Description
Descrizione di una invenzione industriale avente per titolo: Description of an industrial invention entitled:
Microbisturi chirurgico con scale graduate di riferimento. Surgical micro scalpel with graduated reference scales.
The present invention refers to a diamond blade micro scalpel for microsurgery in general. The micro scalpel is equipped with two graduated scales: the first is positioned on the fixed external support foot of the micro scalpel and the second is positioned at the end of the mobile support, adjustable for the cutting depth, on which the diamond blade is fixed. In cataract ophthalmic surgery, the first graduated scale is used to perform the calibrated corneal incision, or pre-cut, while the second graduated scale is used to create the tunnel with a predetermined width. The micro scalpel can also be used for glaucoma, astigmatism, refractive surgery and microsurgery in general. With non-limiting reference to ophthalmic surgery relating to cataract operations, it is known that, in sequential phases, calibrated corneal incisions or pre-cut incisions (pre-cut incision), the creation of tunnels of determined length (tunnel incision) and the service entrances (side pores). For glaucoma operations, the so-called scleral flaps (glaucoma surgery) with calibrated depths must instead be created. For microsurgery interventions in general, incisions of various kinds must be made, with different extension and depth, subject to particular dimensional checks.
It is also known that sets of scalpels specifically dedicated to the various phases are currently used to perform these operations. Each of these instruments, even in the specific and qualified structural and functional conception, can be used only for one or, at most, for some of these operations, and not for all, requiring surgeons to replace them during the rest of the operations. .
Another drawback consists in the fact that, for example, during the creation of tunnels relating to cataract interventions, surgeons can only operate with calibrated diamond blades of fixed width.
A further drawback is due to the fact that the complete sets of these scalpels, of very high quality as they are equipped with diamond blades with a stainless steel or titanium support foot and a multi-position cutting depth selector, have a low cost. indifferent.
The object of the present invention is to eliminate the aforementioned drawbacks. The invention, as it is characterized by the claims, solves the problem by means of a single multifunctional micro scalpel with a diamond blade, by means of which the following results and advantages are obtained: the two graduated scales available to the micro scalpel allow to carry out a constant and effective control of the length of the incisions, during all the possible sequential phases of the interventions; in the ophthalmic field, relating for example to cataract operations, the steps of incision and dimensional control of the pre-cut, of the formation of the tunnel, and of the creation of the service entrances, are performed using the same suitably adjusted micro scalpel; in the same way the micro scalpel can be adopted for the realization of any incision for which it is essential to check not only its depth but also its extension; despite its greater complexity, the micro scalpel is in any case cheaper than the current sets of differentiated diamond scalpels.
The invention is described in detail below, according to one of its embodiments given solely for illustrative and non-limiting purposes, with reference to the attached drawing, in which:

fase dì pretaglio, relativo ad un intervento di cataratta, con controllo dell’estensione sulla scala graduata del piedino di appoggio, e pre-cutting phase, relating to cataract surgery, with control of the extension on the graduated scale of the support foot, and
6 a-b-c represents a micro scalpel according to the invention, in the phase of creating the tunnel, relating to a cataract surgery, with control of the extension on the graduated scale placed at the end of the adjustable mobile support, on which the diamond blade is fixed.
In the following, the invention is described with reference to the specific use in ophthalmic surgery relating to cataract surgery; the application is given only by way of non-limiting example. The characteristics of the micro scalpel with double graduated scale are such as to allow its advantageous use for any microsurgery.
With reference to the figures, the micro scalpel 1 is substantially constituted by a manual gripping stem 2 on which a known type of cutting depth selector device 3 is included, with control located at its rear end. Said cutting depth selector is connected and activates the mobile support 4 which slides inside the rod 2. The cutting blade 6, preferably made of diamond, is constrained to the front end 5 of the mobile support 4. The front end 7 of the stem 2, which constitutes the external support foot of the instrument, has a linear, orthogonal transversal extension, on which a first graduated scale 8 is engraved or shown, while in correspondence with the attachment of said blade 6, the front end 5 also has a linear transversal extension, orthogonal, on which a second graduated scale 9 is engraved or shown, perfectly identical and aligned in a parallel and axial direction to the first.
According to purely indicative, non-limiting configurations, for use in ophthalmic surgery and / or microsurgery in general, the aforementioned graduated scales 8 and 9 can have an extension, in metric units, from 0 (zero) to 4 (four) mm, divided in: 0 - 1 - 2 - 3 - 3.2 - 4 mm, while the depth of cut selector 3 can include the following preselections: 0.15 - 0.20 - 0.25 - 0.30 -0.37 - 0.50 - 4.50 / 5.00 mm.
However, it is not excluded that both the graduated scales 8 and 9 and the cutting depth selector 3 can be made with steps and / or intervals and / or units of measurement different from those mentioned above, according to needs and / or preferences. different, at the request of the surgeons and the technical evolutions of the intervention.
An example of a diamond blade 6 mounted on the micro scalpel, particularly suitable for use in ophthalmic surgery, may have the following characteristics, in metric units: thickness 200 micron - length 4.5 / 5.0 mm - blade width 1 mm, with a flat cutting base of 550 microns - front side vertical cutting edge of 4. 5 / 5.0 mm - back side with first cutting part oblique by 20 ° starting from the flat base and vertical for the rest - front vertical side cutting edge 10 aligned to 0 (zero) of the graduated scales 8 and 9.
Other types of blades 6, mounted on the same micro scalpel, with different shapes or sizes, preferred and / or suitable both for ophthalmic surgery and for different uses of microsurgery in general, can be made and applied on the same type of micro scalpel, without leaving the scope of the invention.
Again with reference to ophthalmic applications for cataract surgery, the graduated scale 8, of the external support foot 7, is used in the pre-cut step 11, to establish its exact extension, while the graduated scale 9, of the anterior end 5 of the mobile support 4, is used to control the extension of the tunnel 12 which forms in the cornea 13, until the anterior chamber 14 is reached.
Assuming that the incision 11 to be made on the cornea 13 must have a calibrated length, for example 4 mm (section C in figure 5), the starting point A is marked on the tissue, taking into account the exact width b of the blade 6 (for example b = l mm). From this starting point A, seen by the vertical cutting edge 10 of the blade 6 and, consequently, by the 0 (zero) of the aligned graduated scales 8 and 9, (figure 5-a) the calibrated pre-incision of the cornea is first developed in depth , according to the arrow V, for a value corresponding to the part s of the blade protruding from the external support foot 7 according to the established selection, for example s = 250-300 microns, (figure 5-b), then in length, according to the arrow L, making the blade slide so that the graduated scale 8 translates accordingly, keeping itself aligned with the same starting point A. The constant depth s is guaranteed by the sliding of the foot 7 on the fabric to be engraved 13. The pre-cut incision it ends when the aforementioned point A_ is aligned with the point of the graduated scale 8 (for example 4 mm minus the width b of the blade 6), corresponding to the desired extension C of the cut (figure 5-c).
In the next step, the same micro scalpel can be used to make the incision intended to form the tunnel 12. For this reason the blade 6 is made to protrude by the desired amount, for example 4.5 mm, by making the mobile support 4 slide forward by means of the selector. rear 3. With the adjustment of the extension of the blade 6, the graduated scale 9, present on the end of its mobile support 4, also advances by the same length, overlapping and completely covering the graduated scale 8 of the foot 7 and arranging in a position clearly visible to the surgeon who has to perform the operation taking into account the calibrated length of the pre-cut. Basically, the graduated scale 9 simply replaces the one 8 below, guaranteeing identical previous operating and control conditions. As for the creation of the pre-cut, Figure 6 illustrates the salient stages of the creation of the tunnel, with the blade 6 extended and with the use of the graduated scale 9.
Always with the blade extended (for example 4.5 / 5.0 mm) the same tool is used for the creation of the service entries at full depth; for this reason the blade is made with a preferential width of 1 mm, as normally required in phacoemiilsifìcation surgery. For different uses, the aforementioned width may, of course, be different.
In the intermediate extensions of the blade 6, useful or necessary for other types of ophthalmic surgeries or for other uses of ir microsurgery. in general, the graduated scale 8 or the graduated scale 9 will be used, depending on the best visual conditions for checking the interventions.
The micro scalpel, according to the invention, can be made with a diamond blade fixed on the right or left side (for left-handed) and with the external vertical cutting side always in alignment with the 0 (zero) point of the external side of the mobile support 4 with scale. graduated 9. The support foot 7 is preferably made of titanium and the points of its graduated scale 8 are perfectly aligned with those of the graduated scale 9 of the mobile support 4.
While the present invention has been described and illustrated according to an embodiment thereof given solely by way of non-limiting example, it will be evident to those skilled in the art that various modifications to the shapes, to the details and to the assembly can be made without thereby departing from the its scope and purpose.
