A device for use in filling in football pool and like coupons or forms com. prises a pair of stencil elements 10, 11. one of the elements 11 constituting a form entry element provided with a plurality of openings 13, 14, 15 some of which, at 14, constitute entry openings and at least one of which, at 13, constitutes a client identity opening, the other stencil element 10 constituting a periodical record entry element provided with a number of record entry openings 16, 17, means 20, 21 being provided for removably holding behind each stencil 10, 11 separate plain or substantially plain sheets 23, 22 of paper or the like which constitute entry forms 23 or record sheets 22, the holding means 20, 21 permitting the removal of the sheets 22, 23 independently of each other, and the stencil elements 10, 11 being connected together, as by a fold line 12 between them, so that they may be disposed in side by side relation so that the sheets 23 exposed through the openings are simultaneously visible and held by the holding means 20, 21 so that the user can complete the entries on the entry form 23 through the openings 13, 14 and 15, record the entry on the record sheet 22 through the openings 16, 17 and thereafter remove the entry form 23 and despatch it. As shown, the parts of the device are of cardboard, the holding means 20. 21 being connected to the stencils 10, 11 along fold lines 18, 19, but thin aluminium sheet or plastic sheet may be used and hinges provided. The openings 16, 17 are for monetary sums and the names of the horses or the like involved and numbers of points, the one record sheet 22 relating, for instance, to four days of the week for each of which an entry form 23 has been completed through the openings 13, 14 and 15. An outer cover 24 is provided, connected along the fold line 12 to the stencils 10, 11, and a data sheet 25 may be foldably connected thereto, the sheet 25 being placed in front of the stencil 11 when entries are being made on the record sheet 22. The stencils 10, 11 may have suitable printing adjacent each of the openings, which openings may be varied as desired for different purposes. The resulting booklet as shown in Fig. 2 may be inserted in an outer casing for convenience. Additional sheets 22, 23 may be held behind the ones in use by the holding means 20, 21, the outer faces of which may be printed with the rules of the competition or other activity concerned.
