【発明の詳細な説明】
【０００１】
【発明の属する技術分野】本発明は、プロテーゼ（補て
つ物）構造に関し、特に、そのプロテーゼのステムがそ
の近位端を骨窩（bone socket）にセントラライズする
ための手段と共に供給されたプロテーゼ構造に関する。
【０００２】
【課題を解決しようとする手段】本発明によれば、セメ
ントを用いて骨窩に固定するためのステムを供給された
プロテーゼに、骨窩の内壁に接触し、その中にステムの
近位端をガイドする様に寸法が決定された複数或いは多
くの成形した突起がステムの近位部に供給されている。
好ましい構造において、前記突起は、最初の植え込みの
後にステムが骨窩内で更に下方に移動する場合には無効
となり得るように取り外し可能な取り付け手段により前
記ステムに固定されている。
【０００３】前記突起は、ステムの近位端または遠位端
に向いた涙滴の形を有しうる。取り付け手段は、取り外
す可能な接着剤、シャーピンにより、または突起の平滑
面とステムの平滑面間の相互粘着により供給できる。突
起は、ポリメチルメタクリレート（ＰＭＭＡ）の様な任
意で好都合な材料により製造可能である。一つ好ましい
構造として、三角形、正方形または長方形断面を有する
突起はステムの角に配置されている構造がある。

【発明の実施の形態】本発明は、種々の方法により実施
可能で、以下、幾つかの実施態様を具体例として、添付
図を参考に本発明を説明する。図１に示すように、大腿
骨プロテーゼ１に本発明に従って装備されている。プロ
テーゼ１は、患者の大腿骨５の骨窩４にセメント３で固
定するためのステム２を含む。ステム２の近位端６に
は、プロテーゼステムの周囲に環状に配置された多く
（４個）の成形した突起７が供給されている。図１に示
すようにステムの一般的な断面は長方形で、突起７が各
面に供給されている。
【０００５】各突起は、裏面８が平らな涙滴の形状を有
する。この構造において、突起７は取り外せる接着剤の
薄層９により供給される、取り外し可能な取り付け手段
によりステム２に固定されている。突起は、その外面が
骨窩４の内壁１０に接触し、その中にステム２の先端６
をガイドする働きをするようにその寸法が決定される。
除去しうるセメントは、プロテーゼの挿入時、そして骨
窩の内壁４と密着するまで、突起を適所に保持するのに
十分な接着力がある。表示されているプロテーゼは、シ
ョルダーレスタイプで、ステム７が骨窩にて最初の植え
込み後に下方に移動する場合には、接着剤は接着力を失
いこの更なる移動を可能にする。
【０００６】突起は、金属または合成プラスチック材料
の様な任意で好都合な材料のから製造可能で、ポリメチ
ルメタクリレート（ＰＭＭＡ）から製造できるのが好ま
しい。その後下方に移動することを意図されたプロテー
ゼのステムは、通常高度に研磨されており、この場合、
各突起の裏側壁面には同様に平滑な面が提供されている
ため、２つの平滑面間の相互接着力により、突起が適切
に配置でき、またその位置が保持される。しかし、もし
もその後ステムが下方に移動する場合には、両面間の接
着力は解消される。
【０００７】図２は、平滑な裏面１２を有する別の涙滴
の形の突起構造を示す。各突起には、ステム２の壁の穴
１４に密着する小さなシャーピン１３が供給されてい
る。シャーピン１３は、突起を適切な位置に保持するに
は十分であるが、ステムがセメントで固定された骨窩で
下方に移動すると折れる程度に弱い。別の実施例（表示
されていない）において、シャーピン１３は穴１４に配
置されているが、そこから突出している別の部品で、突
起１１はそれを受け入れる類似の穴を持つ。
【０００８】図３に示す別の構造において、図１に使用
されている部品と類似部品を示すために同じ番号が使用
されているが、この構造においては、成形した突起は、
ステム２の近位端６に配置されるように成形され、寸法
が決定されたバンドまたはカラー１５に設置されてい
る。バンドはステム２が先細になっているため適切な位
置に保持され、突起７は図２に示す接着剤またはシャー
ピンの設置により、適切な位置に同様に保持することが
できる。バンドは金属または合成プラスチック材料の様
な好任意で都合な材料から製造可能である。
【０００９】上記構造に示すように、涙滴の形の突起
は、細い先端がステムの近位端を向くように配置されて
いるが、細い先端がステムの先端を向くように配置する
こともできる。特定の状況においては、この後者の構造
により、ステムを骨窩に挿入し易くなる。
【００１０】図４は、ステム２の先端断面図を示し、こ
の場合突起７はステムの角１５に配置されている。従っ
て、突起の断面は、Ｌ形または三日月形となる。この場
合も、突起は涙滴の形で、細い先端はステムの近位また
は遠位端を向く。同様に、これらの突起は、図１、２ま
たは３に説明されている手段のいずれによっても固定で
きる。長方形断面のステムに関して説明されているが、
この構造の機能は、ステムの角に突起を供給することな
ので、ステムは三角形、正方形または長方形、あるいは
その他の好都合なあらゆる形態が可能である。
【００１１】上記の構造において、ステムは少なくとも
２枚の平らな側面を有する傾向があるが、本発明は、ど
の様な断面のステムにも、たとえ楕円または円形のよう
に湾曲しているステムにも応用できる。特定の構造にお
いて、ステムの近位部は、ステムが挿入される骨窩の中
心に来なくても構わず、従って、突起７の形状は、目的
通り確実にガイドするために、ステムの周囲に異なる寸
法で配置することができる。

  1. 【請求項１】 骨窩の内壁に接触し、その中にステムの
    近位端をガイドする様に寸法が決定された複数の成形し
    た突起がステム近位部に供給されている、セメントを用
    いて骨窩に固定するためのステムを有するプロテーゼ。
  2. 【請求項２】 前記突起が最初の植え込み後にステムが
    骨窩内で更に下方に移動する場合には取り外せる取り付
    け手段により前記ステムに固定されていることを特徴と
    する請求項１に記載のプロテーゼ。

    向いた涙滴の形であることを特徴とする請求項１または
    請求項２の何れかに記載のプロテーゼ。
