Description
Descrizione dell’invenzione industriale dal titolo: Description of the industrial invention entitled:
Vite di guarigione per impianti dentali ed impianti dentali che la comprendono Healing abutment for dental implants and dental implants that include it
Campo dell’invenzione Field of the invention
The present invention relates to the field of dental implants.
Stato del’arte State of the art
As is known, when a tooth has to be implanted, replacing an extirpated one, the first step consists of inserting an implant into the crestal bone ending at one end (the one destined to protrude from the bone) with a neck to which the replacement tooth is applied.
The implant consists of an essentially cylindrical body hollow inside and possibly threaded.
The end of the implant intended to be inserted into the crestal bone is closed while the opposite end is open and has an external neck intended to protrude from the bone.
At present, two types of systems apply:
- monofasici, cioè trasmucosi, in cui il collo è di forma tronco/conica, leggermente svasata e tale da accogliere i tessuti molli crestali; - monophasic, i.e. transmucosal, in which the neck is truncated / conical in shape, slightly flared and such as to accommodate the crestal soft tissues;
- biphasic in which, after integration or at the time of insertion, a so-called healing screw is housed which is then removed, once the soft tissues have adapted around the implant neck, and replaced by the definitive prosthetic abutment on which the replacement tooth is mounted.
Implants are also known in which a removable neck is applied to the protruding end once the soft tissue adaptation has been achieved. The main objective of modern implantology is the achievement and maintenance of osseointegration, that is, the direct connection between bone and implant surface in order to guarantee the stability of the implant over time; it is also important to ensure the adaptation of the supracrestal soft tissues around to the healing screw that is inserted in the implant neck, right from the application of the implant, so that even from an aesthetic point of view the distinction between natural dentition and inserted tooth is the minimum possible.
In light of the foregoing, the importance of having a system that allows the above objectives to be achieved is evident.

.Descrizione dettagliata dell’invenzione. . Detailed description of the invention.
The present invention allows to overcome the aforementioned problems thanks to a scalloped and removable healing screw for dental implants to be inserted into the implant with the relative tightening screw (hereinafter referred to as screw-cap).
As can be seen from Figure 1, the implant is constituted by the hollow body 10 closed at one end and whose other end ends with a neck 11, which can possibly be tapered, which allows it to be engaged with the healing screw 12 which is press fit when placing the implant in the bone prior to suturing.
The internal cavity of the body 10 can have a thread or be shaped in an essentially frusto-conical shape.
In the first case the implant will engage with an abutment having the corresponding threaded end, while in the second case the abutment will have the corresponding end essentially frusto-conical in order to engage with the implant by simple pressure.
The neck 11 can be of a regular cylinder shape or preferably tapered in a truncated cone shape in order to facilitate its engagement with the healing screw.
The healing screw 12 comprises a solid body 13 having a shaped plan section according to the possible shape of the tooth as shown by the examples in Figure 2 (a - c) in which:
(a) rappresenta la forma di un dente centrale; (a) represents the shape of a central tooth;
(b) di un premolare; (b) of a premolar;
(c) di un molare. (c) of a molar.
The healing screw according to the invention therefore has an external profile capable of following in its coronal portion the profile of the dental section of the element that will be affixed to it to rehabilitate the maxillary edentulous space.
In other words, the external lateral surface of the healing screw will have a course corresponding to that of the gingival tissues which, in relation to their relationship with the implant, will have a different thickness, i.e. greater in correspondence with the interproximal surface and less in the lingual and buccal ones.
The lateral surface of the external healing screw is also preferably tapered and shaped so as to adapt to the walls of the peri-implant tissue.
The healing screw then has, in an essentially central position of the solid body 13, a through hole 14 which extends outside the solid body 13 in a neck 15 capable of engaging in pressure with the neck 11 of the body 10.
Said hole 14 allows to house the cap screw (not shown in the figure) with which the healing screw is closed at the time of implant placement and, once the cap screw has been removed, the metal abutment 16 on which it is applied the tooth, usually in ceramic material.
The external walls of the healing screw are perfectly smooth in order to guarantee perfect adhesion of the connective tissue and the peri-implant membrane.
The neck 15 may also be slightly tapered to better adapt to the neck 11 of the implant and thus minimize possible gaps.
The abutment has an upper part 17 possibly shaped according to the profile of a tooth, a central part 18 shaped so as to engage with the hole 14 and a threaded terminal part 19 capable of engaging with the internal thread of the hollow body 10.
According to a particular embodiment of the invention, in the case in which the upper end (i.e. destined to protrude towards the outside) of the body 10 is also closed and a polygonal cavity (normally hexagonal) is present on its surface, which allows better positioning the abutment, the terminal part of the abutment will have a polygonal protuberance (hexagonal or dodecagonal) capable of engaging with the aforementioned cavity by simple pressure and thus helping to position the abutment itself correctly.
In this case obviously the healing screw will not have the neck 15 and will rest on the upper surface of the hollow body 10 in which the aforementioned polygonal cavity is formed.
As can be seen, therefore, the implant as a whole has an axial truncated cone shape, in sagittal section its profile is such as to perfectly collimate with the crest platform of the implant while in the apico-coronal direction it becomes concave so as to be able to accommodate the supracrestal connective. and then becomes convex so as to simulate the coronal / root anatomy at the level of the amelo-cement line.
With the implant according to the invention, the mucous sleeve, resuming the adjustment of the gingiva at the crest level and adapts more consistently to the vestibular and interproximal surfaces where the physiological aspect of the mucous scallop and the interproximal papilla is recreated, thus obtaining an optimal biological width.
The usefulness and practicality of the healing screw according to the invention is immediately evident.
Once the implant has been positioned in the crestal bone, it will be sufficient to apply pressure to the healing screw according to the invention in the hollow body of the implant and finally screw the cap screw to close the central hole of the healing screw.
Subsequently, once the healing of the soft tissues has been completed and the implant stabilized in the bone, the cap screw will be removed and the abutment will be inserted into the hole of the healing screw either by screwing or by pressure depending on the grafting system adopted. . The healing screw according to the invention is made of materials normally used for this purpose such as titanium or zircon oxide.
