Abstract
A femoral prosthesis comprises a straight stem (10) and an upper body part (12), a neck (13) extending from the body part (12) and, at the base of the neck a lip (14) extending outwardly along three sides of the prosthesis. The body part (12) may also carry a radially-projecting axially- extending fin (15). Between its body part (12) and lower stem (10) the prosthesis stem is formed with a tapered section (16) which narrows downwardly and provides an angled shoulder. The prosthesis can be formed as a single finished forging from titanium, steel or, preferably. from titanium/vanadium/aluminium alloy. and the part (12) and the length (11) are coated with calcium hydroxy- apatite (Ca5(PO4)3. The upper part (12) can have a recessed hexagonal socket which extends downwardly to a bore (18), to allow possible extraction of the prosthesis. <IMAGE>
Description
SPECIFICATION
Femoral prosthesis
This invention relates to orthopaedic prostheses and, more specifically, to a straightstem femoral prosthesis.
Femoral prostheses have been known and used, with considerable success, for a good number of years in so-called 'total hip replacements'. Very briefly, the operation involves extracting the head of the patient's femur from the acetabular cavity in the pelvis, replacing the neck and head of the femur with a femoral prosthesis, securing a cup prosthesis into the acetabular cavity, and inserting the head of the femoral prosthesis into the cup.
The preferred form of femoral prosthesis is now the straight-stem prosthesis which has replaced the earlier curved-stem or 'banana" types, and other improvements have evolved over the years. For example, apatite coatings have been applied to the prostheses stems to promote ingrowth of bone and anchor the prosthesis in the femur shaft without the use of bone cement. As a result, the success rate in hip-joint replacement operations is extremely high, reaching well over 90% under appropriate conditions.
One residual problem however has been that some cemented femoral prostheses have become slightly loose and unstable after a time; it is an object of this invention to provide a femoral prosthesis which will produce generally improved results and, specifically, will remain firmly in position on a more-or-less permanent basis.
According to the present invention there is provided a femoral prosthesis having a straight stem provided at its upper end with a neck and with a lip adapted to engage the upper end of the severed femur, characterised in that the stem is formed at an intermediate location in its length with a tapered section which narrows downwardly and provides an angled shoulder to engage a similarly shaped section formed inside the tubular femur shaft.
Preferably, the upper end of the stem is formed with a radially-projecting axially-extending fin to engage in the shaft of the femur which has been appropriately shaped, to resist rotation of the prosthesis.
Preferably, also, the prosthesis is formed from titanium/vanadium/aluminium alloy, and the straight stem is coated with calcium hydroxy-apatite.
According to a preferred further feature of the present invention, an upwardly-opening tapped bore is formed in the upper part of the stem to receive a screwed-rod instrument and so provide for possible extraction of the prosthesis. The tapped bore may be formed as a downward continuation of a shaped, usuallyhexagonal, socket in the upper part which is adapted to receive an Allen key instrument to facilitate rotary manipulation of the prosthesis.
Embodiments of the present invention will now be described, by way of example, with reference to the accompanying drawings in which:
Figure 1 is a side elevation showing a femoral prosthesis according to the present invention;

Figure 4 is a side view showing the prosthesis, with its ceramic head, after insertion in the shaft of a femur.
Referring to Figs. 1 to 3 of the drawings, the femoral prosthesis comprises a straight stem 10 having a length 11 of circular section and an upper body part 12. A neck 13 extends from the body part 12 and, at the base of the neck, a lip 14 extends outwardly along three sides of the prosthesis. The body part 12 is also formed with a radially-projecting axially-extending fin 15 and, between its body part and circular-section length, the prosthesis stem is formed with a tapered section 16 which narrows downwardly and provides an angled shoulder.
The prosthesis is formed as a single finished forging from titanium, steel or, more desirably, from titanium/vanadium/aluminium alloy, and the part 12 and the length 11 are coated with calcium hydroxy-apatite, Ca#(PO4)3 OH.
As clearly shown in Fig. 3, the upper part 12 is formed with a hexagonal socket 17, and the socket extends downwardly as a tapped bore 18.
When inserted in an upper end of the tabular shaft 19 of a patient's femur (Fig. 4), which has been suitably prepared after removal of the neck with its head, the lip 14 engages the upper, severed face of the femur and, also, the angled shoulder provided by section 16 engages a correspondingly-prepared part in the femur. The combination of these two engagements, assisted by the bottom, rounded end 20 of the stem jamming in the shaft, plus the rotational stability afforded by the fin 15 which creates a slot in the femur, act to hold the prosthesis firmly and stably in position. After a period of a few weeks, the patient's bone will grow onto the apatite coating to ensure permanent fixation of the prosthesis. It is believed that this effect arises to a substantial extent due to the aforementioned stabilizing shoulder (16), and that this shoulder in combination with the lip 14, the rounded lower end 20 and the fin 15, ensure that the prosthesis will remain firmly and stably in position for a considerable number of years.
It might be added that the neck 13 includes an outer slightly-tapered section 21 which receives a ceramic head 22 (Fig. 4) adapted to be received by a prosthetic cup (not shown) which is fitted into the prepared acetabular cavity of the patient's pelvis. Such a cup is illustrated in British Design Registration No.
1030276, and comprises a polyethylene socket resting in an apatite-coated metal shell.
Ceramic heads (22) bored to alternative depths are provided for appropriate compatibility with the required length of the replaced neck of the femur.
Claims (5)
1. A femoral prosthesis having a straight stem provided at its upper end with a neck and with a lip adapted to engage the upper end of the severed femur, characterised in that the stem is formed at an intermediate location in its length with a tapered section which narrows downwardly and provides an angled shoulder to engage a similarly shaped section formed inside the tubular femur shaft.
2. A prosthesis as claimed in Claim 1, wherein the upper end of the stem is formed with a radially-projecting axially-extending fin to engage in the shaft of the femur which has been apropriately shaped, so as to resist rotation of the implanted prosthesis.
3. A prosthesis as claimed in Claim 1 or 2, which is formed from a titanium/vanadium/aluminium alloy, and the straight stem is coated with calcium hydroxy-apatite.
4. A prosthesis as claimed in Claim 1, 2 or 3, wherein an upwardly-opening tapped bore is formed in the upper part of the stem to receive a screwed-rod instrument and so provide for possible extraction of the prosthesis.
5. A femoral prosthesis constructed substantially as hereinbefore described with reference to the accompanying drawings.
