診断及び治療のための放射能標識したソマトスタチン受容体リガンド
本発明は、罹患した細胞のソマトスタチン結合性表面受容体に選択的に結合さ
れるような、腫瘍組織の診断及び/又は破壊のための各種金属化合物のための担
体物質に関し、その際ソマトスタチン−14アゴニストに巨大環キレート化剤が
結合している。
非常に多種多様な腫瘍細胞の上でのペプチド受容体の高められた発現は核医療
的な局所化診断の枠内で放射能標識されたペプチドの利用に対する分子的基礎を
なす。この種の第1の分子種の1つ、放射性ヨード標識されたN-ホルミル-Nle-L
eu-Phe-Nle-Tyr-Lys(1)が、好中性の血球の上の走化性受容体への結合に基づ
く膿瘍の位置決定のために開発された。次いで、(D)Phe-Cys-Phe-(D)Trp--Lys-T
hr-Cys-Thr(o1){Tyr3-オクトレオチド:2)}及び111In-DTPA-(D)Phe-Cys-Phe-(
D)Trp-Lys-Thr-Cys-Thr(o1){(DTPA)-(D)Phe1-オクトレオチド:3)}のようなペ
プチドが神経内分泌系腫瘍、乳癌及びリンパ腫のような悪性疾病の位置決定のた
めに使用された。
ウィーンにおいて開発された、放射性ヨードで標識された天然由来のペプチド
であるVIP{血管作働性腸管ポリペプチド:4)}を用いるシンチグラフィーの
技術が種々の腫瘍の表示のためのもう一つの方法を与える。この放射性リガンド
(すなわち123I-VIP)は神経内分泌系腫瘍に高い親和性で結合するばかりで
なく、中でも、111In-DTPA-(D)Phe1-オクトレオチドによっては可視化できない
多数の腺癌にも結合する。しかしながら注目すべきは、カルチノイド及びインス
リノーマのような一連の初期腫瘍が123I-VIPによってのみならず111In-DTPA-
(D)Phe1-オクトレオチドによっても表示されることである。この場合に、ウィー
ンにおいて興味ある現象、すなわちVIP及びオクトレオチド/ソマトスタチン
が腫瘍細胞膜への結合について交叉競合的であることが観測された。
今日まで、異なった5つのヒト−ソマトスタチン受容体(hSSTR1-5)及び異な
った2つのVIP受容体を特定してクローニングすることができている。形
質転換された(transfektiert)ペプチド受容体の使用のもとに、ソマトスタチ
ン受容体の中でSSTR 3サブ型が腫瘍組織の中でソマトスタチンに対して最も拡が
った結合部位であることが見出された。また、VIPがSSTR 3に結合すること、
及びソマトスタチンとVIPとの初期腫瘍細胞への結合の交叉競合が存在するこ
とも示された。
ペプチドの腫瘍受容体への結合は腫瘍の局在診断(Lokalisationsdiagnose)
のためのみならずその転位の診断にも用いることができる。すなわち、診断の目
的に用いられる111In-DTPA-(D)Phe1-オクトレオチドは既に、積極的な放射能療
法のために受容体リガンドとして高い投与量でも使用された。
ヨーロッパ特許EP 0714911 A2から、通称オクトレオチドのオクタペプチドと
巨大環キレート化剤1,4,7,10-テトラアザシクロドデカン-N,N',N",N'"-4酢酸(
DOTA)との抱合体が放射性核種を、これが生体内で種々の腫瘍の治療のため
に使用できるように錯化合物化できると言うことが示されている。この特許は、
前述した物質の治療的使用に限定されている。オクトレオチドは、その間に見出
されたソマトスタチン−14サブ型受容体の若干とのみ高い親和性で結合する。
生体内では111In-DTPA-(D)Phe1-オクトレオチドは腎臓、肝臓及び脾臓において
も取り込まれ、これはこれらの器官の不必要に高い放射線負荷に導く。
本発明は、できるだけ広いソマトスタチン−14サブ型(hSSTR 2ないし5)識
別プロフィルを有し、そして健康な体組織に対する放射線負荷が僅かであり、従
って良好な生体分布(Biodistribution)を有するような、本文頭書に述べた類
の担体分子を得ることを課題とするものである。その効果は、腫瘍組織における
高い線量及び健康な組織、中でも脾臓及び腎臓の比較的低い放射線負荷であろう
。
同様なことは、放射性核種の代わりに重金属原子又は常磁性金属を用いた場合
にも当てはまる。
本発明はこの課題を、ソマトスタチン−14アゴニストとして下記一般式
のオクタペプチドがそのC末端又はN末端を介してその巨大環キレート化剤に結
合されている(例1)ことによって解決する。このオクタペプチドは「ランレオ
チド」の表示のもとに知られているが、巨大環キレート化剤との結合においては
知られていない。本発明のペプチドは金属を含まない緩衝液の中で放射性核種の
塩によって高められた温度において標識させることができる(例2ないし4)。
この高い標識効率が調剤キットの調製のための出発基盤を構成する。本発明のペ
プチドは種々の生理学的条件のもとで111In、90Y又は67/68Gaと安定なキレート
を形成する。例えば、このキレートはヒト血清の中で、又はジエチレントリアミ
ン5酢酸(DTPA)の1万倍の過剰の存在のもとで安定である。本発明のペプ
チドは稀土類(161Tb、153Sm等)とも安定なキレートを形成する。
ヨーロッパ特許EP 0 714911 A2は、90Y又は161Tbで標識された形で、及びα−
粒子、β−粒子又はオージェ電子を放射する放射性核種で標識された形でDOT
Aオクトレオチドを使用することをカバーしている。特に、異性体遷移を経た放
射性核種(99mTcのような)、電子を捕獲した放射性核種(すなわち、67Ga、111I
n)又は陽電子を放出する放射性核種(68Ga)は言及されていない(例8)。本発
明のペプチドはいずれの場合にも111In又は67/68Gaで安定に標識されることがで
き、また従って治療のためのみならず、放射線診断薬(SPECT及びPETの
ための)として使用するためにも適している。
本発明に従うオクタペプチドは広いソマトスタチン受容体識別プロフィルを有
する。特別には、この低いナノモル領域のKd値を有する放射能標識されたペプチ
ドはSSTR 2、SSTR 3、SSTR 4及びSSTR 5にも結合する(例5)。その放射能標識
されたリガンドはPC3及びDU145前立腺癌細胞、PANC1及びHT29腺癌細胞、A431上
皮細胞、ZR75-1及びT47D乳癌細胞並びに518A2黒色腫細胞にも高い結合親和性を
示す。その上に、このペプチドは乳癌、黒色腫、腺癌、リンパ腫、肝細胞腫瘍、
甲状腺腫瘍及び種々の神経内分泌系腫瘍におけるような一連の初期腫瘍に結合す
る。その腫瘍において発現される最も重大なソマトスタチン受容体はSSTR 3であ
るもののようであるが、このものは本発明のペプチドに対する標的−Rである(
例6)。実験において、このオクタペプチドの結合挙動と、巨大環キレート化剤
に結合したオクタペプチドのそれとは若干異なっ
ていたことが示されているが、これはこのオクタペプチドの結合された状態と非
結合の状態とにおける異なった親油性に帰せられるべきものであろう。
90Yで標識されたペプチドのSprague Dawleyラット(180-220g、4MBq、160pmol
)における生体分布は、血液からのこの物質の迅速な排除及び生理学的態様でSS
TR発現組織(膵臓のような)における高い吸収並びに貯留を示した。これに対し
て、通常は全く、又はほんの僅かしかSSTRを発現しない器官における吸収は放射
能の非常に僅かな吸収によって際立っていた。すなわち、例えば骨についての算
定は注射の48時間後においても注射量1g当り僅かに約0.01%の吸取しか与え
なかった(例7)。
上に上げた放射線核種に代えて、原子番号20ないし32、42ないし44、45及び57
ないし83の重金属原子又は金属イオンも医療的造影診断のために使用することが
できる。磁気共鳴造影のためには原子番号21ないし29、42、44又は57ないし71の
常磁性金属又は金属イオン、中でもGd3+、Mn2+又はDy3+を使用することができる
。更に、調剤目的のためにはアルカリ金属、アルカリ土類金属又は遷移金属、或
いはこれらの金属のイオン、中でもNa+、K+、Ca2+、Fe2+/3+、Zn2+及びMn2+を使
用することができる。
キレート化剤としては1,4,7,10-テトラアザシクロドデカン-N,N',N",NH'"-4
酢酸(DOTA)が有利に用いられた。ランレオチドとDOTAとから得られた
抱合体は比較的親油性であり、そしてヒトにおいて他のソマトスタチン14類似
体よりもよりゆっくりとした血液からの排除及びより高い腫瘍内貯留を示す。D
OTAの親水性に基づいてその用いたオクタペプチドの親油性は低下し、それに
よって一方においてそれら腫瘍の良好な造影性が維持されるとともに他方におい
て罹患していない器官、すなわち非標的の脾臓や腎臓のような器官においてはこ
の物質の比較的低い吸収が達成される。ヒトにおいて本発明の111In-標識された
ペプチドを直接111In-DTPA-(D)Phe1-オクトレオチドと比較した。本発明のペプ
チドの静脈内適用(3mCi、6nmol)の後で111In-DTPA-(D)Phe1-オクトレオチド
よりも、より高い腫瘍内吸取、より低い腎臓内貯留、並びに低下した脾臓内への
吸収が見出された(例8)。
有利には、診断のために放射性核種として111In又は67/68Gaがそのキレ
ート化剤に結合されていることができ、これはこの放射性核種が診断に適して身
体には比較的低い放射線負荷しか伴わないγ線を放出すると言う利点を有する。
腫瘍組織の破壊のためには放射性核種として90Yがキレート化剤に結合されてい
ることができ、このものは公知のように腫瘍細胞の破壊をもたらすβ線を放出し
、その際このオクタペプチドの特定的な結合に基づいてその放射性核種の作用は
本質的にその罹患した細胞に限定されて留まる(例8)。90Yはその放射線到達
距離(β-線、2.3MeV)に基づいて比較的大きな腫瘍の処置に良好に適している
が、比較的小さな腫瘍は、同様に本発明のペプチドに結合されていることのでき
る161Tb(β-線、0.5及び0.6MeV)又は153Sm(β-線、0.7及び0.8MeV)のような
他のβ線放射性核種によってより良好に処置することができる。この理由から、
腫瘍の患者(その腫瘍はSSTR 2ないし5、中でもSSTR 3を発現する)を本発明の
ペプチドよりなる混合物で処置するこことを考えることができる(90Yで標識さ
れた、及び161Tbで標識された本発明のペプチドのような、放射線療法用の放射

治療に随伴する調整のためにソマトスタチン受容体陽性の腫瘍疾病又は内分泌
学的疾病の評価のためにこの放射能標識されたペプチドを繰り返し使用すること
ができる。実施例

[ε-Boc-lys5]-ランレオチドを15mlのH2Oと30mlのN,N-ジメチルホルム
アミドとに溶解する。100mgのN,N'-ジ−シクロヘキシルカルボジイミドを

チドをエーテルの使用のもとに沈殿させ、そしてC18逆相HPLCカラムによ
り水/アセトニトリル1/0.1%トリフルオロ酢酸溶剤混合物の使用のもとに
精製する。水/アセトニトリル/1%酢酸の溶剤混合物の使用のもとに第2のH
PLC精製段階を引き続いて行ない、本発明の純生成物を酢酸塩として得る。

例2:このペプチドの111Inによる放射能標識化
本発明のペプチドを0.2モル/lの酢酸アンモニウム緩衝液(pH7、金属
を含まない)に溶解し、そして0.05モル/lのHClの中の111InCl3(担体な
し、Me++<0.1μg/mCi,<1μg/ml)によりインキュベーションする。0.5mCi111
In/1nmol DOTA−ランレオチドの比率が作られ、そしてこの反応混合物を
100℃において30分間にわたり煮沸する。1部を薄層クロマトグラフィーに

例4:ペプチドの67/68Gaによる放射能標識化
本発明のペプチドの放射能標識化のために111InCl3の代わりに67/68GaCl3を用
いる。
例5:111In/90Yで標識したペプチドの試験管内での結合
本発明の標識された物質及び非標識物質を飽和実験並びに競合実験において試
験管内で分散した腫瘍細胞(初期腫瘍、生存細胞)へのそれらの結合並びにソマ
トスタチン受容体サブ型SSTR 1ないしSSTR 5へのそれらの結合について試験した
。このためにその各サブ型の受容体をcDNAプローブの使用のもとにCOS-7細胞
の上に発現させた。比較のための試験は他のソマトスタチン受容体アゴニストわ
用いて行なった。比較試験においても放射性リガンドとして125I-Tyr11-ソマト
スタチン−14を用いた。本発明の111Inで標識されたペプチドと90Yで標識され
たペプチドとの間に結合挙動についてなんらの本質的差異は見出されなかった。
しかしながら、下記のデータは、本発明のペプチドと111In-DTPA−(D)Phe1-オク
トレオチドとの間にSSTR 3並びにSSTR 4への結合について本質的な差異が存在す
ることを明らかに示している。例6:ソマトスタチン受容体サブ型3を発現する腫瘍
胃癌、大腸及び直腸の癌、皮膚癌、乳癌、膵臓、甲状腺腫瘍、リンパ腫、肺癌
、種々の類癌腫のような分散性神経内分泌系腫瘍、胃癌、褐色細胞腫、前立腺癌
、腺癌、及びこれらの腫瘍の娘腫瘍。
例7:動物実験における90Yで標識されたペプチドの生体分布
ラットにおいて本発明の90Yで標識したペプチドの生体分布を試験した。その
際この放射能標識したペプチドをSprague Dowleyラット(180-220g、4MBq、

及び後方投射における全身撮像並びにより正確な腫瘍位置の決定及び大きさの評
価のためにシングルフォトン放射トモグラフィー(SPET)を含んだ。個別の
器官及び腫瘍並びに転位における相対的撮像は「目的領域」の照準によって求め
た。
線量測定データはMIRDプログラムを用いてコンピュータ計算した。
111In-DTPA-オクトレオチドと111In-DOTA-ランレオチドとの間の比較の結果は
添付図の第1A図ないし第1D図にグラフとして示してある。
第1A図から、本発明の物質は111In-DTPA-(D)Phe1-オクトレオチドに比して
