多発性硬化症を治療するための
ペントキシフィリンとI型インターフェロン類の併用
本発明は、間欠性および続発性の進行性多発性硬化症を含む間欠性の多発性硬
化症(以下MSと称す)を治療するためのペントキシフィリンI型インターフェ
ロン類の併用に関する。詳しくは、本発明は間欠性MSを治療するための皮下注
射によるβ―インターフェロンおよび経口投与がなされる医薬の形態のペントキ
シフィリンの併用に関する。
併用および併用製剤の形態で、本発明は後記特許請求の範囲に詳細に述べる特
徴を有する。
間欠性および慢性両者の進行型MSを治療するのにペンントキシフィリンを使
用することは、ドイツ特許第4343034C1号明細書によって公知である。
現在、β―インターフェロンによる予防治療法(1日おきに8×106単位を皮
下投与する)が、MSのエピソードを少なくする唯一の知られている治療法であ
る。
上記ドイツ特許第4343034C1号明細書によれば、ペントキシフィリン
とコルチコステロイド類は併用投与すると、これら2種の医薬の併用には相乗効
果があるので、MS
エピソードを短時間で治療するためこれら2種の医薬の組み合せを投与すること
ができる。急性多発性硬化症のこの種治療法に用いる好ましいコルチコステロイ
ド類は、プレドニゾン、プレドニゾロン、プレドニゾリン−21−アセテート、
プレドニリデンおよびメチルプレドニゾロンであり、特にメチルプレドニゾロン
が好ましい。
多発性硬化症(MS)は、世界のこの地域で最も多い神経障害の一つである。
MSは、通常、成人期の早期に発症し、絶え間なくまたは間欠的に進行する。い
まのところ、その正確な発現機序は分かっていない。しかし、遺伝子配列および
環境要因の両者が役割を演じているようである。
MSの公知の臨床像の一つは患者の中枢神経系(CNS)における脱髄の藩種
性斑である。この脱髄は、炎症が原因であるが、中枢神経系中に間欠的に起こり
、MSの症状の特徴である神経機能喪失を伴うエピソードをもたらす。その上に
、患者の約1/3は、着実に進行する新しい段階を発現し、この段階は明確に限
界を決定することができる。進行性脱髄以外の病理機序もこの後期段階で役割を
演じていると考えられる。
上記の新しいMSの治療方法以外に、MSは、従来、エピソードが起こったと
きに、コルチコイド類を投与することによって(例えば3日間にわたってメチル
プレドニゾロンを1
日当たり1g静注する)、症状だけを治療することができた。すなわちこの投与
によってエピソードが短くなる。しかし、コルチコステロイド類を長期間投与す
ることによるいくつもの重篤な副作用はよく知られていることにかんがみて、こ
れらのホルモン類で連続して予防治療を行うことは推奨できない。このことに関
連して、コルチコステロイド類の投与が有効である他の疾患、例えばI型真性糖
尿病にかかっている患者を、副腎皮質ホルモン類またはその誘導体で治療しよう
という考えに対して別の重大な疑問点がある。
このような医薬による治療は、極めて困難でかつ危険である。副作用と治療の
問題点を回避するため、いくつもの他のグループの医薬がMSに対するそれら医
薬の有効性について検査されている。
いくつもの研究結果は、例えばアザチオプリンまたはシクロスポリンAなどの
免疫抑制剤でMSを長期間治療しても、MSの発現に対し著しい効果がないこと
を明確に示している。しかし、組換え大腸菌β―インターフェロンを皮下投与す
ると、最初は、エピソードが顕著に減少した。しかしながら、患者に対してβ―
インターフェロンを投与することは、いくつもの副作用がありかつ非常に高価な
ので困難である。
したがって、本発明の目的は、MSの治療法またはMS治療のための医薬の使
用法を提供するものであり、これらの方
法はそれぞれ、上記の欠点を克服している。本発明の前記目的は、驚くべきこと
には、間欠型のMSを治療するのに、皮下に投与されるI型インターフェロン例
えばα―インターフェロンおよび/またはβ―インターフェロンならびに経口投
与されるペントキシフィリンの併用治療法を用いることによって達成される。
これら2種の有効成分、すなわちI型インターフェロンとペントキシフィリン
の併用法は、104〜108単位、特に106〜16×106単位のβ―インターフ
ェロンまたはα―インターフェロンを皮下に投与し、一方100〜1600mg
または2000mgのペントキシフィリンを、場合によっては徐放型で、1日に
1〜5回経口で投与することからなる方法である。インターフェロンとペントキ
シフィリンの投与量は治療を受ける患者の全身状態と体重によって決まる。原則
として、インターフェロンの投与量は、皮下投与を行う場合、体重1kg当たり
、1.5×105〜30×105I.U.であり、好ましくは3×105〜20×105I
.U.であり、そしてさらに好ましくは、皮下または筋肉内に投与される場合1.5
×105I.U.である。0.3×105、1×105、2×105、2.5×105、3.
0×105、さらに3.5または4.0×105I.U.までの投与量を、溶液で皮下ま
たは筋肉内に投与することができる。106の単位で示した8×106の単位の投
与が好ましいが、患者1名当た
り、2、4、6、10、12、14、15、16またはさらに18〜20×106
単位までの量を投与することができる。
天然起源および遺伝子工学的起源の両者のインターフェロンを治療に使用でき
る。この有効成分の濃度は、1回投与用の1本のアンプルまたはバイアル当り1
04I.U.〜108I.U.になる濃度である。アンプルで提供される場合、医薬は、液
体に溶解された形態、または凍結乾燥物としてすなわち注射用の凍結乾燥された
水溶性粉末として提供される。
上記注射溶液または凍結乾燥物の生理学的pH値を調節するために使用される賦
形剤は、暖衝系であり、例えばグリシン/水酸化ナトリウム、リン酸二水素ナト
リウム/リン酸水素二ナトリウムもしくはそのカリウム塩、トリス(ヒドロキシ
メチル)アミノメタン/HClまたはナトリウム/酢酸がある。適切な溶媒は、
適用可能な薬局方の規定による注射用の水または蒸留水である。マンニトール、
デキストラン、白糖、ヒトアルブミン、乳糖およびポリビニルピロリドンなどの
ような構造用薬剤が凍結乾燥物を製造するのに必要である。
注射溶液および凍結乾燥物の両者は滅菌状態で関連する注射用バイアルまたは
アンプルに充填される。
個々のペントキシフィリンの投与量は、経口投与がなされる場合、各々少なく
とも400mgであるが、1回投与当り300、500、600、800、10
00、1200、1
500〜1600またはさらに2000mgでもよい。経口医薬の形態で投与さ
れる場合、徐放型の製剤が好ましい。一日の投与量は300もしくは400mg
〜4、500mgの範囲内で変えることができる。1名の患者当り1日の投与量
としては1200mgが好ましい。しかし、1名の患者に対する1日の全投与量
は、500、600、1,000、1,200、1,500、2,000、2,
500、3,000、3,500、4,000または4,500mgでもよい。
500mg/日〜2,500mg/日に範囲内が好ましい。体重に基づいた、1
回投与用の投与量は、少なくとも3〜300mg/kgであり、3〜18mg/
kgの範囲が好ましく、6mg/kgの一回投与量が特に好ましい。1日当りの
合計投与量は、一度に全部を投与してもよいが、1日に2〜5回で投与する方が
好ましい。
ペントキシフィリンの徐放または通常行うことができる投与形態は、錠剤、コ
ーチング錠、カプセルまたは顆粒である。投与形態1個当りのペントキシフィリ
ンの投与量は200〜1,000mgまたは2,000mg以下である。下記の
賦形剤は徐放形の投与形態にとって適している。すなわち、エチルセルロース;
アクリル酸メチル類、アクリル酸およびメタクリル酸のエステルとトリメチルア
ンモニウムメタクリレートとの共重合体;アクリレート系分散剤;アルギン酸類
;
ヒドロキシプロピルメチルセルロース;エチルセルロース;セチルステアリルア
ルコール;またはステアリン酸を混合した水素化ヒマシ油である。これらの物質
は、通常のガレヌス結合剤類;充填剤類;潤滑剤類;および徐放剤類、例えば乳
糖、デンプン類、微結晶性セルロース、リン酸二カルシウム、リン酸三カルシウ
ム、コロイドシリカ、タルク、ステアリン酸マグネシウム、ポリビニルピロリド
ンまたはメチルセルロースなどと組合せて使用される。
可能なカプセル製剤は軟質または硬質のゼラチンカプセルの製剤である。カプ
セル剤を製造するのに適切な賦形剤は、通常の担体、徐放性物質、潤滑剤および
希釈剤またはゼラチンなどのカプセル充填剤である。高投与量のペントキシフィ
リン医薬製剤を製造する場合、摂取し易いように長円形のカプセルが好ましい。
また本発明は、包装容器中に、別々にインターフェロンを含有する注射溶液ま
たは凍結乾燥物の入ったアンプル、ならびに、場合によっては徐放型で、錠剤、
コーチング錠、カプセルまたは顆粒の形態のペントキシフィリンが入っており、
そして両有効成分の適切な投与単位が上記範囲内に入っていることを特徴とする
併用医薬製剤を提供するものである。
今までのところ、我々の研究は、インターフェロンとペントキシフィリンの併
用による相乗効果についてなにも説明し
ていない。これまで分かっていることは、γ―インターフェロンが誘発する効果
[自己免疫性T細胞の治性化、接着分子および組織適合抗原の誘発、ならびにT
NF−α(腫瘍瘍壊死因子)の放出]の大部分に対してα―またはβ―インター
フェロンが拮抗するということだけである。インターフェロンによる治療の初期
に、症状がかなり悪化し、重篤な副作用、例えば発熱、インフルエンザ様の症状
、倦怠感、関節の痛みなどが起こることは、各種の実験が示している。これらの
症状の一部は、インターロイキン1およびTNF−αなどの内因性発熱因子の放
出およびIFN−γを産生する細胞の比率の増大によって説明できる。生体外の
実験がインターフェロン、例えばIFN−αおよび特にIFN−βとペントキシ

パク質(MSの関連自己抗原)に対して誘導されるTNF−αおよびγ―インタ

ら2種の有効成分の併用による効果を示すため、下記の症例研究を提供する。症例1
25歳の男性の患者が、3年前、多発性硬化症であると診断された。最初のエ
ピソードが6ヵ月間隔で起こったがコルチゾンを投与後、迅速に改善された。最
初、左脚の麻痺、歩
行の失調、尿と糞便の間欠的失禁、および一過性複視が診断された。昨年はずっ
と、コルチゾンによって治療している際に寛解していた部分にのみエピソードが
起こり、そして歩行の失調障害の増大が観察された。安定期に、β―インターフ
ェロン[8×106単位のBetaseron(登録商標)〕を皮下に投与し、同時に3×
400mgのペントキシフィリンを徐放投与する治療を開始した。この治療中、

を示さなかった。どの患者も、ペントキシフィリンとの併用治療でβ―インター
フェロンを十分に投与した際に、著しい副作用を経験しなかった。他の症例で説
明したβ―インターフェロンによる治療による副作用と比較して、またはさきに
概略を述べた医薬治療に関連して、生体外の実験で見出された望ましい相乗効果
が、併用医薬治療中の患者にも観察できると考えられる。MSの免疫病理学的発
