Annessa a domanda di brevetto per INVENZIONE AZIENDALE avente per titolo Attached to a patent application for COMPANY INVENTION having the title
This patent application relates to footwear as an aid to the treatment of gait disorders associated with neurological disorders (parkinsonism, spasticity, ataxia and neuropathies, etc.) by means of a plantar mechanical stimulation device. The invention concerns a shoe with a plantar vibration system that facilitates walking through a proprioceptive physiological stimulation facilitating the activity of the nerve centers of walking.
Walking disorders are common to many neurological diseases, affect a high percentage of the population, contribute significantly to the disability and impaired autonomy of patients. Having a negative impact on the quality of life and their social functioning.
One example is Parkinson's disease. Its incidence is about 3 per thousand of the world population. In Europe, the World Health Organization (WHO) estimates that 0.5 per cent of the population is affected, for a total of about one million people. In Italy there are about 400,000 patients, with an average of 1,200 new cases a year. Most patients with Parkinson's disease, particularly in the advanced stages of the disease, have severe gait disturbances.
La deambulazione. Walking.
Walking is a functional ability that is gradually acquired and developed by the individual in early childhood assuming automatic-reflex characteristics. Walking disorders are responsible for about 53% of falls, after the age of 65 about 1/3 of the elderly are at risk of falls, with a 50% increase after the age of 80.
Walking is a complex and not yet fully understood motor function, mostly unconscious and automatic. This motor function has significant as well as obvious repercussions on relational life, on the social functioning of the individual. It is the movement allowed to the body by the lower limbs, which allows the individual to move. It takes place according to well-defined parameters such as the linear direction, the oscillation of the upper limbs, the homogeneity of the individual steps. Walking is under the control, of the cerebellum brain, and of the inner ear, of sight, hearing, psychological factors, the spinal cord and the osteoarticular system. It can therefore undergo alterations in case of disease or lesion of these structures, such as in cerebral palsy (from stroke), in diseases of the cerebellum (e.g. ataxia) and vestibule, in diseases of the spinal cord (e.g. multiple sclerosis or trauma ).
Lack of coordination is associated with cerebellar disorders and leads to a loss of motor control and the ability to walk.
Most neurological diseases can result in gait disturbances through various mechanisms. The characteristic example is Parkinson's disease. Gait disturbances and gait freezing are particularly common in Parkinson's disease. The phenomenon of "freezing" is a walking disorder for which effective treatments are not available in most patients to date. Patients describe freezing as a sensation where the feet remain glued to the floor as if frozen. This disorder often causes falls resulting in traumatic injuries. The term "freezing" refers to transient episodes, usually lasting a few tens of seconds, in which the patient is unable to initiate or continue movement in an otherwise relatively good motor skill situation. It is an extremely variable phenomenon in appearance and severity and which appears more often late in the clinical history of the disease with an incidence ranging from 50 to 90%. The disease affects 2% of the population over the age of 65 and in 10-15% of cases it can have a juvenile onset. The prevalence is 150-200 people per 100,000 inhabitants. In Italy the number of Parkinsonians is about 150,000, with 15,000-20,000 patients still young and working. In advanced stages, freezing can become the prevalent disorder, causing significant disability with a high impact on quality of life and daily activities (ADL). Despite the practical and functional relevance of freezing for patients, the different therapeutic strategies used in most patients have not led to satisfactory results.
The need for an alternative strategy therefore appears clear: the proposed invention consists of an 'anti-freezing shoe', an innovative tool, simple to use and useful above all because it can be used in any place and at any time. , outdoors or indoors, without the system being visible from the outside and therefore without embarrassment for the patient wearing the device.
Therefore, the specific object of the present invention is a shoe with a polyurethane sole, equipped with a radiofrequency control system capable of activating a plate integrated in the inner sole able to provide a localized vibration under the sole of the foot. In addition to freezing, the invention also improves the 'start esitation', that is the hesitation when starting the march, which also occurs in situations such as a sudden change in the direction of travel, moving in confined spaces or in the act of reaching a target such as the bed or the chair.
La presente invenzione sarà ora descritta a titolo illustrativo ma non limitativo con riferimento ai disegni allegati in cui: The vibration is generated by an eccentric mass rotary motor capable of providing accelerations of about 11 G, the same motor is made integral with a carbon plate by mechanical fixing by means of "torx" screws of the motor support made of high structural polyethylene. density (HDPE). Risultati preliminari Preliminary results
The shoe object of this patent was tested in prototype form in Parkinson's disease which is the paradigmatic example of a pathological condition whose clinical picture is characterized by walking disorders. The sample consisted of 5 patients (1 woman; 4 men) aged 70-80 years, with severe gait disturbance. All patients were subjected to an experimental protocol for the assessment of walking which involved the use of footwear with a plantar vibration system.
The experimental protocol included a clinical / motor evaluation and a subjective evaluation.
The Visual Analogue Scale (VAS) was used for the subjective assessment: this linear scale is the visual representation of the extent of the improvement that the patient believes he feels. The amplitude is represented by a 10 cm long line, which represents the measure of the particular modality to be quantified for the subjective evaluation of two characteristics of walking 'difficulty' and 'fatigue' on a scale from 0 to 10 (maximum ease = 0, maximum difficulty = 10; no fatigue = 0, maximum fatigue = 10;),
The scale is completed by the patient who is asked to draw a sign on the line that represents the level of improvement experienced.
Patients were clinically assessed with UPDRS III by a blinded rater via video, and subjectively and with VAS.
Patients showed approximately 50% improvement on subjective rating scales.
These preliminary results indicate that footwear with plantar vibration system improves walking in patients with Parkinson's disease and freezing of the gait.
