Description
Le système veineux a pour fonction de ramener le sang appauvri en oxygène et riche en toxines, au coeur. Les contraintes de ce retour veineux sont plus importantes au niveau du pied et de la cheville qui supportent tout le poids de la pesanteur de la colonne veineuse +90 mmHg, contre 40 mmHg à mi-cuisse. La conjonction de plusieurs phénomènes assure le retour veineux au niveau des jambes - La présence de valvules qui permettent uniquement le sang de circuler de la surface vers la profondeur, du pied vers le coeur, - Une première impulsion donnée par le réseau veineux du pied, - Une seconde impulsion produite par la contraction des muscles du mollet, - Et enfin un phénomène d'aspiration coeur poumon. Pendant la marche, l'écrasement de la voute plantaire et la contraction musculaire du mollet compriment les veines et propulsent le sang vers le coeur. Au niveau des membres inférieurs, les deux principaux acteurs du retour veineux sont donc la semelle plantaire, les muscles du mollet et de la cuisse qui vont jouer le rôle de pompe d' éj ection. Dans le cadre du traitement de l'insuffisance veineuse des membres inférieurs, la performance éjectionnelle des muscles de la jambe est largement bonifiée par l'application de bas compressif. Cette contention/compression bien encadrée par un cahier des charges précis du LPPR (Liste des prestations et produits remboursables) a démontré une réelle efficacité. Pour être pris en charge, la réglementation du LPPR impose : - un tricotage tramé, - des grosseurs de fils, - des pressions mesurées à la cheville 13 à 17 hpa pour une classe I, - 20.1 à 27 hpa pour une classe II, - une dégressivité de la cheville en haut du mollet ou de la cuisse de 75% pour une classe I, - 70% pour une classe II mais aucune pression n'est exigée pour la partie du pied ! Au niveau de la pompe veineuse du pied, siège des plus hautes pressions, aucune aide significative n'est actuellement apportée, et ce par aucun produit actuel. C'est là une lacune 30 importante Une étude anatomique récente, effectuée par les Docteurs Berthier, Prévoteau et Gillot, a permis d'affirmer que la véritable pompe veineuse plantaire est profonde, intermusculaire, constituée par les veines plantaires. Nous pouvons affirmer que la partie chaussante de tous les produits actuels, sans réelle pression ne peuvent apporter l'aide souhaitée. - 2 - Le réservoir sanguin du pied que l'on peut mobiliser par une compression manuelle forte de la plante du pied ou qui se vidange lors de l'appui du pied au sol à la marche correspond bien aux veines plantaires. Son éjection rapide est responsable du pic de reflux observé au doppler dans les veines tibiales postérieures. Ces données anatomiques expliquent l'importance de la pompe veineuse du pied, première étape du retour veineux, et la nécessité d'un traitement compressif adapté à son rôle. Le premier but de notre brevet est d'améliorer les performances du drainage veineux au niveau du pied en associant selon un cahier des charges précis, des fibres à effet compressif et des fibre à effet contentif ( Fig. 1), pour tricoter en double épaisseur un véritable chausson drainant ( Fig. 1), point de départ de la fabrication de chaussettes, de bas cuisses et de collants de contention. Selon notre invention, ce bas est composé tout d'abord d'un chausson compressif débutant à la racine des orteils enveloppant totalement le pied et remontant jusqu'au tendon d'Achille ( Fig. 2 et Fig. 3), afin d'avoir une action circulaire sur toutes les veines du pied et notamment sur les veines plantaires latérales. Le chausson compressif remonte de chaque coté des malléoles pour encadrer les veines tibiales postérieures et antérieures afin d'assurer une chasse veineuse profonde. The first aim of our patent is to improve the performance of venous drainage at the foot by combining according to precise specifications, compressive fibers and fibers with a protective effect (Fig. 1), to knit in double thickness. a real draining slipper (Fig. 1), starting point for the manufacture of socks, thighs and compression tights. According to our invention, this stocking is composed first of all of a compressive slipper starting at the root of the toes completely wrapping the foot and going up to the Achilles tendon (Fig 2 and Fig 3), in order to have a circular action on all the veins of the foot and in particular on the lateral plantar veins. The compression slipper goes up on each side of the malleolus to frame the posterior and anterior tibial veins to ensure a deep venous hunt.
