Description
The invention relates to a gelling gelatin formulation based on ketamine, its use in the treatment of pain, and its method of preparation and administration.
It is characterized by a general anesthetic effect and rapid action. It can be used as a unique anesthetic agent: it is particularly suitable for short-term interventions and, thanks to repeated injections or its use as an intravenous infusion, it also allows prolonged anesthesia for several hours. It can also be used either as an anesthetic inducer before administering other anesthetic agents, or as a potentiator of low-potency anesthetic agents, such as nitrous oxide. It is also used outside the framework of the Marketing Authorization (MA), as an analgesic agent, especially for the treatment of acute pain, especially in pediatrics. Ketamine-based pharmaceutical specialties currently on the market in France are injectable solutions for intravenous or intramuscular administration, which are difficult to access in children. It has now been developed a new formulation to overcome the disadvantages of current pharmaceutical specialties. More particularly, the invention relates to a ketamine formulation, advantageously capable of being sprayed in the form of a liquid spray into the oral cavity and of instantaneously gelling in contact with the mucous membranes of the mouth, and to allow rapid and sustained release of ketamine. Compared to a solution, the presence of a gel texture allows prolonged contact of the ketamine with the surface of the mucous membranes. This formulation offers several advantages: it is first of all easy to apply and makes it possible to increase, on the one hand, the exchange surface in the oral cavity, when it is sprayed in liquid form, and on the other hand increase the time of presence of the active ingredient in the oral cavity by its gel form; it provides access to a better bioavailability of ketamine; it can also be easily prepared and packaged to be kept under sterile conditions, and may not contain preservatives. Finally, this formulation makes it possible to provide first 5 care quickly, safely and economically, especially in emergencies in children. Thus, according to a first aspect, the invention relates to a gelling formulation comprising ketamine or a pharmaceutically acceptable salt thereof, a poloxamer, and water. For the purposes of the present description, the term "gelling formulation" means a formulation capable of gelling, in particular in which the concentration of the poloxamers or poloxamers is sufficient to allow the aqueous solution to gel at a given temperature. especially in contact with the skin, especially the mucous membranes of the oral cavity, in particular the mucous membranes of the tongue and cheeks. Ketamine is the compound of the following structure: According to a preferred embodiment, the gelling formulations according to the present application comprise a pharmaceutically acceptable salt of ketamine, in particular ketamine hydrochloride. Ketamine may be used in its racemic form, or in enantiomerically enriched form. Preferably, the S - (+) enantiomer of ketamine, also referred to as esketamine, or a pharmaceutically acceptable salt thereof is used. The formulations according to the present application preferably comprise from 2 to 20 g / 100 ml of ketamine or a pharmaceutically acceptable salt thereof, more preferably from approximately 2 to 10 g / 100 ml of formulation, and in particular approximately 8 W100. mL of ketamine As used herein, the term "poloxamer" refers to an organized tri-block copolymer comprising or consisting of a polyoxypropylene (also called polypropylene glycol, POP) core chain grafted on both sides by a polyoxyethylene chain (also called polyethylene glycol, POE). The poloxamers are generally designated by the letter "P" (for poloxamer) followed by three digits: the first two digits multiplied by 100 give the molecular weight of the polyoxypropylene core, and the last digit multiplied by 10 gives the percentage of the content. made of polyoxethylene. By way of example, P407 corresponds to a poloxamer whose polyoxypropylene core has a molecular weight of 4000 g / mol and a polyoxyethylene content of 70%. The poloxamers which are useful according to the invention are thermosensitive poloxamers which, depending on their concentration in solution, are in the liquid state at ambient temperature, and especially between + 8 ° C. and + 33 ° C., and in the state of gel to. a temperature greater than or equal to the gelling temperature (Tg), especially under physiological conditions, and in particular between + 30 ° C and + 40 ° C. The gelling temperatures of the poloxamers (Tg) can be determined by conventional methods or are available in reference books such as the Handbook of Pharmaceutical Excipients. More particularly, these poloxamers are present in a sufficient concentration in the aqueous solution of ketamine or a pharmaceutically acceptable salt thereof to allow it to gel when the temperature is greater than or equal to their gelation temperature (Tg), especially in physiological conditions. Preferably, the formulation gels at a temperature of between + 30 ° C. and + 37 ° C., more preferably in contact with the skin or the mucous membranes, in particular at a temperature of between + 31 ° C. and + 34 ° C. The formulations according to the present application generally comprise from 15 to 20 g / 100 ml of poloxamer, preferably from 16 to 19 g / 100 ml. As an example, mention may in particular be made of poloxamer 407, poloxamer 188 or a mixture thereof. The formulations according to the present application may further comprise one or more pharmaceutically acceptable excipients selected from mucoadhesives, flavor masking agents, acidic agents, basic agents, preservatives and sodium chloride or the like. potassium chloride. Examples of muco-adhesive agents that may be mentioned include alginates (for example Keltone, Manugel LBA®, Satialgine®), gellan gums (for example Gelzane), xanthan gums (for example Satiaxanee), gum tragacanth, Karaya gum, carrageenans (eg Satiagel®), pectins, chitosans (eg Chitoclear®), hyaluronic acid and its salts, polymers of acrylic acid (eg Carbopol®, Noveon®), copolymers Methylvinylether and maleic acid (eg Gantreze), polyvinylpyrrolidone and its derivatives (eg Kollidon 90F0), as well as cellulosic derivatives (HPMC, HPC, HEC, CMCNa). Examples of taste-masking agents include low-calorie sweeteners such as sucralose, and stevia and more particularly its extract, 97% rebaudioside A (eg Rebaten 970). Aromas such as tutti frutti, strawberry, raspberry, caramel, cola, citrus (orange, lemon) can also be used to mask the taste of the formulation. Examples of acidic agents useful for adjusting the pH of the solution include hydrochloric acid, acetic acid and boric acid. As examples of basic agent, also useful for adjusting the pH of the solution, mention may be made of solutions of soda and potash. Examples of preservatives that may be mentioned include sorbic acid, boric acid. According to one embodiment, the formulations of the invention comprise about 17 g / 100 ml of poloxamer 407, and between 0.04 and 2 g / 100 ml of mucoadhesive agent such as, for example, sodium alginates, xanthan gums, carrageenans. Advantageously, the formulations according to the invention are sterile and can be prepared by sterilizing filtration, by radio sterilization, or under aseptic conditions. More particularly, this solution can be prepared and / or stored under sterile conditions in a bottle provided with a suitable sprayer. In another aspect, the invention relates to a pharmaceutical composition comprising or consisting of a gelling formulation as defined above. According to yet another aspect, the invention relates to a gelling formulation as defined above for its use in the treatment of pain, intended in particular to be administered in the oral cavity, sublingually. Preferably, the formulation according to the present application is sprayed into the oral cavity, and instantly forms a gel on the application zone, preferably sublingual. According to another aspect, the invention relates to a process for the preparation of a gelling formulation as defined above, said process comprising the steps of: i) cold dissolving the poloxamer in water, especially at a temperature of between + 8 ° C and + 16 ° C; ii) Addition and dissolution of one or more mucoadhesive agents, preferably at a temperature of between + 40 ° C and + 70 ° C; Iii) Addition and dissolution of ketamine or a salt thereof in the solution obtained in step i), preferably at room temperature, especially at a temperature of between + 15 ° C and + 25 ° C ; and iv) recovering the resulting formulation.
