解決するための手段】これらの要望を解決する
ため、本発明は、代替すべき長骨の計測の誤差を補償す
るために患者の身体内で容易に調節することができる調
節可能な長骨プロテーゼを検討する。本発明に係るプロ
テーゼは第１の長尺部品を有し、この第１の長尺部品
は、例えば肘関節のような第１の人工関節に係合するよ
うに形成された第１の端部と、第１の関節形成部分を持
つ他端部とを有する。また、第２の長尺部品が設けられ
ており、この第２の長尺部品は、例えば肩関節のような
第２の人工関節に係合するように形成された第２の端部
と、第２の関節形成部分を持つ他端部とを有する。これ
らの二つの関節形成部分は互いに調節可能に嵌め合うこ
とができ、これにより第１の長尺部品および前記第２の
長尺部品の合計長さが、代替される長骨の長さに近似で
きるように調節可能である。
【００１１】本発明の一つの形態では、第１の関節形成
部分は細長い心棒部を有しており、第２の関節形成部分
は環状体を有しており、環状体は螺刻された外面と、細
長い心棒部が圧力嵌めされるように形成された内面を有
する。このプロテーゼはさらにロックナットを有してお
り、このロックナットは環状体の螺刻された外面に螺合
するように螺刻されたテーパ状の内面を有する。この形
態によれば、ロックナットを環状体に螺合させることで
環状体をその内部の細長い心棒部の周囲に押しつけて、
これらの二つの部材の相対的な軸方向および回転方向の
位置を固定する。
【００１２】本発明によれば、第２の関節形成部分は、
環状体の内面に連続する寸法を持つ貫通しない孔も有す
る。この貫通しない孔は細長い心棒部にピッタリした隙
間嵌め（close running fit）する寸法を持ち、これに
より心棒部は孔に対して自由に回転または平行移動する

は、両者の間に互いに噛み合う境界面を有する。互いに
噛み合う境界面は、孔に対する心棒部の割出回転または
割出平行移動が可能なように形成されていてもよい。あ
るいは、互いに噛み合う境界面は、これらの部品間の相
対回転または相対平行移動を防止または限定するように
形成されていてもよい。ある実施の形態では、互いに噛
み合う境界面は、心棒部および孔に存在する多数の軸線
方向のリブを有する。
【００１３】環状体の内面および心棒部の外面はとも
に、心棒部の周囲の環状体の圧力固定状態すなわち嵌め
合いを増強するように表面処理を施されていてもよい。
この表面形状は、部品間の互いに噛み合う境界面の形状
でもよいし、変形可能な突起群の形状でもよい。
【００１４】本発明の一つの目的は、長骨の代替として
容易に使用できるプロテーゼを提供することである。さ
らなる目的は、患者の身体内でのプロテーゼの長さの調
節が容易に行えるようにするという本発明の特徴に存在
する。
【００１５】本発明の一つの長所は、長さの調節を可能
にする能力と、長さの調節が可能なプロテーゼにおける
強固な係合を達成する能力により実現される。他の長所
は、部品が多くの骨格構造および多くの長骨の代替のた
めに容易に適することにある。さらなる長所は、このプ
ロテーゼの部品が、患者の解剖学的構造に応じて、選択
された部品のうちから組み合わせおよびマッチングする
ことができることにある。

【発明の実施の形態】本発明の原理の理解を容易にする
ために、図面に示され下記に説明される実施の形態を以
下に参照する。理解すべきこととして、実施の形態によ
って本発明の範囲が限定されることを意図するのではな
い。さらに理解すべきこととして、本発明は、当業者が
通常想起する程度の範囲にある限り、例示の実施の形態
のあらゆる改変および修正を包含するとともに、本発明
の原理のさらなる応用を包含する。
【００１８】本発明は、最も好ましくは本来の場所でプ
ロテーゼの長さを調節することが可能な調節可能なプロ
テーゼを検討し、患者の長骨、例えば上腕骨の代替とい
う用途を有する。しかし、本発明の調節可能な特徴は、
例えば人工関節のような他のプロテーゼのためにも実施
することができる。本発明の一つの態様では、調節可能
なプロテーゼは、プロテーゼの関節形成部品同士の間の
圧力嵌めをもたらすための螺刻された環状体およびテー
パナットを利用する。
【００１９】図１および図２に示されるように、調節可
能なプロテーゼ１０は、下方の関節形成部分１２を備え
ており、下方の関節形成部分１２は上方の関節形成部分
１４に調節可能な関節状に屈曲する形式の係合をするよ
うに構成されている。例示の好適な実施の形態では、プ
ロテーゼ１０は、上腕プロテーゼとして意図されてい
る。従って、これらの二つの関節形成部分１２，１４
は、患者の上腕骨に近似するように意図されている。下
記の説明では、下方の関節形成部分１２は肘上腕部品１
２、上方の関節形成部分１４は肩上腕部品１４とも呼
ぶ。もちろん、これらの上方および下方の部分は、プロ
テーゼの性質に応じて修正することができる。例えば、
プロテーゼは他の長骨の代替に提供されてもよい。同様
に、部分１２，１４の一方または両方は、既存の骨に係
合するように修正してもよい。例えば、プロテーゼ１０
は、長骨の途中部分の代替として使用してもよい。
【００２０】例示の実施の形態では、肘上腕部品１２
は、その自由端に形成された肘関節部材１８を有する。
同様に、肩上腕部品１４は、その自由端に形成された肩
関節部材２０を有する。ある実施の形態では、肘関節部
材１８は、米国特許第６，２９０，７２５号に開示され
たモジュール式の肘の部分を構成するのでもよい。この
文献の関連部分、特にモジュール式の肘の上腕部品を説
明する部分は言及されることによりこの明細書の一部を
なす。同様に、肩関節部材２０は、米国特許第５，３１
４，４７９号に開示されるように形成されていてもよ
い。米国特許第５，３１４，４７９号の関連部分、特に
関節ヘッド部材に係合するように形成されたプロテーゼ
本体を説明する開示の部分は言及されることによりこの
明細書の一部をなす。
【００２１】図１に示されるように、肘上腕部品１２は
下方の長尺部材２５を有しており、肩上腕部品１４は上
方の長尺部材２７を有する。上方の長尺部材２７は貫通
しない孔３０を画定しており、孔３０は上方の長尺部材
２７の一端部（肩関節部材２０の反対の端部）の内部を
延びている。下方の長尺部材２５は、細長い心棒部３２
を有しており、心棒部３２は孔３０の内部にピッタリし
た隙間嵌め（close running fit）するような寸法を有
する。心棒部３２は下方の長尺部材２５における肘関節
部材１８と反対の端部に形成されている。
【００２２】本発明の重要な態様によれば、上方の長尺
部材２７はさらに環状体部分３５を有する。環状体部分
３５は上方の長尺部材２７の貫通しない孔３０のある方
の端部に組み込まれている。図３に示されるように、環
状体部分３５は、溝３８で区分された多数の環状体フィ
ンガー３７により形成されている。環状体フィンガー３
７は、上方の長尺部材２７の貫通しない孔３０に実質的
に連続する孔３９を画定する。好ましくは、孔３９およ
び貫通しない孔３０は、各々が同一の直径を持つように
形成され、最も好ましくは、共通するドリルおよびリー
マ仕上げ作業で形成される。
【００２３】図１および図２に示されるように、環状体
部分３５の周囲にはロックナット１６が配置される。さ
らに具体的には、環状体部分３５は、その外面に一連の
外側ネジ山４１を有しており、外側ネジ山４１は環状体
部分３５のほとんどの長さにわたって延びている。ロッ
クナット１６は、環状体部分３５の外側ネジ山４１に螺
合するように形成された雌ネジ４４を有する。
【００２４】本発明によれば、ロックナット１６、環状
体部分３５および下方の長尺部材２５の心棒部３２の組
み合わせは、下方および上方の関節形成部分１２，１４
の間の圧力嵌めすなわち圧入を可能にする。さらに具体
的には、環状体部分３５の内面５８（図５）は、環状体
部分３５の外側ネジ山４１がロックナット１６に螺合す
ることにより、心棒部３２の外面に向けて押しつけられ
る。この圧入機能を達成するために、環状体部分３５
は、孔３９の直径が心棒部３２の外径未満に減少するよ
うに環状体部分３５が半径方向に縮減することが必要で
ある。この好適な実施の形態では、この縮減は、ロック
ナット１６をテーパナットとすることにより達成され
る。さらに具体的には、ロックナット１６の内部孔４５
は、環状体部分３５側の端部で大きい方の直径を持ち、
肘上腕部品１２側の端部で小さい方の直径を持つように
先細になっている。最も好ましくは、ロックナット１６
内の内部孔４５の小径部分は、環状体部分３５と心棒部
３２の間の締まり嵌めを起こすことができるように十分
に小さい。このようにすることによって、圧入力の大き
さは、ロックナット１６が環状体部分３５の周囲に螺合
される程度に依存する。
【００２５】この好適な実施の形態では、環状体部分３
５は、ロックナット１６の雌ネジの全長を受け入れるこ
とができるように十分な長さまで延びる外側ネジ山４１
を有することが期待される。この方式によれば、プロテ
ーゼが患者の身体内に埋め込まれると、環状体部分３５
を完全にロックナット１６の内部に隠すことができ、軟
組織を傷つけるおそれが減少する。
【００２６】最も好適な実施の形態では、ロックナット
１６の内部孔４５は先細に形成されている。ただし、他
の実施の形態では、環状体部分３５の雄ネジが螺刻され
た面４１が先細で、ロックナット１６の雌ネジ４４が一
定の直径であってもよい。同様に、環状体部分３５の孔
３９が先細で、環状体部分３５の外径およびロックナッ
ト１６の内径が一定であってもよい。これらのどの構成
によっても、環状体部分３５に対するロックナット１６
の螺合により、心棒部３２に対する内側への圧縮圧力つ
まり圧縮力が引き起こされ、二つの関節形成部分１２，
１４が一体にロックされる。ある実施の形態では、形成
される角度は約１°〜３°のモールス角度でよい。ロッ
クナット１６と環状体部分３５の両方が、ロックナット
１６と環状体部分３５の間の固定状態を向上させるため
に同一のモールス角度を使用してもよい。
【００２７】本発明の好適な実施の形態では、心棒部３
２は、貫通しない孔３０および環状体の孔３９に対して
回転および平行移動の両方ができるようにピッタリした
隙間嵌め（close running fit）するような寸法を有す
る。換言すれば、好適な実施の形態では、肘上腕部品１
２は、肩上腕部品１４に対して、回転方向および延長方
向の両方に動かすことができる。このようにすることに
よって、調節可能なプロテーゼ１０の長さが患者の本来
の骨の長さに近似するように調節でき、肘関節部材１８
と肩関節部材２０の間の角度の向きは、孔３０に対して
心棒部３２を回転させることにより調節できる。ある実
施の形態では、心棒部３２と孔３９との間のピッタリし
た隙間嵌め（close running fit）は、半径方向に約
０．１０インチ（２．５ｍｍ）である。
【００２８】二つの部品１２，１４の間の角度の向きを
固定するのを補助するために、心棒部３２のリブ５１と
貫通しない孔３０のリブ５２の相互の噛み合いを使用し
てもよい（図３〜図５）。好ましくは、この実施の形態
によれば、リブ５１，５２は、その対応する表面上に僅
かに突出するだけである。このようにすることによっ
て、互いに噛み合うリブ５１，５２は、肘上腕部品１２
および肩上腕部品１４の間の相対回転を邪魔することが
ない。あるいは、リブ５１，５２は回転運動の際に周期
的な間隔をおいてわずかな抵抗をもたらしてもよく、さ
らには回転の際の個々の割出角を表すクリック音を発生
してもよい。例えば、互いに噛み合うリブ５１は心棒部
３２の直径の周囲に１０°の角間隔をおいて配置しても
よい。特に聴取可能な目印だけが必要な場合には、二つ
の部品１２，１４の一方に単一のリブを形成してもよ
い。あるいは、二つの部品１２，１４の一方に他方より
も多数のリブを形成してもよい。好ましくは、ある実施
の形態では、リブ５１，５２は、半径方向の隙間嵌めの
半分よりも僅かに大きい高さ、例えば０．０６インチ
（１．５ｍｍ）の高さを有していてもよい。
【００２９】ある場合には、二つの部品１２，１４の相
対的回転は不適切で、さらには有害なことがある。この
種のプロテーゼの実施の形態では、互いに噛み合うリブ
５１，５２は、対応する要素に対してさらに大きく突出
させてもよい。換言すれば、リブ５１，５２は、二つの
部品１２，１４の間の回転が不可能なように形成しても
よい。ある実施の形態では、互いにロックするスプライ
ン構造をリブ５１，５２の代わりに使用してもよい。こ
の構造によれば、貫通しない孔３０には、長尺の溝また
は直立したスプラインのいずれかが設けられてよく、心
棒部３２はこれに補完するように直立したスプラインま
たは溝が形成されてよい。従って、一旦心棒部３２が貫
通しない孔３０の内部にまで延びたなら、部品１２，１
４の間の角度の向きは環状体部分３５上のロックナット
１６の位置いかんに関わらず固定される。あるいは、他
の実施の形態では、孔と心棒部の形状は、相対回転を防
止するように、非円形の相互補完する形状、例えば楕円
または正方形でもよい。
【００３０】同様の形式で、これらの関節形成部分に
は、長さの調節を割出式で行うために、周方向リブを設
けてもよい。従って、心棒部３２のリブ５１は、所定の
軸線方向の間隔をおいて心棒部の全周の周りに延びてい
てもよい。貫通しない孔３０のリブ５２は、孔３０の入
口に位置する単一のリブすなわち単一の突起として修正
してもよく、この単一のリブ５２は、心棒部３２が孔３
０に進入することに対して最小の抵抗を引き起こす。こ
こでも、このように修正されたリブは、上腕部品１２，
１４の割出長さ位置を表すように聴取可能なクリック音
を発生させてもよい。
【００３１】さらなるオプションとして、心棒部および
孔には、患者の身体内に埋め込まれる前に、部品同士を
一体に保持するのを補助する保持機構を設けてもよい。
この保持機構は、心棒部の端部に位置する周方向リブ
と、孔の端部に位置する対応する周方向リブを有してお
り、これらの二つのリブは締まり嵌めを引き起こす。心
棒部を孔に最初に押し入れる時に両者の干渉は打ち負か
される一方で、心棒部に通常の操作がされている時には
両者の干渉は心棒部を保持するのに十分である。
【００３２】心棒部３２は、心棒部３２に環状体部分３
５を強い圧力で固定して保持するために形成された表面
形状５５を有していてもよい。類似の表面形状は、環状
体部分３５の内面５８に設けてもよい。ロックナット１
６が環状体部分３５に完全に螺合する時に、部品間の圧
力での固定状態を高める（すなわち増強させる）表面処
理という条件で、様々な表面処理も考えられる。例え
ば、表面形状５５は、刻印パターン、周方向および／ま
たは長手方向のスプライン、または変形可能な突起群を
有していてもよい。環状体部分３５の内面５８にも類似
の処理を施してもよい。
【００３３】図１および図２ならびに図５の分解図を参
照した上記の説明より理解されるように、調節可能なプ
ロテーゼ１０は、プロテーゼが患者の身体内にある場合
でも容易に調節することが可能である。調節を行うため
には、ロックナット１６を環状体部分３５に対して締め
付けない状態にしておくことだけが必要である。調節可
能なプロテーゼ１０の全長は、貫通しない孔３０の内部
に心棒部３２が延びる量を変更することにより調節でき
る。従って、上腕部品１２，１４の組み合わせの全長
は、個々の患者にとって解剖学的に精確なプロテーゼ長
さを確保できるように、患者の身体内において個別に補
正することができる。さらに、二つの部材１８，２０の
間の角度の状態も調節することができる。
【００３４】これらの部品が所定の位置に配置された
ら、ロックナット１６を環状体部分３５に螺合させるこ
とができる。好ましくは、ロックナット１６は、外側レ
ンチ面４６を有することによりレンチに嵌まるように形
成されている。本発明の好適な形態では、外側レンチ面
４６は丸められた角部のみを有しており、これにより周
囲の軟組織に外傷を与えるおそれを減少せしめている。
図３に示されるように、外側レンチ面４６は、六角レン
チに嵌まるように形成されている。ただし、当然ではあ
るが、様々なレンチに嵌るように、他のレンチ面の形状
も使用してもよい。
【００３５】外側ネジ山４１と雌ネジ４４の螺合の精度
の粗さは、しっかりした固定状態を達成するのにロック
ナット１６を何度も回転させなくてもよいように、補正
できると好ましい。他方、部品の固定状態を増強し、ロ
ックナット１６が環状体部分３５から離脱するおそれを
減少させるためにも、ネジのある程度の精度が望まれ
る。このような不適切な出来事が起こるのを防止するた
めの保証として、ロックナット１６と環状体部分３５の
間には生体適合性のある接着剤を使用してもよい。さら
に、ロック部分および／または関節形成部分の間にモー
ルステーパ面を設けてもよい。さらなる代替として、ロ
ックナット１６の逆回転を防止するためにロックナット
１６に螺合させることの可能な追加のロックナットによ
ってロックナット１６を補完してもよい。
【００３６】本発明のさらなる特徴は、図２および図５
を参照することにより理解されるであろう。具体的に
は、肩上腕部品１４、特にその上方の長尺部材２７は、
特定の外径を有する。好ましくは、この外径は、代替さ
れる長骨の直径に近似する。さらに、この上方の長尺部
材２７は、調節可能なプロテーゼ１０の全長の大部分を
占める。プロテーゼの全体の連続性を確保するために、
ロックナット１６は上方の長尺部材２７の直径よりもあ
まり大きくない外のり寸法を有するようにしてもよく、
本来は上方の長尺部材２７よりも小さい直径を有するの
が好ましい。同様に、肘上腕部品１２の下方の長尺部材
２５は、上方の長尺部材２７の外径以下の外径を有する
ようにしてもよい。このようにすることによって、プロ
テーゼ１０の全体の輪郭が円滑になり、周囲の患者の組
織を必要よりも傷つけることが少なくなる。
【００３７】最も好ましくは、部品１２，１４およびロ
ックナット１６は、生体適合性のある金属材料、例えば
チタンから形成される。また、材料はＸ線不透過であっ
て、プロテーゼの存続性を後段階のＸ線検査で判定でき
るようにすると好ましい。プロテーゼ１０が上腕骨の代
替に使われることを意図したある実施の形態では、肩上
腕部品１４は、直径が約０．７５インチ（２０ｍｍ）、
肩関節部材２０の底面から環状体部分３５の底面までの
長さが約４．２５インチ（１１０ｍｍ）でよい。環状体
部分３５は、上方の長尺部材２７の端部から約１．０イ
ンチ（２５ｍｍ）延びていてよく、その外径が約０．５
インチ（１３ｍｍ）、その孔の直径が約０．４インチ
（１０ｍｍ）でよい。
【００３８】肘上腕部品１２、特にその下方の長尺部材
２５は、肘関節部材１８から心棒部３２の底辺までの長
さが約２．１インチ（５３ｍｍ）、その直径が０．５５
インチ（１４ｍｍ）でよい。心棒部３２は孔３９の直径
よりも僅かに小さい直径を有し、下方の長尺部材２５の
端部からの長さが約１．５インチ（３８ｍｍ）である。
ロックナット１６は、約０．９５インチ（２４ｍｍ）の
長さを有していてよい。
【００３９】理解すべきこととして、本発明の重要な特
徴は、意図的にプロテーゼを埋め込んだ後に本来の場所
でのプロテーゼの調節を可能にすることにあるが、この
同じ特徴により、プロテーゼの長さおよび／または向き
をその後に調節することも可能となる。従って、時間の
経過または衝撃的な荷重のためにある程度の滑りが発生
した場合には、ロックナット１６を緩めて、下方および
上方の長尺部材２５，２７の再調節を行うことができ
る。さらに理解すべきこととして、プロテーゼ１０がモ
ジュール式になっているという特徴により、他の部品１
２，１４の組に交換することが可能になる。例えば、別
の肘関節部品を要する新規な肘関節が開発された場合に
は、肘上腕部品１２をその新規に設計された部品に交換
することが可能である。この能力は、患者の解剖学的構
造から決定される様々な肩部品および肘部品の組み合わ
せおよびマッチングを可能にするので、初期の外科手術
でも便利である。
【００４０】以上、本発明を詳細に図示して説明した
が、上記の説明および図面は例示としてみなすべきであ
り特徴点を限定するものと考えてはならない。理解すべ
きこととして、単に好適な実施の形態が説明され、本発
明の趣旨に包含される変更、修正およびさらなる応用は
保護されるべきであると希望する。
【００４１】例えば、本発明は、長尺部材２５，２７の
両方を患者の一つの連続した骨の髄内観の内部に嵌まる
ように形成することも意図している。従って、プロテー
ゼの一端部をある骨部分に嵌めて、他端部を関節部品に
嵌めるようにしてもよい。同様に、両端部をある骨部分
に嵌め、プロテーゼが既存の骨の途中部分の代替品をな
すようにしてもよい。上記のように、例示の実施の形態
は上腕骨の代替に関するが、本発明の原理は他の骨、特
に人間の長骨、例えば大腿骨、脛骨、腓骨、尺骨、およ
び橈骨にも応用できる。
【００４２】さらに、貫通しない孔３０および心棒部３
２は、上方の部材２７および下方の部材２５の各々に一
体化するものとして説明した。ただし、孔および心棒部
は、反対側の部材に移し替えてもよく、その場合でも本
発明の機能性をなお保有することは明らかである。

Claims (3)

    【特許請求の範囲】
  1. 【請求項１】 第１の長尺部品と、第２の長尺部品と、
    ナットとを備え、 前記第１の長尺部品は自由端と他端を有しており、この
    他端は細長い心棒部を有しており、 前記第２の長尺部品は自由端と他端を有しており、この
    他端が前記細長い心棒部に調節可能に嵌まることが可能
    な関節形成部分を有することにより、前記第１の長尺部
    品および前記第２の長尺部品の合計長さが長骨の長さに
    近似できるように調節可能であり、 前記関節形成部分が環状体を有しており、前記環状体
    が、螺刻された外面と、前記環状体が前記細長い心棒部
    の周囲に押しつけられると前記細長い心棒部が圧力嵌め
    されるように形成された内面とを有しており、 前記ナットは前記環状体の前記外面に螺合する螺刻され
    た内面を有しており、前記環状体および前記ナットの少
    なくとも一方が、前記ナットを前記環状体に螺合させる
    ことで前記環状体を前記細長い心棒部の周囲に押しつけ
    るように形成されたテーパ面を有する、調節可能な長骨
    プロテーゼ。
  2. 【請求項２】 骨の少なくとも一部を代替する調節可能
    なプロテーゼであって、 第１の部品と、第２の部品と、ナットとを備え、 前記第１の部品が細長い心棒部を有しており、 前記第２の長尺部品が前記細長い心棒部に調節可能に嵌
    まることが可能な関節形成部分を有することにより、前
    記第１の長尺部品および前記第２の長尺部品の合計長さ
    が代替される骨の部分の長さに近似できるように調節可
    能であり、 前記関節形成部分が環状体を有しており、前記環状体
    が、螺刻された外面と、前記環状体が前記細長い心棒部
    の周囲に押しつけられると前記細長い心棒部が圧力嵌め
    されるように形成された内面とを有しており、 前記ナットは前記環状体の前記外面に螺合する螺刻され
    た内面を有しており、前記環状体および前記ナットの少
    なくとも一方が、前記ナットを前記環状体に螺合させる
    ことで前記環状体を前記細長い心棒部の周囲に押しつけ
    るように形成されたテーパ面を有する、調節可能なプロ
    テーゼ。
  3. 【請求項３】 第１の長尺部品と前記第２の長尺部品を
    備え、 前記第１の長尺部品が、第１の人工関節に嵌まる第１の
    端部と、第１の関節形成部分を持つ他端とを有してお
    り、 前記第２の長尺部品が、第２の人工関節に嵌まる第２の
    端部と、第２の関節形成部分を持つ他端とを有してお
    り、 前記第２の関節形成部分が前記第１の関節形成部分に調
    節可能に嵌まることが可能であるように形成されている
    ことにより、前記第１の長尺部品および前記第２の長尺
    部品の合計長さが長骨の長さに近似できるように調節可
    能である、調節可能な長骨プロテーゼ。
