Abstract
Description
Stable hydroalcoholic foam for antiseptic and skin cleansing.
Περιγραφή της εφεύρεσης Description of the invention
The correct observance of hand hygiene rules was and is one of the main points of prevention of the transmission of various viruses and bacteria. Rapid antisepsis, i.e. hand application with antiseptic agents, is considered more effective than hand washing with simple soap. The present invention relates to a stable hydroalcoholic foam for skin antiseptic and cleansing.
Hand hygiene is a part of the daily life of citizens since it is one of the most important measures to prevent the spread of germs and viruses in the community. Many of the antiseptics on the market are difficult to use and cause dryness of the skin which is often accompanied by itching and a burning sensation. Hand cleaning and disinfection, however, are effective when applied at the right time and with the right method and technique. For this reason, the antiseptic agent used must have a rapid effect, be easy to use, effective, not irritate or dry the hands and not have an unpleasant smell.
Hand disinfection according to the European Standard ΕΝ 1500 (hygienic handrub): a study with Gram-negative and Grampositive test organisms, Int. J. Hyg. Environ. Health 204, 123 ± 126 (2001); Gunter Kampf, Efficacy of ethanol against viruses in hand disinfection, J Hosp Infect 2018 Apr;98(4):331 -338.] . Ο οργανισμός FDA έχει χαρακτηρίσει τόσο την αιθανόλη σε περιεκτικότητα από 60% έως 95%, όσο και την ισοπροπανόλη σε περιεκτικότητα από 70% έως 91,3% ως ασφαλείς και αποτελεσματικές [Reichel Μ et. al. Alcohols for Skin Antisepsis at Clinically Relevant Skin Stes, Antimicrobial agents and chemotherapy, 2009; 4778-4782] , Μελέτες έχουν δείξει ότι τα αλκοολούχα διαλύματα περιεκτικότητας 60-80% έχουν εντονότερη αντιμικροβιακή δράση έναντι όμοιων διαλυμάτων περιεκτικότητας σε αλκοόλη πάνω από 80%, λόγω του ότι το νερό δρα ενισχυτικά στην μετουσίωση των πρωτεϊνών [WHO Guidelines on hand hygiene in health care (Advanced draft)., Global patient safety challenge 2005-2006] . Επίσης αναστέλλουν την βλάστηση των σπορίων αλλά δεν έχουν υπολειμματική δράση [Me Donnell G and Russell A.D, Antiseptics and Disinfectants: Activity, Action, and Resistance, Clinical Microbiology Reviews, Jan. 1999, p. 147-179] , Για τον λόγο αυτό πολλά αλκοολούχα προϊόντα περιέχουν και άλλα βιοκτόνα όπως χλωρεξιδίνη, η οποία παραμένει στο δέρμα και μετά την εξάτμιση της αλκοόλης αυξάνοντας την αποτελεσματικότητα του προϊόντος [Bush, L. Ε., L. Μ. Benson, and J. Η. White.. Pig skin as a test substrate for evaluating topical antimicrobial activity. J. Qin. Microbiol. 1986;24:343-348] . Rapid antisepsis, i.e. hand application with antiseptic agents, is considered more effective than hand washing with simple soap. Alcohols with 2-6 carbon atoms in relatively high concentrations are one of the basic chemical means of sterilization and antiseptics. The water-alcohol solutions owe their antimicrobial effect to their ability to cause protein denaturation while at the same time solubilizing lipids thus causing the lysis of the cell wall of microorganisms. Alcohols have a rapid disinfecting and antiseptic effect against gram positive and gram negative bacteria, fungi, mycobacterium tuberculosis and various viruses [Gorony-Bermes P. Hand disinfection according to the European Standard EN 1500 (hygienic handrub): a study with Gram-negative and Gram-positive test organisms, Int. J. Hyg. Environ. Health 204, 123 ± 126 (2001); Gunter Kampf, Efficacy of ethanol against viruses in hand disinfection, J Hosp Infect 2018 Apr;98(4):331 -338.] . The FDA has classified both ethanol in a content of 60% to 95%, and isopropanol in a content of 70% to 91.3% as safe and effective [Reichel M et. al. Alcohols for Skin Antisepsis at Clinically Relevant Skin Stages, Antimicrobial agents and chemotherapy, 2009; 4778-4782] , Studies have shown that alcoholic solutions with a content of 60-80% have a stronger antimicrobial effect compared to similar solutions with an alcohol content of more than 80%, due to the fact that water acts as an enhancer in the denaturation of proteins [WHO Guidelines on hand hygiene in health care (Advanced draft)., Global patient safety challenge 2005-2006] . In general, alcohols exhibit rapid, broad-spectrum antimicrobial activity against bacteria including mycobacteria, against viruses and fungi. They also inhibit spore germination but have no residual activity [Me Donnell G and Russell A.D, Antiseptics and Disinfectants: Activity, Action, and Resistance, Clinical Microbiology Reviews, Jan. 1999, p. 147-179] , For this reason many alcoholic products contain other biocides such as chlorhexidine, which remains on the skin even after the alcohol evaporates, increasing the effectiveness of the product [Bush, L. E., L. M. Benson, and J. H. White.. Pig skin as a test substrate for evaluating topical antimicrobial activity. J. Qin. Microbiol. 1986;24:343-348].
Alcohols, despite their antimicrobial effect, cannot be used directly on human skin because they cause irritation and dryness. For this reason, various moisturizing/emollient agents are added to the alcoholic solutions to make the composition easy to use, tolerable and skin-friendly while maintaining their effectiveness against microbes.
Όλες οι παραπάνω είναι ασφαλείς και χρησιμοποιούνται ευρέως σε φαρμακευτικά σκευάσματα [Mary Ann Liebert Final Fteport on the Safety Assessment of Stearyl Alcohol, Oleyl Alcohol, and Octyl Dodecanol. Journal of the American college of toxicology, 1985;4 (5): 1 -29; Sepulveda E et. al. Ftelationship Between Internal Phase Volume and Emulsion Stability: The Cetyl Alcohol/Stearyl Alcohol System, Pharmaceutical development and Technology, 2003; 8(3): 263-275] . Stabilizing agents are used in said compositions to adjust the viscosity and structurally stabilize the foam formed. Fatty alcohols such as hexadecane-1-ol (cetyl alcohol), octadecane-1-ol (stearic alcohol), dodecanol (lauric alcohol), 1-tetradecanol (myritic alcohol), cis -9-octadecen-1-ol (oleic alcohol) and docisodanol (bechenyl alcohol). All of the above are safe and widely used in pharmaceutical formulations [Mary Ann Liebert Final Fteport on the Safety Assessment of Stearyl Alcohol, Oleyl Alcohol, and Octyl Dodecanol. Journal of the American college of toxicology, 1985;4 (5): 1 -29; Sepulveda E et. al. Relationship Between Internal Phase Volume and Emulsion Stability: The Cetyl Alcohol/Stearyl Alcohol System, Pharmaceutical development and Technology, 2003; 8(3): 263-275].
Ως εκ τούτου, οι επιφανειοδραστικές ουσίες έχουν την ιδιότητα μείωσης της επιφανειακής και διεπιφανειακής τάσης και επηρεάζουν τον τρόπο με τον οποίο τα άλλα μόρια συμπεριφέρονται στις διεπιφάνειες και στο διάλυμα [Hamme J.D et. al. Physiological aspects Fhrt 1 in a series of papers devoted to surfactants in microbiology and biotechnology. Biotechnology Advances 24 (2006) 604-620] . Ζ. Fton High- and lowmolecular-mass microbial surfactants, Appl Microbiol Biotechnol (1999) 52: 154± 162] , Surfactants or solubilizers are molecules composed of a hydrophobic and a hydrophilic part. Therefore, surfactants have the property of reducing surface and interfacial tension and influence the way other molecules behave at interfaces and in solution [Hamme J.D et. al. Physiological aspects Fhrt 1 in a series of papers devoted to surfactants in microbiology and biotechnology. Biotechnology Advances 24 (2006) 604-620]. Their hydrophilic part is differentiated and depending on its nature, surfactants are divided into anionic, cationic, nonionic and amphoteric. Nonionic surfactants carry no charge at their hydrophilic end, are less irritating and have a high emulsifying potential. Nonionic surfactants such as ethoxylated aliphatic alcohols and polyoxyethylated derivatives are widely used in the pharmaceutical industry as emulsifiers to increase product stability and as solubilizers to increase the degree of solubility of ingredients [ Ftosenberg E. Z. Fton High- and lowmolecular-mass microbial surfactants, Appl Microbiol Biotechnol (1999) 52: 154± 162] ,
Τα φυτικά εκχυλίσματα και βότανα είναι πλούσια πηγή βιταμινών, αντιοξειδωτικών, πρωτεϊνών, τερπενοειδών, αιθέριων ελαίων και πληθώρα άλλων βιοδραστικών συστατικών τα οποία συμμετέχουν σε πολλές βιολογικές λειτουργίες του κυττάρου και προάγουν την υγεία του δέρματος [Ribeiro AS et al. Main Benefits and Applicability of Rant Extracts in Skin Care Products, Cosmetics 2015, 2, 48-65] . The use of herbal soothing and emollients in products for human skin is beneficial. Plant extracts and herbs are a rich source of vitamins, antioxidants, proteins, terpenoids, essential oils and a multitude of other bioactive components that participate in many biological functions of the cell and promote skin health [Ribeiro AS et al. Main Benefits and Applicability of Rant Extracts in Skin Care Products, Cosmetics 2015, 2, 48-65] .
Μεταξύ άλλων διαθέτει αντκρλεγμονώδεις, μυκητοκτόνες, αντισταμινικές και αντισηπτικές ιδιότητες [Gupta S. C et al. Neem (Azadirachta indica): An indian traditional panacea with modern molecular basis, Phytomedicine 2017; 34:14-20] . To θυμάρι ( Thymus vuigarid) είναι ένα άλλο φυτό ευρέως χρησιμοποιούμενο στις φαρμακευτική βιομηχανία για τις καταπραϋντικές και μαλακτικές του ιδιότητες αλλά και για τις αντκρλεγμονώδεις, αντιοξειδωτικές, αντί βα κτηριακές και αντιμυκητιακές ιδιότητες του [Hosseinzadeh S. et. al. The Application of Medicinal Rants in Traditional and Modern Medicine: A Review of Thymus vulgaris, International Journal of Clinical Medicine, 201 5, 6, 635-642] . Μελέτες έχουν δείξει ότι η αλόη διεγείρει την παραγωγή κολλαγόνου και υαλουρονικού οξέος από τους ανθρώπινους ινοβλάστες βελτιώνοντας την ελαστικότητα, και την ενυδάτωση του δέρματος και προάγοντας με αυτόν τον τρόπο την διατήρηση του υγιούς δέρματος [Tanaka Μ et al. Effects of Aloe Sterol Supplementation on Skin Elasticity, Hydration, and Collagen Score: A 12-Week Double-Blind, Randomized, Controlled Trial Skin Pharmacol Physiol 2016;29:309-317] , Οι θεραπευτικές ιδιότητες του φυτού καλέντουλα (Calendula officinalis) έχουν αναγνωρισθεί από την αρχαιότητα. Διάφορα εκχυλίσματα του φυτού έχουν επιδείξει ποικίλες φαρμακευτικές ιδιότητες συμπεριλαμβανομένου αντκρλεγμονώδης, αντιιοξειδωτικής, αντιϊκής, αντιβακτηριακής και αντιμυκητιακής δράσης [Khalid A et. al, Biology of Calendula officinalis Linn.: Focus on Pharmacology, Biological Activities and Agronomic Practises, Medical and aromatic plant science and biotechnology 2012; 6 (1)12-27] , Επίσης, μελέτες έχουν δείξει ότι εκχύλισμα από φλοιό πεύκου (Pinus pinaster) το οποίο περιέχει ποικιλία αντιοξειδωτικών συμπεριλαμβανομένου βιοφλαβονοειδών, κατεχινών, προκυανιδινών και φαινολικών οξέων εκτός του ότι ενυδατώνει την επιδερμίδα βελτιώνει τις λειτουργίες του δερματικού φραγμού και την ομοιόσταση [Susanne Grether-Beck et. al. French Maritime Pine Bark Extract (Pycnogenol<®>) Effects on Human Skin: Clinical and Molecular Evidence. Skin Pharmacol Physiol 2016;29:13-17] A plant with valuable properties used for this reason is Azadirachta Melia (Neem) which belongs to the mahogany family and since ancient times has been a panacea for many diseases. Among other things, it has anti-inflammatory, fungicidal, antihistamine and antiseptic properties [Gupta S. C et al. Neem (Azadirachta indica): An Indian traditional panacea with modern molecular basis, Phytomedicine 2017; 34:14-20] . Thyme (Thymus vuigarid) is another plant widely used in the pharmaceutical industry for its soothing and emollient properties but also for its anti-inflammatory, antioxidant, antibacterial and antifungal properties [Hosseinzadeh S. et. al. The Application of Medicinal Rants in Traditional and Modern Medicine: A Review of Thymus vulgaris, International Journal of Clinical Medicine, 201 5, 6, 635-642] . The beneficial properties of aloe mainly for the skin but also for many diseases have been known since ancient times. Studies have shown that aloe stimulates the production of collagen and hyaluronic acid by human fibroblasts improving the elasticity and hydration of the skin and thus promoting the maintenance of healthy skin [Tanaka M et al. Effects of Aloe Sterol Supplementation on Skin Elasticity, Hydration, and Collagen Score: A 12-Week Double-Blind, Randomized, Controlled Trial Skin Pharmacol Physiol 2016;29:309-317], The therapeutic properties of the calendula plant (Calendula officinalis) have recognized since antiquity. Various extracts of the plant have demonstrated various medicinal properties including anti-inflammatory, antioxidant, antiviral, antibacterial and antifungal activity [Khalid A et. al, Biology of Calendula officinalis Linn.: Focus on Pharmacology, Biological Activities and Agronomic Practices, Medical and aromatic plant science and biotechnology 2012; 6 (1)12-27] , Also, studies have shown that pine bark extract (Pinus pinaster) which contains a variety of antioxidants including bioflavonoids, catechins, procyanidins and phenolic acids in addition to moisturizing the skin improves skin barrier functions and homeostasis [Susanne Grether-Beck et. al. French Maritime Pine Bark Extract (Pycnogenol<®>) Effects on Human Skin: Clinical and Molecular Evidence. Skin Pharmacol Physiol 2016;29:13-17]

Although there are many antiseptic products available in various forms, there is a need for a fast-acting skin antiseptic that is easy to use, does not dry the skin, does not dehydrate the skin, instead has an emollient effect and leaves a pleasant sensation. Antiseptic products in the form of gel leave the skin with an unpleasant sticky feeling after repeated use, due to the concentration of coagulation factors contained in them. Antiseptic solutions show a very high rate of evaporation when applied to the hands, as a result of which their antiseptic effect is significantly reduced during use, while in addition, due to their high fluidity, they are less easy to use.
The above problems have been solved by a stable hydroalcoholic composition, in the form of a foam for use as a skin antiseptic, which contains one or more alcohols with two to six carbon atoms, stabilizing agents, one or more surfactants and one or more soothing agents with antimicrobial activity.
Said composition may additionally contain and/or perfumes and/or propellant gases.
The prior art has not disclosed a hydroalcoholic composition in foam form for use as a skin antiseptic which contains an alcohol or a mixture of alcohols with two to six carbon atoms, stabilizing agents, nonionic surfactants and soothing agents and/or perfumes and/or propellant gases and keep its texture unchanged until it reaches the temperature of the human skin, keeping the percentage of alcohol constant until then without evaporating.
Although alcohols and surfactants are very irritating to the skin and it is not obvious how to overcome the problem, the addition of the appropriate soothing agents drastically alleviates the problem of dryness and the resulting skin irritations.
The pH of the compositions ranges from 5.0 to 8.0 without the use of buffer solutions, usually around 7.0 to be tolerated by the skin.
We have been surprised to find that the foam formed from the composition of the present invention remains unchanged until it reaches the temperature of the human skin, keeping the percentage of alcohol constant until it evaporates.
Additionally, we were surprised to find that the composition of the present invention, despite containing fatty alcohols as stabilizing agents, does not leave a greasy feeling on the skin.
Additionally, we have surprisingly found that the foam formed from the composition of the present invention is extremely stable and maintains its volume and consistency until the time of application.
Also, we found with surprise that the composition of the present invention does not dry, nor irritate but instead leaves the skin soft by exerting a softening effect on it.
In addition, we were surprised to find that the composition of the present invention has a pleasant smell, without the characteristic smell of alcohols, despite the high percentage of alcohol it contains even without the addition of perfume.

Composition of the water-alcohol formulation fill solution The following table summarizes the components of the water-alcohol formulation fill solution of a proposed embodiment of the present invention.
Table 1 : Filling solution composition.
The fill solution of Example 1 has a density of 0.870 g/mL at 35°C.
The filling solution is encapsulated in the presence of a propellant gas mixture of propane/isobutane/n-Butane in a ratio of 55:15:30 at a rate of 5% by weight of the total composition in foam form in a metal container with a continuous flow expansion valve.
The antiseptic foam formed upon expansion of the composition of Example 1 has a density of 0.0923 g/ml at 20-22°C.
The viscosity of the foam formed at room temperature (20-22°C) was measured using a Brookfield DVE-RV viscometer with T-bar spindle “E” (S95) at 12 rpm and found to be 16701.7cRs (Centipoise).
The combination of density and viscosity makes the resulting foam extremely stable at room temperature (20-22°C).
Table 1a shows the mass percentage of foam that remains over time at room temperature 24-25°C compared to a hydroalcoholic solution and gel of the same ethanol content.
Table 1a: Foam mass percentage over initial mass over time for 70% v/v hydroalcoholic solution, 70% v/v hydroalcoholic gel and the foam formed from Example 1, at a temperature of 24-25°C.
Keeping the percentage of alcohol at such high levels up to 1 hour after the foam is exposed to the environment makes it extremely stable compared to both water-alcohol solutions and gels of the same alcohol content which makes it more easy to use and effective even if its application to the hands is not immediate.
