TECHNICAL FIELD The invention will find application in the field of maxillary implant surgery in the indication of the immediate placement of dental implants in cases of maxillary sinus filling with a residual sinus bone height of less than 3 mm.
STATE OF THE PRIOR ART Maxillary sinus cavities have long constituted anatomical zones to be preserved during the placement of dental implants in clinical situations of lateral maxillary edentulousness. Numerous techniques and implant systems have been developed to circumvent the maxillary sinuses, thus avoiding their break-in, and to allow the creation of posterior abutments when the residual sinus bone height was insufficient for the implantation of dental implants. The appearance of sinus filling techniques has opened prospects for unexpected treatment at the maxillary level, thus constituting an alternative to these implant systems, represented by pterygoid implants or zygomatic implants; the latter having, moreover, seen their indications diminish appreciably to almost reduce themselves to clinical situations which contraindicate sinus surgeries. Sinus filling techniques benefit from a significant decline, the first publications dating from the 1980s. The large number of publications has, therefore, made it possible to establish a treatment planning flow chart according to the sinus anatomy to be filled, based on literary reviews as well as the experience of surgeons. It is, in this case, established to resort to a surgical technique in two stages when the crestal height is less than 3 mm increase of the sinus floor by lateral approach with delayed implantation. There is therefore the need to provide a dental implant specially designed to allow to propose an operative protocol in a surgical time for the immediate placement of implants during surgical procedures for filling maxillary sinuses with a residual bone height below sinus of less than 3 mm .
DESCRIPTION OF THE INVENTION The drawing, showing a side view of the implant, serves as an example and is not limiting of the invention. The implant is made of titanium without welding or addition. The type of manufacturing material is not limiting because it is also possible to manufacture it in zirconia.
The implant is intended to be placed in a helical motion, preferably in an axial direction, through the sinus sinus crest. The upper part of the coronal trunk-cone (5) has a taper angle of 53 ° over a length of 1 mm, by a smooth surface which must be maintained above the surface of the bone to prevent a possible resorption residual cortex due to the risk of inflammatory reaction at the implant / screw overlap. The diameter of the plate of the head of the implant is therefore greater than that of the body of the implant, which has the advantage of avoiding the possibility of a vertical displacement of the implant towards the sinus cavity in case of loss of primary stability in the early phases of bone healing. The upper plate of this coronal trunk cone may provide an internal connection or an external connection to connect the implant to a wide variety of prosthetic fittings for the realization of supra-implant prostheses.
The anchoring zone of the implant consists of the part (1). The apical trunk-conical portion (2) and the cylindrical middle portion (3) are intended to be in contact with the bone filling biomaterial placed in the sinus cavity to allow the regeneration of neoformed bone tissue ; at the end of the bone healing, the parts (2) and (3) will serve as anchoring in the neoformed bone.
5 10 15 20 25 The lower part of the coronary truncated cone (4) provided with self-tapping micro-turns will make it possible to obtain anchoring and primary stability of the implant in the maxillary bone at the time of its placement. Its slight taper, with a cone angle of 15 ° open in the coronal direction, will exert an increasing pressure of the implant body on the bone tissue during the screwing of the implant and will optimize the stability of the implant in the implant. bone of the sinus crest under sinus. The height of this lower portion of the coronal trunk-cone (4) being 2mm that if exclusively in contact with the bone if the thickness of the bone crest is 2mm. In clinical cases where the residual sinus bone thickness is less than 2 mm, this lower part of the coronary trunca cone (4), which must be completely buried below the bone level, will have its lower part in contact with the filling material placed in place in the sinus cavity, in the space created between the sinus membrane and the lower wall of the sinus bone crest, to leave room for neoformed bone The lower plate of the implant (6) has a diameter slightly lower than the wellbore so that screwing can be initiated without vertical pressure to avoid collapse of the bone ridge which can be fragile because it is very resorbed; only the lateral pressure exerted by the turns on the osseous walls makes it possible to guide the placement of the implant. The apical truncal conical self-drilling portion (2) allows the screwing of the implant at the wellbore. It has an angle of taper of 26.2 ° open in the coronal direction to go from a diameter of 2.2mm, at the diameter of the apical plate of the implant, to a diameter of 5mm, at the level of the cylindrical body of the implant, over a length of 6mm. This low taper allows the walls of the implant to exert progressive pressure on the bone tissue during the screwing process necessary for anchoring and stabilizing the implant. This self-drilling apical truncate conical portion advantageously has 3 equidistant section edges located every 120 °. These allow the evacuation of bone chips and irrigation fluids out of the wellbore. 5 10 15 20 25
