Description
The present invention relates to an osteosynthesis clip of hyperelastic or shape memory material, particularly intended for corrective surgery of the hallux valgus. It can however be used for any other osteosynthesis technique for which it would prove effective. The invention also relates to a drilling guide that can be used for the installation of this clip. To maintain the position of two bone parts after an osteotomy, especially during a corrective surgery hallux valgus, it is well known to use hyperelastic staples or shape memory. Such a clip comprises two lateral branches and a central branch connecting these lateral branches, each lateral branch being intended to be inserted into one of the two parts of bone. A known staple comprises, at the level of its central branch, a eyelet passing, by shape memory, from an elongate shape before implantation to a contracted shape after implantation, so that the two lateral branches of the staple are brought closer together. one of the other. The latter also pivot slightly after implantation, so that their free ends converge towards each other, realizing a certain compression of the interfaces of the two bone parts generated by the osteotomy. This staple has the disadvantage of not being perfectly applied against the bone parts after implantation and thus to have a troublesome size for the surrounding soft tissues. In addition, the pressure exerted by the pivoting of the lateral branches on the cortical of the two parts of bone generates a tendency of this clip to expel. In addition and above all, this staple only imperfectly maintains the two parts of bone, particularly with respect to the pivoting of one portion of bone relative to the other in the longitudinal direction, and the bone interfaces are not not perfectly maintained in application against each other. Another known staple comprises a central branch "S" or broken line, whose inflections are accentuated after implantation, by shape memory, also achieving a rapprochement of the two lateral branches of the staple.
The disadvantages of this clip are substantially the same as those of the previous clip, with the addition of a deformation of the central branch that may include some twisting, which can cause a defective positioning of the two parts of bone relative to the other.
To put one or other of these staples, the practitioner must perform the osteotomy and osteosynthesis of the two parts of bone, which is not very easy since the distal fragment is unstable and the Drilling guide used to prepare the insertion holes of the lateral branches of the staples should be applied to these unstable fragments. As a result, this drilling guide is often poorly centered relative to the bone parts, forcing the practitioner and his assistant to use either forceps or temporary fixing pins, thus inducing additional gestures and a congestion of the operating site, and creating a risk of poor positioning of the two parts of bone relative to each other.
The present invention aims to remedy all of these disadvantages. The staple it concerns comprises, in known manner, a central branch and two lateral branches connected to the central branch, these two lateral branches being intended to be each inserted into one of the two parts of bone to be joined. ; the central branch is shaped to be able to adopt a first form, in which the two lateral branches are distant from each other, and a second form, in which the two lateral branches are brought closer to each other, and these lateral branches are substantially parallel to each other in said first form and converge towards each other towards their free ends in said second form. According to the invention, the central branch has, in said first form, a substantially rectilinear shape, and in said second form, an arcuate or angular shape, such that this central branch forms a concave curvature on the side opposite to that on which extend the lateral branches, or a "V" opening on the side opposite to that on which these lateral branches extend, the curvature or the angulation of the central branch in said second form being such that this central branch is extends, after implantation, near the cortical bone parts in which this staple is implanted.
Thanks to the aforementioned arcuate shape or angulation of the central branch after implantation, and also thanks to the aforementioned pivoting of the lateral branches, the clip according to the invention makes it possible to compress the zones of the bone interfaces close to this central branch. while having a low tendency to expulsion. In addition, the arcuate or angulated shape of the central branch of the staple corresponds substantially to the angulation of the two parts of bone to be joined, so that this staple projects little beyond the cortex of the bone parts. and does not particularly conflict with the surrounding soft tissues.
Preferably, the central branch comprises two holes formed through it, on either side of its central zone, each intended to receive a fixing screw of the staple to the corresponding bone part. These screws make it possible to perfectly secure the fastening of the staple to the bone parts. In addition, when the staple is shape memory, the aforementioned pivoting of the lateral branches is carried out after placing the screws and allows these lateral branches to come to press against the fixing screws in the direction tending to bring the free ends of these screws from one another, which makes it possible to compress the zones of the bone interfaces remote from the central branch of the staple. The staple thus makes it possible to ensure perfect compression of the bone interfaces, which is an essential condition for the successful completion of the osteosynthesis. The drilling guide that can be used for the installation of this clip comprises, according to the invention, a first block comprising a first drilling guide hole and a second block comprising a second drilling guide hole, these two blocks being able to slide one relative to each other in the direction of the approximation or removal of said holes. This drilling guide thus makes it possible to pierce the holes in the bone before performing the osteotomy, so simply and easily, in stable bone parts. Preferably, the sliding means of the two blocks are such that each of the two blocks can come into contact with the other block at interface transverse faces that comprise the blocks, and - on each block, the distance separating the block piercing guide hole and the corresponding interface transverse face is equal to the distance separating each lateral branch of the staple and the central zone of the central branch of this staple.
The drilling guide according to the invention thus makes it possible, by means of said transverse interface faces, to locate on the proximal cortical the width of the osteotomy to be produced, and to arrange in the bone the receiving holes of the lateral branches. of the staple, at the proper distance to the bone interfaces that will result from the osteotomy.
Perfect positioning of the receiving holes of the lateral branches of the staple is thus obtained regardless of the width of the osteotomy to be performed at the proximal cortical level. In practice, the two blocks are spaced apart from one another according to the proximal width of the osteotomy to be produced and the piercing guide is applied against the proximal cortical bone; the width of the osteotomy to be produced is then identified at the right of said interface transverse faces, then the receiving holes of the lateral branches of the staple are pierced. The piercing guide is then removed and the osteotomy is performed. After pivoting two bone parts thus formed, the staple is put in place by engagement of its lateral branches in the above-mentioned receiving holes, the central transverse zone of the central branch coming to be positioned exactly in line with the interfaces of FIG. osteotomy. The staple then takes said second form, effecting the compression of the proximal zones of the bone interfaces by the accentuated curvature or the accentuated angulation of its central branch and by the pivoting of its lateral branches, and realizing the compression of the zones. distal bone interfaces. When the clip includes fixing screws, these fixing screws are put in place after implantation of the clip; the support of the lateral branches against the screws following the pivoting of these lateral branches promotes the compression of the distal zones of the bone interfaces and the maintenance of the screws in the bone. Preferably, the faces of the piercing guide intended to come into contact with the bone have a concave shape enabling them to adapt to the convex shape of the bone, which facilitates the positioning of this guide on this bone.

For simplicity, parts or elements of an embodiment that are identical or similar in another embodiment will be identified by the same reference numerals and will not be described again. Figures 1 to 8 show an osteosynthesis clip 1 particularly intended for corrective surgery hallux valgus. The correction of a hallux valgus consists, as appears in FIGS. 10 to 12, in performing an osteotomy 101 on a bone 100 (see FIG. 10), then in performing an osteosynthesis of the two parts of the bone 100 a , 100b thus generated, by keeping these two parts of bone in the angular correction position (see FIGS. 11 and 12). The two bone parts 100a, 100b are maintained in this correction position by means of the clip 1.
FIGS. 1 to 3 show a clip 1 made of a hyperelastic material, comprising a central branch 1a and two lateral branches 1b connected to the central branch 1a, each lateral branch 1b being intended to be inserted into one of the two parts of the bone 100a, 100b to assemble.
The clip 1 is deformable between a normal non-deformation state, shown in FIGS. 1 and 2, and an elastic deformation state, shown in FIG. 3. In its non-deformation state, the central branch 1a of the staple 1 forms an open "V", opening on the side opposite to that on which the lateral branches 1b extend, and the lateral branches 1b of the staple 1 converge towards each other towards their ends free. In its state of elastic deformation, the staple 1 is brought into a shape in which its central branch is rectilinear or forms a "V" much more open than in the normal state of no deformation, so that the two branches lateral are more distant from each other than in the state of no deformation, in a direction parallel to the length of the central branch la. In this same state of elastic deformation, the lateral branches 1b of the staple 1 are substantially parallel to each other. The staple 1 is brought into this state of elastic deformation at the time of its implantation, using a suitable instrument. Figures 4 to 6 show a clip 1 very similar to that shown in Figures 1 to 3, except that the central leg, instead of forming a "V", has an arcuate shape with its concave curvature turned the opposite side to the one on which the side branches 1b extend.
Figures 7 and 8 show a staple 1 of shape memory material, in particular nickel-titanium alloy known under the name "nitinol". This clip 1 has, at the time of its introduction, the form shown in Figure 7, wherein its central branch is straight or forms a "V" very open and in which its side branches lb are parallel; the staple 1 adopts, after implantation, by shape memory, the form shown in FIG. 8, in which its central branch forms a closed "V", bringing the lateral branches 1b close together, and in which its lateral branches 1b converge towards each other towards their free ends.
The clip 1 further comprises two holes 2 arranged through its central branch 1a, for the establishment of screws 5 for fixing the clip 1 to the bone 100, visible in FIG. 12. FIG. a piercing guide 10 that can be used for laying the staple 1. This guide 10 is formed by two blocks 11, 12, connected to one another, delimiting transverse interface faces 13 and coming faces 14 in contact with the bone. The block 11 comprises a hole 15 for piercing opening into the face 14 of this block, and the block 12 comprises a hole 16 for drilling guide also opening in the face 14 of this block. On each block 11, 12, the distance separating the hole 15, 16 and the corresponding interface transverse face 13 is equal to the distance separating each lateral branch 1b of the staple 1 and the central zone of the central branch 1a of this clip. The two blocks 11, 12 are slidably connected to one another so that the two blocks can slide relative to one another in the direction of the approaching or the mutual distance of the holes. 15 and 16. This sliding can be achieved by any appropriate sliding means, in particular in the form of pads integral with one of the blocks, slidably engaged in sliding grooves arranged in the other block. These sliding means are such that each of the two blocks 11, 12 can come into contact with the other block at said interface transverse faces 13. The faces 14 have a concave shape enabling them to adapt to the convex shape of the bone 100.
In addition, the block 11 is secured to a gripping rod 17, facilitating the handling of the guide 10. For the implementation of a shape memory clip as shown in Figures 7 and 8, as shown on FIGS. 10 to 12, the two blocks 11, 12 are spaced apart from one another according to the proximal width of the osteotomy 101 to be produced (a graduation arranged on one of the blocks and a mark on the other block may be provided to accurately measure the spacing of the two blocks); the spacing of the faces 13 is then a benchmark of the width of the osteotomy 101 to achieve; the piercing guide 10 is then applied against the proximal cortex of the bone 100 and landmarks are arranged on this cortical to the right of the faces 13; the holes 102 for receiving the lateral branches 1b of the staple 1 are then drilled, using the holes 15 and 16 as a drill guide (FIG. 10). The guide 10 is then removed and the osteotomy 101 is performed. After pivoting two bone portions 100a, 100b thus formed, the staple 1 is placed on the bone 100 by engagement of its lateral branches 1b in the holes 102, the median transverse zone of the central limb 1 coming from position precisely perpendicular to the osteotomy interfaces 103 (Figure II). The fixing screws 5 are then put in place, after possible drilling 10 pre-holes for receiving them. The staple 1 then resumes, by shape memory, its second form shown in FIG. 12, effecting the compression of the proximal zones of the osteotomy interfaces 103 by the accentuated angulation of the central branch 1a of the staple, which brings the lateral branches lb closer to one another and by the pivoting of these lateral branches 1b so that they converge towards their free end. This pivoting allows these side branches lb to come to press against the screws 5 in the direction tending to bring the free ends of these screws to one another, which makes it possible to compress the distal zones of the interfaces 20 The invention thus provides an osteosynthesis clip of hyperelastic or shape memory material, particularly intended for corrective surgery of the hallux valgus, and a piercing guide for the application of this staple, which remedy in a decisive way to the disadvantages of the homologous equipment of the prior art. The invention has been described above with reference to embodiments given purely by way of examples. It goes without saying that it is not limited to these embodiments but that it extends to all other embodiments covered by the appended claims.
