La figure 3 illustre un formulaire 1OC, du type liste, le formulaire comprenant une zone d'en-tête réduite 17 et une zone liste 19 comprenant plusieurs items. Dans l'exemple illustré, la liste comprend onze items (iteml - item 11). Chaque item est avantageusement un objet ou un produit que l'expéditeur 2 souhaite se faire livrer par un destinataire de la liste. Ainsi, le formulaire peut être envoyé à la suite d'un formulaire 10A. A chaque item et à chaque identité est associé un identifiant alphanumérique 21, et des moyens d'identification automatique 22. L'identifiant alpha numérique 21 est destiné à une lecture directe, notamment par l'expéditeur ou le destinataire du message. Les moyens d'identification automatique sont prévus pour une reconnaissance automatique, notamment par le centre de traitement 11. Dans les exemples illustrés, les moyens d'identification automatiques ont la forme d'un code graphique 22. Dans les exemples illustrés, chaque code graphique 22 comprend dix barres 24 disposées verticalement relativement au sens de lecture et parallèlement à la direction D d'introduction des formulaires dans la machine 8. Les barres sont régulièrement espacées entre elles, toutes de même épaisseur E24. Leurs longueurs L24 sont un multiple par 1, 2 ou 3, d'une longueur de base LB24. Les barres d'un même code s'étendent toutes vers le haut du formulaire correspondant, à partir d'une même horizontale, c'est-à-dire à partir d'une même transversale au formulaire, perpendiculaire à la direction D. Ainsi, dans l'exemple illustré, Monsieur Dupont, l'expéditeur 2 est identifié par un code 22 comprenant, de gauche à droite, neuf barres 24 de longueur simple L24 = 1 x LB24 et d'une dixième barre de longueur triple io L24 = 3 x LB24 et peut être illustré numériquement par la suite de chiffre 1111111113. De même, sur le formulaire liste 10C, le code représentant l'item iteml l comprend, de gauche à droite, une barre de longueur triple, six barres de longueur simple, une barre de longueur double L24 = 2 x LB24, une barre de longueur simple, et, finalement, une barre de longueur triple, et peut, par 15 exemple, être illustré par la suite de chiffres 311111213. Devant chaque identifiant alphanumérique le formulaire comprend des moyens 26 pour activer le code 22. Dans l'exemple illustré, les moyens d'activation 26 sont des cases 26. Ainsi, lorsqu'une case 26 est cochée ou noircie par l'expéditeur 2, le code 22 correspondant est considéré actif par le 20 centre de traitement 11, lorsqu'il reçoit le formulaire correspondant. Dans l'exemple illustré à la figure 2, seule la case 26 de la ligne 2 du formulaire 10A est noircie. Ainsi, lorsque le centre de traitement reçoit le message, il détermine que le message correspondant doit être automatiquement retransmis à contact2, uniquement, et qu'il ne nécessite pas de traitement 25 manuel. Lorsque le code 22 est décodé par le centre de traitement, il permet d'accéder directement aux coordonnées de contact2 dans la base 13, afin que le message lui soit retransmis au travers du réseau distal 14, selon un mode de retransmission choisi par contact2. De cette façon, l'expéditeur n'a pas à connaître les coordonnées ni le mode de retransmission de contact2. En outre, un changement de coordonnées de contact2 peut être enregistré dans la base 13 et rester totalement transparent pour l'expéditeur 2. Dans l'exemple illustré à la figure 3, seule la case 26 correspondant à l'item iteml est noircie. Ainsi, lorsque le centre de traitement 11 reçoit le message, il détermine que dans la liste (item 1 - itemll), seul l'item item 1 est demandé par l'expéditeur 2. Par exemple, si l'item iteml est un médicament, la base de données 13 peut prévoir que le message soit retransmis à une pharmacie 6 prédéterminée, par exemple une pharmacie qui possède une copie des ordonnances au nom de l'expéditeur 2, sans que l'expéditeur 2 ait à lo mentionner la pharmacie en tant que destinataire du message. Dans ce cas, le formulaire 10C peut être envoyé seul, sans un formulaire d'en-tête 10A. Ainsi, un formulaire selon l'invention permet d'adresser des messages de façon simplifiée, au travers d'un système de communication tel que décrit à la figure 1. Il suffit que l'expéditeur connaisse les seules coordonnées du centre 15 de traitement 11. De façon plus perfectionnée, les coordonnées du centre de traitement 11 peuvent être préenregistrées dans la machine d'envoi 8, de sorte que, par exemple, l'appui sur un bouton d'envoi dédié permette l'adressage automatique du message au centre 11. Les codes d'identification 22, autres que celui de l'expéditeur 2 20 peuvent être spécifiques en eux-mêmes. Ainsi, le code 22 de l'item itemll (311111213) peut en lui-même représenter la commande d'une baguette de pain, quel que soit l'expéditeur du formulaire 10C. Au contraire, les codes d'identification 22, autres que celui de l'expéditeur 2, peuvent n'être spécifiques que par association à celui de 25 l'expéditeur. Ainsi, le code 22 de l'item itemll (311111213) peut représenter la commande d'une baguette de pain pour l'expéditeur 2, et la commande d'une plaquette de beurre pour un autre expéditeur.
