Abstract
Description
The present invention relates to a range of intramedullary humeral nails. Fractures of the humerus, whether due to patient age or trauma, are frequent accidents that must be treated in the most efficient manner. To do this, several treatments are available depending on the importance of the fracture and the degree of bone degradation. If the neck of the humerus and the head of the humerus are not affected, the placement of a total prosthesis can be avoided. The purpose of the treatment is then simply to re-bond the different bone fragments to the body of the humerus by means of a suitable intramedullary fixation assembly. Such a fixation assembly comprises, in known manner, on the one hand an intramedullary nail intended to be impacted in the medullary canal of the humerus and comprising a plurality of through-holes formed in its proximal and distal portions, and on the other A plurality of fastening screws are provided for engaging the through holes in the nail to immobilize the nail and the various bone fragments relative to the body of the humerus. There are so-called short intramedullary nails for the treatment of proximal humeral fractures, and so-called long intramedullary nails for the treatment of medial humeral fractures. In order to position these two types of intramedullary nails, it is known to use two types of anchoring devices, namely a first adapted for placing intramedullary nails called short and a second adapted for placing so-called intramedullary nails. . The use of two different anchoring tools makes the surgical procedure more complex. The present invention aims to remedy these disadvantages. The technical problem underlying the invention therefore consists in providing a range of humeral intramedullary nails which makes it possible to simplify the surgical procedure. To this end, the invention relates to a range of intramedullary humeral nails of the type comprising a proximal portion, a distal portion, and a plurality of through holes formed in the proximal portion and intended for the passage of fixing screws, the various through holes 35. extending substantially perpendicular to the axis of the intramedullary nail and being longitudinally offset from one another, the range comprising: - a first series of humeral intramedullary nails of different sizes for the treatment of proximal humeral fractures, - a second series of humeral intramedullary nails of different sizes for the treatment of medial humeral fractures, characterized in that each intramedullary nail belonging to the first series has at least four through holes formed in its proximal portion, in that each intramedullary nail belonging to the second It has at least three through-holes in its proximal portion, in that the distance between the proximal end of an intramedullary nail and the nearest through-hole is substantially the same for all the nails belonging to the first and second series, in that the orientation of the through hole closest to the proximal end of an intramedullary nail is substantially identical for all nails belonging to the first and second series, and in that the third through hole from the proximal end of each intramedullary nail belonging to the first series and the second through hole from the proximal end of each intramedullary nail belonging to the second series are located in an orientation and a distance from the proximal end of the corresponding intramedullary nail respectively substantially identical. These arrangements allow the positioning of the intramedullary nails belonging to the first and second series with a single 25 ancillary pose, which facilitates the surgical procedure. Advantageously, the distance separating the proximal end of an intramedullary nail and the through hole closest to the latter is substantially equal to 10 mm for all the nails belonging to the first and second series. According to one embodiment of the invention, the third hole from the proximal end of each intramedullary nail belonging to the first series and the second hole from the proximal end of each intramedullary nail belonging to the second series. extend substantially perpendicular to the axis of the through-hole closest to the proximal end of the corresponding intramedullary nail.
Preferably, the through hole closest to the proximal end of an intramedullary nail is located substantially in the anteroposterior plane of the latter for all the nails belonging to the first and second series.
Advantageously, for each intramedullary nail belonging to the first series, the second through hole, the third through hole and the fourth through hole from the proximal end of an intramedullary nail are oriented respectively at approximately 30 °, 90 ° and - 30 ° relative to the through hole closest to the proximal end of said intramedullary nail. Preferably, the second through hole, the third through hole and the fourth through hole from the proximal end of an intramedullary nail are oriented respectively at 30 °, 90 ° and -30 ° clockwise with respect to the hole. passing through said proximal end for a nail to be implanted on a left humerus, and the second through hole, the third through hole and the fourth through hole from the proximal end of an intramedullary nail are respectively oriented substantially at 30 °, 90 ° and -30 ° counterclockwise relative to the nearest through hole of said proximal end for a nail to be implanted on a left humerus. According to one embodiment, the adjacent through holes in the proximal portion of each intramedullary nail belonging to the first series have a substantially identical for all intramedullary nails belonging to the first series, the spacing is preferably about 5 mm. Advantageously, the length of each intramedullary nail belonging to the first series is between 110 and 170 mm, and preferably about 150 mm. Advantageously, intramedullary nails belonging to the first series are substantially cylindrical and have diameters of between 6 mm and 11 mm and varying in steps of 1 mm between each intramedullary nail size. Preferably, for each intramedullary nail belonging to the second series, the second through hole and the third through hole from the proximal end of an intramedullary nail are respectively substantially perpendicular and parallel to the nearest through hole of the proximal end of said intramedullary nail. Advantageously, the fourth through hole from the proximal end of each intramedullary nail belonging to the first set and the third through hole from the proximal end of each intramedullary nail belonging to the second set are located at a distance from each other. at the proximal end of the corresponding intramedullary nail respectively substantially identical. According to one embodiment, the length of each intramedullary nail belonging to the second series is between 180 and 300 mm, and advantageously varies in steps of 20 mm between each intramedullary nail size. Preferably, the intramedullary nails belonging to the second series are substantially cylindrical and have diameters of between 7 mm and 10 mm and varying in steps of 1 mm between each intramedullary nail size. According to one embodiment of the invention, each through-hole formed in the proximal portion of each intramedullary nail belonging to the first and second series is smooth, and each intramedullary nail belonging to the first and second series comprises a rib projecting therein each transverse hole, transversely thereto, each rib having dimensions such that it is adapted to serve as support for the thread of a fastening screw for passing through the through hole. Each of these ribs thus forms a stop for locking the nail relative to the corresponding screw in the axial direction of this screw. After implantation, the possibilities of movements of the nail relative to each screw are thus significantly limited, without a particular type of screw is imposed so far since this rib adapts to both threads cortical bone screws and spongy bone screw threads.
Preferably, each rib is arranged substantially at the middle portion of the corresponding through hole. Each rib advantageously extends substantially perpendicularly to the axis of the corresponding hole. Advantageously, each rib has a width of about 1 mm and a peak located about 1.1 mm from the axis of the corresponding hole.
According to one characteristic of the invention, the intramedullary nails belonging to the first and second series comprise identical indexing means arranged in their proximal portion and intended to cooperate with complementary indexing means provided on an ancillary device designed for the application of said nails. intramedullary. Preferably, the indexing means provided on each intramedullary nail comprises an indexing groove opening into the proximal end of said intramedullary nail and of axis substantially parallel to the axis of the through hole closest to said proximal end.
The range of intramedullary humeral nails according to the invention comprises a first series of humeral intramedullary nails 2 of different sizes intended for the treatment of proximal humeral fractures, and a second series of humeral intramedullary nails 2 'of different sizes intended for the treatment of medial humeral fractures. . It should be noted that nails 2 'belonging to the second series can be used for the treatment of medial humeral fractures associated with simple proximal humeral fractures. Each intramedullary nail 2, 2 'belonging to the first and second series is made of an osteoconductive metal, such as a titanium alloy, and comprises, as shown more particularly in FIG. 1, a proximal portion 3, 3 ', a distal portion 4, and a plurality of through holes 5, 5', 6 formed in the proximal and distal portions of the nail and intended for the passage of fixing screws. The various through holes 5, 5 ', 6 extend substantially perpendicular to the axis of the intramedullary nail and are offset longitudinally with respect to each other. Although not shown in FIG. 4, the distal portion of an intramedullary nail 2 'is similar to that of an intramedullary nail 2 and therefore comprises through-holes similar to those shown in FIG. 1. Each intramedullary nail 2 belonging to the first series has four through holes 5a, 5b, 5c, 5d formed in its proximal portion 3, while each intramedullary nail 2 'belonging to the second series has three through holes 5'a, 5'b, 5'c formed in its proximal portion 3 '. The distance D between the proximal end 7, 7 'of an intramedullary nail 2, 2' and the through hole 5a, 5'a closest to the latter is substantially identical for all the nails belonging to the first and second series, and preferably substantially equal to 10 mm. In addition, the orientation of the through hole 5a, 5'a closest to the proximal end 7, 7 'of an intramedullary nail 2, 2' is substantially identical for all the nails belonging to the first and second series . Preferably, the through hole 5a, 5'a closest to the proximal end 7, 7 'of an intramedullary nail 2, 2' is located substantially in the antero-posterior plane of the latter for all nails belonging to the first and second series. For each intramedullary nail 2 belonging to the first series, the second through hole 5b, the third through hole 5c and the fourth through hole 5d from the proximal end 7 of an intramedullary nail are respectively oriented at approximately 30 °, 90 And -30 ° relative to the through hole 5a closest to the proximal end 7 of said intramedullary nail.
