The present invention relates to the dental sector and more precisely refers to a composition for the treatment of dentin hypersensitivity, the disinfection and protection of the teeth and periodonlus. Such a composition can be used as a solution.
Breve descrizione della tecnica nota Brief description of the prior art
Dentin hypersensitivity is a symptom that the dentist frequently detects in the oral history. The patient reports it as an annoying disorder caused by the intake of hot or cold, sugary or acidic foods, as well as by brushing it.
In correspondence of the hypersensitive dental areas, the inspection often highlights an abrasion or erosion of the enamel with more or less severe superficialization of the dentin, in other cases it is the gingival recession that reveals the most sensitive part of the tooth, that is the junction enamel-concrete. It is emphasized that normal dentin is peripherally coated with enamel (in coronal dentin) and cementum (in root dentin). Dentin under these conditions is not actually peneable. It must be remembered that the pulp is richly innervated. From this plexus the nerve fibers travel centrifugally towards the dentin. Some of these form an arborization of delicate terminal fibers that penetrate more or less deeply into the odontoblast layer, others still can cross the predentine without giving collaterals and enter the dentin through the dentinal tubules. In fact, the dentine is crossed radially by innumerable dentinal tubules, which appear to contain the dentinal fluid. As it enters the dentin (centrifugally), the percentage of tubules that contain snow fibers decreases rapidly. The nerve fibers then penetrate the dentin no more than a few micrometers, so the dentin is largely devoid of nerve fibers. There are several hypotheses that try to clarify the mechanisms through which the transmission of stimuli from the external surface of the tooth to the sensory structures of the pulp takes place.
To date, the most accredited theory is the hydrodynamic one of dentinal sensitivity. In summary, this theory asserts that small displacements of the tubular fluid content occur in response to tactile, thermal or osmotic stimuli. With the application of various stimuli (dehydration, heat, cold, or hyperosmotic solutions) there will be a consequent movement of the dentinal fluid inside the tubules and a consequential agitation, of the protoplasmic extensions of the odontoblast within the tubule itself and therefore of the sensory nerves that are closely associated with such extensions. This rapid displacement of the contents of the dentinal tubules is responsible, through the stretching of the aforementioned nerve fibers, for the excitation of the nerve mechanoreceptors located in the area between pulp and dentin, thus producing an electrical response and therefore the painful stimulus.
Therefore a tacit assumption of the hydrodynamic theory is that the dentinal tubules have open tucking (by abrasion or erosion of the enamel): the sensitive dentin is therefore permeable dentin. When the fluid is removed from the ends of the dentinal tubules, for example by dehydration with an air jet, the capillary forces produce a rapid movement of fluid towards the outside. Nerve stimulation is more easily transmitted when the openings of the external tubules are large and the dentinal fluid is easily accessible. Conversely, dentinal sensitivity can be considerably reduced by obliterating the orifices of the tubules, so as to decrease their conductance. Any substance capable of occluding the dentinal tubules should decrease sensitivity. This occurs physiologically when, for example, the tubules are occluded at their ends by tartar, or by intratubular precipitation of salivary mineral salts, or by the formation of collagen plugs, etc. All these processes would tend to occlude the dentinal tubules and decrease dentinal sensitivity. However, there are substances such as nitrate or potassium chloride capable of decreasing dentinal sensitivity without however possessing the ability to decrease the fluid flow through the dentin. These observations can be explained by hypothesizing that potassium nitrate leads to desensitization due to an increase in the concentration of potassium ions at the extra-cellular level. The potassium ions, presumably, depolarize the piasmatic membrane of the axon, causing an initial increase in the action potential. After the initial depolarization, the nerve fibers cannot repolarize due to persistent high levels of extra-cellular potassium. So the effect of increasing the extra-cellular potassium concentration consists in reducing nervous excitability. The nerve fibers remain inexcitable for as long as the extra-cellular potassium concentration remains high. Another substance that appears to reduce nervous excitability is eugenol. In clinical practice, chemical compounds are used (in the form of solutions, gels or pastes) or physical treatments to try to mechanically occlude the opening of the dentinal tubules. Among the former we remember ferric and potassium oxalate (through a reaction with the ionized calcium present in saliva, calcium oxalate would form which precipitating would occlude the orifices of the dentinal tubules), silver nitrate (also through the precipitation of complexes of silver), stannium florin (with precipitation of stannium complexes), strontium salts etc. Or insoluble salts such as calcium phosphate, hydroxyapatite, colloidal silica, etc. which directly occlude the tubular orifices. Glass ionomer cements and some types of resins are currently used for this purpose.
Among the physical means we can remember the laser or the ultrasonic scalers that cause the formation of a consistent amount of debris (smear layer) with consequent occlusion of the tubules. Currently in practice, therefore, the desensitization of dentin is carried out by means of chemical or physical tubular occlusion to block the movement of fluids inside the dentinal tubules, or by applying substances capable of blocking pulp nervous activity by altering the excitability of the fibers. sensory nerves that are inserted inside the dentinal tubules such as eg. Potassium Chloride, Potassium Nitrate, or 1 Potassium Oxalate. In any case, all the methods of occlusion of the dentinal tubules using soluble or insoluble salts used up to now have had poor response rates and a relatively short duration even if they are simple to apply. Likewise, compounds with depolarizing action on nerve fibers have a limited duration of action (linked to the maintenance of a high concentration of potassium ions in the dentinal tubule, which, however, being patent, makes the dentinal fluid and with it the potassium ions escape).
Newer methods, still not widespread in clinical practice, which use glass ionomer cements or photoinduced resins lead to better results as regards the duration of desensitization, but are complex in practical application and indicated for very deep erosions. Treatment with laser or ultrasonic scaler leads to results that are comparable, such as response rates and duration, to those obtained with topical treatment with the aforementioned soluble or insoluble salts, also requiring a significant expense for the purchase of the equipment. The infectious pathology of the mucous membranes of the oral cavity, in its clinical evolution, may first present itself as gingivitis which can subsequently evolve into periodontitis and then become chronic, presenting in its progression an aggravation of the infection and inflammation of the deep tissues of the periodontium.
The infectious process can cause detachment of the periodontal tissue with gingival recession and exposure of the collar. In this condition, in the presence of bacterial plaque, abrasions and erosions of the cement and dentin in the cervical area are frequently observed: all this is the main cause of hypersensitivity.
The pathognomonic signs of periodontal disease, due to the destruction of the periodontal fibers of the cervical border, are gingival pockets with loss of epithelial attachment, Γ exposure of the collar, bone resorption with apical proliferation of the junctional epithelium, abrasions at the level of the enamel with painful symptoms caused by both inflammation and dentinal sensitivity.
The primary etiological factor, both of caries and inflammatory periodontal diseases, is bacterial plaque, a heterogeneous microbial accumulation consisting of aerobic and anaerobic bacteria and an intercellular matrix.
Whatever the triggering cause of dentinal hypersensitivity, the infectious process determines a detachment of the periodontal tissue with gingival recession and exposure of the collar, and in the presence of gingival recession, abrasions and erosions of the cement and dentin in the cervical area are frequently observed.
In fact, the exposure of the amelocementitious junction and of the cement, following a gingival recession, causes a softening of the exposed cement or dentin. This is due to decalcification and maceration of the cement and dentin exposed to oral fluids. This condition frequently leads to hypersensitivity phenomena.
An important chapter of dentinal hypersensitivity is that which appears after a cavity filling performed with definitive material (amalgam or composite). However, while for amalgam there are no problems in the use of desensitizers, as the stabilization mechanism of the amalgam filling in the cavity is of the retentive and non-adhesive type, for composite fillings this procedure cannot be performed as the dentinal tubules must be open for the adhesion that the adhesive must exert on the dentin. There is also the problem of maintaining the seal between the composite and the dentin. It is known in the literature that enzymes (metallo proteinases) tend to weaken the adhesion between composite and dentin.
While the efficacy of desensitizing products currently used in the clinic is satisfactory, the need is felt for a product that possesses optimal desensitizing characteristics combined with disinfectant and protective properties from metalloproteinases.
Sintesi dell’invenzione Summary of the invention
It is a composition for dental use for the desensitization disinfection and protection of exposed dentin, and in particular for the desensitizing disinfectant and protective treatment of dental cavities suitable for accommodating composite fillings, to be used in place of substrates, or for the treatment disinfectant and protective desensitizer of abutments when applying prosthetic elements, after scaling and grinding of the roots, after whitening treatments. This composition comprises a solution which can be applied topically with a brush or micro-sponge on the aforementioned areas. It is an object of the present invention to provide a composition used in the therapy of desensitization disinfection and dentinal protection which has a strong capacity in the deep occlusion of the dentinal tubules (leaving the last 3-6 microns open) in addition to the depolarization capacity of the dentinal nerve fibers, and protection of the collagen present in the hybrid layer of dentin (pre-treated with orthophosphoric acid), from metalloproteinases, allowing a high desensitizing / protective response and therefore a longer adhesive stability over time.
This purpose is achieved by the composition according to the invention, the characteristic of which is to include a disinfectant, an acrylic polymer, oxalic acid, and / or a fluorine salt.
Preferably, the disinfectant selected is from the family of cationic disinfectants, in particular: benzalkonium chloride, benzethonium chloride, chlorhexidine, cetrimide, cetylpyridinium chloride. Chlorhexidine is chosen in preference among the disinfectants.
The soluble fluorine salts are preferably selected from:
In preference among the soluble salts of potassium and chosen is the potassium fluoride.
In preferenza tra i polimeri acrilici è scelto ΓΗΕΜΑ (2-idrossietilmetacrilato) . Preferably ΓΗΕΜΑ (2-hydroxyethyl methacrylate) is chosen from among the acrylic polymers.
The pharmaceutical composition according to the invention allows to overcome the aforementioned problems related to the partial properties present in the desensitizers used in clinical practice. The association of a cationic disinfectant together with an acrylic polymer, chlorhexidine, with oxalic acid and / or a fluorine salt surprisingly allows to obtain high desensitization and protection activities of the dentin. This surprising activity is linked to a synergistic action of the cationic disinfectants with oxalic acid together with the fluorine ion, and the acrylic polymer. In fact, in the literature, a desensitizing activity of disinfectants is known when applied to exposed dentin, as these by reducing bacterial plaque also reduce all the synthesis or degradation products of the bacteria themselves that can penetrate into the dentinal tubules, pertaining to the exposure of the dentin. In particular, chlorhexidine would also have inhibiting properties of metalloproteinases, while oxalic acid and fluorine salts would occlude the tubules in depth.
HEMA would allow the stabilization of calcium oxalate and calcium fluoride crystals formed deeply in the dentinal tubules.
The potassium ions would act on the nerve fibers contained within the dentinal tubules thus reducing the sensitivity threshold.
La proporzione ponderale generale per la soluzione è la seguente: The general weight ratio for the solution is as follows:
HEMA 5-40% HEMA 5-40%
Clorexidina gluconato 0.1-5% Chlorhexidine gluconate 0.1-5%
Acido ossalico 0,5-5% Oxalic acid 0.5-5%

Alcool etilico 96° 1-40% Ethyl alcohol 96 ° 1-40%

The use of a cationic disinfectant plus an acrylic polymer plus oxalic acid and / or soluble fluorine salts is not known in the technique of the dental sector, where instead it is optimally usable according to the present invention.

Floruro di potassio 3% Potassium Floride 3%
Clorexidina gluconato 2% Chlorhexidine gluconate 2%
2-idrossietilmetacrilato (HEMA) 35% 2-hydroxyethyl methacrylate (HEMA) 35%
Acqua depurata q.b a 100 Purified water q.s. to 100
Tali composizioni vengono utilizzate nel modo seguente per il trattamento desensibilizzante disinfettante e protettivo: Such compositions are used in the following way for the disinfectant and protective desensitizing treatment:
Clean the surface of the dentin to remove bacterial plaque; in the treatment of cavities after cleansing and application of 37% orthophosphoric acid etching gel, wash thoroughly with water. Dry carefully with a jet of air for about 10 seconds, if there is severe hypersensitivity, simply dry with a cotton swab; Application of the liquid: squeeze the contents of the single-dose directly onto the area to be treated or into a well of a disposable base (if the product is packaged in vials with dropper cap). Then pick up the solution with a brush or micro-sponge and massage gently for about 10 seconds.
The composition is particularly suitable in cavities under composite fillings, but it can generally be applied to those dental regions where there is erosion or wear of the enamel, or in those patients where a gingival recession is present, the amelo-cement junction. The composition can also be applied on prosthetic abutments, after scaling and root planning, or after whitening treatments.
Variations and / or modifications may be made to the composition according to the present invention without thereby departing from the protective scope of the invention itself.
