Abstract
Description
The present invention relates to an osteotomy surgical device intended more particularly to treat the risk of knee osteoarthritis in varus, by precisely modifying the angulation of the tibial plateau. The tibial approach used is the most widespread and the safest; however, there is nothing to prevent the present device from being adapted for a femoral approach.
When diagnosed in a frontal plane, the clinical signs of such treatment are due to a misalignment of the leg which reveals an abnormal angulation between the tibia and the femur. This defect when in varus leads to very lateralized forces at the knee joint when walking. Such a situation results in premature wear of the internal articular cartilage. Hence the interest of surgical treatment before reaching this last stage. For an en.valgus defect, natural forces show that there are no concentrations of additional stresses in the cartilages, hence the very minimal risk of osteoarthritis for the latter case. However, if the treatment of this valgus proves necessary, the present device is perfectly suitable for performing the appropriate osteotomy.
Following the diagnosis, the surgeon assesses and calculates on a frontal radiographic film of the stretched leg, the misalignment of the knee with respect to the axis passing through the center of the femoral head and the middle of the patella (between the two tibial spines ). The situation and the angulation of the femorotibial line with respect to this axis, determines the site and the value of the corrective osteotomy, taking into account an ideal morphotype, where the natural angulation is in valgus of 20.
The angular value thus calculated of this misalignment then serves as a working basis for calculating the bone ablation under the condylar in the form of a wedge, of angle at the apex aO, necessary for the realignment of the leg thus ensuring subsequently a homogeneous distribution of the articular forces in knee.
If the practitioner determines without too much difficulty on the radiographic film the value of the angle desired for the osteotomy, he is however deprived of precise spatial references, in the operating room to practice the ablation of a bony dihedral. , of angle cro, on the leg which can be, moreover, folded and not stretched according to the working position adopted. Only the art of the practitioner then makes it possible to locate the two planes of bone section, and to carry out the osteotomy.
To achieve a quality osteotomy, it is first of all advisable that the two cutting planes, P1 and P2, of the dihedral, of angle aO, meet along an edge located in A in the vicinity of the periosteum of the second cortex and not very much below, 8, or far beyond,
This result is achieved in accordance with the invention by the fact that the saw blade will be able to bear on two plane supports P1 and P2 of the device effectively forming an angle cLo and necessarily contributing in QA.
Other characteristics and advantages of the invention will appear from the description and the appended drawings which show a preferred embodiment of the invention.

Figure 3 is an overview of the device when placed in position relative to the tibia.
FIG. 4 is a partial view of the device when it is placed in position relative to the upper end of the tibia, after reduction of the osteotomy by means of a plate blade.
The osteotomy device shown in Figure 2 includes
- A circular rule (1), provided with a light (la), with radius of curvature R which can advantageously be taken equal to 10 cm, graduated in degrees.
This rule, rigid but light, can advantageously be made of titanium alloy for example.
- Trois curseurs (2) (3) (4), supportant des guides. - Three sliders (2) (3) (4), supporting guides.
- A surgical pin (10) blunted at its distal end and provided with a mark (10b) located at the distance R from this end.
- A guide (2a) secured to the central cursor (2) and provided with a slide in the form of a dovetail for example.
- Two orientable spindle guides (5) fixed on the top (3) and bottom (4) sliders.
FIG. 2 also makes it possible to describe the manner in which the device will preferably be used to carry out an osteotomy, at the level of the upper epiphysis of the tibia for example.
The device is set up as follows
- The spindle (10) is inserted into the tibia while being located in a first osteotomy plane, P1, provided by the practitioner. The spindle is inserted until reaching point QA which is flush with the periosteum of the second cortex crossed. The practitioner can check that the point QA is reached by the spindle with the usual techniques, that is to say either manually or by means of an image intensifier for example. The spindle then occupies a position in the space designated by A1A.
- A stop ring (l0a) is then brought into contact with the first cortex and then made integral with the spindle (10), by means of a screw for example, this i prevents the end of the spindle from going beyond the point Has previously defined.
- A non-orientable spindle guide (6) is fixed, by a screw, in the slide (2a) of the central cursor (2).
- the circular rule (1) is now threaded along the spindle (10). This sliding is ensured by the barrel (8) of the non-orientable spindle guide (6), up to the mark (IOb) bringing the cursors to the distance R from the point QA. This reference may preferably be the end of a colored part of the spindle which is brought to the level of one end of the barrel (8).
- The positioning of the circular rule (1) relative to the central cursor (2), can possibly be corrected by sliding in the light (la), before being fixed by means, for example, of a screw ( 2b).
- The lower cursor (4) being positioned as desired by the practitioner enQCî for example, the lower spindle guide (5) is oriented so as to insert a second pin C1C into the tibia, so that the direction C1C is not parallel to direction A1A. As of now, the device is stable and the circular rule cannot approach the point QA, due to the angulation existing between A1A and C1C.
- the upper cursor (3) being positioned as desired by the practitioner in B1, for example, the upper spindle guide (5) is oriented so as to insert a third pin B1B into the tibia, so that BlB is not not parallel to C1C.
- It is now possible to remove the first pin (10), since the stability of the device is ensured by B1B and C1C.
- The spindle holder (6) of the central cursor (2) is then replaced by a saw blade support (7) also sliding in the guide (2a) before being blocked by a screw. The plane of this support is the P1 plane which passes through
- A cut is then made along the plane P1, by means of a surgical saw, the practitioner preferably having to stop his action a few millimeters before QA.
- The saw blade is removed and the cursor (2) is moved by the angle aO provided.
- A second cut is then made along the plane P2, the practitioner preferably having to stop his action a few millimeters before A QA The stops at the planes P1 and P2 are intended to maintain better stability of the device-shin assembly. The cuts beyond the QA edge do not however jeopardize the objectives of the device.
The osteotomy dihedral is therefore, at this stage of the procedure, performed with precision with regard to the positioning of the QA edge and the value of the calo angle. It is now appropriate to place an osteotomy plate blade (12), drawn on the diagram in FIG. 4, according to the usual technique and to extract the bony corner of angle cXo by finishing the sections according to the plans
P2 and P1.
It now remains to reduce this artificial fracture that is the osteotomy. This reduction can be carried out either manually or with the aid of the low fixing which appears in FIG. 3. In both cases, FIG. 4 will facilitate these descriptions.
- In the first case, without low fixation, the practitioner moves the cursor (3) manually brings the two bony planes P1 and P2 in coincidence. It remains to install the two usual transfixing screws (13) before dismantling the entire device.
- In the second case, the practitioner uses the low attachment complementary to the device, which has been put in place, either before or after the introduction of the pins and which consists of the elements appearing in FIG. 3.
- A beam (9), straight or not, articulated by means of universal joints (11) and comprising two elements (9a) and (9b) sliding one inside the other.
A stirrup (14) provided with shells (15) resting on the malleoli,
It will be noted that all the mobile elements of the device comprising locking screws, the device is connected in a very stable manner to the tibia and that, consequently, the bone cuts according to P1 and P2 can be carried out directly beyond the point QA without affecting the stability of the assembly. The low fixing therefore constitutes a non-essential but recommended part for the device.
In the case where the low fixation is used, the displacement of the cursor (3) to reduce the fracture then the installation of the transfixing screws (13) are only facilitated.
The reduction is almost perfect since the proximal and distal parts of the fracture are at all times stable and guided at will in space by the practitioner.
It goes without saying that the embodiment which has just been described constitutes a preferred and nonlimiting example of the invention.
Various variants are possible, which do not depart from the scope of the invention.
Thus it would be possible to provide for the sliding of the sliders along the circular rule by means of a sliding ring and not through the light (la).
This is how it would be possible to robotize the manual movements described here.
