Description

Description [0001] The invention relates to a pharmaceutical composition for use in the prevention or treatment of pain comprising a first pharmacologically active ingredient selected from (1r,4r)-6’-fluoro-N,N-dimethyl-4-phenyl-4’,9’-dihydro-3’H-spiro[cyclohexane-1 ,T-pyrano[3,4,b]indol]-4-amine and the physiologically acceptable salts thereof, and a second pharmacologically active ingredient selected from gabapentinoids and the physiologically acceptable salts thereof.
[0002] (1r,4r)-6’-fluoro-N,N-dimethyl-4-phenyl-4’,9’-dihydro-3’H-spiro[cyclohexane-1,T-pyrano-[3,4,b]indol]-4-amine and its corresponding physiologically acceptable salts as well as methods for their preparation are well known, for example, from W02004/043967 and W02008/040481. The compounds exhibit analgesic properties and are particularly suitable for the treatment of acute, visceral, neuropathic or chronic (nociceptive) pain. Additionally, in W02004/043967 the use of the compounds described therein for the treatment of epilepsy and anxiety is mentioned.
[0003] Gabapentinoids, such as pregabalin, gabapentin and vigabatrin are anticonvulsant drugs that are used for the treatment of various neurological and psychiatric disorders. EP 1977744 A1 describes a specific crystalline form of pregabalin and mentions its use for the treatment of pain, anxiety and epilepsy. In US 2008/153874 A1 pharmaceutical compositions are described which comprise a pain-relieving anti-convulsant and an alpha-2B receptor agonist.
[0004] Though both of the aforementioned substance classes are therapeutically effective, side effects may occur, especially upon prolonged use or when administered at high dosages.
[0005] It is further known that specific combinations of pharmacologically active compounds exert supra-additive (synergistic) therapeutic effects upon administration. An advantage of these special cases is that the overall dose and accordingly the risk of undesired side effects may be reduced.
[0006] In a further aspect, two pharmacologically active compounds exerting a synergistic effect may be combined in one single pharmaceutical dosage form, e.g. a tablet, thus enhancing patient compliance.
[0007] It is an object of the invention to provide pharmaceutical compositions which have advantages compared to pharmaceutical compositions of the prior art. In particular, the pharmaceutical compositions should provide rapid therapeutic effects, but also should have a high tolerability, good compliance and safety.
[0008] This object has been achieved by the subject-matter of the patent claims.
It has been surprisingly found that a pharmaceutical composition comprising (1r,4r)-6’-fluoro-N,N-dimethyl-4-phenyl-4’,9’-dihydro-3’H-spiro[cyclohexane-1,1’-pyrano[3,4,b]indol]-4-amine and a gabapentinoid is useful for the treatment of pain, especially chronic pain, in particular neuropathic pain.
[0009] Further it has been surprisingly found that said composition exhibits a synergistic therapeutic effect upon administration. Therefore, the overall administered dose may be lowered, so thatfewer undesired side-effects will occur.
[0010] A first aspect of the invention relates to a pharmaceutical composition for use in the prevention or treatment of pain comprising: a) a first pharmacologically active ingredient selected from (1r,4r)-6’-fluoro-N,N-dimethyl-4-phenyl-4’,9’-dihydro-3’H-spiro[cyclohexane-1,1’-pyrano[3,4,b]indol]-4-amine and the physiologically acceptable salts thereof, and b) a second pharmacologically active ingredient selected from gabapentinoids and the physiologically acceptable salts thereof.
[0011] The pharmaceutical composition according to the invention comprises a first pharmacologically active ingredient selected from (1r,4r)-6’-fluoro-N,N-dimethyl-4-phenyl-4’,9’-dihydro-3’H-spiro[cyclohexane-1,T-pyrano[3,4,b]indol]-4-amine and the physiologically acceptable salts thereof.
[0012] For the purpose of specification, (1r,4r)-6’-fluoro-N,N-dimethyl-4-phenyl-4’,9’-dihydro-3’H-spiro[cyclohexane-1,1’-pyrano [3,4,b]indol]-4-amine is the compound according to formula (I) which can also be referred to as 1,1-(3-dimethylamino-3-phenylpentamethylene)-6-fluoro-1,3,4,9-tetrahydropyrano[3,4-b]indole (trans)
(I).
[0013] The definition of the first pharmacologically active ingredient includes (1r,4r)-6’-fluoro-N,N-dimethyl-4-phenyl-4’,9’-dihydro-3’H-spiro[cyclohexane-1,1’-pyrano-[3,4,b]indol]-4-amine in form of the free base, i.e. the compound according to formula (I), in any possible form including solvates, cocrystals and polymorphs, and its physiologically acceptable salts, in particular acid addition salts and corresponding solvates, cocrystals and polymorphs.
[0014] The pharmacologically active ingredient (1r,4r)-6’-fluoro-N,N-dimethyl-4-phenyl-4’,9’-dihydro-3’H-spiro[cy-clohexane-1,1’-pyrano[3,4,b]indol]-4-amine may be present in the pharmaceutical composition according to the invention in form of a physiologically acceptable salt, preferably an acid addition salt, whereby any suitable acid capable of forming such an addition salt may be used.
[0015] The conversion of (1r,4r)-6’-fluoro-N,N-dimethyl-4-phenyl-4’,9’-dihydro-3’H-spiro[cyclohexane-1,T-pyrano[3,4,b]indol]-4-amine into a corresponding addition salt, for example, via reaction with a suitable acid may be effected in a manner well known to those skilled in the art. Suitable acids include but are not limited to hydrochloric acid, hydrobromic acid, sulfuric acid, methanesulfonic acid, formic acid, acetic acid, oxalic acid, succinic acid, tartaric acid, mandelicacid, fumaricacid, lactic acid, citric acid, glutamic acid and/or aspartic acid. Salt formation is preferably effected in a solvent, for example, diethyl ether, diisopropyl ether, alkyl acetates, acetone and/or 2-butanone. Moreover, trimeth-ylchlorosilane in aqueous solution is also suitable for the preparation of hydrochlorides.
[0016] In a preferred embodiment, the first pharmacologically active ingredient is (1r,4r)-6’-fluoro-N,N-dimethyl-4-phenyl-4’,9’-dihydro-3’H-spiro[cyclohexane-1,1’-pyrano[3,4,b]indol]-4-amine in form of the free base, i.e. the compound according to formula (I).
[0017] In another preferred embodiment, the first pharmacologically active ingredient is (1r,4r)-6’-fluoro-N,N-dimethyl-4-phenyl-4’,9’-dihydro-3’H-spiro[cyclohexane-1,1’-pyrano[3,4,b]indol]-4-amine in form of a physiologically acceptable acid addition salt, in particular the hydrochloride, hemicitrate or maleate salt.
[0018] Unless explicitly stated otherwise, all amounts of the first pharmacologically active ingredient specified in the following are given according to the corresponding amount of (1r,4r)-6’-fluoro-N,N-dimethyl-4-phenyl-4’,9’-dihydro-3’H-spiro[cyclohexane-1,1’-pyrano[3,4,b]indol]-4-amine in form of the free base, i.e. the compound according to formula (I).
[0019] The pharmaceutical composition according to the invention comprises a second pharmacologically active ingredient selected from gabapentinoids and the physiologically acceptable salts thereof.
[0020] The definition of the second pharmacologically active ingredient includes gabapentinoids in any possible form including any enantiomers, if applicable, solvates, prodrugs, cocrystals and polymorphs, and their physiologically acceptable salts, in particular acid addition salts and corresponding solvates, cocrystals and polymorphs.
[0021] Preferred gabapentinoids according to the invention are γ-aminobutyric acid derivatives, i.e. amino acids. Amino acids are amphoteric and thus, may be transformed into the corresponding acid addition salts as well as the corresponding metal salts. This may be effected in a manner well known to those skilled in the art, for example, via reaction with a suitable acid or base as well as metal salt. Suitable acids include but are not limited to hydrochloric acid, hydrobromic acid, sulfuric acid, methanesulfonic acid, formic acid, acetic acid, oxalic acid, succinic acid, tartaric acid, mandelic acid, fumaricacid, lacticacid, citricacid, glutamicacid and/or aspartic acid. Moreover, trimethylchlorosilane in aqueous solution is also suitable for the preparation of hydrochlorides. Suitable bases include but are not limited to the hydroxides of sodium, potassium, calcium and/or magnesium. Suitable metal salts include but are not limited to alkali salts such as sodium, potassium or lithium phosphate, sulfate, methanesulfonate, formate, acetate, oxalate, succinate, tartrate, man-delate.fumarate, lactate, citrate, glutamate, aspartate and/or silyls, as well as alkaline earth salts, in particular magnesium and calcium salts, including their phosphate, sulfate, methanesulfonate, formate, acetate, oxalate, succinate, tartrate, mandelate, fumarate, lactate, citrate, glutamate, aspartate and/or silyl salts. Salt formation is preferably effected in a solvent, for example, diethyl ether, diisopropyl ether, alkyl acetates, acetone and/or 2-butanone.
[0022] As prodrugs of gabapentinoids, amides and esters are particularly preferred. Suitable methods for selecting and preparing a prodrug of a given substance are, for example, described in "Textbook of Drug Design and Discovery, 3rd edition, 2002, chapter 14, pages 410-458, Editors: Krogsgaard-Larsen et al., Taylor and Francis.
[0023] Preferably, the second pharmacologically active ingredient is a γ-aminobutyric acid or a physiologically acceptable salt thereof.
[0024] In a preferred embodiment, the second pharmacologically active ingredient is selected from the group consisting of pregabalin, gabapentin, vigabatrin and the physiologically acceptable salts thereof. Pregabalin, gabapentin and viga-batrin are γ-aminobutyric acid derivatives and have the structures according to formulas (II), (III) and (IV), respectively:
[0025] Unless explicitly stated otherwise, all amounts of the second pharmacologically active ingredient specified in the following are given according to the corresponding amount of the amino acid, i.e. the compounds according to one of formulas (II) to (IV).
[0026] In a preferred embodiment, the second pharmacologically active ingredient is a gabapentinoid, preferably according to one of formulas (II) to (IV), in form of the free amino acid.

[0036] Since the second pharmacologically active ingredient according to one of formulas (I I) to (IV) contains a carboxyl group, it may react with the first pharmacologically active ingredient according to formula (I) forming a salt which incorporates both pharmacologically active ingredients.

Animals developed tactile allodynia at the ipsilateral paw. One to four weeks after the operation the tactile allodynia threshold baseline (withdrawal threshold) was measured on the ipsilateral and contralateral hind paw by an electronic von Frey anaesthesiometer(Somedic, Schweden). After test and measurement of the baseline, the first pharmacologically active ingredient (1r,4r)-6’-fluoro-N,N-dimethyl-4-phenyl-4’,9’-dihydro-3’H-spiro[cyclohexane-1,1’-pyrano[3,4,b]indol]-4-amine in form of the hemicitrate salt, and the second pharmacologically active ingredient in form of (S)-pregabalin according to the invention were each dissolved in a mixture of DMSO (10 %), Cremophor (5 %) and glucose solution (85 %) and injected by the intravenous (i.v.) route (application volume 5 ml/kg). The first and the second pharmacologically active ingredient were either administered as the respective single substance or both at the same time. Animals were randomly assigned to groups of 10 for each test dose and vehicle (DMSO (10 %), Cremophor (5 %) and glucose solution (85 %)) and tactile withdrawal thresholds were tested 0.5 h before administration and on several time points (0.5, 1 and 3 hours) after intravenous administration. Ipsi- and contralateral hindpaws were tested. The median of the withdrawal threshold for each animal at a given time is calculated from five individual stimulations with the electronic von Frey filament. Withdrawal thresholds of the injured paws are expressed as % MPE (Maximum possible effect) comparing predrug threshold of Chung-Animals (= 0 % MPE) and control threshold of sham-animals (100 % MPE). A cut-off is set at 100 % MPE. The effect of each compound and vehicle is calculated for each testing time point as interindividual % MPE value.

Claims 1. A pharmaceutical composition for use in the prevention or treatment of pain comprising: a) a first pharmacologically active ingredient selected from the compound according to chemical formula (I) and the physiologically acceptable salts thereof
(I), and b) a second pharmacologically active ingredientselectedfromgabapentinoidsand the physiologically acceptable salts thereof. 2. The pharmaceutical composition for use in the prevention or treatement of pain according to claim 1, wherein the first pharmacologically active ingredient is the compound according to chemical formula (I)

The pharmaceutical composition for use in the prevention or treatement of pain according to claim 1 or 2, wherein the second pharmacologically active ingredient is selected from the group consisting of pregabalin, gabapentin, vigabatrin and the physiologically acceptable salts thereof. 4. The pharmaceutical composition for use in the prevention or treatement of pain according to any of the preceding claims, which contains the first and the second pharmacologically active ingredient in such a weight ratio that they will exert a synergistic therapeutic effect upon administration to a patient. 5. The pharmaceutical composition for use in the prevention or treatement of pain according to any of the preceding claims, wherein the relative weight ratio of the first pharmacologically active ingredient to the second pharmacologically active ingredient is within the range of from 1 : 30 to 1 : 1,000,000. 6. The pharmaceutical composition for use in the prevention or treatement of pain according to claim 5, wherein the pain is: - peripheral, central or muscle skeletal pain; and/or - acute, subacute or chronic pain; and/or - moderate to severe pain; and/or - neuropathic or psychogenic or nociceptive or mixed pain; and/or - low back pain, visceral pain or headache; and/or - post-operative (post-surgical), cancer or inflammatory pain 7. A pharmaceutical dosage form for use in the prevention or treatement of pain comprising the pharmaceutical composition according to any of the preceding claims. 8. The pharmaceutical dosage form for use in the prevention or treatement of pain according to claim 7, which contains the first pharmacologically active ingredient in a dose of from 10 to 1,200 μg. 9. The pharmaceutical dosage form for use in the prevention or treatement of pain according to claim 7 or 8, which contains the second pharmacologically active ingredient in a dose of from 0.05 to 5 g. 10. The pharmaceutical dosage form for use in the prevention or treatement of pain according to any of claims 7 to 9, wherein the dosage of the first pharmacologically active ingredient is within the range of from 1:20 to 20:1 of the amount which is equieffective to the dosage of the second pharmacologically active ingredient. 11. The pharmaceutical dosage form for use in the prevention or treatement of pain according to any of claims 7 to 10, which is for oral, intravenous, intraperitoneal, transdermal, intrathecal, intramuscular, intranasal, transmucosal, subcutaneous, or rectal administration. 12. A kit for use in the prevention or treatement of pain comprising a first pharmaceutical dosage form comprising the first pharmacologically active ingredient as defined in claim 1 or 2, and a second pharmaceutical dosage form comprising the second pharmacologically active ingredient as defined in claim 1 or 3. 13. The kit for use in the prevention or treatement of pain according to claim 12, wherein the first and the second pharmaceutical dosage form are adapted for simultaneous or sequential administration, either by the same or a different pathway of administration.
Patentansprüche 1. Pharmazeutische Zusammensetzung zur Verwendung bei der Vorbeugung oder Behandlung von Schmerzen, umfassend: a) einen ersten pharmakologisch aktiven Inhaltsstoff, der ausgewählt ist aus der Verbindung gemäß der chemischen Formel (I) und den physiologisch annehmbaren Salzen davon:
(I), und b) einen zweiten pharmakologisch aktiven Inhaltsstoff, der ausgewählt ist aus Gabapentinoiden und den physiologisch annehmbaren Salzen davon. 2. Pharmazeutische Zusammensetzung zur Verwendung bei der Vorbeugung oder Behandlung von Schmerzen nach Anspruch 1 .wobei der erste pharmakologisch aktive Inhaltsstoff die Verbindung gemäß derchemischen Formel(l)ist:

Pharmazeutische Zusammensetzung zur Verwendung bei der Vorbeugung oder Behandlung von Schmerzen nach Anspruch 1 oder 2, wobei der zweite pharmakologisch aktive Inhaltsstoff aus der Gruppe ausgewählt ist, die aus Pregabalin, Gabapentin, Vigabatrin und den physiologisch annehmbaren Salzen davon besteht. 4. Pharmazeutische Zusammensetzung zur Verwendung bei der Vorbeugung oder Behandlung von Schmerzen nach einem der vorhergehenden Ansprüche, die den ersten und den zweiten pharmakologisch aktiven Inhaltsstoff in einem derartigen Gewichtsverhältnis enthält, dass sie bei Verabreichung an einen Patienten eine synergistische therapeutische Wirkung ausüben. 5. Pharmazeutische Zusammensetzung zur Verwendung bei der Vorbeugung oder Behandlung von Schmerzen nach einem der vorhergehenden Ansprüche, wobei das relative Gewichtsverhältnis des ersten pharmakologisch aktiven Inhaltsstoffs zum zweiten pharmakologisch aktiven Inhaltsstoff im Bereich von 1:30 bis 1:1.000.000 liegt. 6. Pharmazeutische Zusammensetzung zur Verwendung bei der Vorbeugung oder Behandlung von Schmerzen nach Anspruch 5, wobei die Schmerzen Folgendes sind: - periphere, zentrale oder Muskelskelettschmerzen; und/oder - akute, subakute oder chronische Schmerzen; und/oder - mäßige bis starke Schmerzen; und/oder - neuropathische oder psychogene oder nozizeptive oder gemischte Schmerzen; und/oder - Kreuzschmerzen, viszerale Schmerzen oder Kopfschmerzen; und/oder - postoperative (nach einer Operation auftretende), krebsbedingte oder Entzündungsschmerzen. 7. Pharmazeutische Darreichungsform zur Verwendung bei der Vorbeugung oder Behandlung von Schmerzen, umfassend die pharmazeutische Zusammensetzung nach einem der vorhergehenden Ansprüche. 8. Pharmazeutische Darreichungsform zur Verwendung bei der Vorbeugung oder Behandlung von Schmerzen nach Anspruch 7, die den ersten pharmakologisch aktiven Inhaltsstoff in einer Dosis von 10 bis 1.200 μg enthält. 9. Pharmazeutische Darreichungsform zur Verwendung bei der Vorbeugung oder Behandlung von Schmerzen nach Anspruch 7 oder 8, die den zweiten pharmakologisch aktiven Inhaltsstoff in einer Dosis von 0,05 bis 5 g enthält. 10. Pharmazeutische Darreichungsform zur Verwendung bei der Vorbeugung oder Behandlung von Schmerzen nach einem der Ansprüche 7 bis 9, wobei die Dosierung des ersten pharmakologisch aktiven Inhaltsstoffs im Bereich von 1:20 bis 20:1 derMenge liegt, diegleichwirksamwiedie Dosierung des zweiten pharmakologisch aktiven Inhaltsstoffs ist. 11. Pharmazeutische Darreichungsform zur Verwendung bei der Vorbeugung oder Behandlung von Schmerzen nach einem der Ansprüche 7 bis 10, die zur oralen, intravenösen, intraperitonealen, transdermalen, intrathekalen, intramuskulären, intranasalen, transmukosalen, subkutanen oder rektalen Verabreichung vorgesehen ist. 12. Kit zur Verwendung bei der Vorbeugung oder Behandlung von Schmerzen, umfassend eine erste pharmazeutische Darreichungsform, umfassend den ersten pharmakologisch aktiven Inhaltsstoff, wie in Anspruch 1 oder 2 definiert, und eine zweite pharmazeutische Darreichungsform, umfassend den zweiten pharmakologisch aktiven Inhaltsstoff, wie in Anspruch 1 oder 3 definiert. 13. Kit zur Verwendung bei der Vorbeugung oder Behandlung von Schmerzen nach Anspruch 12, wobei die erste und die zweite pharmazeutische Darreichungsform zur gleichzeitigen oder aufeinanderfolgenden Verabreichung entweder auf dem gleichen oder einem unterschiedlichen Verabreichungsweg ausgelegt sind.

Composition pharmaceutique destinée à être utilisée dans la prévention ou le traitement de la douleur selon la revendication 5, la douleur tétant : - une douleur périphérique, centrale ou musculo-squelettique ; et/ou - une douleur aiguë, subaiguë ou chronique ; et/ou - une douleur modérée à sévère ; et/ou - une douleur neuropathique ou psychogène ou nociceptive ou mixte ; et/ou - une lombalgie, une douleur viscérale ou une céphalée ; et/ou - une douleur post-opératoire (post-chirurgicale), liée au cancer ou de nature inflammatoire. 7.
