Parfois les praticiens n'obtiennent pas de contact osseux malgré une certaine profondeur 15 d'injection (souvent chez les patients ayant un axe de branche mandibulaire ouvert) : ils risquent alors d'injecter au-delà de l'espace inter-ptérygoïdien, (dans l'espace parotidien) ce qui peut avoir comme conséquence une paralysie faciale temporaire chez ces patients.
