血液調節化合物
発明の分野
本発明は、血液調節活性を有する化合物であって、造血を刺激し、ウイルス、
真菌および細菌感染症の治療に用いることができる新規な化合物に関する。
発明の背景
造血系は終生に及ぶ細胞再生プロセスであり、このプロセスにより特定の幹細
胞集団が少なくとも9種の異なる細胞系統(赤血球、血小板、好酸球、好塩基球
、好中球、単球/マクロファージ、破骨細胞およびリンパ球)のより大きな集団
の成熟分化血液細胞(Dexter TM.Stem cells in normal growth and diseas
e.Br.Med.J.1987;195:1192-1194)を生じさせる(Metcalf D.,The Molecu
lar Control of Blood Cells.1988;Harvard University Press,Cambrid
ge,MA)。さらに、幹細胞は、最終的に、細胞毒性物質で治療した後または骨
髄移植後の骨髄の再生に関与している。
大部分の標準的な抗腫瘍薬の用量−限定の(dose−limiting)主な毒性は骨髄
抑制に関連付けられ、これが重度であるかまたは長引くと、生命を脅かす感染性
および出血性合併症を引き起こしうる。骨髄抑制は予測可能であり、単一薬剤の
I相試験細胞毒性化合物の50%以上で用量−限定的であると報告されている(
Merrouche Y.,Catimel G.,Clavel M.,Hematopoietic growth factors
and chemoprotectants; should we move toward a two-step process for phas
e Iclinical trails in oncology?Ann Oncol 1993;4;471-474)。感染の危険
性は、好中球減少症の重篤度および期間により測定される骨髄抑制の程度に直接
関係する(Brody GP,Buckley M,Sathe YS,Freireich EJ.Quanti
tative relationship between circulating leukocytes and infections with a
cute leukemia.Ann InMed 1965;64:328-334)。
造血作用の調節は、初期多機能幹細胞および成熟循環エフェクター細胞を含む
、
造血カスケードの種々の段階での種々のサイトカインおよび増殖因子の相互作用
に影響を与える。これらの調節分子として、顆粒球コロニー刺激因子(G−CS
F)、顆粒球−マクロファージ刺激因子(GM−CSG)、マクロファージ−コロ
ニー刺激因子(M−CSF)および宿主防御において主要な役割を果たす重複す
る、付加的かつ相乗的作用を有する種々のインターロイキンが挙げられる。機構
的には、これは、顆粒球およびマクロファージの産生の向上、ならびにエフェク
ター細胞機能の活性化により達成される(Moore MAS.Hemopoietic growthf
actor interactions: in vitro and in vivo preclinical evaluation.Cancer
Surveys 1990;9:7-80)。これらの活性を調整することで、細菌、ウイルスおよ
び真菌感染症を克服するのに必要な最適宿主防御が支持される。
好中球減少症の重篤度および骨髄毒性を予防または軽減させる方法として、造
血増殖因子および/または他の造血サイトカインの使用が挙げられる。このよう
な治療法は、抗腫瘍薬の効能を改善させるかもしれない細胞毒性薬の用量を増加
させる可能性を付与し、抗腫瘍薬の使用に伴う罹患性を減少させるため、該治療
法は一般的に行われるようになってきている(Steward WP.,Granulocyte an
d granulocyte-macrophage colony stimulating factors,Lancet 1993;342:15
3-157)。臨床研究により、G−、GM−および/またはM−CSFが、細胞毒性
化学療法を受けている悪性腫瘍の患者または骨髄移植後の感染の危険性が高い患
者において好中球減少症の期間を減少させ、骨髄性回復を促進し、好中球減少症
に伴う感染症および他の感染性合併症を減少させうることがわかった(Steward
WP.,Granulocyte and granulocyte-macrophage colony stimulating factor
s,Lancet 1993;342:153-157およびMunn DH,Cheung NKV.Preclinical
studies of macrophage colony-stimulating factor.Semin Oncol.1992;19:3
95-407)。
合成ペプチドは、骨髄基質要素からのm−CSFを含め、造血メディエイター
の合成および放出を誘発すると報告されている。米国特許出願番号08/001
905を参照のこと。
本発明者らは骨髄造血細胞に対して刺激効果を有するある種の新規な非ペプチ
ド化合物を見いだした。これらは、組織反応を抑制する免疫抑制治療による、す
なわち骨髄移植手術における骨髄機能が低下した患者を含め、骨髄損傷、無顆粒
球症および再生不能性貧血を含む骨髄造血活性の減少に苦しむ患者における骨髄
造血の刺激に有用である。これらはさらに腫瘍およびウイルス病に関する細胞増
殖抑制性化学療法および放射療法を行った後の骨髄のより迅速な再生の促進に用
いられる。これらは、患者が骨髄不全後の免疫応答の欠如による重い感染症を有
する場合に特に有用である。これらはウイルス、真菌および細菌病の治療および
予防において有用である。
発明の要約
本発明は、血液調節活性を有し、造血作用の刺激ならびに細菌、ウイルスおよ
び真菌病の予防および治療に用いることができる、後記に式(I)で表す、化合
物を含む。
これらの化合物は、種々の臨床的状況、例えば手術により誘発された骨髄抑制
、AIDS、ARDS、先天性脊髄形成異常、骨髄および臓器移植に由来の細胞
数の減少した患者における白血球の回復;白血球減少症の患者の感染症からの防
御;重症の火傷患者の治療;いくつかの細胞周期特異性抗ウイルス剤について観
察される脊髄抑制の軽減;骨髄移植を受けた患者、特に移植片対宿主疾患の患者
における感染症の治療、結核の治療およびヒトおよび動物における原因不明の熱
病の治療において有用である。化合物はさらに免疫抑制および「正常」対象の両
方におけるウイルス、真菌および細菌感染症、特にカンジダおよびヘルペスの治
療および予防においても有用である。これらはグラム陰性およびグラム陽性生物
により引き起こされる敗血症の治療において有用である。
これらの化合物はさらに同時係属の米国出願番号07/799465および米
国特許第4499081号(出典明示により本発明の一部とする)の脊髄抑制剤
と組み合わせて用いて、骨髄細胞における活性の高低の交互のピークを得、造血
の自然の約24時間周期のリズムを促進できる。このように、細胞増殖抑制療法
を低骨髄活性の期間に適用することができ、かくして骨髄損傷の危険性が軽減さ
れ、一方で次の活性のピークにより再生が促進されるであろう。
本発明はさらに、式(II)の化合物と医薬上許容される担体とを含んでなる医
薬組成物を提供する。
本発明はさらに、ヒトを含む動物の骨髄造血系の刺激法であって、これを必要
とする動物に、有効量の式(I)または式(II)の化合物を投与することからな
る方法を提供する。
本発明はさらに、免疫抑制および正常な、ヒトを含む動物におけるウイルス、
真菌および細菌感染症の予防および治療法であって、これを必要とする動物に有
効量の式(I)または式(II)の化合物を投与することからなる方法を提供する
。
発明の詳細な記載
本発明の化合物は、構造式I:

リドニル、ピロリジニルまたはN−メチル−2−イミダゾリルであり;
R2は、独立して、水素、C1-4CO2HまたはC1-4アルキルであり;

本発明はまた、人を含む動物の骨髄造血を刺激する方法であって、これを必要
とする動物に有効量の式(II)の化合物を投与することからなる方法に関する。
C1-4アルキル基は直鎖であっても分枝鎖であってもよい。
本発明の化合物は1またはそれ以上の不斉炭素原子を含有し、ラセミおよび光
学活性な形態で存在してもよい。これらのすべての化合物およびジアステレオマ
ーは本発明の範囲内にあると考えられる。
前記の式(II)におけるR1は、置換されていてもよいピロリル、イソピロリ
ル、ピラゾリル、イソイミダゾリル、トリアゾリル、イソキサゾリル、オキサゾ
リル、チアゾリル、イソチアゾリル、オキサジアゾリル、ピリジニル、ピリダジ
ニル、ピリミジニル、ピラジニル、ピロリジニル、ピペラジニル、トリアジニル
、モルホリニル、インドリル、インドレニニル、イソベンザゾリル、ピリンジニ
ル、イソインダゾリル、インドキサジニル、ベンズオキサゾリル、キノリニル、
イソキノリニル、シンノリニル、キナゾリニル、ナフチリジニル、ピリドピリジ
ニル、テトラヒドロキノリニル、テトラヒドロイソキノリニル、キノキサリニル
、インドリニル、2−ピロリドニル、イミダゾリル、イミダゾリジニル、イミダ
ゾリニル、ピペリジル、テトラゾリル、キヌクリジニル、アゼチジニル、または
プリニルを意味する。
式(I)および式(II)の好ましい化合物は、R1がキノリニル、2−ピリン
ジニルまたは2−ピロリドニルであり;R2が水素またはC1-4CO2Hであり;
Yが(CH2)n、(CH2)mC≡C(CH2)sまたは(CH2)mCH=CH(CH2)sであ
り;nが3ないし5の整数であり;mが1または2であり;sが1または2であ
る化合物である。
式(I)および式(II)のさらに好ましい化合物は、R1が2−ピリンジニル
または2−ピロリドニルであり;R2が水素であり;Yが(CH2)nまたは(CH2)m
CH=CH(CH2)sであり;nが3または5であり;mが1であり;sが1で
ある化合物である。
式(I)の好ましい化合物は:
N,N’−ビス(ピコリノイル)−1,5−ジアミノペンタン;
(E)N,N−ビス(ピコリノイル)−1,4−ジアミノブト−2−エン;
N,N’−ビス(ピコリノイル)−1,4−ジアミノブタン;および
(R,R)−N,N’−ビス(2−ピロリドン−5−カルボニル)−1,3−
ジアミノプロパン;または
3,10−ビス(ピコリノイル)−3,10−ジアザ−1,12−ドデカンジ
オイックアシッド
である。
上記の式(I)の好ましい化合物に加え、式(II)の好ましい化合物には、N
,N’−ビス(ピコリノイル)−1,3−ジアミノプロパンが含まれる。
調製法
R2が水素であり;R3がC1-4アルキルであり;YおよびR1が式(I)および
(II)について定義したとおりである式(I)および式(II)の化合物は、スキ
ーム1に記載したのと類似した方法により調製される。
スキーム1 a)EDC、ピリジン
適当なジアミン(スキーム1における2など)を、適当な極性非プロトン性溶
媒(ピリジンなど)中、活性化剤(EDCなど)を用い、適当な複素環酸(スキ
ーム1における1など)でビス−アシル化する。
R2がC1-4アルキルであり;R3がC1-4アルキルであり;Y、R1およびnが
式(I)および(II)について定義したとおりである式(I)および式(II)の
化合物は、スキーム2に記載したのと類似した方法により調製される。
スキーム2 a)(BOC)2O、CH2Cl2;b)NaH、THF;c)MeI;

適当なジアミン(スキーム2における1など)を通常のアミン保護基(t−ブ

ついで、アルキル化剤(ヨウ化メチルなど)で処理して、N−アルキル化する。
標準的な酸性条件(4NHCl/ジオキサンなど)下でスキーム5における3の
窒素保護基を除去し、ついで、得られたアミンを適当な溶媒(ピリジンなど)中
、適当な酸(スキーム5における4など)でアシル化し、保護されたチオールを
得る。適当な酸源(HFとアニソールなど)を用い、ベンジル基を除去する。つ

ど)中、アルケニルハライド(アリルブロマイドなど)の存在下、強塩基(水素
化ナトリウムなど)と反応させる。スキーム6における2のビス−オレフィンを
酸化剤(NaOMe/MeOH中のオゾンなど)で酸化的に開裂させ、スキーム
6における3のビス−エステルを得る。標準的な酸性条件(4NHCl/ジオキ
サンなど)下、末端のジアミノ−保護基を除去し、ついで、得られるアミンを適
当な溶媒(ピリジンなど)中、活性化剤(EDCなど)を用いて適当な酸(ピコ

トン性溶媒(DMFなど)中、ハロ−エステル(メチルブロモアセテートなど)
の存在下、強塩基(水素化ナトリウムなど)と反応させる。標準的な酸性条件下
(4NHCl/ジオキサンなど)、スキーム7における2の末端のアミノ保護基を
除去し、ついで、得られるアミンを適当な溶媒(ピリジンなど)中、活性化剤(
EDCなど)を用い、適当な酸(ピコリン酸など)でアシル化する。標準的な条
件(トリフェニルホスフィンなど)下、硫黄保護基を除去し、得られるチオール
を標準的な手段(pH7.8における空気暴露など)により酸化し、スキーム7

れる。
適当に保護されたジアミン(スキーム8における1など、スキーム4に示され
るように調製される)を非プロトン性溶媒(THFなど)中、強塩基(水素化ナ
トリウムなど)と、ついで、ハロアルケン(アリルブロマイドなど)と反応させ
、ビス−N−アルキル化する。スキーム8における2のビス−オレフィンを酸化
剤(NaOMe/MeOH中のオゾンなど)で酸化的に開裂させ、スキーム8に
おける3のビス−エステルを得る。標準的な酸性条件(4NHCl/ジオキサン
な
ど)下、末端のジアミノ−保護基を除去し、ついで、得られるアミンを適当な溶
媒(ピリジンなど)中、活性化剤(EDCなど)を用いて適当な芳香族酸(ピコ
リン酸など)でアシル化する。中央のアミノ保護基を適当な触媒(10%Pd/
Cなど)の存在下、標準的な還元条件(MeOH中のH2など)下で開裂させ、
スキーム8における4のアミノエステルを得る。メチルエステルを塩基性水性条
件(1NNaOHなど)下、けん化し、スキーム8における生成物5を得る。
式(I)および(II)の化合物またはその医薬上許容される塩をヒトおよび他
の哺乳動物の治療に用いるためには、通常、標準的な製薬慣習にしたがって医薬
組成物として処方される。
本発明のさらに別の特徴により、活性成分として、1またはそれ以上の前記し
た式(II)の化合物またはその生理学上適合しうる塩を医薬担体または賦形剤と
合わせて含んでなる医薬組成物が提供される。本発明の組成物は、たとえば経口
、経鼻、非経口または経直腸投与に適した形態で提供してもよい。
本明細書において用いる場合、「医薬上」なる用語は、本発明の獣医学的用途を
包含する。これらの化合物を経口投与用にカプセル化、錠剤化あるいはエマルジ
ョンまたはシロップに調製する。医薬上許容される固体または液体担体を添加し
て、組成物を増強したり、安定化したり、あるいは組成物の調製を容易にするこ
とができる。液体担体は、シロップ、落花生油、オリーブ油、グリセリン、食塩
水および水を包含する。固体担体は、デンプン、ラクトース、硫酸カルシウム二
水和物、白土、ステアリン酸マグネシウムまたはステアリン酸、タルク、ペクチ
ン、アカシア、寒天またはゼラチンを包含する。担体はまた、単独でまたはワッ
クスを含む、グリセリルモノステアラートまたはグリセリルジステアラートなど
の徐放性物質を包含する。固体担体の量は変わるが、好ましくは、投与単位当た
り約20mgないし約1gの間である。医薬製剤は、錠剤形の場合、粉砕、混合
、顆粒化、および必要ならば圧縮を含み;またハードゼラチンカプセル形の場合
、粉砕、混合および充填を含む通常の調剤技術にしたがって調製される。1また
は数種の活性成分を含有するカプセルは、例えば、活性成分をラクトースまたは
ソルビトールなどの不活性な担体と混合し、混合物をゼラチンカプセル中に充填
す
ることにより調製される。液体担体を用いる場合、製剤はシロップ、エリキシル
剤、エマルジョンまたは水性または非水性懸濁液の形態である。このような液体
製剤は、直接経口投与されるか、またはソフトゼラチンカプセル中に充填される
。臓器特異性担体系もまた用いられる。
別法として、本発明の化合物またはその誘導体の医薬組成物は、非経口投与用
凍結乾燥粉末の溶液として処方してもよい。粉末は、使用前に適当な希釈剤また
は他の医薬上許容される担体の添加により復元される。液体処方は、一般に緩衝
化された等張水溶液である。適当な希釈剤の例は、通常の等張塩溶液、標準的5
%水中デキストロースまたは緩衝酢酸ナトリウムまたはアンモニウム溶液である
。このような製剤は非経口投与に特に適しているが、経口投与用に用いることも
できるし、吸入用に計量吸入器またはネブライザー中にいれてもよい。ポリビニ
