The present invention relates to instruments for performing a surgical intervention to treat Dupuytren's disease of the hand.
A very complete and documented article is known which deals with Dupuytren's disease, presented by Doctors Marie-Pascale Manet, Jean-Pierre Teyssedou, Sophie Lahalle and Jean-Marc Ziza.
Parts of this article are reproduced below, in italic letters, for the understanding of the instrument according to the present invention.
This so-called Dupuytren's disease affects the palm of the hand and the palmar surface of the fingers. It is an abnormal fibrous thickening (or fibrosis) of a membrane located under the skin called palmar fascia. This fibrosis deforms the surface of the palm where bumps or nodules develop, skin folds and strings (fibrous bridles) that form in the axes of the fingers. The flanges can retract, flexing the fingers gradually. These flexion retractions are irreducible and can fix the fingers in hooks or claws. They generate, by the progressive impossibility of opening the hand and relaxing certain fingers, functional difficulties which can be severe: embarrassment to give a handshake, to spread the fingers, to put on a glove, to slip his hand in his pocket, for toilet or hygienic gestures, for carrying an object or holding a tool handle, for typing on a computer keyboard and for using the mouse, for playing a musical instrument, for practice certain sports.
Dupuytren's disease, described in the 1830s, had, for 140 years, only surgery as a sole remedy - a cumbersome and costly method of care, a source of long unavailability for patients, burdened with sometimes severe complications and frequent recurrences .
Alors, un rhumatologue français, JeanIn the 1970s, new needles appeared. These new needles, single use, fine (5 / 10th of a mm), silicone, have a bevel with two sharp sides. So, a French rheumatologist, Jean
Luc Lermusiaux had the idea to take advantage of this technical innovation. Indeed, introduced through the skin, these needles, used as “mini scalpels”, could allow, by back and forth movements, to cut the fibrous cords and to dissect the nodules. The same needle first used to inject the product for local anesthesia, and, at the end of the sequence, a firm and controlled maneuver to extend the treated finger and break the remaining fibers.
This method has been called percutaneous needle aponeurotomy. A new treatment therefore appeared, simple and inexpensive, perfectly ambulatory, achievable in the treatment room, requiring the cessation of activity for only a few days in manual workers. In addition, this treatment, due to its benignity and simplicity, could be repeated and therefore lent itself to the treatment of recurrences.
»First considered as an alternative to surgery, percutaneous needle fascia has become, over the decades, the initial and main treatment of Dupuytren's disease, that is to say the treatment to be considered first and applicable from the outset to the majority of cases including forms affecting the fingers. "
The present invention aims to achieve an improvement to this instrument for treating Dupuytren's disease, aiming to facilitate and simplify the work of the practitioner in order to be able to carry out the operations described above with even more safety and less arduousness for the practitioner .
More specifically, the subject of the present invention is an instrument for carrying out an intervention on the hand for treating Dupuytren's disease, comprising a needle defined along a longitudinal axis between a proximal end and a distal end, this distal end being shaped at least a cutting edge, and a cylindrical through hole made in said needle between its proximal and distal ends along said longitudinal axis, characterized in that it further comprises:

With reference to the two appended figures, the present invention relates to an instrument for performing an intervention on the hand to treat Dupuytren's disease.
This instrument comprises a needle 10 defined along a longitudinal axis 100 between a proximal end 11 and a distal end 12, this distal end being shaped as at least one cutting edge 13, and a cylindrical through-hole 14 made in the needle between its ends proximal and distal along its longitudinal axis.
This instrument also comprises a rigid cylindrical rod 20, for example made of stainless steel or the like, having a cross section equal to that of the through hole 14 so that this rod is able to be inserted by sliding into this through hole,
Means 30 are also provided for removably securing the rod 20 with the proximal end 11 of the needle 10 when the latter is inserted in the through-hole 14, so that one end 15 of this rod 20 is located at the less near the cutting edge 13 of the distal end 12 of the needle.
This configuration of means makes it possible to use this rod 20 again after sterilization, but with another needle because these needles are generally for single use.
The definition given above of the position of the end 15 of the rod 20, according to which it is located at least near the cutting edge 13 of the distal end 12 of the needle, means that this end is located at the level of the plane of the cutting edge, slightly projecting therefrom or, preferably, advantageously slightly recessed inside the through-hole 14, as in the embodiment illustrated in FIG. 1.
Also provided are gripping means 40 associated with the needle 10, able to be gripped manually, these gripping means being mounted in cooperation on the outer lateral wall of the needle.
The means 30 for removably securing the rod 20 when it is inserted in the through-hole 14 with the proximal end 11 of the needle 10 can be formed in several ways. It is possible to cite the following systems: (i) part of the rod is force-fitted into a cavity made at the proximal end of the needle, (ii) part of the rod is screwed into a tapped cavity made at the proximal end of the needle, (iii) part of the rod is force-fitted by a conical fitting into a complementary cavity made at the proximal end of the needle, (iv) part of the rod is temporarily secured to the needle in its through hole, by a temporary fixing or a reversible.
As mentioned above, the instrument according to the invention also comprises gripping means 40 able to be gripped manually. These means 40 comprise at least one sleeve 41 integrally surrounding a median part of the external lateral wall 18 of the needle 10, FIGS. 1 and 2, for example by force pressing, gluing, etc.
This sleeve 41 comprises, on its side wall, FIG. 2, at least two grooves 42, 43, each of concave shape defined so that a finger of a human being, namely the practitioner, fits relatively well to the bottom of the groove, so that the instrument is perfectly held and without fatigue by the practitioner's hand. Advantageously, as in the illustrated embodiment, the sleeve 41 has, on its side wall, at least three grooves 42, 43, 44, each having a shape like that described above. This number of grooves allows the practitioner to grasp and hold the instrument optimally in order to orient the instrument and handle it so that the cutting edge is positioned as desired.
According to a preferred embodiment, Figure 2, the axis of the concave grooves 42, 43, 44 is substantially parallel to the longitudinal axis 100, so that the practitioner can grasp this instrument in the manner, for example, of a pen or similar.
To facilitate its positioning relative to the organ to be operated, the instrument further comprises, mounted in association with a part of the needle 10 which is close to its proximal end 11, for example on the needle itself or on the sleeve 41, a visible mark of the practitioner when he operates and indexed relative to the orientation of the cutting edge, by specifying that the distal end 12 of the needle 10 formed into a cutting edge 13 is beveled.
According to always a preferred embodiment, the proximal end 11 of the needle 10 is shaped as a Luer-type attachment tip 111, known in itself, so as to be able to connect it to the conventional syringes known for injecting the anesthesia product. before inserting the rigid rod 20 and proceeding with the operations as defined in the preamble to this description.
On reading the description given above, it appears that the aims and advantages of the present invention are well achieved, namely essentially: obtaining an instrument for carrying out an intervention on the hand to treat Dupuytren's disease which is, by compared to the instruments known from the prior art, more rigid and therefore providing more security in operations, and also easier to hold and to handle, thus causing less fatigue and pain in the hand of the practitioner in charge of the operation.
