Claims (1)
- 【特許請求の範囲】 1.被験者の呼吸波形をモニターするステップと、 モニターした呼吸波形から被験者のピーク吸気流量およびピーク吸気加速の うちの一つを決定するステップと、 決定されたピーク吸気流量およびピーク吸気加速のうちの一つから被験者の 呼吸ドライブを決定するステップとからなる、生体被験者の呼吸ドライブを測定 する方法。 2.呼吸波形をモニターするステップが、被験者の胸郭の動きおよび腹部の動き を測定するステップと、胸郭の動きおよび腹部の動きの合計から一回呼吸量を決 定するステップとからなり、ピーク吸気流量およびピーク吸気加速のうちの一つ を決定するステップが、測定された胸郭の動きと測定された腹部の動きと決定さ れた一回呼吸量の一つに応答して選択された所定の時にピーク吸気流量およびピ ーク吸気加速の一つを決定することからなる請求項1の方法。 3.呼吸波形をモニターするステップが、被験者の一回呼吸気量および気流の一 つを測定することからなる請求項1の方法。 4.呼吸波形をモニターするステップが、肺活量計、呼吸記録器、身体体積記録 器、鼻口サーミスタおよび鼻口熱電対の一つを用いて被験者の呼吸波形をモニタ ーすることからなる請求項1の方法。 5.呼吸波形をモニターするステップが、被験者の体表に外側からあてがわれる 装置を用いて被験者の呼吸を測定することからなる請求項1の方法。 6.呼吸波形をモニターするステップが、呼吸誘導体積記録器、ジャーキン(je rkin)体積記録器、線形差分変換器、磁気計、ふいご型呼吸記録器、ひずみゲー ジ、圧電装置、インダクタンス周囲変換器、インピー ダンス呼吸記録器および胴の動きの画像変換の一つを用いて被験者の呼吸波形を モニターすることからなる請求項1の方法。 7.被験者のピーク吸気流量およびピーク吸気加速のうちの一つを決定するステ ップが、気道内圧、胸腔圧、横断横隔膜圧、首誘導体積記録器および呼吸音測定 の一つを用いてピーク吸気加速を決定することからなる請求項1の方法。 8.被験者のピーク吸気流量およびピーク吸気加速のうちの一つを決定するステ ップが、被験者の吸気の開始から±300msec.の領域内のピーク吸気加速 を決定することからなる請求項1の方法。 9.被験者のピーク吸気流量およびピーク吸気加速のうちの一つを決定するステ ップが、呼吸毎に時間平均でピーク吸気流量およびピーク吸気加速のうちの一つ を決定することからなる請求項1の方法。 10.CO2および低O2ガス混合物のうちの一つの呼吸による被験者の脳の呼吸 中枢の完全性の査定、被験者の覚醒および睡眠状態にわたる被験者の基線量呼吸 中枢活性の査定、閉塞性の無呼吸/呼吸低下時の被験者の呼吸中枢ドライブの査 定、睡眠時の被験者の外的呼気抵抗負荷の有無の検知、被験者が鼾と上気道抵抗 症候群の一つを持つ場合、被験者の正の気道圧の滴定のための設定点の提供、被 験者の気管支収縮の有無のモニター、通常呼吸ドライブを低下させる麻酔剤、鎮 静剤および鎮痛剤の被験者への作用の測定、被験者の疾病状態の尺度としての被 験者の呼吸ドライブの類別、被験者の機械的換気時の呼吸ドライブの状態の査定 、被験者が行う疲労の可能性のある作業時の被験者の呼吸筋の状態の査定、新生 児被験者の低酸素血症発生の間接的な標識、被験者の滞留気管気管支分泌物の標 識、被験者の睡眠時の夜間気管支痙攣の診断、過度に高い呼吸ドライブを低下さ せるために被験者に麻酔剤を投与した後の被験者の呼吸中枢抑制の検知、被験者 の内因性の正の呼気終期圧(positive end expiratory pressure PEEP)の有無のモニターおよび有害なガス環境で作業する 被験者の呼吸困難の検知のうちの一つのために、決定された呼吸ドライブを用い るステップをさらに含む請求項1の方法。 11.一回呼吸量、吸気流量および吸気加速のうちの一つが閾値を超えたときト リガーパルス信号を発生するステップをさらに含む請求項2の方法。 12.トリガーパルスを発生するステップが、胸郭の運動および腹部の運動のう ち主導のものに由来する一回呼吸量、吸気流量および吸気加速の一つを用いるこ とからなる請求項11の方法。 13.手動選択および自動選択のうちの一つを用いて胸郭の運動および腹部の運 動のうち主導のものを選択するステップをさらに含む請求項12の方法。 14.トリガーパルスに応答して機械的人工呼吸器による膨張を開始するために 、被験者に接続された機械的人工呼吸器にトリガーパルス信号を送るステップを さらに含む請求項11の方法。 15.呼吸速度と一回呼吸量との積から換気量を決定するステップと、息切れの 尺度としてピーク吸気加速の換気量に対する比を計算するステップとをさらに含 む請求項2の方法。 16.呼吸ドライブおよび呼吸ドライブの換気量に対する比のうちの一つに応答 して、被験者に接続された連続正空気圧(CPAP)装置のために最適圧力レベ ルを決定するステップをさらに含む請求項15の方法。 17.最適圧力レベルを決定するステップが、被験者が閉塞性睡眠時無呼吸、呼 吸低下、上気道抵抗症候群および動的過度膨張の一つを患っている場合、被験者 に接続されたCPAP装置の最適圧力レベルを決定することからなる請求項16 の方法。 18.呼吸波形をモニターするステップが、呼吸誘導体積記録器を用い て呼吸波形をモニターすることからなり、前記方法が、呼吸波形から平均吸気流 量を測定するステップと、ピーク吸気流量を平均吸気流量で割って計算された値 に応答して、被験者に接続された連続的正空気圧(CPAP)装置の圧力レベル を制御するステップとをさらに含む請求項1の方法。 19.呼吸波形をモニターするステップが、呼吸誘導体積記録器を用いて呼吸波 形をモニターすることからなり、前記方法が、呼吸波形から平均呼気流量とピー ク呼気流量とを測定するステップと、ピーク呼気流量を平均呼気流量で割って計 算された値に応答して、被験者に接続された連続的正空気圧(CPAP)装置の 圧力レベルを制御するステップとをさらに含む請求項1の方法。 20.被験者の胸郭の動きと腹部の動きとを測定するステップと、 測定された胸郭の動きと測定された腹部の動きとの合計から被験者の呼吸器 系の一回呼吸量を決定するステップと、 胸郭の動き、腹部の動きおよび一回呼吸量の一つから被験者のピーク吸 気流量およびピーク吸気加速のうちの一つを決定するステップと、 ピーク吸気流量およびピーク吸気加速のうちの一つから呼吸ドライブを決定 するステップとからなる、被験者の呼吸ドライブを測定する方法。 21.胸郭の動きと腹部の動きとを測定するステップが、被験患者の体表に外側 からあてがわれた装置を用いて前記動きを測定することからなる請求項20の方 法。 22.胸郭の動きと腹部の動きとを測定するステップが、呼吸誘導体積記録器、 ジャーキン(jerkin)体積記録器、線形差分変換器、磁気計、ふいご呼吸記録器 、ひずみゲージ、圧電装置、インダクタンス周囲変換器、インピーダンス呼吸記 録器および胴の動きの画像変換の一つを用い ることからなる請求項20の方法。 23.被験者のピーク吸気流量およびピーク吸気加速のうちの一つを決定するス テップが、被験者の吸気の開始から±300msec.の領域内のピーク吸気加 速を決定することからなる請求項20の方法。 24.被験者のピーク吸気流量およびピーク吸気加速のうちの一つを決定するス テップが、呼吸毎に時間平均でピーク吸気流量およびピーク吸気加速のうちの一 つを決定することからなる請求項20の方法。 25.被験者の吸気流量と吸気加速を測定するステップと、一回呼吸量、吸気流 量および吸気加速のうちの一つが閾値を超えたときトリガーパルス信号を生成す るステップをさらに含む請求項20の方法。 26.トリガーパルスを生成するステップが、胸郭の動きおよび腹部の動きのう ち主導のものに由来する一回呼吸量、吸気流量および吸気加速の一つを用いるこ とからなる請求項25の方法。 27.手動選択および自動選択の一つを用いて胸郭の動きおよび腹部の動きのう ち主導のものを選択するステップをさらに含む請求項26の方法。 28.トリガーパルスに応答して機械的人工呼吸器による膨張を開始するために 、被験者に接続された機械的人工呼吸器にトリガーパルス信号を送るステップを さらに含む請求項25の方法。 29.被験者の呼吸速度を測定するステップと、呼吸速度と一回呼吸量との積か ら被験者の換気量を決定するステップと、無呼吸の尺度としてピーク吸気加速の 換気量に対する比を計算するステップとをさらに含む請求項20の方法。 30.呼吸ドライブおよび呼吸ドライブの換気量に対する比の一つに応答して、 被験者に接続された連続的正空気圧(CPAP)装置のために最適圧力レベルを 決定するステップをさらに含む請求項29の方法。 31.最適圧力レベルを決定するステップが、被験者が閉塞性睡眠時無呼吸、呼 吸低下、上気道抵抗症候群および動的過度膨張の一つを患っている場合、被験者 に接続されたCPAP装置の最適圧力レベルを決定することからなる請求項30 の方法。 32.被験者の平均吸気流量を測定するステップと、ピーク吸気流量を平均吸気 流量で割って計算された値に応答して、被験者に接続された連続的正空気圧(C PAP)装置の圧力レベルを制御するステップとをさらに含む請求項20の方法 。 33.被験者の平均呼気流量とピーク呼気流量を測定するステップと、ピーク呼 気流量を平均呼気流量で割って計算された値に応答して、被験者に接続された連 続的正空気圧(CPAP)装置の圧力レベルを制御するステップとをさらに含む 請求項20の方法。 34.被験者の胸郭の動きと腹部の動きとを測定するステップが、被験者に接続 された呼吸誘導体積記録器を用いることからなり、前記方法が、測定された胸郭 の動きと測定された腹部の動きに由来する吸気波形および呼気波形の形状を示す 指標を決定するステップと、決定された指標に応答して、被験者に接続された連 続的正空気圧(CPAP)装置を減衰閉鎖ループ制御システムを用いて制御する ステップとをさらに含む請求項20の方法。 35.被験者の胸郭の動きと腹部の動きとを測定するステップが、被験者に接続 され、少なくとも毎秒200ポイントのサンプル速度を有する体積記録装置を用 いることからなる請求項20の方法。 36.被験者の胸郭の動きと腹部の動きとを測定するステップが、被験者に接続 され、クロストークを減らし、これによってオシレータが生成した多数の信号を 収容するために、装置のトランスデューサバンド上に直接位置させたオシレータ を有する体積記録装置を用いることからなる 請求項20の方法。
