Abstract
Description
CORSET ORTHOPEDIQUE ET PROCEDE DE REALISATION D'UN TEL CORSET ORTHOPEDIQUE ORTHOPEDIC CORSET AND METHOD FOR PRODUCING SUCH AN ORTHOPEDIC CORSET
The invention relates to a method for producing an orthopedic brace on molding or on measurements for supporting and maintaining a patient in a given position. More specifically, the invention relates to a method for producing an orthopedic brace that can adapt to the changes and specific needs of the patient, particularly from the point of view of the morphology, the chosen angulation, and the desired position. The method according to the invention allows in particular to make a seat-brace to position and maintain different body segments, such as thighs, hips and trunk so as to maintain the patient in a sitting position. Similarly, the method according to the invention allows for a so-called vertical orthopedic brace, allowing the rectilinear upright position of all or part of the body segments and in particular the feet, the legs, the pelvis, the trunk, etc. to be maintained. The invention also relates to such orthopedic corsets. The use of a corset by a patient is recommended in the case where the patient is not able by itself to support and maintain his trunk in a given position. For example, a seat corset is used to hold a person with a static, neurological, muscular, genetic, or other disabling condition in a seated position. The seat corset then allows stability in a sitting position and support of the spine. It must also prevent the risk of orthopedic deformities, such as joint deformities of the spine, pelvis and hips. Currently, the realization of a corset, seat or other, consists of successively molding on a mold representing the shape of the patient in the desired position, a comfort foam thermoformable polyethylene, then a rigid plastic shell thermoformable polyethylene also. The shell obtained after demolding is monolithic. Alternatively, it is possible to inject flexible foamed polyurethane foam between the mold representing the shape of the patient in the desired position and a rigid polyethylene shell thermoformed beforehand on the patient's mold, to obtain a seat corset or a comfort stand up. In any case, the orthopedic brace obtained can not be adapted to the specific needs of the patient if they change between the moment when the mold on the patient is made and the delivery of the orthopedic brace. Similarly, in the case where modifications, for example following analyzes of differential needs of the patient, take place, it is necessary to proceed to the realization of a new complete seat. Finally it is practically impossible to inject or to dig mosses of different densities.
In the invention, it is sought to provide a novel method of producing an orthopedic brace for supporting and maintaining in a selected position of the trunk of a patient, and which does not have all or some of the disadvantages mentioned above. . For this, according to the invention, it is proposed to make the corset from a block of comfort foam, not obtained by molding, but instead dug strictly to the shape or measurements of the patient. The comfort foam used may be more or less flexible depending on the final destination of the corset. Similarly, according to the invention, it is possible to use several different foam blocks, especially from the point of view of density, in order to adapt the densities and therefore the rigidity and / or the flexibility of the foam, to the needs local specific to each part of the patient's body and according to the position to support and maintain. The foam blocks can be regrooved, added, removed according to the specific needs of the patient. Furthermore, according to the invention, the external structure of the corset is composed of different modules that are adjustably fixed with each other, in order to be able to adapt the rigid structure of the corset to the specificities of the foam structure at the time of conception, but also according to the needs of the patient which may vary over time, for medical reasons and / or physiological particular. Thus, the orthopedic brace according to the invention has great adaptability to the morphological changes of the patient as the changes in the prescription of the doctor at the time or after completion of the corset. In the case of a seat corset, this adaptability continues with the possibility of making a seat corset for two different seating positions namely the classic sitting position and the tailoring seat position, by means of two different interchangeable seat cushions for the same structure. The subject of the invention is therefore a method for producing an orthopedic brace, for supporting and maintaining in a given position at least one body segment of a patient, characterized in that it comprises the following steps: a) casting and / or morphological measurements of the at least one body segment of the patient in the desired position; b-modeling the shape of the at least one body segment of the patient in said position; c-program a machining device capable of digging the foam, according to the modeling of step b; at least one block of foam is hollowed out with the aid of the machining device according to the virtual shape of the at least one body segment of the patient, so as to form an internal shell conforming to the external contours of the at least one segment; body of the patient; an outer shell is mounted around the inner foam shell so as to form a reinforcing structure, said outer shell comprising a main main reinforcement part, and lateral side reinforcing side parts, said main and secondary parts being assembled so as to form a reinforcing structure; scalable. By body segment, is meant any segment to be maintained in a specific position for medical reasons and / or comfort in particular. Thus, the segments concerned may be the thighs, the hips, the trunk, the feet, the pelvis, etc. of a patient. Modeling, for example numerically, of the shape of the patient makes it possible to program the machining device, such as a digital milling machine, so as to take into account the exact data relating to the morphology of the patient and to the position that one wish to make him take. Modeling of the shape of the patient can be done by any known means, in particular by using computer modeling programs and in particular CAD / CAM, so as to obtain a virtual mold of the patient from which the various parts of foam constituting the catheter will be hollowed out. envelope of the corset.
Advantageously, it is possible to machine different densities of foam, in order to locally provide more or less flexible support zones, depending on the part of the body concerned. For example, it is possible to use a polyethylene foam density 28 kg / m3 for sensitive areas or strong support, and a polyurethane foam density 33 kg / m3 for the holding zones where said foam should not to be crushed. The outer shell forms a modular reinforcing shell constructed to accommodate the outer volume of the inner foam shell.
Preferably, this structure is composed of thermoformed plastic parts. In particular, in the case of a seat corset, the first part may be a flat plate whose folding or articulation by hinges can constitute respectively the seat and backrest reinforcements of the orthopedic corset according to the invention. Angles, or lateral cheeks, intended to support the inner foam shell on the sides at the level of the thorax and thighs, are then thermoformed, for example under vacuum, in order to create in addition to the angle of the ribs in the material . These ribs strengthen the structural rigidity while reducing the final weight of the device. The different parts of the outer shell are then assembled, either by riveting or by slide systems. Advantageously, it is possible to leave moving parts for instant adjustments, such as the adjustable inclination of the back or the width adjustment of the seat volume. Furthermore, all the additions provided for in the nomenclature of orthopedic seat corsets are adaptable, in particular the headrests, the leg segments, the pedal segments, the tablets, etc. In the case where the orthopedic corset according to the invention is a seat corset intended to support and maintain the trunk and the hips in a seated position, the parts of the outer shell composing the seat and the backrest can be joined by lateral struts of junction forming a reinforcement of the seat structure. According to the invention, the orthopedic brace can support and maintain at least one body segment in the upright position. By straight position is meant a position in which patient's body segments are held rectilinear. For example, according to the invention, one can maintain the rectilinear trunk, so that the back and the hips extend longitudinally. This upright position allows the vertical, inclined or horizontal position of the patient to be maintained. In this case, the main structure of the outer shell of the corset may comprise, in addition to the backrest reinforcement portion, two parts each forming a reinforcement of a thigh. The reinforcing portions of the thighs may be integral with the backrest reinforcing portion, or conversely may be adjustably attached to said backrest reinforcing portion. Otherwise, the corset according to the invention can help maintain any body segment of a patient, including legs, arms etc., in the upright position. Advantageously, according to the invention, it is possible to provide all or part of the following additional steps: g-again hollow at least one block of foam of the inner shell, after a first fitting on the patient, so as to take into account variations between the modeling of the at least one body segment of the patient and the at least one body segment of said patient; h- adding at least one additional block of foam to the inner shell, after a first fitting on the patient, so as to take account of variations between the modeling of the at least one body segment of the patient and the at least one body segment of said patient, said additional foam block having been dug taking into account these variations; i-one adapts the inner shell, by repositioning, additional digging and / or adding block (s) of foam, so as to vary at least one anatomical angulation. Thus, according to the invention, it is possible to validate the accuracy of the molding or measurements by fitting on the patient. The required adjustments are then made by digging or adding blocks of foam. It is also possible to vary the anatomical angulations, for example the flexion of the hips or adduction of the thighs, by repositioning or reworking parts of foam. Once excavated, the foam block (s) are covered with one or more protective covers. Such a cover may be in particular waterproof textile and / or allowing good ventilation of the foam, so as to facilitate the maintenance of the orthopedic corset; k- at least one block of foam forming the seating of the orthopedic corset forming seat, so as to allow a seated sitting position of the patient. Thus, it is possible to easily vary the sitting position of the seat corset, between a conventional sitting position, in which the legs hang down from the trunk and a tailor sitting position in which the legs are brought back and maintained at the height of the seat. basin. It suffices for this to invert the foam blocks forming the seat of the inner shell of the seat corset according to the invention. The shape of the foam blocks forming the seat is adapted to the particular positioning of the legs, and the support hands, support angles of the foam blocks, in varying numbers are arranged according to the desired therapeutic goals.
Finally, once the adjustments and adjustments on the patient made, the outer surfaces of the inner foam shell can be cut to refine the overall shape of the seat and obtain posterior, lateral and / or lower flat surfaces. The outer shell, modular, is built to fit the outer volume of the inner foam shell. This modular outer shell is composed of different parts, preferably thermoformed plastic material. The different parts constituting the outer shell are assembled, by riveting, slide systems or any other means allowing to leave moving parts for instantaneous adjustments such as the adjustable back tilt or the width adjustment of the seat volume. The invention also relates to an orthopedic brace for supporting and maintaining in a given position at least one body segment of a patient comprising: - an inner foam shell intended to conform to the morphological contours of at least one segment body of a patient, the inner shell having at least one block of foam dug according to the morphological contours of the at least one body segment of the patient; an outer shell comprising a main part forming a backrest reinforcement, and side reinforcing side portions, said main and secondary parts being assembled in a flexible manner.
Modular means that the different parts of the outer shell are assembled using any means allowing all or parts of said parts to remain movable relative to each other, for subsequent adjustments. Thus, the different parts of the outer shell can be fixed by riveting, slide systems etc. Such attachment allows in particular the adjustment of the inclination of the back, or the width adjustment of the seat volume. The orthopedic corset according to the invention may be a seat corset, advantageously provided with instantaneous adjustment devices. In this case, preferably, the inner shell comprises a seat portion, and the main part of the outer shell comprises a seat reinforcement structure, so as to support and maintain the trunk in a sitting position. Advantageously, the outer shell may be adjusted in width, so as to adapt to the width of the backrest and / or the seat of the inner shell. Preferably, the inner shell comprises at least two foam blocks of different densities, so as to locally adapt the stiffness of the corset depending on the body parts of the patient to support and specific needs.
The invention will be better understood on reading the description which follows and on examining the figures which accompany it. These are presented as an indication and in no way limitative of the invention. The figures represent: FIG. 1: a schematic representation of an internal foam shell, for an orthopedic corset forming a seat according to an exemplary embodiment of the invention, excavated according to the morphological contour of a patient; - Figure 2: a schematic representation of an embodiment of an outer shell for an orthopedic corset forming seat according to the invention; - Figure 3: a schematic representation of an embodiment of an orthopedic corset forming seat according to the invention; - Figure 4: a rear view of the orthopedic corset forming seat of Figure 3. The embodiment described here, with the help of Figures 1 to 4, relates to an orthopedic corset forming seat made for a patient with strong deformations due to a pathology of cerebral palsy. More precisely, the patient presents a strong right dorsal gibbosity with a rotation of the trunk characterized by an antépulsion of the left shoulder. The basin is rotated and tilted to the right. The right femoral flexion is limited to 70 degrees, the left to 85 degrees. There is also a very prominent sacrum presenting a risk of eschar. The patient therefore needs a seat corset allowing the hips to be maintained in strong abduction and taking into account the important body deformities, notably the gibbosity and the thoracic rotation.
The seat corset 1 according to the invention, as shown in its version mounted in Figure 3, is custom made for the patient, depending on its morphology, stature and deficiencies to correct. In a first step, the patient's trunk is molded. For example, in the case of strong deformations, the molding can be performed using a seat positioning ball bag completed by a 3D body survey of the trunk using a digital beam scanner laser. The laser views are then assembled to virtually reconstitute the volume and shapes of the patient.
The molding is then worked to provide the desired orthopedic corrections, adjust the hip flexions according to the angles measured at the time of molding, slightly increase the volume of the gibbosities or growths to create areas of relief support in the shell, reduce the volume of certain areas of the molding, for example under gibbositaire, in order to create areas of corrective or support support in the shell, correct the position of the patient relative to the vertical, smooth the final shape, etc. This step can be carried out for example by means of a sculpting software connected to a sensory arm with force feedback, which allows the manipulator to feel in his hand all the reliefs of the molding and to transform it using tools held they also virtually in hand and whose action we feel and dose depending on the feel. Once the model is completed, for example, using software, the positioning of the molding in the digital milling machine is determined. The digital milling machine digs the patient's shape 3 into a block of foam 2 (Figure 1). Otherwise, as shown in Figure 3, several foam blocks 4, 5 can be used, having different densities, to adapt to the requirements of orthopedic maintenance and comfort of the patient. Thus, in FIG. 3, the foam blocks forming the inner shell consist of a first block of foam 5 used for the seat part, of polyurethane foam, density 28 kg / m 3, and of a second block of foam 4 used for the dorsal part, in polyethylene, density 33kg / m3. In general, given the large volume of a seat and the need to limit material losses, it is preferable to slice the model, to mill them separately in moderately sized foam blocks, and then to gather and glue together at the end of the digital milling machine. The first version of the inner foam shell of the seat 1 corset can then be tested on the patient.
Preferably, in order to be able after fitting on the patient, to make the necessary adjustments, the first version of the inner shell is made on the basis of lengths of dimension increased compared to those initially recorded on the patient. Thus, in the case for example where the seat is too long and / or supports axillary too high, it is possible to make adjustments by cutting on the spot surplus material. Conversely, in the case of monolithic shells, such readjustments require complete remolding of the inner shell. According to the invention, on the contrary, it is sufficient to rework the block or blocks of foam so as to dig deeper, redécouper, add pieces of foam etc, in order to adjust exactly to the morphology of the patient and to the position one wishes to maintain. Thus, it is possible to readjust the inner shell so as to adapt the position according to the orthopedic needs. For example, in the case where the maintenance obtained is too straight, it is possible to open the angle of the hips by a simple incision in the foam at said hips. Since the change in positioning may alter the location of the patient's rib hump and underlying concavity relative to the seat, it is then possible to regress the foam and / or add pieces of foam that the we stick to the main blocks of the inner shell. The finishing work is then carried out. The foam can be resized by the outside with a hot wire, to refine the volume and aesthetics.
The outer shell 6 (FIG. 2) of thermoformed plastic is shaped and fitted on the inner shell 2. More precisely, as can be seen in particular in FIG. 2, the outer shell 6 comprises a main part 7 forming a backrest reinforcement 8 and reinforcement seat 9, the two reinforcements 8, 9 being fixed to each other by hinges 10, possibly adjustable. The outer shell 6 also has upper lateral reinforcements 11 and lower lateral reinforcements 12. The upper lateral reinforcements 11 are mounted in an adjustable or fixed manner (in FIG. 2, the two possibilities are shown on the left and on the right, respectively). in said FIG. 2) on the backrest reinforcement 8 of the main part 7. Similarly, the lower lateral reinforcements 12 are mounted in an adjustable or fixed manner (respectively to the left and to the right in FIG. 2) on the seating reinforcement 8 of the main part 7. The covers intended to surround the foam blocks 4, 5 of the inner shell 2 are then chosen in order to increase the wear resistance of said foams, to facilitate maintenance and to increase hygiene. of the seat corset 1, and improve the overall aesthetic of said seat corset 1. It is still possible, after the achievement of these purposes, to easily modify the corset seat 1, in particular to take into account the specific environmental constraints that the patient faces, or according to its growth. Thus, in the case where the patient must be able to use devices with restricted fixed dimensions, such as an elevator, it is possible to specifically modify the seat corset 1 to adapt to environmental constraints while maintaining the orthopedic requirements. It is thus possible to reduce the angle of separation of the hips, by simply cutting and gluing in the foam of the seat portion 4, in order to tighten the legs, to move the lateral reinforcing cheeks of seat, so to follow the change of shape of the foam shell, and to fix them again by riveting or screwing on the structure, to adapt it to the new configuration. Finally, the cover is taken again to adjust it to the new dimensions of the inner shell 2. Moreover, as can be seen in FIG. 4, the outer shell 6 is adjustable so as to adjust as closely as possible to the dimensions of the shell. inner shell 2. Thus, it is possible to adjust, by a sliding system or the like, the width of the backrest, bringing closer or at least one of the two upper lateral reinforcements 11 on the part of the outer shell 6 forming a backrest reinforcement 8. Similarly, it is possible to adjust the width of the seat, bringing closer or away at least one of the two lower lateral reinforcements 12 on the part of the outer shell 6 forming a seat reinforcement 9. The seat corset 1 according to the the invention can be mounted on any structure usually used with a seat corset, and be completed by any commonly used apparatus, such as a headrest, a tablet etc.15
